Fitness & Nutrition Assessment

Ages 6 months-11 years. Supervised care for your children while you work out. Kids Zone features toys, computers and staff trained in CPR/AED and First Aid. Reservations are required and maximum stay is 2 hours. Reservations can be booked at kidszone@csparks.org or call 630-784-6115.

Relax in our saunas after a tough workout only accessible to all-inclusive members. Our premium locker rooms also include keyless lockers (no need to bring a padlock) and towel service.

Holiday - Jan 1

A free introduction to cardio, strength and selectorized equipment is available to all-inclusive members. Learn how to operate your favorite machines and receive guidance on proper form and technique. Members ages 12-13 must complete an equipment orientation before utilizing the fitness center. Occasional Care Flex Pass

Good for five Before or After School admissions $65. Flex Passes cannot be utilized for Monday, After School. Flex Passes can be used for either Before and/or After School ActivKids. To use the Flex Pass for After School ActivKids, call your child’s school office and site a minimum of 24 hours in advance to ensure enough staff and supplies. No call is necessary for Before School ActivKids. Same day usage is not allowed for After School.

How to Register To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available online, at your school’s ActivKids site, or at the Fountain View or Simkus Recreation Centers. Registration must be at least 24 business hours prior to your child’s start date. A minimum of two weeks' advance notice is required if your child needs special assistance from a WDSRA aide (1:1 support staff). More information can be found in the WDSRA section of this guide.

Parent Code of Conduct By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at the preseason parent meeting. Meet and Greet The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms. Player Evaluation Players get rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend. Playing up Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to: The player must register at his or her age or grade. A request to play up must be made to the program supervisor. The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation). Placed on a Specific Team No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. New Swimmers Select a swim class level based on your child’s age and previous lesson history or attend a skill test day. Returning swimmers — select a swim class level based on your child’s previous swim lesson report card. Custom and Private Lessons These lessons are specially scheduled for you. Should you fail to show up for a lesson without notifying the instructor at least 24 business hours in advance, you forfeit the lesson. Cancelling a lesson for any reason requires 24 business hours. One make-up class is allowed if above communication is properly followed. Lessons not finished by the end of the session expire. Combined Classes Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Coordinator at 630-784-6187. Parent Attendance Stay for the entire swimmer’s class, in case of emergency where consent is required. Report Cards Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. First Day First day arrive 15 minutes early for check-in. Bring — towel, swimsuit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swimsuit; report card from previous session. Do not bring — swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons. Refunds To receive a full refund withdrawals and transfers must be completed 24 business hours prior to the first day of lessons. Refunds and make-ups will not be given for classes missed. A volunteer-based program for youth who want to join the world of competitive swimming. Dolphins compete in the Northern Illinois Swim Conference along with 14 other park district teams. Competition is from October through mid March. All Swimmers must be able to swim 25 yards without stopping and without assistance to join swim team. A swim assessment may be required. Graffiti and spray paint on playgrounds, damaged park amenities, burned port-o-lets, tire ruts on ball fields these are just some of the incidents we deal with weekly as we work to maintain our parks and playgrounds. Repairing damage caused by vandalism costs us money in staff time and in replacement or repair of property. Park District property is community property - parks, playgrounds, ballfields - for your enjoyment. You help us pay for it on your tax bill. Help us protect our shared property. If you see vandalism happening, call 911. The police need to know when vandalism is happening so they can catch the culprits. If you see vandalism after it has happened for example, you’re with your kids in the morning at your local park and you see obscenities sprawled on playground equipment – call us at 630-784-6100 so we can photograph and report the damage to police and work to clean up the vandalism.

By Mail: Complete a registration form and mail the form and check to Simkus Recreation Center, 849 W. Lies Rd, Carol Stream, IL 60188.

Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks. Activities with recurring or monthly payments must have an EZ Pay Agreement on file. Your payments are charged to your credit/debit card. For more information, please visit our Registration Services Team at Simkus or Fountain View Recreation Centers to complete an EZ Pay authorization form.

Photography Policy

Carol Stream Park occasionally takes photographs and videos of participants at our parks or facilities, in park district programs, classes or events for marketing purposes.

Once the maximum enrollment is met for a class, a waiting list will be formed. If space becomes available or additional classes are created, you will be contacted. Payment is due when being waitlisted. Credits will be issued after the first class for those who are not able to be registered.

Inclusion Services

Inclusion Services are designed to provide the least restrictive environment while providing the maximum opportunity to participate in regular park district programs. There is no charge for WDSRA’s support. To register for services, simply indicate that special accommodations are needed on the park district registration form. Two-week advance notice is requested for inclusion support.

Insurance

The Park District does not carry medical or accident insurance for program participants or park patrons. Review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial Assistance

Most classes require a set minimum number of participants. Register at least a week prior to the start date to avoid cancellation.

We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications available at Fountain View Fitness Center and Simkus Recreation Center.

Program Capacity

Smoking Policy

All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

The District’s philosophy is to promote health and a clean air environment. The use of any and all tobacco products is prohibited in all park district buildings and facilities and at all parks and trails. Carol Stream Park District joins other park districts in Illinois with a tobacco-free policy. To read the full policy, go to csparks.org.

Refund and Withdrawal Policy

It is the District’s intent to provide quality recreation programs and services. The District recognizes however, that there may be occasions when the event/service doesn’t match a customer’s needs, or when the customer is unable to participate. For those occasions, the following withdrawal policy will apply. (a) All requests for refunds will be assessed a ‘Service Fee’ of 10% of the original, total fee - $5 minimum / $50 maximum, or 10% of the remaining credit balance - $5 minimum/$50 maximum. (b) No refunds under $5; credit will be placed on account. (c) No refunds after 2nd meeting of class/program. (d) For monthly/quarterly payments, or memberships, there is no refund of the current period; a cancellation of the future months/period charges will be allowed. (e) Forever Young Programs – Service Fee will apply after the payment deadline, and refunds may be issued as long as Supervisor approval is given. (f) No refunds after event. (g) No refund of annual memberships including but not limited to Walking Track, Lap Swim, Coral Cove Water Park or Bark Park. Exceptions in which a Withdrawal Service Fee will not be assessed: • Medical note exception is valid any time prior to the start of the last class and will not be subject to a ‘Service Fee’; a prorated refund or credit will be issued. This does not apply to events/trips, and does not include costumes, uniforms, meals, or tickets. • Transferring to another program or date in lieu of receiving a refund. If the class has begun, the credit available to transfer towards another program will be calculated on the prorated balance from the original registration. • Leaving on Account in lieu of receiving a refund. Funds should be used within a 6 month period. Any credit that is returned will be less the applicable ‘Service Fee’. No Refunds are offered: • If participant did not attend all or any portion of the class/program (forgot about class, moved and did not withdraw); • For Summer Camp, ActivKids, or Preschool because participant’s vacation, illness, for any other absence; • For returned or unused (new or used) uniforms or costumes; • For Enrollment, Administration, Registration, or Annual Fees; nor for Tickets; • For Punch Passes (that have no expiration date) includes Pool, Open Gymnastics, Open Baton • For “Private” Recreational Packages: Personal Fitness Packages, Customized Private Swim Lesson Packages, Private Voice, Private Gymnastics Lessons • For trips – no refund unless customer provides a replacement. If replacement is found, Service Fee will still be assessed to the original participant, • For Adult Athletic Leagues – if a team chooses to drop out • For events/activities under $5 – money can be left on account.

Additional Notes: No prorated registration, enrollment, or administration fee for late registrants.

Meet Your Park Board of Commissioners The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait ― a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. MAKE A DIFFERENCE IN YOUR COMMUNITY The Carol Stream Parks Foundation has a donation GOAL of $5,000 to support the Carol Stream Park District Scholarship Program. The Scholarship Program has financially assisted 52 families, 15 adult/seniors and 101 children. Recipients of Scholarships are Carol Stream residents well below the standard USDA poverty level who need financial assistance to participate in Park District programs. Without the Scholarship Program, children would not be able to participate in sports or programs with their peers, and seniors or those who are disabled due to health issues that need therapy, would not be able to take part in physical or warm water activities.
