Rentals & Parties Event or Meeting Fountain View Fountain View Recreation Center is the perfect venue for birthdays, receptions, showers, graduation/sweet sixteen parties or any special occasion. With scenic lake views, access to the outdoor patio and state-of-the-art AV equipment, this is the perfect place for your party or meeting. The banquet room can accommodate 160 people and can be split into two or three individual rooms, each with AV capability and a variety of room set up options. Ask about our new amenity options, including a stage riser. $65 per hour Tables and Chairs Party Host (Add Catering Kitchen for $25)

Host a smaller gathering at the newly renovated rooms at the Simkus Recreation Center! These rooms are the perfect place for a party, bridal/baby shower, graduation or holiday event. Each room holds up to 60 people and includes a refrigerator, freezer, and sink and countertop area. This site also features closer parking for you and your guests! (capacity 60)

Within three business days a staff member will contact you to confirm your booking. Full payment is required within 7 days of booking confirmation. Discounted rates apply for residents and civic groups or room rentals only. Hourly fee increases $25/hr after regular operating hours.

Party Wagon

Wow the crowd at your next party or picnic with our Party Wagon! The fun-making trailer is stocked with 10 tables, 100 chairs, a 10’ by 10’ pop up canopy, a supersized charcoal grill, ladder toss, bocce set, volleyball net, jump ropes and bean bag game. We deliver right to your Carol Stream home. The Party Wagon is available April - October. Reservations may be taken starting February 1, 2017. Cost is $125 per rental.

ActivKids Before & After School Care Kids have a blast learning new games, sports or creative activities at ActivKids, our Before and After School recreation program for kids in grades K-5. Kids love ActivKids for the fun and friends and parents love knowing their children are safe, supervised and feel at home in the familiar setting of their own school.

ActivTime will keep your children actively engaged in learning new activities each month. New for the 2016-17 school year: Northbrook Park District Team Building and Carol Stream Park District Dance-n-Style!

Days Off Trips ActivKids Day Off Club dates follow the District 93 school calendar. Most dates coincide with other school districts such as, U-46, D200, and D2. Take part in field trips, special events, and enjoy crafts, games and physical activity. For complete field trip list see csparks.org/activkids. Location: Simkus Recreation Center Grades: K-6th (open to students of all school districts) Register 48 hours prior to the start date of the program to receive the lowest rates. All registrations are based on program space, staffing, and enrollment. Registrations may be accepted less than 48 hours in advance if there are program openings.

Receive One Free Week Pay all program fees in full at time of registration. Discount applyed to enrollment fee.

Your monthly payment will be calculated based on the days of the week your child is registered for. Payment amounts already include discounts for school holidays, breaks and non-attendance days. EZ pay form required, automatic withdrawals will be processed on the 1st of every month. If your child is registered for less than 5 days per week your monthly payment is less.

After September 1, 2016

One time registration fee of $20 plus payment for the current month, prorated if needed, is due at registration. All subsequent payments will be automatically withdrawn with EZ Pay on the 1st of each month.

Occasional Care Flex Pass

Good for 5 Before or After School admissions - $65. Flex Passes cannot be utilized for Monday, After School. Flex Passes can be used for either Before and/or After School ActivKids. To use the Flex Pass for After School ActivKids, please call your child’s school office and site a minimum of 24 hours in advance to ensure enough staff and supplies. No call is necessary for Before School ActivKids. Same day usage is not allowed for After School.

How to Register

To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available online, at your school’s ActivKids site, or at the Fountain View or Simkus Recreation Centers. Registration must be at least 24 business hours prior to your child’s start date. A minimum of two weeks' advance notice is required if your child needs special assistance from a WDSRA aide (1:1 support staff). More information can be found in the WDSRA section of this guide. The goal of this challenge is to walk, jog or run 500 miles within one year (52 weeks), which means you will have to average around 1.4 miles per day or 9-10 miles per week to stay on pace. The park district strives to provide trips and activities for everyone. Most Forever Young programs are accessible; however, participants using walkers or wheelchairs should confirm the trip location’s accessibility in advance. Return the registration form with your trip selections to the Simkus Recreation Center at 849 W. Lies Rd, Fountain View Recreation Center at 910 N. Gary Ave in person, by mail, by fax to 630-289-1972, or register at csparks.org. We cannot accept registrations over the phone. Please make sure we receive your registration form and payment by the indicated deadline otherwise your spot will not be guaranteed. Space is often limited, so complete your registration early!

Trip Cancellations

We understand that trip cancellations sometimes cannot be avoided. Withdrawals are only processed after the payment deadline if you find a replacement or a participant on the waitlist takes your spot. If a friend is taking your place on a trip, they must be paid with a registration form and waiver completed in advance. Passengers who are not on our roster are not permitted to attend.

The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants registering for Forever Young trips, outings and activities must recognize that there is an inherent risk of injury arising out of this activity. Parent Code of Conduct

Register at csparks.org or pick up a registration packet at the Simkus Recreation Center. By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at the preseason parent meeting.

What is the meet and greet?

The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms.

What is the player evaluation?

Players get rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend.

How do I register for soccer and tee ball leagues?

Registration is by grade level. Players are to be registered for the grade the player will be in for 2016/2017 school year. Preschoolers must be 4 years old by program start date. Participants can participate in Instructional Tee Ball and Instructional Soccer without conflict.

How do I register for youth baseball?

Registration is by age. Players are to be registered based on their age as of April 30, 2017.

How do I register for youth softball?

Registration is by age. Players are to be registered based on their age as of December 31, 2016.

What happens if I register after the deadline?

Players placed on a wait list are not guaranteed placement on a team. If you register before team formation the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.

Can my child play up?

Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to: • The player must register at his or her age or grade. • A request to play up must be made to the program supervisor. • The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation).

Can I request for my child to be placed on a specific team?

No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However, one mutual friendship request will be honored for players in an instructional league, Pinto (8U) baseball, or grade 1 and 2 soccer. A park district request form must be used for these requests and the guidelines listed on the form must be followed. These forms are available at Simkus Recreation Center and on csparks.org. Request deadlines are the same as registration deadlines (not early bird).

Can my business sponsor my son or daughter's sports team?

Yes, if your business would like to sponsor a youth sports team, contact Pam at 630-784-6123, pamf@csparks.org. Specific sponsorship requests based on availability.

Swimming Class Info

• Location: Fountain View Recreation Center, 910 N.

Gary Ave., Carol Stream, in the 25-yard indoor pool and therapy pool. • New swimmers — select a swim class level based on your child’s age and previous lesson history or attend a skill test day. Returning swimmers — select a swim class level based on your child’s previous swim lesson report card. • Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Coordinator at 630-784-6187. • First day arrive 15 minutes early for check-in. • Stay for the entire swimmer’s class, in case of emergency where consent is required. • Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. • Please bring — towel, swimsuit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swimsuit; report card from previous session. • Do not bring — swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons. • To receive a full refund withdrawals and transfers must be completed 24 business hours prior to the first day of lessons. Refunds and make-ups will not be given for classes missed.

DOLPHINS WINTER SWIM TEAM

A volunteer-based program for youth who want to join the world of competitive swimming. Dolphins compete in the Northern Illinois Swim Conference along with 14 other park district teams. Competition is from October to March and swimmers may join any time before February 1. Parents are required to volunteer at swim meets. Swimmers must be able to swim 25 yards without stopping before being allowed to compete.

Who We Are

The Carol Stream Parks Foundation is a non-profit organization dedicated to developing additional financial support for public parks and recreation programs of the Carol Stream Park District. It is managed by a board of directors who live or work locally and are passionate about providing a strong financial foundation for parks and recreation in the community.

What We Do

We build and strengthen the financial resources of the District by receiving gifts, grants, bequests and endowments on the District’s behalf. Contributions from individuals and businesses are used to improve or add to existing park projects or programs, or to help fund future projects.

Make a Donation

Your donation will help us fund our Scholarship Program, Parks & Facility Improvements, Program Enhancements and Green Initiatives. As a 501(c)3 corporation, all contributions to the organization are tax deductible. Identified donors will receive a written receipt.

Leave a Legacy

Including a gift of any size in your will is a way you can leave a legacy for your parks.

Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks.

Photography Policy

Carol Stream Park occasionally takes photographs and videos of participants at our parks or facilities, in park district programs, classes or events for marketing purposes.

Oops!

The Carol Stream Park District staff has made every effort to prepare our marketing material as accurately as possible, however, errors can occur. Circumstances may require that adjustments be made to programs, fees, schedules, etc. The Park District reserves the right to make any such adjustments. The Carol Stream Park District apologizes for any inconvenience these errors may cause.

Lost & Found

Contact us if you lost something at one of our parks (630-784-6107) or facilities (630-784-6100).

Avoid Cancelation

Most classes need a set minimum number of participants to be able to run them. Please register at least a week before the start date.

Program Capacity

All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting Lists

Once the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you willelass for those who are not able to be registered.

Refund and Withdrawal Policy

It is the District’s intent to provide quality recreation programs and services. The District recognizes however, that there may be occasions when the event/service doesn’t match a customer’s needs, or when the customer is unable to participate in an activity. For those occasions, the following withdrawal policy will apply. All requests for refunds will be assessed a ‘Service Fee’ of 10% of the original, total registration fee - $5 minimum / $50 maximum. No refunds under $5; credit will be placed on account. For monthly/quarterly payments, or memberships, there is no refund of the current period; a cancellation of the future months’ charges will be allowed. Forever Young Programs – Service Fee will apply after the payment deadline, and refunds may be issued prior to the event as long as Supervisor approval is given. No refunds after event. Exceptions in which a Withdrawal Service Fee will not be assessed: Medical note exception is valid any time prior to the start of the last class and will not be subject to a ‘Service Fee’; a prorated refund or credit will be issued. Transferring to another program in lieu of receiving a refund. The credit available to transfer towards another program will be calculated on the prorated balance from the original registration. Leaving on account in lieu of receiving a refund. If funds are not used by the end of any calendar year, the credits will be returned less the applicable ‘Service Fee’. No Refunds are offered: if participant did not attend all or any portion of the class/program (forgot about class, moved and did not withdraw) for Summer Camp, ActivKids, or Preschool because participant’s vacation, illness, or any other absence for returned (new or used) uniforms or costumes for Enrollment, Administration, Registration, or Annual Fees

There is no charge for WDSRA’s support. To register for services, simply indicate that special accommodations are needed on the park district registration form. Meet Your Park Board of Commissioners The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait ― a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly.
