(Re: What’s His Name?, Issue 221) In the article you published about the sources of common names, it was written that there are names that we assume are Yiddish but are not (the examples brought included the names Getzel, Hertz, etc.). The article asserts that they are German names. May I mention that so are the last names Rosenberg, Spitzer, Eisenberg, etc.? In fact almost all Ashkenazi last names are of German origin… and most Yiddish first names — like Liba, Faiga, Gitta — are of German origin, too. In fact, all original Yiddish words and names, with the exception of Lashon Hakodesh words that are used in the Yiddish language, originated from Old German. As Yidden spread outward from Germany and France and settled in Eastern European countries, many Slavic words entered the Yiddish vocabulary, too. In our generation, those of us living in the English-speaking countries use many English loan words mixed into our Yiddish, and Yiddish speakers in other countries use loan words from their native language, thinking that they are speaking purely Yiddish. YOUR VOICE A DANGEROUS DRIVE Recent letters about school has prompted me to speak up in the hopes that with public awareness we can create a safer environment for our children. Within the past year or so, I have noticed a disturbing trend that seems to be growing — of school buses with children turning around by backing up in side streets in order to return to the direction they were coming from. It is illegal and very unsafe for a bus to be doing that! I understand that bus drivers are on a very tight schedule, and the growth of Monsey with the traffic situation and ever-expanding coverage areas for bus routes only compound the race against timelines. However, to be saving a minute or two that it would take to drive around the block on the account of safety is unacceptable. Think of the winter with compromised traction, uphill/downhill around town, and little kids trekking in the snow running toward home not necessarily paying attention to the direction their bus is taking after they were dropped off... Let’s unite and voice our objection to this compromise to our children’s safety. If you see a bus make a U-turn or backing up in any which way, do yourself and others a favor, please: Take note of the bus number and report it to the respective bus company/operator. Hopefully they’ll address our concern by conveying the intolerance of such behavior to their drivers. Thanks for being the platform for voicing the views of Monseyites. A Concerned Neighbor

While driving around Monsey, I too often witness a bothersome sight: Mothers with baby carriages trying to cross the street first push out their baby’s carriage to the busy road, and only then check to see if cars are passing. This can be very dangerous, chas v’shalom! Parents should rather hold onto the carriage from the side, on the sidewalk, and then peek out to the road to see if it’s safe to cross the street. I’ve seen too often carriages almost getting hit from a passing vehicle — while the child was inside! A carriage is way less noticeable than a person checking if the roads are clear to cross the street safely. A.D.

Fifteen years ago, at the height of the intifada, Kever Rachel was a desolate place. Parshas Lech Lecha By: Malka Schneps

This week’s parsha famously begins, “Lech lecha mei’artzecha u’mimoladitecha u’mibeis avicha…” (Bereishis 12:1). Hashem commanded Avraham Avinu to leave behind his land, his birthplace and his father’s house, and blindly set out on a journey to a destination not yet revealed. Essentially, Avraham was commanded to leave behind every external factor that had any influence on who he was, from the nature with which he was born, to his lineage, to the way he was raised, to the environment in which he’d lived until that time, and set out on a journey toward his highest self. Ultimately, Avraham would move so far beyond the boundaries that seemingly defined him that his name was changed. But why did Avraham Avinu have to take a physical journey in order to accomplish this change? Could he not have transcended himself in the place where he was born? There are two answers to this question: (1) The physical journey itself was an integral aspect of Avraham Avinu’s growth; and (2) The destination of that journey was necessary for Avraham’s growth as well. Regarding the physical journey, Hashem sent Avraham Avinu out to travel without revealing to him the final destination of the journey. The Sfas Emes explains that the uncertainty rendered by such a blind journey would remove from a person any false sense of autonomy. When a person is settled in one place and has consistency and predictability in his life and live-

lihood, he will not feel his dependency on Hashem to the same extent as a person who does not know from one day to the next where he will be or how he will earn his living. A settled person can formulate plans and goals, and seek to do his own will to a far greater extent than a person who is traveling, especially when he does not know his destination. Second, regarding the necessity of Avraham’s destination, the Ramchal explains that while Avraham Avinu was able to perceive Hashem’s existence and His Oneness while living in Ur Kasdim, which was in chutz l’aretz, Avraham would only be able to attain a deeper understanding in Eretz Yisroel, the Eretz Hakodesh. This is the deeper meaning of the lashon in Hashem’s command, “Lech lecha… el ha’aretz asher areka — Go… to the land I will show you.” Hashem is telling Avraham to go to the land where Hashem will show Avraham the deeper perspective of Hashem and Hashem’s hashgacha in this world that Avraham sought. The Ramchal further explains that Eretz Yisroel is the specific place where Avraham Avinu can gain this perspective. Avraham’s characteristic middah is chesed, and Eretz Yisroel is the place that reveals Hashem’s chesed most clearly. Thus, davka in Eretz Yisroel can Avraham be bentched, “And I will make you into a great nation, and I will bless you, and I will make your name great, and you shall be a blessing” (Bereishis 12:2).

In order to achieve his fullest potential, it was necessary for Avraham Avinu to go on a journey in which his dependence on Hashem was felt most keenly, and travel to a place where it would be possible to attain the spiritual heights and closeness to Hashem that he sought. What does this mean for us, Avraham’s progeny? After all, Chazal say, “Maaseh avos siman l’banim — The deeds of the fathers are signs for their children.” The Sfas Emes quotes the Zohar Hakadosh that Hashem is constantly calling out the command “Lech lecha” to all people, at all times. Hashem was not speaking only to Avraham. Only Avraham heard the call. Only Avraham perceived Hashem’s command and acted upon it. Indeed, Hashem did not speak to Avraham because of Avraham’s greatness; Avraham’s greatness was that he chose to listen. Hashem is calling every one of us at all times — He is calling us right now: “Lech lecha.” Do what is right, not what is comfortable and easy. Learn Torah. Do mitzvos. Strive for closeness to Hashem. Do not feel bound by the nature with which you were born, nor the nurture with which you were raised, nor the environment in which you grew up. Every single one of us is capable of rising above these physical limitations to reach our highest potential. It is encoded within our spiritual DNA to succeed; we need only follow in our father’s footsteps.

New Car Seat Law Requires Kids Under Two to Face Rearward A newly enacted law will require car seats for nearly all children under the age of two to be positioned facing the back of the vehicle. USA Today reported that the law was intended to promote greater safety for babies and toddlers and applies to all children two years old and under who do not exceed the height and weight guidelines outlined by their car seat’s manufacturer. Previously, New York State law did not specify which way car seats should face, although manufacturers had long advocated for placing infant car seats facing rearward to better protect their occupants. The law was signed by Governor Andrew Cuomo in October 2017 and went into effect on Friday, November 1. According to the bill, children under the age of two are 75 percent more likely to be injured if they are sitting in forward-facing car seats instead of rear-facing seats. The risk factor increases five times for children between the ages of one and two years old. The American Automobile Association reported that car crashes are the second leading cause of injury and deaths in children ages one to four. In year 2017, 675 children under 12 died in crashes, with 116,000 others injured. Senator Joseph Robach sponsored the bill in the State Senate. “By ensuring that children under the age of two are restrained in a rear-facing car seat, we can limit the ef-

fects a car accident may have on their head and neck and increasing their safety as a whole,” said Senator Robach. Four other states already have laws on the books requiring rear-facing car seats for children under two: California, New Jersey, Oklahoma and Pennsylvania.

Kosher Food Court Just One of Many Amenities As Record-Breaking Shopping Center Opens in New Jersey All eyes are on East Rutherford, New Jersey, where an mega-mall featuring shopping, entertainment and a kosher food court recently opened to the public, with its owners hoping to attract 40 million visitors per year. Located next to MetLife Stadium, the massive entertainment complex known as American Dream had a limited opening at the end of October. Built by a Jewish family from Minnesota, the 3 million-square-foot mall is a $5 billion project that will ultimately include an indoor ski slope, a Legoland Discovery Center, an aquarium, a Ferris wheel, a miniature golf course and over 300 stores, including Hermes, Saks Fifth Avenue, Zara and Century 21. Kosher Today reported that a kosher food court will be among the American Dream’s many dining options with pizza, fried chicken and both Israeli and Chinese cuisine to be available six days a week. The mall is a project of the Triple Five Group, a Canadian real estate firm owned by the Ghermezian family, whose portfolio includes two other massive shopping complexes — Mall of America in Minnesota and West Edmonton Mall in Alberta. American Dream president Don Ghermezian said that visitors to the mall will find it to be “the realization of their wildest dreams.” Currently, the only sections of the American Dream that are open to the public are the ice skating rink and the largest indoor theme park in the Western Hemisphere, featuring over 35 rides for children of all ages and the world’s steepest roller coaster. Triple Five plans to continue opening sections of American Dream in stages, with the first retail stores expected to open this coming spring.

The man in M.R. Stefanik Airport in Bratislava shifted his small valise to his right hand. Even its relatively low weight was heavy for him. Either his age was catching up with him, or this trip was just not doing him much good. But he was the one responsible for the fact that he had come here, so he had no one else to blame. “Mr. Joe Ludmir?” A taxi driver leaning on his car stood up when he approached. “That’s you, right?” “Right.” The man nodded at him. “I see that you recognized me, despite the years that have passed.” “Of course,” the driver, a local, replied. “You’ve hardly changed.” Sure, only fifteen years had been added to his age since his last visit here, but giving compliments to a tourist who hired one as a driver for his entire trip could surely be very profitable. Joe’s lips curved into a halfsmile, and he let the driver put his valise into the trunk. “Is that all?” the driver asked in surprise. “Like then?”

“You remember well; I don’t like traveling with too many things.” He ran his fingers through his gray-white hair and straightened his small yarmulke. “If I lack for something, I’m sure you’ll be very happy to buy it for me.” The streets hadn’t changed much in the last fifteen years, and Joe couldn’t decide if he remembered them so well from his wandering around during that visit, or from the many photos he had taken to capture every single corner of the city of his birth. He leaned back and looked out the window in silence. Every so often, between the buildings, he could see the glistening waterline of the Danube, with the Bratislava Castle rising up behind it. “Did you visit the castle last time you were here?” the driver asked when he saw his guest gazing at the castle’s famous turrets. Joe nodded. He didn’t know what it was, but something about this city enveloped him in a cloak of depression. “We’re going to Mamaison, right?” the driver confirmed. “You reserved a room in that hotel, yes?”

“Yes, I have a confirmed reservation,” the tourist answered heavily. “How do you like it there?” “It’s excellent. You eat only kosher, right?” “Right.” Joe fell silent again. The taxi drove up into the parking lot of the former Jewish Hospital, which had been converted to a hotel in recent years. The driver took the valise out of the trunk and followed his passenger. He put the suitcase down in front of the reception desk, and Joe turned to him. “Thank you,” he said as he pulled out his wallet. “Where to tomorrow and when?” the driver asked. “Do you want to go where all the tourists like to go? The Bratislava Castle, the Museum of Clocks, the National Gallery, or the —” “No, not at all.” Ludmir looked at the sky, which was turning pink, through the open door. “In the morning I’ll have some research to do. I’ll let you know around lunchtime when we’ll be going and where.” “No problem,” the driver said, and Joe turned back to the desk clerk. “First time here?” the desk clerk asked, swiping the credit card. “Oh, no, I see that it’s your second time. Allow us to accept that as a compliment.” Joe smiled. “Last time wasn’t my first visit in this city, either,” he said. “I was born here. Like most Jewish children in Bratislava of those years.” *****

The second time was not as hard as the first; the third time was even easier, and by the fourth time, he entered the room without even pausing at the threshold. He put down his hat and jacket and sat down on the chair next to the bed. Shabsi crossed the room to bring over the other chair, and Binyamin stared for a moment at the ceiling fan. Another once or twice on this job, and he’d be able to buy himself a new hat. The grayish hat he had seen the first time still remained etched in his memory, but he could not pinpoint what exactly had frightened him so much then. For a fraction of a second, his gaze flitted to the tallis resting on the ground, near the opposite wall. He even felt a flash of heat run through him. Hastily, he opened his pocket Mishnayos. “Rich people,” Shabsi whispered to him as he took a seat beside Binyamin. “Mmm…?” Binyamin queried. “It’s not the time or the place to talk about it, but you can tell that they won’t need to fight over the yerushah here. There’s plenty to go around.” “Nu, uh,” Binyamin concluded the discussion. “You’re right, tzaddik. Let’s learn.” Shabsi wanted to add that he had the feeling they’d get a good tip here, but then it certainly wouldn’t be nice on their part to waste time chatting idly. Instead he took out his own Mishnayos. A quarter of an hour passed, then half an hour, then forty minutes. “Listen a minute, Binyamin.” Shabsi closed his sefer. “I’m really exhausted. I didn’t sleep much last night, and it’s hard for me to make up lost sleep during the day. I’m going out for some fresh air, okay?” Binyamin shrugged and nodded. If it would have been his first night on the “job,” he would have probably been unnerved at the idea of staying alone with a niftar. But now it no longer fazed him. The part that he found hard was interacting with the grieving families, but he really tried to avoid that as best he could. Shabsi was the one who took care of all the practical details before and after; he just showed up, said Tehillim or learned Mishnayos, and that was it. He heard footsteps approaching the room. Shabsi must be coming back, he thought. But the footsteps stopped at the door, and no one entered the room. Instead, he heard quiet, hoarse sobbing. Someone close to the niftar. Probably a family member. Binyanim trained his eyes at his sefer and kept learning quietly. “Excuse me,” the voice, a woman’s, choked out from the door. “I… I just got here from the airport, from London. I want… to take leave of my father, privately. Do you mind stepping out of the room? It will just be a few minutes.” Binyamin got up with a lowered gaze and slipped out the door. Yes, Mr. Katz’s apartment was opulent, and he was hardly familiar with such living standards. But what did he know about fancy apartments? He wanted to move away from the room; he couldn’t bring himself to hear what the daughter would tell her deceased father as she parted from him. Still, his sense of responsibility for the job didn’t let him go too far. Out of the corner of his eye, he saw the profiles of two silent men in the dining room. One stood near the open window, and the other was resting his head on a nearby wall. He didn’t want to be too close to them either. He stood like a potted

plant in the middle of the small hallway and murmured the first perek of Tehillim, then the second and third… and then perek dalet… Just as he finished perek hey, he heard someone clearing her throat from the doorway of the room. He understood that he was supposed to go back and take up his position again. But sitting on the chair where he had been a few moments earlier was a thick envelope. He looked at the chair, wondering what this was about. Mr. Katz’s daughter spoke up from where she stood. “We’ll be leaving here in about two hours, and I don’t want to take that envelope with me. But I don’t want to leave it here unguarded either…” She was crying again, and Binyamin fixed his gaze on the tips of his shoes. They were pretty rubbed out, though hopefully a good polish would help… “I… I want to ask if you can guard the envelope for now, without talking about it… because it’s very precious… And after the levayah…” She sniffed. “After the levayah, I’ll send someone to pick it up from you.” At the end of her sentence, she choked up again; a few seconds later, she was gone. The envelope was a bit heavy and full of lumps. Binyamin bit his lip but went over to the bed and stuck the envelope into the inside pocket of his jacket. Then he went back to his place and opened his Mishnayos again. A minute later Shabsi walked in. “It’s almost neitz,” he said, somewhat cheerfully, and sat back down. “That means that we should be finished here soon. I think that the rest of the family is starting to arrive. I think I saw the

Binyamin did not know how his friend was going to find someone to speak to in the subdued hubbub that filled the house. But Shabsi succeeded; he always did. And with a grim expression that matched the atmosphere in the house, he emerged to the stairwell. Once

Because I would actually like to personally meet this person; people say that whatever he gives Ohel Torah — and trust me, it’s a fancy place — is not even a hundredth of what he has. I wonder if it was him, or if they haven’t arrived yet.” “I’m trying to decide if I should go to sleep for a little bit now,” Binyamin said, not sure why he wasn’t telling Shabsi that the couple from London had indeed arrived. “Something like forty minutes, and then I’ll get up for minyan in yeshivah. Or maybe I should go find an in-between minyan, after neitz but before the regular ones, and take a nap afterward…” “I’m going to sleep now,” Shabsi said. “Not for a little bit, but for a lot! Bernie Sanders told the crowd that “I would use the leverage of $3.8 billion” to pressure Israel to change course and improve the living conditions for the Palestinians in Gaza. He also stated that part of the Israeli aid should go toward humanitarian assistance in Gaza. Senator Elizabeth Warren also appeared open to withholding American aid to Israel as a means of pressuring the Israeli government to change its policies in the West Bank. She spoke of ways “to apply pressure and create consequences for problematic (Israeli) behavior as previous Democratic and Republican presidents have done.” Her remarks come on the heels of her comments in Iowa last week, where she responded to a question regarding applying pressure on Israel over Palestinian issues by saying that “everything is on the

table.” South Bend Mayor Pete Buttigieg echoed a similar sentiment concerning Prime Minister Netanyahu’s pre-election pledge to annex West Bank settlements. He said that he feels a responsibility “to ensure that U.S. taxpayers’ support for Israel does not get turned into taxpayer support for a move like annexation.” However, when a Wall Street Journal reporter asked leading Democratic candidate and former Vice President Joe Biden about the comments of his Democratic rivals, he was unequivocal in his condemnation of such policy. He said, “The idea that we would draw military assistance from Israel, on the condition that they change a specific policy, I find to be absolutely outrageous,” and “I would not condition it, and I think it’s a gigantic mistake. And I hope some of my candidates who are running with me for the nomination; I hope they misspoke, or they were taken out of context.” THE IMPACT: The policy face-off between Sanders,

Warren and Buttigieg on one side and Biden on the other exposes the deep rift in the Democratic Party between its progressive and centrist elements. Biden has a long record of support for Israel. He has referred to himself as a “Zionist” and credits his father for teaching him about the importance of supporting Israel. In remarks he made at Tel Aviv University and later repeated at the 2011 Beth Yehuda Dinner in Detroit, he said that “American support for Israel is not just an act of friendship; it’s an act of fundamental national self-interest on the part of the United States, a key component of our broader efforts to secure this region and the wider world as well as secure our own security.” Biden represents the centrist wing of the Democratic Party, which sees support for Israel as a fundamental principle despite any policy differences that they may have with the country. However, the progressive wing of the party has been on the ascendancy over the past few years. Many of progressive persuasion tend to be sympathetic to the Palestinian view and are often hostile to Israeli positions. They also see Israel under Netanyahu’s leadership as acting overtly political in support of Republican candidates and positions. This has led to an erosion of once-broad public support for Israel among all leading presidential candidates.

Pelosi Warns That Progressive Policies May Cost the Democrats the Election THE PULSE: In an interview with Bloomberg News, Speaker of the House Nancy Pelosi issued a warning to

Democratic candidates. She said that progressive policies might serve to fire up the base and may help a candidate win a primary, but they will not play well in a general election. “What works in San Francisco does not necessarily work in Michigan.” She criticized expensive Medicare for All policies currently being promoted by some candidates and also those who advocate for the implementation of the Green New Deal. Pelosi encouraged candidates to focus on workers’ rights and sharing prosperity. She said that candidates should focus on shoring up Obamacare and a pay-as-you-go health system. “We cannot just keep increasing the debt,” she said. THE IMPACT: Pelosi is seeking to moderate Democratic policies before the general election. She is sincerely concerned that even though President Donald Trump has a negative approval rating, many Americans will choose to vote for him over a progressive Democratic candidate. She fears that extreme liberal positions will further alienate the working-class voters whom Trump won in 2016. However, those on the other side of the argument believe that Pelosi’s approach of appealing to the political center is out-of-date. They believe that President Trump has shown that the path to victory lies in appealing to the base. A fired-up and energetic core is the key for voter turnout, and they believe that high turnout will result in victory in tightly fought elections in swing states throughout America.

O’Rourke Withdraws From Presidential Race THE PULSE: Presidential Candidate Beto O’Rourke announced that he is withdrawing his candidacy for the Democratic presidential nomination. O’Rourke is a former congressman who rose to prominence in his tightly fought 2014 Senate race against Senator Ted Cruz (R-TX). He was considered a rising star in the Democratic party and a serious contender for the Democratic nomination. He was outspoken in his support for strict gun control measures. Ultimately, his campaign suffered from lack of organization, and he could not meet his fundraising goals. THE IMPACT: O’Rourke’s withdrawal is the first withdrawal of a candidate once considered to be among the top tier of Democratic candidates. It is an indication that the field is tightening and coalescing around the top three candidates: Biden, Warren and Sanders. Other candidates seeking a breakout moment will need to focus their energies on Iowa or New Hampshire and hope for an early primary surprise or be eclipsed by the leading candidates in the field.

INGREDIENTS

2 cups cranberry juice

2 cups milk

2 cups half-and-half

1 cinnamon stick

6 oz. white milk chocolate, chopped

4 whole cloves Half a lemon, thinly sliced 1 cup fresh cranberries

DIRECTIONS 1. Place all ingredients into a pot. Bring to a boil over medium heat. Allow to simmer for five minutes. 2. Remove fruits and spices. Serve warm.

Pinch of salt 2 T. Walders creamy vanilla liqueur

DIRECTIONS 1. Add all ingredients except for the liqueur to a pot set over medium-low heat. 2. Heat, whisking frequently, until the chocolate is completely melted, but do not let mixture come to a boil. 3. Remove from flame and stir in the liqueur.

DIRECTIONS 1. Heat milk and half-and-half over medium heat until steaming, but not boiling. Remove from heat. 2. Add tea bags and steep for ten minutes. Remove tea from pot. 3. Return milk to the flame, and add remaining ingredients, besides for whipped cream. 4. Cook, stirring frequently until smooth and slightly thickened, taking care not to allow milk to boil. Pour into mugs and top with dairy whipped cream spray.

They said it was two harrowing encounters with death — tragedies that doctors felt could have been avoided with the proper medical facilities — that set Ida Barry Ryan on the path to establishing a hospital in Suffern.

She lived on an impressive mountaintop estate on the outskirts of Suffern — in the Montebello Mansion — but she was not one too high on her horse to turn a blind eye to the plight of her neighbors.

IT WAS AT THE TURN OF THE TWENTIETH CENTURY, and the small village of Suffern, secluded as it was, was burgeoning. The Erie Railroad had put Suffern on the map, just as it did Monsey and Spring Valley, and visitors were quick to notice its rural grandeur. Slopes overlooking the vistas of untouched nature provided impressive views and enchanting beauty, and travelers were attracted to its charm. It took but a few short years for resort hotels to spring up and the upper crust New Yorkers to avail themselves of the sweeping expanses of undeveloped land. While the super-wealthy would rent out estates in the area or purchase land on which they’d build private summer resorts, many came to visit just for the afternoon, or for a short getaway at one of the hotels in the area. Some 20 times a day, a train would stop at the grand Victorian depot in Suffern, and out would spill

tourists coming for their share of natural beauty in the cooler rural atmosphere. What this secluded village had in natural beauty, it sorely lacked in medical care. At that time, any necessary medical treatment was administered by one of Suffern’s three local physicians. And if one’s condition was beyond their expertise, the patient would need to travel to one of the nearby hospitals in Nyack, Middletown or Paterson, New Jersey. In those cases, patients would be transported via horse and wagon to the hospital over the rugged dirt erect a stately summer home. They named their estate roads. After this incident, Mrs. Ryan could no longer passively look on as lack of proper medical care got in the way of people’s lives. She got to work, dug into her fortunes and proceeded to underwrite the establishment of Good Sam Hospital. and from there the patients kept coming. Ten patients were seen in the following month, and by year’s end, the hospital had to be expanded. A two-story addition was constructed to the left side of the mansion. Back in the day, hospital rates were $1 per day for the wards and $10 to $14 per day for a private room. With the growth both of the population and the hospital’s popularity, the patient base as well as the hospital facilities continued to expand, with bed capacity reaching 26 in 1907, and 144 patients treated there in that year. The staff had also grown to ten physicians and seven nurses. In the following decade, numerous improvements were made, and by 1917, the space had increased to accommodate 35 patients, and three specialists were added to the staff. The horse-drawn ambulance was also replaced with a new motorized model! In 1926, the Spring Valley ladies auxiliary funded the furnishings for the nursery and a maternity ward in 1927. By 1937, the hospital had expanded to a medical team of 20 doctors and now had 40 beds for patients, and the estate was bursting at its seams. Funds were raised for a new building. At the very end of 1938, the first hospital building on Orange Avenue closed its doors, and the institution relocated to Lafayette Avenue — its present site — where the new structure that was built accommodated 72 patients. Coming full circle, the new location had actually once been the property of Mrs. Ryan.

1902, Mrs. Ryan purchased a Civil War–era estate situated on Orange Avenue at East Park Place in Suffern, across the train tracks, as the home for a hospital that would provide emergency service to the Suffern population of 1,800 and the tourists who frequented the area. Mrs. Ryan negotiated with the doctor who occupied the estate to vacate the property immediately by paying up the lease in just five days. Fifteen days after its opening, on November 27, the first patient was admitted to the hospital, a “Willie Mann, age 12, from Hillburn,” who fractured his femur. Today, the road off Montebello Road leading to Estate Drive, where the Montebello Mansion stands, is Ryan Mansion Drive, the location of Ryan Mansion Estates, a gated 55+ community. Behind the Mansion is another street with beautiful homes names Fortune Way. Perched on this hilltop in Montebello, Ryan’s original mansion still stands in its stateliness. It is currently used as commercial space and is for sale with an asking price of $12 million.

The sounds of suffering combined with the pungent smell of antiseptic and the ever-present beep-beepbeep of machinery are enough to make most people pick up their feet and flee the hospital at the first opportunity — but not so Rabbi Yisroel Kahan. For him, a day in the hospital is just another day doing what he loves: helping Yidden when they need it most. “You know what they say, ‘Find work you love and you’ll never work a day in your life’?” Rabbi Kahan asks me. “Well, that’s what happened with me.” What started out nearly 19 years ago as a spare-time mitzvah evolved into a full-time occupation. Initially, Rabbi Kahan volunteered through Bikur Cholim at Westchester Medical Center, visiting patients, stocking food cabinets and comforting families. “I was still in kollel at the time, but this was my special mitzvah,” he tells me. “I was at the hospital whenever I had the chance.” But it was his being offered a job as the administrator of Monsey Health Center that really kickstarted Rabbi Kahan’s career in health care. “I began dealing with doctors and emergencies, and became busy ensuring patients were well taken care of.” Over time, Rabbi Kahan became a familiar face in many New York–area hospitals, though Rockland and Westchester County hospitals received the bulk of his attention. “One day, Good Sam Hospital offered me the job as an official liaison to the frum com-

munity. They wanted someone who could serve as a bridge between Orthodox patients and the medical staff, and to help the hospital understand the needs of the frum patient. I was already doing that, so I said, ‘Why not? I’m here anyway! Why not get paid for it?’” Rabbi Kahan explains that the hospital tries to go out of its way to accommodate the heimishe population to the greatest extent possible. As one example, he tells me that while the new mother-and-baby wing was being constructed, the engineers wired the unit with motion sensors in the bathrooms in order to help the Yidden avoid the issue of flipping switches on Shabbos and yom tov… “When I was given a tour of the new unit, I had to explain to them that motion sensors were very problematic… They then rewired the whole unit and put in regular switches. It was a well-intentioned effort to make things more comfortable for us.” It’s not only an example of Good Sam’s accommodation of the frum population; it’s also an example of Rabbi Kahan’s role as a liaison. He’s there to serve in any manner that helps Orthodox patients have the easiest time in a

place where hardship abounds. But his work isn’t just limited to the frum community. “In my other role as a liaison to the local government, I’ve made many contacts with people outside of the frum community. Plenty of nonJewish acquaintances have contacted me for guidance while in the hospital. If there’s someone in need and I have the ability, I’m more than happy to help.”

OVERCOMING THE COMMUNICATION GAP

When they hung up and relayed the message to the doctor, he looked at me in surprise. This advice showed a lack of medical understanding, because babies fight very hard until the last minute, and they might appear to be hanging on, but then in a second things can spiral out of control. There’d be no time to call for a psak then. The family needed to be prepared. “I asked the family if I could call a rav wellversed in these situations and we got him on the line. He ended up speaking with the doctor and giving his psak. When he hung up, the doctor turned to me and asked where the rabbi had

That can involve translating or explaining complicated medical terminology in plain language. THE WAITING GAME gone to medical school, as he was so knowledgeable.” Sadly, tragic stories are not uncommon in any hospital setting, but at least in Good Sam, there’s a frum liaison who helps families navigate the tempest. “I’m here to help them cross the choppy waters,” Rabbi Kahan tells me.

FINDING THE BEST FIT “I sometimes get calls from family members or doctors who explain a situation and ask which hospital is best for a patient to be sent to. It’s important to understand that both small and large hospitals have benefits. “University hospitals such as Columbia, Mt. Sinai, Westchester, etc., certainly have more advanced care in regard to machinery, and they have burn units, for instance, which smaller, local hospitals don’t have. They’re equipped to handle much sicker patients. But for more standard issues, the quality of relationship with doctors is better at local hospitals.” Rabbi Kahan tells me that one of the benefits of a community hospital is that one’s primary care physician will have privileges that enable him to call the shots and monitor the treatments the entire time. “Your local doctor will have his hand in the system. He’ll coordinate

It’s hard to sit and wait, especially in this instant gratification generation.” Perhaps understanding how the ER operates can help people tolerate what seems to be a slow-moving environment. The first thing to understand, he tells me, is that each patient entering the emergency room is evaluated at triage to determine the severity of their condition. That, combined with the occupancy of the ER and the severity of other patients’ conditions, determines wait time. “A sprained ankle isn’t going to be of the same priority as someone having difficulty breathing. It’s not always first come, first served. Realize that people may have come in with more serious or life-threatening conditions.” It’s also important to understand that even when nothing seems to be happening once one has already entered the treatment area, there’s a lot going on behind the scenes. “Sometimes I will receive a call in which someone says, ‘I’m waiting for over an hour and nothing is happening.’ What they don’t know is that the doctor is waiting for the blood results to come back before taking the next step. For instance, if a doctor wants to order a CT scan with contrast, he’ll first check the health of the kidneys with a blood test to ensure that the body can tolerate the contrast. So while the patient may think that nothing is being done, there are actually things happening behind the scenes. “If a nurse says that results will be available in about 45 minutes, give them some extra time before you approach and ask for results… It’s important to be a mentch and not, chas v’shalom, to make a chillul Hashem. There was once someone who came into the emergency room and was frustrated with the wait. Within a half hour, he had contacted the CEO of the hospital and sent him a nasty letter! To make things worse, the person who did this was an EMT who should have known better. It created such a bad impression of frum Yidden. “Of course, if there is any concern that a patient is in danger, absolutely, go ahead and get attention. Don’t sit by idly. You must advocate for yourself and your loved ones when needed. There is a time and place for patience, and the moment of an emergency is not one... But if there is no danger to life, just wait. Also know that it’s okay to ask questions and get explanations, but not every family member has to ask the nurse for updates. Choose one representative, and stay in your room until approached. Don’t hound the nurse. She has a job to do and other patients to treat.”

But if an adult walks into a university hospital for appendicitis, it’s as if they’re walking in with a big sign that says: ‘Please let your residents practice on me!’ Because it’s a relatively simple surgery; it’s one of the first surgeries that residents do.” It’s important to evaluate the nature of a condition when choosing which hospital will provide care. But what if, after being admitted to a hospital, the condition is recognized as needing a different type or level of care? That’s where transfers come in. But even before a transfer is put into motion, Rabbi Kahan suggests considering a consult between the hospital’s doctors and specialists elsewhere. “There are times that we can avoid the physical and monetary burdens that come with a transport by having local doctors consult with top specialists nationwide. When it comes to infection, for instance, sometimes it’s just a matter of getting the right treatment plan, but the treatment itself can be administered in the original location without physically stressing the patient.”

FROM ONE HOSPITAL TO ANOTHER During an emergency every second counts, and oftentimes, even people in complicated, extreme conditions will be taken to a local hospital to be stabilized before being sent to a medical center better equipped for intensive care. “Not a week goes by that I don’t transfer patients,” Rabbi Kahan tells me. “We’re not just talking about local transfers; patients can be sent to Johns Hopkins, Cleveland… to whichever hospital is best equipped to handle a patient’s specific needs.” To set up an interfacility transfer, the doctors at the initial hospital contact the transfer center

at the potential receiving hospital to discuss the case. Once deemed an appropriate transfer, the logistics planning can proceed. When it comes to cases of emergency, no prior authorization is required with insurance companies, but a request for a non-emergency transfer must first receive insurance approval, which may take several days. Once all is approved, transportation must be arranged and a bed made available. Some hospitals, such as Cleveland Clinic, have their own airplane and medical crew for transporting patients. Other hospitals will say, “Send us the patient,” and if insurance approves the transfer, the first hospital will arrange transport; otherwise the family will need to contact a private company such as Vital One to get the patient safely to their destination. Because transfers can be complicated and stressful on top of an already nerve-wracking situation, Rabbi Kahan is there for families, helping them through the ordeal. “Whether a patient needs a simple procedure or intense medical intervention, I consider it my honor to help them receive the best, most dignified treatment available. And watching people be helped? That’s the most rewarding part of my job; seeing people who came in broken, extremely sick, or having suffered a severe trauma getting back on their feet. Iâ&#x20AC;&#x2122;ll never forget the day an unanchored food closet fell on my four-year-old, knocking her over and coming very close to causing untold harm. Terror ran through me when I saw her little body pinned under the heavy closet, and I could do little more than screech for my older children to summon our next-door neighbor while I bumbled about, trying to correctly punch in Hatzolohâ&#x20AC;&#x2122;s phone number. Trembling from head to toe, my wits were not about me, but eventually I reached the dispatcher and help was on the way.

B B’chasdei Hashem, Hatzoloh pronounced my daughter unharmed. And while she was being examined, my dear neighbor quietly snuck into the kitchen and began cleaning up the spilled oil, sugar and other foodstuffs that were spread far and wide across the floor. Once Hatzoloh left, she bundled up my children and sent them to her house, telling me to rest a while to regain my shattered equilibrium. Such neighbors are a gift from Heaven, and irreplaceable. Blessed with a sixth sense of exactly what to do and when, these types of people are able to comfort and help in ways that may not be obvious to every bystander. Unfortunately, not everyone has such neighbors. When an ambulance pulled up in front of my friend’s house late one afternoon, the neighborhood children gathered around for the show. It was Erev Shabbos and everyone was home. Living on a busy block means that privacy is hoped for, but not always a given. However, even people living on a very busy block should respect the dignity of persons undergoing an emergency. In this case, though, that didn’t happen. Not only did children gather around to see the action; parents, too, stood gawking. Feeling exploited, my friend’s children

began to cry — their tears no longer solely for their dear brother, who was being rushed to the hospital, but also for the shame of being given public attention they’d never asked for. To make matters worse, a particularly rambunctious boy climbed onto the ambulance and opened the door to get a better view of what was happening inside... Such poor conduct is rare, but great behavior can’t be expected to happen naturally. Our children (and the adults among us for whom that sixth sense of proper emergency decorum is lacking) need to be trained how to react when emergencies occur. Here are some tips to review with your children (and to keep in mind yourself!). STAY OUT OF THE WAY OF ACTION Being rachmanim bnei rachmanim means that we truly care about what is happening to the people around us — causing us to naturally stop, look, question and feel concern when we see an ambulance pull up. But how do we avoid becoming gawkers and actually prove ourselves valuable in an emergency? The first thing to remember is that paramedics need room to enter and exit a house, as well as space to park their vehicles. Never loiter around a home where an emergency is happening, and do gather together any bystanders (especially children) and herd them further down the block, away from the site of action. DIGNITY MATTERS! Emergencies often involve people losing control over their bodily functions or having the patient appear in public dressed in a manner they’d not like others to

On the way down to wait with the medics for the ambulance (every second counts during a stroke), I knocked on the door of a neighbor, who I knew is an early riser. I explained that we had to go to the hospital and asked if they could please take care of our guests. She wasn’t able to, and it was too early to knock on other neighbors’ doors, so I asked the medic if he could possibly get a message to the girls that they should find another place to eat the seudah. Instead, he came back to our apartment after davening, stood outside and waited for the girls to come... and took them to his house for the meal!” -D.S.

witness. Moreover, simply having an emergency removes many aspects of privacy from a person, alerting all the neighbors to a situation at hand. When people stand around, waiting for the ill person to emerge from the house, dignity is compromised in a significant way. If possible, not only herd bystanders down the block, but also encourage them to completely disperse. Certainly don’t give in to the desire to watch the goings-on!

down, there are still many ways neighbors can help. Children may need to be delivered to playgroup, picked up from school, fed supper or bathed. Can you offer to help in any way? So, too, homework help is often appreciated. Sometimes, even the simple act of styling a child’s hair in the morning so they look mentchlich can be a chesed of tremendous proportions. I know this, because when I once stayed in the hospital with a sick baby, my own amazing neighbor made sure my daughter’s hair was neatly pulled back every morning as my husband found making a ponytail difficult.

NOT A TOPIC OF CONVERSATION Patients are not a topic of conversation; they are people. And people usually prefer not to be spoken about. If you are concerned about a neighbor, pull out a Tehillim and encourage others to do the same. Don’t stand around talking about the patient! Yes, it’s hard to not be “in the know,” but that doesn’t make it right to discuss other people.

NEVER BE NOSY! Just because a patient has been released from the hospital doesn’t mean that the situation has been instantly resolved. It also doesn’t mean that the medical issues are up for discussion. One friend with a medically fragile child was approached by a neighbor and asked if her child was “all better” after returning home from the hospital. Flabbergasted, my friend didn’t know how to respond. Even though the immediate issue at hand had been calmed, there were still many more to contend with. If you are not a close confidante, please don’t pretend you are. Send your warm wishes for smooth sailing, but don’t try to get too personal.

OFFER SPECIFICS Do you have something valuable to offer a family in their time of need that the paramedics won’t be covering? (See sidebar for stories of how others have done this.) Only if you know that your offer will be appreciated at that specific moment should you consider popping over. Otherwise, give wide berth to the area. The bystanders watched in horror as three Hatzoloh volunteers hoisted the car midair while two others crawled underneath and gingerly extracted a gangly teenage girl. The feelings of shock only intensified as the teenager attempted to walk away from the scene. It took ten minutes of arguing and extreme powers of persuasion until the Hatzoloh members successfully cajoled the teen into the waiting ambulance, so that she could be properly examined for potential internal injuries. It took a full 30 minutes for the tension to thaw. Only then did the crowd disperse. ------------------------It was on a Tuesday many years prior that Avi Simons* had spotted the poster on which Hatzoloh was showcasing their fundraising endeavor of the year. The daily expense of running Hatzoloh’s activities amounted to $3,600. Hatzoloh was now soliciting the generosity of the local residents to claim a day of significance and shoulder the expenses of the day. The idea appealed to Avi, and choosing a date was a no-brainer. His daughter Ruchele was getting married on Rosh Chodesh Cheshvan. It was just a matter of date availability. Not one to procrastinate, Avi was on the phone within minutes arranging the credit card details with a campaign rep. Let this be a zechus for the couple, he thought. In the merit of my donation,

the afternoon traffic. As the car marked for chase picked up speed, so did the patrol car. They bumped and swerved, missing all the human obstacles by mere centimeters. Until the miss turned into a hit. BAM! Avi didn’t give the matter much thought until the following year, same time. When he saw the number of Hatzoloh on his caller ID, his heart skipped the proverbial beat. The wedding was all but a distant memory, albeit a joyous one. He had signed up as a zechus for the couple’s impending wedding. The hesitation lingered for a few moments, and then, on a whim, he decided to stick to his date l’kavod the couple’s upcoming anniversary. His card was charged. The years came and went. Every year, he would get the perfunctory call, hesitate about the date, and subsequently renew the charge for Rosh Chodesh Cheshvan. ------------------------The three teenage girls chatting at the corner were oblivious to the police chase gaining momentum in the not-too-far-off distance. It was a typical cops and robbers scenario: a police car chasing an alleged perpetrator through

metal chassis. Chaos erupted. Emergency vehicles. Crime tape. Crowds. The whole nine yards. Amid all the confusion a Hatzoloh member shrieked, “She’s under the car. Hoist!” The vehicle was hoisted. And there, under the massive hunk of steel, lay Devorah Simons. ------------------------Still in a trance of astonishment, Avi Simons allowed himself to be directed to the curtained cubicle in the ER, where Devorah was being examined. “She’s good to go,” the attending doctor informed him. “Are you sure?” Avi struggled to collect his thoughts, which were playing ping-pong in tune to his rapidly beating heart. “She was pinned under a vehicle!” “Sir, you can take a look at the tests.” The doctor handed him the documents along with the discharge papers. “Squeaky clean.” Avi stared blankly at the documents in his hands. None of the test results meant anything to him. He closed his eyes as he fervently whispered, “Thank You, Hashem.” And he instantly recalled that he had in fact already recited Hallel that day. Of course, he had. It was Rosh Chodesh Cheshvan — Ruchele’s anniversary.

DRESSED FOR THE JOB Originally established for both hygiene and identification, the traditional nurse’s uniform consisted of a long-sleeved white dress, an apron and a cap. Ironically, although the uniform was intended to protect against fever and infection, nurses in those times wore neither masks nor gloves!

During World War II, revisions to the uniform were made in an effort to scrimp on material. The cap, which was mainly used to cover the hair, was next to go, since newer nursing caps were more of a fashion statement and were quite a hindrance with their tendency to fall off nurses’ heads at inopportune times.

Since 1990, countries including Western Europe and North America have moved toward the modern alternative of scrub tunics, with the scrub dress worn in emergency and operating rooms. Scrubs are disliked by many, who argue that nurses are no longer recognizable and are perceived by patients as cleaners or surgeons, but they are said to be a lot simpler for hospitals to sanitize.

The UK’s version of nurse uniforms remains traditional and includes dark blue trousers and a tunic-style top, designed to prevent crossinfection. They also wear a dress in the same color as the tunic, its color representing the amount of years they’ve trained for. And although it used to be common for male nurses to wear white tunics, today they wear shades of blue or dark green — colors that signify authority to the general public.

THE RED CROSS The American Red Cross was founded in 1881 by Clara Barton — famous for her energetic nursing efforts during the Civil War — as a disaster relief and nurse training agency. Today, they have evolved into an organization that continues to assist both U.S. soldiers and families during natural disasters, provides training courses in both first aid and swimming to the public, and is responsible for over 40 percent of blood donations in the country!

UNADORNED? Ever wonder why nurses often lack a basic watch or bracelet? Nurses are advised to refrain from wearing hand jewelry to prevent distraction or their getting caught on patients’ skin during care. That is also the reasoning behind the fob (pendant) watches parents of young nurses traditionally gift their children when they move away from home for their residency.

NURSING THROUGHOUT HISTORY Early nurses devotedly cared for patients without any formal training, and nursing was one of the only professions dominated by women. In 1883, the first woman graduated from a Canadian medical school, and only later, during the late 1900s, was nursing professionalized with the establishment of training schools in the U.S. and Japan. Unbelievably, between 1873 and 1910, in just 37 years, the number of hospitals worldwide increased from 149 to 4,400! As the public trusted hospitals more and facilities were now equipped to professionally handle intensive health issues, the amount of active graduate nurses grew from 51,000 to 375,000 (1910 to 1940), and later to 700,000 (1970)! Many of the nurses at this time provided care free of charge, in exchange for the hands-on training they received.
