NON-ESSENTIAL DRUGS: NPPA WITHDRAWS PRICE CONTROL ORDER

T

he National Pharmaceutical Pricing Authority (NPPA) has withdrawn its order to fix prices of drugs, which are not under the list of essential medicines, a move that has been welcomed by the pharmaceutical industry. The NPPA had late on Monday issued a statement, saying it has withdrawn guidelines for price control issued under Para 19 of the Drug Prices Control Order (DPCO), 2013. The Paragraph 19 of the DPCO, 2013, authorises the NPPA to control the prices drugs that are not under the NLEM (National List of Essential Medicines) under extraordinary circumstances in public interest. The NPPA had invoked Para 19 of the DPCO to cap prices of 108 medicines, including cardiac, diabetes and HIV/AIDS drugs, last month.

The NPPA capped prices of 108 non-essential drugs in July to improve affordability in a country where more than four-fifths of its nearly 1.3 billion people have no health insurance, and where 70 percent of the population lives on less than $2 a day. But the decision was greeted with protests and has been challenged in courts in India, where prices of generic drugs are already low compared with other markets. “In compliance with the directions received from the government in the Department of Pharmaceuticals...the aforesaid internal guidelines issued by the NPPA on May 29, 2014 under Paragraph 19 of DPCO 2013 are, hereby, withdrawn with immediate effect,” the NPPA said in a statement. The order will be effective prospectively.

Not long ago, the National Pharmaceutical Pricing Authority (NPPA) has capped prices of 43 medicines, including antibiotic ciprofloxacin, BCG vaccine and antidiabetic drug metformin, to make them affordable for the common man. Major drug companies, including Cipla, Ranbaxy, Lupin and Cadila, are likely to be impacted. The drug price regulator said that manufacturers of these drugs having maximum retail price higher than the ceiling price specified shall revise the MRP to an amount not exceeding the ceiling price plus local taxes wherever applicable in accordance with paragraph 13 (1) and 24 of the Drug Prices Control Order, 2013. An official at the Department of Pharmaceuticals confirmed the price cap move. The recommendations will be sent to health ministry by midOctober, he said. Global and Indian drugmakers have

been hit by wide-ranging government-imposed price reductions in the last one year in India. An NPPA official explained that the order meant that the powers the authority drew from the DPCO for fixing caps for nonessential drugs is no longer available. “The plain reading of the order is that a guideline under DPCO 2013 has been withdrawn with immediate effect, that is, from September 22,” he said. Several industry bodies, including the Indian Pharma Alliance (IPA), had criticised the NPPA's move to cap the prices of medicines not under NLEM. Under the Drug Price Control Order (DPCO) 2013, the government already controls the prices of 348 drugs listed in the NLEM. This is a straight-forward, most predictable, overdue action which has been done by NPPA. These medicines join the list of 348 drugs deemed essential and are therefore subject to price caps covering up to 30 per cent of the total drugs sold. Sanofi India has surged 12% to Rs 3,350 on National Stock Exchange after the government withdrew National Pharmaceutical Pricing Authority's power that permitted it to fix prices of non-essential drugs.The Department of Pharmaceuticals has withdrawn guidelines issued on May 29 that gave the National Pharmaceutical Pricing Authority (NPPA) the powers to fix the prices of drugs that are not on the essential medicines list, the latter said in a notice late on Monday. Sanofi India would have the biggest positive impact with around Rs 139 crore gains in sales. Zydus Cadila (Rs 40 crore gain), Ranbaxy (Rs 38 crore gain), Cipla (Rs 19 crore gain), Lupin (Rs 32 crore gain) , Dr Reddy's Laboratories (Rs 14 crore gain in sales) and Sun Pharma (Rs 25 crore) gain in sales), on the basis of AIOCD AWACS. It may be recalled that the NPPA had capped prices of 108 cardiac and diabetes drugs on July 10, 2014. Sanofi India focuses its activities on seven major therapeutic areas, namely – Cardiovascular diseases, Metabolic Disorders, Thrombosis, Oncology, Central Nervous System disorders, Internal Medicine and Vaccines. The government's decision to curb the power of the pricing authority comes in the wake of new Prime Minister Narendra Modi visit to the United States for the first time since taking office. Ties between India and the United States have been strained in recent years because of trade policies and patent disputes.

PHARMACEUTICAL ARTICLE PHASE IIB STUDY OF BTI-320 IN US TO TREAT TYPE 2 DIABETES

B

oston Therapeutics, a developer of complex carbohydrate therapeutics to treat diabetes, has completed enrollment of its phase IIb clinical study on BTI-320 in the United States. The study, SD-002, is being conducted by Accumed Research Associates in Garden City, New York.

The US trial enrolled patients with type 2 diabetes who are currently being treated with metformin. The patients were administered BTI-320 using a randomised, double-blind, placebocontrolled, dose-ranging, three-way cross-over study design. Patients' blood glucose were monitored and their postprandial (after-meal) blood glucose levels were measured following a test meal.

The primary endpoint of the study is the evaluation of the effect of BTI-320 compared to placebo in the area under the curve (AUC) of glucose and secondarily, on insulin levels in the blood for four hours following intake of the meal.

BTI-320 is a non-systemic, new generation alpha glucosidase inhibitor-based tablet developed to reduce post-meal elevation of blood glucose. BTI-320 is a proprietary polysaccharide to be taken before meals and works in the gastrointestinal tract, nonsystemically, to block the action of carbohydrate-hydrolysing enzymes that break down complex carbohydrates into simple sugars, reducing the availability of glucose for absorption into the bloodstream.

PHASE 2B TRIAL OF BRILACIDIN TO TREAT ACUTE BACTERIAL SKIN AND SKIN STRUCTURE INFECTIONS : Cellceutix Corporation, a clinical stage biopharmaceutical company developing innovative therapies in oncology, dermatology, and antibiotic applications, has announced the completion of enrollment in its phase 2b trial comparing Brilacidin to daptomycin (Cubicin) in patients with acute bacterial skin and skin structure infections (ABSSSI). Overall, 215 patients were randomized to one of three dosing regimens of Brilacidin (single dose 0.6mg/kg; single-dose 0.8mg/kg; 1.2mg/kg over 3 days) or 7 days of once daily daptomycin.

Brilacidin is the lead compound among a novel class of antibiotics known as defensinmimetics. These compounds have shown great promise in preclinical and early clinical studies as potent antibiotics that can kill Staph aureus, including methicillinresistant Staph aureus (MRSA).

Moreover, given the unique mechanism of action, among other attributes, there is low, if any, potential for the development of resistance. Finally, the results of the current study may demonstrate that Brilacidin could be given as a single-dose, which provides benefits for both patients and society.

This combination of properties makes Brilacidin unique among antibiotics recently approved or in development for the treatment of MRSA. Because these other compounds come from existing classes of antibiotics, there is the potential for cross-resistance; this is not the case for Brilacidin.

IXEKIZUMAB SUPERIOR TO ETANERCEPT AND PLACEBO IN PHASE 3 PSORIASIS STUDIES : Eli Lilly and Company's investigational medicine ixekizumab was statistically superior to etanercept and placebo on all skin clearance measures in phase 3 studies, the company said in disclosing top-line results from its pivotal UNCOVER studies in moderate-to-severe plaque psoriasis.

"These data are important for people suffering from moderate-tosevere plaque psoriasis, as up to 41 percent of those treated with

ixekizumab were able to achieve clear skin at week 12, with just one injection per dose. In the three UNCOVER studies, patients were assigned to receive either placebo or ixekizumab (80 mg every two or four weeks) for 12 weeks, following a 160 mg starting dose. In the two active comparator studies (UNCOVER-2 and 3), patients could be assigned to receive etanercept 50 mg twice weekly for 12 weeks. In UNCOVER1, responders to treatment were assigned to continue treatment on either placebo or ixekizumab (80 mg every 4 or 12 weeks) for up to 60 weeks.

Patients treated with both dosing regimens of ixekizumab had significantly greater levels of skin clearance compared to placebo and to etanercept at the 12-week endpoint. Skin clearance was measured by standard primary endpoints for psoriasis studies: the Psoriasis Area and Severity Index (PASI) and the Static Physician Global Assessment (sPGA).

Ixekizumab is a monoclonal antibody with high affinity and specificity that binds to and neutralizes the pro-inflammatory cytokine interleukin-17A (IL-17A).

It is the most common inflammatory disease in the United States, affecting as many as 7.5 million Americans and an estimated 125 million people worldwide. Psoriasis can occur on any part of the body and is associated with other serious health conditions, such as diabetes and heart disease.

The most common form of psoriasis, plaque psoriasis, appears as raised, red patches covered with a silvery white buildup of dead skin cells.

Approximately 17 per cent of psoriasis patients have moderateto-severe plaque psoriasis.
