That means intense heat and lots of me outdoors. At this me of year, it’s easy to let go of any physical fitness rou ne, or to put off star ng one un l the weather’s cooler. Moving around while cooling down couldn’t be easier. Jump into the sea and swim around. However, keep out of the sun in the ho est part of the day and slather on sunscreen and wear a hat when you’re at the beach. As the latest campaign puts it, you only get one skin, so look a er it.

Sunburn isn’t the only beach risk. See our feature on how to avoid common problems and what to do about them if you can’t. Do the obvious, keep hydrated and eat well. As the saying goes, you are what you eat – and drink, we might add. The ho er the weather, the more you need to drink and the best you can give yourself is clean, fresh water. That goes for children too. Water’s not just the best hydrant for physical wellbeing, it’s also the safest for their teeth. Read about all this and more in this issue of A&H. Make sure you have good, direct light. If possible, use white light and avoid decorative lamp shades.

Reading, writing, using a computer or staring at a screen will tire and dry your eyes. Every 20 minutes, give your eyes a break by looking away from your book or screen, and blink your eyes 10 times. Close your eyes and move your eye balls clockwise and then anticlockwise. Focus on something at a distance for 10 seconds and then close your eyes for another 10 seconds. Repeat this five times.

Now that the sun is already scorching and the UV index is high, sunglasses are essential to prevent eye damage.

Choose sunglasses that block 99% to 100% of UVA and UVB sun rays. Wraparound lenses help protect your eyes from the side while polarised lenses reduce glare while you drive. Avoid looking directly at the sun as that damages the eyes.

Eye injuries are very common when doing dangerous work or practising sports. There are specific safety glasses to use when doing work around the house and when participating in sports such as hockey or golf.

Lenses must be properly stored in the appropriate cases and solutions. Never sleep with your contacts on. If you don’t use daily lenses, make sure that you change your lenses often.

Certain nutrients are essential to prevent age-related eye problems like macular degeneration and cataracts.

Foods which are eye friendly include green leafy vegetables, salmon and tuna, non-meat protein-rich foods like beans and eggs, pork and citrus fruits. Maintaining a balanced diet also prevents the risk of developing diabetes, which can cause severe eye problems.

Smoking speeds up the risk of certain eye conditions such as cataracts, macular degeneration and optic nerve damage.

People over 40 who do not need spectacles or contact lenses should still go for regular eye check-ups since age-related eye problems are very common. Based on the first check-up, the ophthalmologist will plan the next screenings. KEEP SAFE AND WELL WITH A&H’S GUIDE TO THE SUMMER HOLIDAYS PROTECT

EYES UV rays don’t just harm your skin. They also damage your eyes. Sunglasses should protect your eyes from UV damage. Choose sunglasses for functionality, not just aesthetics. The cheap fun types with brightly coloured lenses might not do the job.

SKIN Buy the best sunblock you can afford, preferably with a high protection factor (35+), and apply it before going out into the sun. Unless you have the patience to continually rub sunblock onto wriggly children, it is best to use a waterproof type for them since it has more staying power and can be re-applied at longer intervals.

FEET Watch out for post-barbeque debris like broken glass, bottle tops, tins and even used syringe needles. Young children could wear “jelly shoes” to the beach for protection.

SPINES AND STINGS Many people worry about shark attacks, but a jellyfish attack is more likely. If this happens, the best thing to do is splash the affected area with vinegar or with seawater (fresh water is not as effective). Then, scrape or ‘shave’ the area gently, using a credit card or something similar, to remove the remaining stings.

this will simply work the stings and poison further into your skin. Sometimes even a mild sting could cause a strong allergic reaction so if the victim feels unusually tired or unwell, or has difficulty breathing, seek medical help.

Sea urchins are another common beach nuisance.

If any spines get under the skin, remove them immediately to avoid infection. If any are deeply lodged, use a traditional onion poultice to encourage the spines to move to the surface where you can remove them easily with tweezers.

TUMMY TROUBLE Beach food is prone to contamination because of heat and exposure. If you take picnic food to the beach, keep it chilled and eat it as soon as possible after you arrive. Temperatures rise very quickly, even in the shade, and good food can turn into tummy trouble sooner than you know it. Take a packet of dry biscuits or galletti for children to nibble on.

Before you buy food at a beach kiosk, check the kitchen. If you see any food left unrefrigerated and uncovered, look for another place to eat. It takes just one contaminated tomato in your sandwich to ruin a perfectly good day.

SAFE DIPS Do not leave children unattended on

airbeds or other flotation devices, since they can easily drift out of reach. Small children in particular tend to wander off easily. Have children keep their armbands on at all times in case they run into the water unexpectedly.

BEAT THE HEAT Watch out for heat induced trouble. Warning signs are feeling weak, dry skin, confusion or dizziness, headache, clouded vision, rapid pulse, nausea and collapse. Children are especially vulnerable, so even if you’re feeling fine, watch out for any signs of trouble.

Drink lots of water frequently, even if you do not feel thirsty. Avoid sweetened or alcoholic drinks because they have a dehydrating effect. Stay home during the hottest time of day. High temperature usually mean more humidity, increasing the possibility of heat exhaustion and heatstroke.

Heat exhaustion can be treated by moving the person to a cool environment and getting him or her to swallow cool, slightly salty drinks such as plain water with a pinch of salt. Heatstroke is a medical emergency and needs professional help. While medical assistance is on the way, move the person to the shade and splash him/her repeatedly with cool water.

he World Health Organisation states that there is no health without mental health. The physical aspect of pregnancy and postpartum recovery has received lots of attention, but little attention has been paid to a woman’s mental health, despite the evidence that depression is the most common complication of pregnancy. Maternal mental health day, marked in May, was created to increase awareness and reduce the stigma surrounding mental ill health.

What is understood by maternal mental health?

Maternal mental health is understood as the mental wellbeing of mothers as they go through pregnancy and the first year postpartum. We usually refer to this period as the perinatal period. Research and experience show that this when mothers are most vulnerable to mental health issues. Perinatal mental health incorporates the emotional well-being of the mother, the father and the infant, as all are deeply effected in this time.

The changes that occur during pregnancy and the postpartum are psychological, social, environmental and physical. All together, these factors increase a woman’s susceptibility to mental health problems. The most common maternal mental health issues are depressive and anxiety disorders, including obsessional compulsive disorder and general anxiety disorder.

Meanwhile, around 80% of women may experience postpartum blues. This is a brief period, usually starting 3-4 days after birth and lasting approximately ten days, where the woman experiences mood swings, sadness and bursting into tears easily, irritability and feeling anxious at times. Normally, there is no need for treatment. However, if the condition does not subside, then a depressive disorder is considered.

There are also other psychiatric complications which can occur in this period such as posttraumatic stress disorder (usually after a traumatic birth or complications during pregnancy and delivery) and postpartum psychosis, which is a rare disorder.

Are mothers sufficiently supported during pregnancy and after giving birth?

During pregnancy and the postpartum period women have frequent contact with health professionals, such as obstetricians, midwives, general practitioners and paediatricians. This provides a window of opportunity for detecting and treating any emotional stress. The mother’s main concern is usually the wellbeing of the child, but the changes she goes through can affect her not only physically but also emotionally. Support during this period is crucial.

Partners, family members, and friends are usually a mother’s first line of support. When this is not sufficient, or not available, there are services to offer support. However, for support services to be useful, disorders or stressors need to be detected as early as possible. We know that this is not always the case and that there are mothers with difficulties which remain undetected and untreated. Unfortunately, reaching out for support carries a stigma. A woman might feel uncomfortable about reaching out for professional support or even about reaching out to family and friends.

That is why we feel it is so important to educate people and raise awareness of the changes, experiences and difficulties a woman may face in the transition to motherhood, a time which is popularly associated with joy and happiness. As professionals, we do not only want to reach out to the mothers, but also to those who are in close contact with her, such as her partner, family members and other professionals.

There is also increasing research into paternal mental health issues. We need to remember to also support the father in this vulnerable time, especially when the mother is psychologically distressed.

Is there enough screening in place to ensure that warning signs do not go unnoticed?

So far, there are no formal screening procedures in the obstetric services. However, midwives ask basic questions regarding the

women’s psychological wellbeing when she registers to give birth. These questions deal with any history of depression and whether she is under any psychiatric treatment. In Malta, there is only one perinatal mental health service that runs once a week.

However, a lot of support is given through other services such as the Parent Craft service, Breastfeeding Clinic, bereavement midwife and discharge liaison service at Mater Dei Hospital.

Internally, research shows that over 75% of women are not diagnosed and do not receive adequate treatment and support. No research has been done in Malta on such statistics, however, we try our best to advocate for early identification and treatment so that we can decrease the negative impact of mental disorders on the mother, the father and the infant.

Would you say there has been an increase in the number of mothers being referred for support and treatment?

Literature in Malta shows that, due to our close communities, mothers are able to receive increased support through the community. Within the perinatal mental health service, we have seen an increase in the number of cases which are referred for support and treatment. This can be interpreted as the result of increased awareness among health professionals working closely with mothers.

Risk factors for mothers not being able to cope well during this time

Support is one of the best treatments for emotional distress and it does not always have to be professional.

include a history of mental health issues, history of antenatal depression or anxiety, problems in the relationship or marriage, lack of support, financial difficulties, history of traumas, isolation, stressful life events and poverty. When these risk factors are identified, the services tackle these issues through referrals to the appropriate services.

It is important to remember that, when a mother feels that she is finding it hard to cope or not coping at all, the services are available and very willing to help. Such difficulties in coping are known to affect not just the mother, but also on a family level, including the father’s wellbeing, the child’s emotional, cognitive and physical development, the couple’s relationship, as well as the attachment between the parents and the child.

What is the way forward?

A perinatal mental health strategy is needed to screen women, both during pregnancy and postpartum, for risk factors and to detect emotional disorders. Early detection is crucial to minimise the distress caused by emotional difficulties during this period.

Health services in Malta generally do not consider the father’s mental 12

health difficulties and increased distress. Including these factors within the services would not only help the father within this period, but also the mother and the child. A fully inclusive practice that can address the family system as a whole is still needed.

Who does one turn to for help?

Support is one of the best treatments for emotional distress and it does not always have to be professional. A supportive family structure is crucial in helping the mother through her distress. Sometimes professional support is needed and there is no shame in that. Reaching out to a trusted health professional, such as your family doctor, who you are comfortable with, can be a start. Your doctor can then refer you to the appropriate services.

Since the mother is usually in close contact with midwives and their obstetricians, turning to them for support and guidance is also an option. Further to this, there is a specialised service within the Antenatal Outpatients Clinic at Mater Dei Hospital. The Perinatal Mental Health Service includes psychiatrists and psychologists and social workers who are available to provide support.

Dr Ethel Felice is a consultant psychiatrist and expert in perinatal mental health. She forms part of The Association for Child and Adolescent Mental Health – ACAMH Malta. The association held a maternal mental conference in May in which President Marie-Louise Coleiro Preca called for stronger synergies between civil society and national authorities to implement policies to safeguard early detection and treatment of maternity-related mental illness.

your face and hands. Most of a lifetime of sun damage to your skin happens before the age of 18.

By your early twenties, the collagen and elastin that keep your skin springy will have started to break down. Some very fine expression lines will begin to form on your face. Keep smiling. You’re still in time to minimise further damage. Take particular care of your face and hands. Because they are exposed all year round, their skin is the most prone to sun damage. Use sun protection daily and, if you have short hair, don’t forget the back of your neck.

In your thirties, cumulative damage can be seen as collagen and elastin breakdown accelerate. Year round sun protection is a must. You can regain a more youthful glow to your face using an alpha hydroxy acid (AHA) cream under a doctor’s guidance. Avoid the sun as AHA makes the skin more sensitive and prone to burning. Use a body scrub at least once a week and take the time to remove harsh skin from your feet as split skin means you can easily pick up infections. Use non-comedogenic products on your face to avoid acne.

In your forties your skin will be much drier and renews itself more slowly. Using a moisturiser more frequently (men too, please note) will compensate for your skin’s reduced

and thinner. Cumulative sun damage can’t be reversed but its worst effects can be masked slightly by using a good moisturiser, exfoliating regularly and using sun protection to slow down further sun damage. Remove hard skin from your feet and use a good foot care lotion to prevent deep drying out.

If an unusual spot appears, visit a dermatologist as it may be an actinic keratosis, a pre-cursor to skin cancer.

Your sixties are the reaping-whatyou-sowed decade. Skin care in a pot will not remove wrinkles though some skin care products are able to disguise them temporarily. Using moisturiser will help to protect your skin from

dryness. If your lines truly bother you, the only way to remove them is by chemical peel, microdermabrasion or a laser procedure that will remove the top layer of skin cells making it look smoother, or by a surgical facelift.

In your seventies, your skin becomes undeniably thinner, age spots are more prominent and subcutaneous fat is lost making the skin look much looser all over. Thread veins are clearly visible, especially on the face. No harsh scrubs and as little direct sunlight as possible will help preserve what little elasticity is left.

Good moisturiser and gentle cleansers will help protect your skin.

Cosmetic creams can mask the effects but they cannot repair deep structural damage. Over time, the skin’s ability to defend and repair itself becomes less efficient so its appearance and health decline.

Sun damage goes deep, affecting even the basal cells that work to replenish the skin. Fewer basal cells working at a lower rate means that skin is refreshed at a much slower rate. Meanwhile, the microvascular system that feeds nutrients and oxygen to the skin and removes waste deteriorates, further reducing the skin’s ability to function properly. The reduced availability of nutrients, cumulative sun damage and slower skin repair and renewal cause the skin to age visibly.

SUMMER SKIN TREATMENT Sun exposure can interfere with skin treatment procedures and, in most cases, will affect post-treatment recovery. If you plan on having cosmetic surgery or other work done, you will need to keep out of the sun for a while beforehand and while your skin heals after treatment.

Acid-based or heat-based treatments should not be done on sunburned skin because it will damage the skin. Tanned skin is also vulnerable and can lead to hyperpigmentation after treatment.

Laser treatment for hyperpigmentation works by ‘burning’ melanin in the skin. If you are tanned, laser treatment cannot work properly and, at worst, can cause other problems.

Laser treatment for hyperpigmentation, hair removal, or skin smoothening, is best done in the winter when your tan has faded and when your skin will be covered up and out of the sun for a while afterwards.

Vein removal is best done in the wintertime. After treatment for varicose veins you will need to wear compression stockings which are uncomfortable in the heat. Sclerotherapy, an injectable treatment for small capillary veins, causes temporary superficial changes in the skin which are aggravated by sun exposure.

Injectable treatments for removing fine lines are wrinkles can work safely in the summer but not if they are the type which is affected by sweating. If you want to have Botox injections, for instance, it’s best to have the work done when the weather is cooler.

t the lowest point on earth, some 400 metres below sea level, the Dead Sea lies in an arid, but visually stunning area flanked by the Judean Mountains to the west and the Moab mountains to the east. It is one of the world’s most saline bodies of water, fed mainly by the River Jordan at its northern tip.

The sea’s unique characteristics make it an attractive destination for climatotherapy. Huge amounts of water evaporate from the sea’s surface, leaving behind a high concentration of minerals and salts in the water and mud bed. Many of the sun’s burning rays are filtered out because of the low level of the area and the heat is tempered by a relatively low level of atmospheric humidity. The combination of air quality, warmth, light exposure, as well as the famous Dead Sea salts and mud make the area an attractive destination for the health and wellbeing traveller.

WHEN TO GO There are more than 300 cloudless days each year in the Dead Sea area, and the annual rainfall is very low. The temperature averages 32C (90F) but because the level of humidity is not very high (35%), the air temperature is bearable, if not always comfortable, except at the hottest time of day. Indoor facilities are usually air conditioned.

WHERE TO GO There are resorts on both the Jordanian and Israeli side of the Dead Sea. With the exception of the Dead Sea Spa Hotel and medical centre, the resorts on the Jordanian side are operated by 5-star hotel chains. All offer spa services in luxurious surroundings, largely aimed at the wellbeing traveler. If you are interested in medical treatment, you’d be better off visiting a medical centre, rather than a hotel spa. If you are taking medical treatment, consult your physician before visiting the Dead Sea.

HOW TO GET THERE Israeli resorts are two hours away from Tel Aviv international airport. Jordanian resorts are one hour away from Queen Alia international airport in Amman. There are no direct flights to Jordan from Malta, but several airlines offer connecting flights through European cities.

PACKAGED PRODUCTS Products based on Dead Sea extracts are readily available over the counter. Mud products including face masks, are most likely to be closer to the “full strength” natural mud than are other preparations such as shampoo, conditioner, shaving cream and so on. However, they are intended for cosmetic use. Do not use them for medical reasons without consulting your physician.

PSORIASIS TREATMENT Climatotherapy at the Dead Sea is particularly beneficial for sufferers of skin conditions such as psoriasis. Published studies show that most psoriasis (80-90%) sufferers can expect significant improvement in their condition and many times the condition clears up altogether. In a small amount of cases (<1%) there is no change at all or the condition worsens. Sufferers of conditions such as eczema also report improvement.

Once the condition clears after treatment at the Dead Sea, the length of remission time varies.

Studies show that remission can last several months but the actual length of time can vary widely from case to case. Patients are recommended to plan to stay for at least four

weeks for the maximum benefit and length of remission. Though improvement can sometimes be seen within two weeks, in most cases significant improvement is not visible for at least three weeks.

The cost of a lengthy stay at any one of the resorts is a factor to consider and it must be weighed up against realistic expectations of the benefits. Using Dead Sea salts in the bath at home may provide some level of relief but is not as effective as receiving treatment at the Dead Sea itself. Although Dead Sea climatotherapy for psoriasis sufferers includes both monitored sessions of sun exposure and bathing in the sea, studies show that it is the unique light exposure that is the most effective part of the treatment regime.

When we cleanse our face daily we realise how much dirt our skin accumulates. The same thing happens to our hair and washing it as often as necessary is basic to good hair care. Use an appropriate shampoo and lightly massage your scalp with your finger tips. To avoid drying out the hair shaft, apply shampoo to your scalp and let the suds run through your hair. Rinse out the shampoo properly and apply a detangler or conditioner, and then let your hair air dry.

Try using the sun’s heat to your advantage. While at the beach, apply a deep conditioning hair mask, twist your hair into a bun, and let the mask work while you relax. Some products offer protection against salt or chlorine. Ask your hairdresser for advice on what’s best for your particular hair type.

Consuming a healthy diet is another factor which contributes to healthy hair. Foods rich in omega 3 fatty acids strengthen the hair and contribute to its shiny look. These foods include salmon, spinach and omega 3 rich eggs.

Use hair spray as infrequently as possible. Many include alcohol and ethanol which dry out the hair, causing it to break and snap easily. If you really must use a hair spray, look for one made of natural ingredients. Your hairdresser should be able to advise you.

Have you ever considered using tea as a hair product? Lemon tea or camomile tea are ideal for blonde hair, black tea for brown or black hair, and red or ginger tea for copper or red hair. Boil the tea and let it cool off. Then relax, and pour the liquid through your hair. When your hair is properly soaked, cover your head with a shower cap and let the tea act for ten minutes. Afterwards, wash and rinse your hair as usual.

Limit the times you blow dry your hair during summer. Use the diffuser attachment and arrange your hair using your own finger-tips instead of a hairbrush. Experiment with different hair styles, tying your hair back or up, so not having

blowdried straight hair won’t be such a problem and a nuisance.

Too much exposure to pool water can damage your hair colour, especially if it is dyed. To avoid colour damage, either keep your hair dry by tying it up or using a swimming cap, or, after every dip in the pool, rinse your hair with sparkling water. The fizz helps clean out any chemicals which would otherwise stick to your hair.

A tip specific to blondes – avoid lightening hair artificually as exposure to the sun lightens your hair naturally. Avoid highlights between May and October, especially if you are a frequent beach goer or if you work outside.

When at the beach, many women brush their hair vigorously as soon as they climb out of the water. Pulling at your hair while it is wet damages the hair shaft as wet hair is more prone to breaking. Instead of using a brush which pulls and tears, try using a wide-tooth comb for detangling.

our body is only as good as what it is fed. Eating gives your body the materials it needs to build healthy tissue. It also tells your body what it should do. Your metabolism keeps your body’s engine going. If you don’t eat the right food, or if you eat too much or too little, your system gets the wrong message and, unless it gets the right materials to work with, you risk developing weight problems and conditions or diseases like diabetes and heart disease.

The processes that keep your body in good health – breaking down food, clearing out waste products, building new tissue, repairing damage, and so on – depend on a steady supply of the right sort of nutrients. Without those nutrients, or if you do

not get enough of the right kind, those functions are compromised. In extreme cases, such as starvation, the metabolic processes slow down or stop altogether.

A lot of popular literature focuses on “bad” foods, i.e. the sort that should be excluded. Butter, margarine, red meat, salt, sugar, have all been tagged as being bad for you. Thinking only about what you shouldn’t eat isn’t very helpful. Cutting some things out of your diet is often necessary for health reasons. Salt, for instance, aggravates problems with high blood pressure. But thinking only about what to cut out, rather than what to eat instead, is frustrating, limiting, and often self-defeating. Nothing induces stronger craving for something than thinking you shouldn’t have it.

BAD DIET HABITS Eating for convenience If you’re rushing around all day without the time or mind to think about food until you’re hungry, you’re more likely to rely on convenience foods. Fast food – burgers, chips, hot dogs, pizza, pastizzi – are conveniently quick, hot, and edible on the go. But their nutrition content is questionable at best. At home, if you’re tired, have a badly stocked fridge and larder, and no inclination to spend time in the kitchen, it’s much easier to rip open a packet and add hot water, or shove a ready 22

meal into the microwave. If that’s a daily routine, you’re not doing your body any favours. Save time by making up double servings of meals and freezing one so whenever you’re rushed or too exhausted to cook, you can still get a home-made meal.

Eating solely for pleasure Food is one of life’s greatest pleasures, but it doesn’t work for you if your pleasure is too much of one thing, not enough of another, and erratic eating times and portion sizes. Yes,

cold, dull weather makes you crave carb-laden, food. When you’re hungry, nothing seems more satisfying than sweet or fatty food. When the weather’s warm, eating anything more than a light salad can seem too much. If your eating pattern is driven only by what you feel like, when you feel like it, you probably are not getting all the nutrients you need. Kick off your day with a reasonable breakfast. It’ll set the tone for the rest of the day, reducing the mid-morning cravings that have you reaching for a chocolate bar or donut.

Relying on processed foods Take a look around your usual supermarket and then look at the amount of fresh food in your shopping trolley. Chances you’ve picked out convenience foods that taste good and are dressed up in attractive packaging, but are nutritional nightmares. Processed foods are lower on natural nutrients than fresh food.

If most of your ingredients or meals come out of a packet or can, you’re asking for trouble.

If your eating pattern is driven only by what you feel like, when you feel like it, you probably are not getting all the nutrients you need

Processed foods have additives Take a look at the labels on those packets. You’ll probably find they are full of additives, flavour enhancers, artificial colours, sweeteners, salt, or

chemically-altered fats. All those things mess up your system. Stock up on fresh, unprocessed food instead.

It means a bit more work, but better health in the long run.

Think nutrition, not just natural Raw vegetables have a higher nutrition content than processed vegetables, right? Not quite. Mass produced food is naturally nutrition-depleted compared to organically grown fruit, vegetables and meat. Super-sized produce of uniform colour and size looks very attractive but its nutrition content may not be as good as the ugly, irregularly shaped produce grown without pesticides and other chemicals.

NUTRITIOUS FOOD Eating well does not have to be boring. As a rule of thumb, everything in moderation and nothing in excess works well for most people. If you have a chronic health condition, are pregnant, overweight or underweight, ask for professional advice on what to eat.

Meat has a high concentration of nutrients, including protein and iron. However, not all meats are equal in terms of health-value and nutrition. Choose lean, organic meat, rather than fatty cuts and keep your intake of red meat to no more than 1-2 times a week.

better than a thick, fatty ribeye steak and can be just as tasty.

Fresh fish provides lowcholesterol, low-fat protein, lots of minerals, and great taste. It is one of the most versatile and nutrient-rich convenience foods. Have your fishmonger gut and descale a fish, put it in the oven for 20 minutes or so and serve with a fresh salad. Oily fish provide essential omega-3 and omega-6 fatty acids. A portion of Atlantic salmon can give you most of your daily vitamin B12 and B6.

Fruit and vegetables pack a nutrient punch, with lots of taste and fibre and zero

preparation time beyond washing and peeling. It’s also portable, and perfect for snacks between meals.

Apples are high in fibre and containt lots of antioxidants.

Bananas are one of the best sources of potassium and have lots of vitamin B6. Oranges are fibrerich, full of vitamin C and antioxidants. Berries are a good source of vitamins and antioxidants. Strawberries are lowcarb, but loaded with fibre, vitamin C and minerals. Blueberries are among the tastiest fruit and packed with antioxidants. Pineapple gives you vitamins A, B6, and C, and a good dose of calcium and other minerals. Buy fresh and seasonal

organic fruit for the best nutritious snack.

Nuts and seeds are loaded with nutrients including protein, manganese, and vitamin E, which is great for skin health. Many are high in fat and calories but their satisfying, crunchy taste and texture can add a nutrient boost even in small amounts. What are your current experiences in this regard and is it just the sugary sweets that are to blame?

I see many cases where parents claim their children eat very few sugary snacks but still have loads of caries. Further questions reveal the children use toothpaste without fluoride. Seeing children with teeth worn away to little more than

stumps due to their mouths being constantly bathed in acidic food and drinks really infuriates me. Dental decay occurs from plaque, a deposit of bacteria that forms on our teeth. This bacterium combines with the sugars and carbohydrates we eat to form an acid. It is this acid that can attack and break down the enamel on teeth and cause a cavity to form. Children’s milk teeth are the direct victims of what we feed them or what we allow them to eat.

popular belief, fluoride is not toxic, unless your child eats a whole tube of toothpaste. It is scientifically proven that fluoride will help children have stronger and more decay-resistant teeth over their lifetime. Fluoride reduces the ability of plaque bacteria to produce acid and can strengthen tooth enamel to resist this kind of acid.

Dental recommendations indicate that children should spit out after brushing, should not rinse their mouth, and should not use water on their toothbrush and toothpaste.

What can be done if a child cannot cope with treatment under local anaesthetic in the dental chair and s/he needs several fillings?

So are our children ingesting key caries-causing foods which are being masked as healthy?

Unfortunately, yes. These include smoothies and fruit juices due to their high content of natural acids, with apple juice being the worst. Then there are fizzy drinks, including diet brands, as well as flavoured fizzy water. If these are ingested, it is important to limit them only to mealtimes and parents should remember that using a straw with drinks helps reduce erosion.

Did you just mention smoothies?

Aren’t they supposed to be healthy?

Parents are inadvertently endangering their children’s oral health by giving them too many concentrated fruit smoothies, which have a high acidity level. Giving a child one fruit in its natural state, say an apple, a fig, or a banana is healthy. But giving the child a concoction of juiced fruits is another matter. Juicing involves the removal of fibre and this removes some of the nutrients that would be found in a whole fruit or vegetable. There is likely to be a more concentrated level of sugars naturally present in the fruit. But if a fruit juice is sipped over a long period of time, the fruit juice, which is quite acidic, can damage dental enamel. In addition, some smoothies are very high in calories, with added whole-milk yoghurt, and even peanut butter or chocolate.

Consider that 81% of Maltese five-year old children have teeth with signs of erosion, which is caused by exposure to fruit-based acidic drinks. I find that 28

many young children are refusing to drink water because they have been given fruit juice from an early age. This is a major dietary problem. Fruit juice, whether freshly juiced, in smoothie form, or in a packet, is very acidic, with a pH value of 2-3.

Consider that teeth are under attack when the pH in the mouth is lower than five. Smoothies are marketed as healthy, claiming they contain one or two portions of the recommended five per day.

However, a major factor is glossed over – these drinks have a high acidity level and are therefore harmful to tooth enamel.

Smoothies and juices should be marketed as treats, rather than health fare, and schools should only offer children milk or water to drink. As things stand, too many children are growing up refusing to drink natural water because they have been fed flavoured water or fruit juice since they were babies. And here I have not gone into the risky business of children become used to other popular caries-causing foods such as tomato ketchup, honey, sugary cereals, raisins, sponge cakes and so on. Moderate consumption is ok. Eating them constantly throughout the day is not.

Should children brush their teeth and use toothpaste differently from adults? Which type of toothpaste is best?

There is another trend to which I referred early on – the trend in the use of natural oral care products like aloe vera toothpaste and fluoridefree toothpastes. This is very risky and may jeopardise healthy teeth for a lifetime. Contrary to

I still see many children below the age of six with a very, very high caries rate and an average of 6-8 holes in their teeth. These teeth MUST be treated as they are usually the primary molars which should be lost naturally when the child is 12 years old. Quite often, very young children do not allow me to treat them under local anaesthesia in the clinic and we have to resort to treating them under general anaesthesia in a private hospital. It is important to point out that, if diagnosed early enough, most baby teeth can be saved under general anaesthesia and this avoids early loss of teeth which has several complications.”

une is usually one of the most anticipated times of year. It signifies exam stress, the start of the holidays, and the opening of Maltaâ&#x20AC;&#x2122;s beach season. This year, June was also as election month. The short, intense campaign drove political enthusiasts into gathering at mass meetings, at discussion meetings in Maltaâ&#x20AC;&#x2122;s towns and villages, and flooding social media with posts about one party or another.

Politics are discussed continuously, not only during election campaigns. Discussions at the workplace, in public places and also within families at times can become really heated. How do couples manage their differences?

Sandra* and Martin*, married for 34 years “I am not bothered at all about politics. I have been a housewife all my life and really no decision taken will ever affect me. I give everything to my family, listen to the news and go to vote for the party that my family has always supported throughout the years,” says Sandra.

Martin, on the other hand, is fervent about his political party. “I work in an industry that has always been a hot issue. I firmly believe in my political party as the only guarantee for my place of work. I argue a lot with Sandra, trying to persuade her that my job can only be guaranteed with my party in government. Over the years, her father has influenced her a lot and so she keeps voting in the same way.’

When asked about how this affects their relationship, Martin said that since Sandra is not interested, they manage to discuss things in a civil way. “He watches all political programmes and debates,” Sandra says. “I watch them too but I don’t pass any comment since I admit that I don’t understand much.”

Martin and Sandra have two children, both of whom are now married. “We don’t influence them, although they understand their father’s position, so most probably they vote like their father,” says Sandra. She adds that she always puts the interest of her family first and so she avoids discussing politics with Martin and with their grown up children and their spouses.

Julie* and Trevor*, partners for 8 years Julie and Trevor are both very involved in their respective political parties, and not only during an electoral campaign. Julie’s full time job deals with politics while Trevor is a candidate’s campaign manager. “When we met, we were already both very involved with our political parties. In fact, at the end of a heated argument, Trevor asked me whether I would like to continue the discussion over drinks,” Julie remembers.

Both she and Trevor say that their situation is far from ideal, not only because they have different political beliefs, but also because they can’t share the details of their day. At times I have sensitive information on my personal laptop while Trevor attends meetings related to strategies being planned by his party,” Julie says. They started living together five years ago and, when they took this step, they both decided not to bring politics into their home.

“It was a bit hard at first since we both breathe politics,” says Trevor. But he also says that they both value their relationship greatly and they couldn’t allow politics and strangers to affect the synergy between them. In spite of all these differences, Julie says that there is one great 32

advantage.

“We understand each other and we don’t judge each other. Politics, when taken seriously, is a means of service. We chose this path and we

Now that the campaign and post-election celebrations are over, Karen and James have put everything behind them and are looking forward to a great summer together. “I just hope that we will manage to talk a bit more about politics and about our beliefs so that we avoid a repetition of tension once an electoral campaign hits us again,” says James. Neither of them expects the other to change political views or to switch allegiance, but James says he would like Karen to be a bit more understanding. *Not their real names.

Political differences are a reality in Malta and, yes, they affect a relationship. Some psychologists say that one way of easing tension is to apply what is known as ‘strategic topic avoidance’. Ignoring what is happening especially when in the presence of each other helps to relieve unnecessary tension. On the other hand, other psychologists say that having different opinions might be an asset to a healthy relationship.

Couples often do not have time to communicate properly due to busy lifestyles.

Having a discussion about politics, even from opposing viewpoints, can still be healthy as it is a way to get to know each other better.

As in every other situation in life, there are

extremes. Some people describe their political beliefs as the foundation on which their character and life are built and this is reflected in how they act in particular situations, even in the way they handle personal relationships.

What surely doesn’t work and should be avoided is one partner trying to change the other partner’s perspective. Everyone has a right to form his or her own opinion. Trying to force another person to believe in what you believe is definitely not a show of love and respect.

Electoral fever fades after a few weeks, but love and respect should be constant. The best policy is focus on what really matters in your relationship.

That’s one policy everyone can agree on, no matter where their political sympathies lie.

This means difficulty in lifting and carrying, walking, bending, sitting or standing, seeing, remembering or concentrating and hearing. Regular physical activity can improve this statistic.

Physical activity can be beneficial to stroke patients on multiple levels

area. However, most people with disabilities face barriers to participation such as imposed limitations on attending leisure activities and mobility difficulties which makes it difficult for them to get out of their home.

Sport and physical exercise benefit everyone, including people with disabilities. The following are some of the common health conditions or disabilities in Malta and how physical movement can help to alleviate them.

Fibromyalgia is a common chronic condition which reduces one’s quality of life. Its effects include widespread pain, cognitive symptoms such as memory failure (both long-term and short-term) and difficulties with attention, sleep, and fatigue. Even though many studies show that aerobic (such as walking) and strengthening exercises are effective.

To help alleviate fibromyalgia, it is recommended to follow a graded and varied exercise programme. This means using varied exercises in the same session or in different sessions, starting from low-intensity and low resistance levels and increasing slowly, approximately by 10% after two weeks of exercise. The intensity and duration of exercise sessions should be reduced when post-exertion pain or fatigue is experienced. Pool-based exercise

Studies show that vigorous or strenuous exercise is associated with decreases in stereotypic (self-stimulatory) behaviours, hyperactivity, aggression, self-injury, and destructiveness.

An example of a vigorous exercise means a 20-minute or longer aerobic

workout, 3-4 days a week. Many autistic children gain weight if they are not active, which causes another set of problems. Since stereotypic behaviour interferes with teaching, an exercise program might improve a pupils’s ability to concentrate, even in a classroom setting.

Attention Deficit Disorder (ADHD) can impact both children and adults. Some typical symptoms of ADHD include restlessness, lack of focus, and excessive fidgeting. Such behaviour can be especially problematic for children in a classroom setting or for adults at their place of work. With daily physical activity, a youngster with ADHD may experience increased cognitive function and a better mastery of personal organisation and self-control.

Using exercise in conjunction with ADHD medications can be especially effective for some patients. Release of excess energy can happen during regular exercising, which can allow a person with ADHD to focus better on their daily tasks and interact well with peers. Studies have suggested that children with ADHD who participate in

With daily physical activity, a youngster with ADHD may experience increased cognitive function

morning exercise before spending time in the classroom may experience fewer problems with moodiness and inattention, and experience higher selfesteem and greater self-confidence.

Muscular Dystrophy (MD) is an inherited genetic condition that gradually causes the muscles in the body to weaken. This condition gets worse over time but regular exercise, focusing primarily on gaining muscle strength and endurance, and flexibility (stretching) exercises to prevent freezing of the joints (contractures) can retain muscle strength while slowing down the progression of weakness and improve breathing. Aerobic exercises, such as walking, rowing, wheeling and arm ergometry for 20-40 minutes 4-6 times a week can help. Swimming can also be a healthy activity for persons with MD as it is a good way to work on muscle tone while the water buoyancy helps protect against muscle strain and injury.

A Stroke is when blood flow to an area of the brain is blocked. When this happens, the brain cells in the affected area are deprived of oxygen and begin to die. When brain cells die during a stroke, abilities controlled by that area of the brain, such as memory and muscle control, are lost. Physical activity

can be beneficial to stroke patients on multiple levels. Studies have demonstrated physiological, psychological, sensorial, strength, endurance and functional effects of various types of exercise and a decrease in the risk of recurrent strokes.

There are three main rehabilitation goals for stroke patients: preventing complications of prolonged inactivity, decreasing recurrent stroke and cardiovascular events, and increasing aerobic fitness. For best results, physical exercises should be started as soon as possible after the stroke attack. Use of treadmill and resistancetraining programmes, together with flexibility exercises to increase range of motion, 2-3 times a week are common recommendations for an improved quality of life.

any Maltese know Victor Calvagna for his work in cancer therapy, especially for children.

Puttinu Cares, the charity that raises funds to support families of children with cancer, kicked off thanks to Dr Calvagna and his team. We had met several times in relation to his work. A few weeks ago, we met up for a completely different reason. Victor and two of his greatest friends, Mark and Robert, are now officially Ironmen, having completed a challenge in Barcelona last May. An Ironman Challenge is a tough physical activity which includes long swims, bike rides and runs. For 59-year old Victor, in spite of his busy weekly schedule, exercise is part and parcel of his day. “I train regularly, not only for the physical benefits that exercise gives, but also mentally,” he explains.

“Exercise helps me relax and clear my thoughts.”

His two training buddies, Mark and Robert, are also

very busy men. Mark, 45, is a high ranking army officer while Robert manages a business. “We got to know each other through exercise,’”says Mark, who first met Victor back in 2010 when they both participated in Life Cycle, an annual fundraising event.

“It was a great experience through which we found each other and from then on we have never stopped training together,” explains Mark, a cycling enthusiast. Apart from taking bike rides together, Mark and Victor started swimming regularly at the national pool where they met Robert. Robert is more into trail running, which means running along trails for long distances. He can easily be described as an ‘ultra-marathoner’ since he runs for distances which go beyond the traditional 42km marathon.

It was Mark’s idea for the three of them to aim to become Ironmen. “We set a target event and immediately started training for it under the guidance of coach Kevin Zammit who co-ordinated all our training, and coach Marthese Fava who took care

of our swimming training,” says Victor. They started training hard last September and aimed to take part in an Ironman Challenge in Barcelona last May. Eight months of hard work, demanding training, exhaustion, and, above all, great determination.

“For these last eight months, we trained for around fourteen hours a week. Each one of us had his preferred area, but the way the training programme was set, we started to improve and do personal bests in all three areas,” says Robert. In the coming months he will hit the trails in a 100km ultra-marathon in the Mont Blanc area. That’s an impressive feat, especially considering all three men have demanding careers, family commitments and do long-duration training.

“It is not easy for sure, but being in the army I have always lived in a disciplined environment,” Mark says.

“To adapt to this hard training was not that difficult for me. To manage time effectively was not a piece of cake. At times, work commitments clashed with training periods and I ended up juggling my days to try and fit in everything.”

SPORT Mark was the first of the three friends to finish the Ironman Challenge in Barcelona. He did it in six and a half hours. The build-up to this event was very well organised and the three of them managed to keep up with the hard training. Unfortunately, mishaps do happen. Mark tore his calf muscle and rehabilitation took around two months.

Then he became unwell.

“Once I hit the road and the sea again, I was sick twice in the last month before the challenge,” he says. “I had to take two courses of antibiotics which affected my fitness a bit. But my determination and will-power were stronger. I was adamant that I would be on that start line.

From the conversation with these three Ironmen, it is evident that the greatest challenge of all is the training, the build-up to the event. “I was always into running, but cycling was not my forte,” Robert says. The Ironman training experience made Robert psychologically stronger: “I feel that nothing scares me anymore. I know that I can get somewhere through determination and hard work.”

Our conversation shifts to the actual challenge in Barcelona last May. “It included a 2km swim in open sea, a 90km bike ride and a half marathon of 21Km, all in that order,” explains Victor.

The three friends arrived in Barcelona around four days before the event to get used to the place and to complete pre-event arrangements such as registration and bicycle setting. The weather was not very welcoming and the sea was quite, but luck was on their side. On the day of the challenge, the 21st of May, the weather was ideal.

“The day before, I was overcome by

fear,” Robert says. “I panicked and felt that I would not make it to the starting line.” With Victor’s and Mark’s

encouragement, Robert overcame his fear and, on the day of the challenge, he was right there at the starting line, one of 2200 participants from all over the world.

“The secret to it all is that you don’t think about the event as a whole, but take it part by part,” explains Mark.

Robert took his friend’s advice. While swimming, he moved from buoy to buoy, trying to avoid the crowds getting into his space and focusing on reaching the 2km mark.

The bicycle ride was particularly difficult as they were not familiar with the route and yet, they had to pace themselves, controlling how hard to push, and where and how much to accelerate. All three saw that the run was the best part of the whole challenge.

“Our names were printed next to our numbers, and it was great to hear strangers calling out our names while we ran,” Victor says. A question I had looked forward to asking was related to their thoughts as soon as they crossed the finish line and officially became Ironmen. Surely there was a sense of achievement?

“It was a sort of an anti-climax,” says

Victor. “You’d have trained for so long, you knew now that you are an Ironman, but then ask yourself the question: ok what’s next?” As soon as the Barcelona challenge was over, all three started thinking of the next one. “You feel satisfied and you only have words of thanks to your coaches. We’re also grateful to our families who supported us throughout and put up with the emotional changes caused by hard training and its resulting fatigue,” says Mark. “Balancing family, work and training is not easy, but having people who back you makes it much easier. For this, I am truly grateful,” Robert adds.

So what is the next challenge for these three Ironmen? “We’re aiming to do a full Ironman Challenge in a year’s time,” Victor says. “We have already started training for it, adding to our other commitments.” Well, that’s impressive and it’s a good lesson learned. The secret to success in physical activity can be summarised in just three words: determination, support and timemanagement. That’s something to keep in mind whether you aim to become an Ironman or Ironwoman or start an exercise programme for a healthier future.
