Message from Ray Friends in the Community, The world of healthcare is complex and everchanging. It’s riddled with acronyms and complicated terminology. New and emerging models of care and reimbursement have turned the way we’ve traditionally treated patients upside down.

At the heart of the dynamic world of healthcare is the patient. At Evergreen Health, our patientcentered approach to care is what helps us stay focused and adapt to the environment around us.

To break it down, we believe it’s all about finding new and innovative ways to connect people to care.

In our 2018 report you’ll learn how we achieve this by establishing new service locations near our targeted populations, using data to identify opportunities, improve outcomes and leverage our resources, and creating partnerships and collaborations whose dividends we’re just beginning to realize.

Finally, we are always thinking of ways to better connect our patients and the community to our brand. We recently launched a new website – www. evergreenhs.org – and I invite you to visit it when you have a moment. Our goal is to provide patients with the same great experience at every point of care, and our website is an integral part of the patient experience.

About Evergreen Health Evergreen Health was founded in 1983 to address the HIV and AIDS crisis in Western New York. As breakthroughs in medical therapy were made for HIV and other sexually transmitted infections, the organization expanded its range of services beyond HIV care to meet the needs of the community.

Today, Evergreen is a comprehensive healthcare delivery system that provides medical, supportive and behavioral health services all under one roof. Its unique model of care treats the whole patient to improve health outcomes.

Services At Evergreen, we understand how overwhelming the healthcare system can feel and we want to make accessing care easy for everyone. We have a primary and specialty care practice onsite, as well as a full-service pharmacy.

Our targeted programs and services help people live their best lives.

Chronic Illness Support Living with multiple chronic illnesses can be challenging. We offer support to help people manage and improve their health, including: care coordination, mental health counseling, housing programs and more.

HIV Prevention and PrEP We are committed to providing HIV education and prevention services to our community. We offer testing and treatment and are the leading Western New York provider for PrEP, a daily pill for HIV prevention.

Sexual Health We offer comprehensive testing and treatment for gonorrhea, chlamydia, syphilis, HIV and hepatitis C, as well as counseling, anoscopy services, and multiple educational and outreach initiatives that provide free condoms throughout the region.

Substance User Health We provide a safe space for people who use substances to get the services and support they need, including: a syringe exchange program, counseling, support groups and medication-assisted treatment.

HIV Treatment and Care Today, HIV is a manageable chronic illness and is often treated with a simple daily prescription pill regimen. We offer comprehensive HIV services to help people manage living with HIV, including medical care and treatment, pharmacy services, insurance enrollment, individual and group counseling, mental health programs, care coordination and more.

LGBQ and Transgender Services We provide sensitive care in a safe, affirming place. We understand LGBTQ concerns and specialize in transgender services, including: medical care, counseling, educational/social groups and more. We are dedicated to helping our transgender patients feel safe, valued and welcome.

Hepatitis C While hepatitis C can be cured, treatment can be hard to find or stick with. We offer testing, treatment, pharmacy services, counseling and more. We can help people get into and succeed in treatment to cure their hepatitis C.

Financials Evergreen’s financial growth in 2018 can largely be attributed to high utilization of our pharmacy, an increase in the number of grants we received, growth in behavioral health services and an increase in the number of patients on PrEP.

Designations Evergreen was designated a 2018 Leader in LGBTQ Healthcare Equality by the Human Rights Campaign.

Evergreen was recognized by the National Committee for Quality Assurance (NCQA) as a New York State Patient-Centered Medical Home in 2018.

Evergreen demonstrated Meaningful Use of certified electronic health record (I) technology to achieve specific measures required by the Centers for Medicare & Medicaid Services program. We were able to attest that all eligible providers met those measures for 2018.

Evergreen has nine providers who are American Academy of HIV Medicine – HIV Specialist Credentialed (AAHIVM).

Evergreen is an Undetectable=Untransmittable (U=U) Community Partner.

Awards Evergreen ranked second in Buffalo Business First’s 2018 Best Places to Work in Western New York awards in the extra-large company category.

Preservation Buffalo Niagara presented Evergreen Lofts with the 2018 Outstanding Commercial Project Award.

The American Institute of Architects Buffalo/WNY Chapter presented Evergreen with the 2018 Community Design Award.

Evergreen received a Brick by Brick Award for New Construction presented by Buffalo Business First.

Evergreen received a Population Health Award in the data-driven category at the Population Health Collaborative’s annual Spotlight on Population Health awards and Expo (SOPHi).

Evergreen was awarded the Homeless Management Information System Award by the Homeless Alliance of Western New York.

Location, Location, Location In the world of real estate, location is everything, right? Homes can decrease or increase in value simply due to where they are located. If we apply this concept to healthcare, we know that people’s health outcomes can be negatively impacted if they live in neighborhoods where they don’t have access to the services they need. Hence location, location, location.

At Evergreen we talk a lot about “meeting people where they are” and we mean that in the literal sense as well. In 2018, we established new office locations and added a mobile unit to our mix of services, which aligns with our unique model of care. We aren’t necessarily waiting for those who need us to find us, we’re finding them.

Community Access Services Moved to Bailey Avenue The Erie County Department of Health’s Erie County New York Community Health Assessment 2017-2019 found that almost half of all Erie County gonorrhea cases are located in a small geographic area. One of the zip codes in this small area is 14215, which is where Community Access Services is now located. Its new location at 3297 Bailey Avenue solidifies our commitment to making a difference in communities of color by decreasing barriers to the type of quality healthcare services they need and deserve. Evergreen also offers testing services at this location.

Expansion of Specialty Care in Southern Tier To meet the healthcare needs of people in the Southern Tier, we recently opened a new location at 320 Prather Avenue in Jamestown, NY. The building houses our specialty care services including HIV care, hepatitis C care, STI testing and PrEP services. The 2,600 square foot office offers a welcoming environment for people in the Southern Tier. Continually improving upon the patient experience is at the core of all we do and we’re excited to bring these services to this region.

Harm Reduction Mobile Unit Our Harm Reduction Center launched a mobile unit that travels to different locations, from Buffalo to Niagara Falls to Hamburg, so people can get the same great service as they would at our Elmwood Avenue Syringe Exchange.

Care Coordinators Make an Impact in Emergency Rooms If we want to reduce emergency room utilization and avoid readmission in our region, well, we need to go see people who are in the emergency room. That’s just what our care coordination team is doing. Our patient health navigators utilize HEALTHeLINK data on a daily basis to identify our patients who are in the emergency room so they can immediately meet with them and the medical staff to provide a discharge planner. In addition, our outreach workers in Buffalo and the Southern Tier work with hospital staff to identify people who may benefit from care coordination services. Our employees are in hospitals every single day meeting people at their point of care. We believe targeted interventions, use of data and collaboration with hospitals can truly make an impact.

Leader in Specialty Care and Prevention At Evergreen, our approach to care is three-fold: testing, treatment, and prevention and education. When people come to Evergreen to get tested, no matter what their test result is, we can connect them to the care they need. If someone tests positive for HIV we can immediately set them up with one of our providers and they can leave with medication that same day. If someone receives a negative test result, we can discuss prevention options so they can determine what is right for their lifestyle. Our model of care allows us to guide a patient through all points of care. Because we are there for people every step of the way, they become more engaged in their own health and wellness, which leads to better health outcomes. A win-win if you ask us!

Testing: Sexually Transmitted Infections We offer testing for HIV, hepatitis C, gonorrhea, syphilis and chlamydia. Our friendly team of test counselors make it easy for people to come in and talk, and get linked to services they may need.

A record-breaking number of sexually transmitted infections (STIs) were reported in the United States in 2017 according to the Centers for Disease Control and Prevention (CDC). Rates of syphilis, gonorrhea and chlamydia have climbed for the fourth consecutive year, reaching the highest number ever reported. We understand the importance of getting tested and our caring and non-judgmental team assures people weâ&#x20AC;&#x2122;re here to help.

Treatment: HIV Care We continue to lead the way in providing compassionate care and treatment to people living with HIV. We have a high success rate in getting our patients to be virally suppressed. People living with HIV with suppressed viral loads who take their medication as prescribed are leading healthy lives and have effectively no risk of sexually transmitting the virus. In a nutshell, we are helping to control the spread of HIV in our community.

Prevention: PrEP Evergreen is the local expert in PrEP, short for Pre-Exposure Prophylaxis.

It’s a prescription medication that, when taken every day, greatly reduces someone’s risk of getting HIV by blocking the virus. As more and more people in the community learn about PrEP, the number of patients on PrEP continues to rise. Our friendly team of specialists and providers make it easy for people to get started on PrEP.

Education: Trans Wellness Conference Transgender care is an important facet of our services. We believe it is important to create ways to share our knowledge with other healthcare organizations that work directly with transgender and non-binary (TGBN) people so they can learn best practices and provide affirming “The people and environment; I’m care. Patient Story Liza Periera It has been 25 years since I was diagnosed with HIV. A few years after my diagnosis, I was introduced to AIDS Community Services, which we know today as Evergreen Health. I walked in thinking that I was going to die and that these people could not and would not help me. Boy was I wrong. I was introduced to some people that really cared about my well-being, that cared about my health and that wanted to help.

Fast forward to today. I’ve been a patient of Evergreen Health for many years.

Evergreen takes care of me in a way that no other medical facility has. I come here for primary care. I receive services from the housing program. I use the pharmacy for all of my medication needs. I have been linked with mental health services for my mental well-being. I have stayed undetectable and have been cured of hepatitis C. But, I’m not just a patient. I also work at the best place in the whole world as a linkage specialist in the Health Education and Engagement Department. Evergreen has opened doors for me and has afforded me some amazing opportunities like becoming one of the first New York State certified peers. I was number three in the whole state in becoming certified in HIV, hepatitis C and harm reduction.

They continue to encourage me, love me and inspire me because of the work that we do. Evergreen Health continues to fan the flame of desire for me to be a healthier and better me.

Population Health Population health refers to the health outcomes of a group of individuals.

It’s an approach to care that focuses on improving the health of a whole population versus individually treating people for a specific condition. There are many factors that play a role in how we can improve the quality of care and health outcomes at lower costs.

Measuring Our Performance HEDIS, or Healthcare Effectiveness Data and Information Set, is one of the healthcare industry’s tools to improve performance. It involves more than 90 measures as a way to evaluate how healthcare organizations are performing. But, what does this mean for patients? Evergreen has a team that works on monitoring and evaluating these measures so we can continually find better ways to deliver care and improve outcomes.

Collaborations: SNAPCAP Evergreen is one of twelve community-based primary care practices that make up the Safety Net Association of Primary Care-Affiliated Providers (SNAPCAP) of Western New York. Its goal is to make sure everyone in WNY has access to the highest quality healthcare available.

Together, SNAPCAP organizations have produced healthcare outcomes that exceed regional and statewide averages in areas such as high blood pressure, diabetes and depression. In 2018 the SNAPCAP Independent Practice Association (SIPA) was established to provide a structure for deeper clinical integration and joint capacity for success in managing populations and value-based payment performance.

Millennium Collaborative Care Evergreen is committed to making sure patients have access to the right care, at the right time, at the right cost. This means making sure they visit their primary care provider for most minor injuries and illnesses instead of going to the nearest emergency room. This is why Evergreen is a proud partner of Millennium Collaborative Care (MCC). With support from the New York State Department of Health, MCC is committed to reducing avoidable hospital utilization by 25 percent by 2020.

This work includes avoiding hospital readmissions by connecting patients with primary care upon discharge. Thanks to support and resources from MCC, we have developed a process to guide patients through the next couple steps after they leave the hospital and make sure they understand their care instructions. We’re here to help them every step of the way.

Value Network: New York State Behavioral Health Care Collaborative The health of the whole person – body and mind – is very important at Evergreen. This is why we are a proud network partner of the WNY Behavioral Health Care Collaborative known as Value Network. Value Network is a coalition of behavioral health providers led by BestSelf Behavioral Health, Endeavor Health Services, Horizon Health Services, and Spectrum Health and Human Services. Participating in Value Network is a community-wide effort to improve behavioral healthcare through stronger care integration across different settings, community-based recovery support services, and data to improve behavioral and physical health outcomes. Just another way we’re making sure people receive the care they need and deserve.

Community Partnerships As we mentioned earlier, the world of healthcare is complex. And while there are many great initiatives underway to improve communication and interaction among providers and other service organizations, the system remains fragmented to some extent. One way to overcome this fragmentation is to ask ourselves, “What else do our patients need? And, who can help us provide it?”

To us, this means forming collaborations with other local entities so we can connect people to the care and services they need to improve their overall health. We know that we can’t do it alone, so we’ll do it together. Read on to learn about two great collaborations Evergreen created in 2018.

University at Buffalo Department of Medicine Early last year, we announced a collaborative partnership with UBMD Internal Medicine to increase the quality of care for individuals living with HIV and hepatitis C and provide an ideal setting for clinical research. The partnership also increases opportunities for student training and gives patients access to the newest treatments.

One of our first joint endeavors was a clinical trial that investigated the safety and efficacy of a regimen to treat HIV that is administered in monthly injections. The injections were administered at Evergreen under the direction of our medical director, Dr. Alyssa Shon.

“Adherence is key to successful HIV therapy, and medication regimens that do not require daily dosing could transform how we treat HIV infection,” said Dr. Shon.

This injectable drug therapy, which is administered every four weeks, differs from most current antiretroviral drug therapy that is taken orally on a daily basis. This clinical trial is one way we are working with UBMD to enhance HIV care in our region.

“Evergreen has developed an innovative approach for providing optimal care for individuals living with HIV, hepatitis C, and for the medically underserved,” said Thomas A. Russo, MD, Professor and Vice Chair for Finance in the Department of Medicine at UB’s Jacobs School of Medicine and Biomedical Sciences and Chief, Division of Infectious Diseases.

“UBMD Internal Medicine’s collaboration with Evergreen will further strengthen this state of the art care model, generate new collaborative research opportunities, and enable our medical students, residents and Fellows in training to experience and learn from this progressive institution that is fulfilling a societal promise to care for these vulnerable populations.”

Core Cuisine When THRIVE Wellness, our program that helps patients manage their chronic health conditions, was looking to partner with a food vendor, it chose Lauren Gresham, owner of Ravelle Culinary Creations, for a variety of great reasons.

Gresham is a chef and educator with a deep understanding of the unique needs of our patients.
