Momentum is critical in the oncology arena, where every day new questions arise, new discoveries are made, and progress is achieved. Cancer is an ever-evolving disease, and so we work in an ever-changing environment — one in which we must lead the field as we strive to outsmart our target.

The accomplishments of this last year reflect Fox Chase Cancer Center’s onward course, compelling us to reach even greater levels of success and to eclipse previously accepted boundaries. With each passing milestone, we reshape what is thought to be possible.

We set higher goals and establish new standards:

Next-Day Access to Care

“

C

atherine Braccia was diagnosed with stage III lymphoma in December 2014. She called several cancer centers to schedule an appointment. However, many were unresponsive, did not accept her insurance, or could not see her for nearly six weeks.

“I felt helpless and in disbelief that in the year 2015, I was not able to receive the care I needed from some of the top-rated cancer centers,” Braccia says.

That is when she decided to call Fox Chase Cancer Center.

Braccia called late on a Monday afternoon and spoke with nurse navigator Cathy Haberle, RN-BC, MSN. “She really listened to me,” says Braccia. “I could hear her typing on the computer, and she was struggling because there really was not a space for me. But she said, ‘I’m not hanging up this phone until I get you in to see somebody.’”

Haberle scheduled Braccia for an appointment the next morning at 8:30 a.m. with Richard I. Fisher, MD, a nationally renowned lymphoma specialist and the Fox Chase president and CEO. One week later, Braccia began chemotherapy.

She also received the research drug lenalidomide, to which she responded well and did not experience adverse effects.

Braccia is one of many patients who have called Fox Chase and benefited from the Center’s Rapid Access Service, which was launched in June 2014 and allows patients to be seen the next business day after their call, whether or not they have a definitive diagnosis.

Fox Chase was the first center in the region to launch this service. “Getting an appointment that quickly saved me mentally — and probably physically,” Braccia says.

Through the Rapid Access Service, a new patient representative or nurse navigator matches patients with the most appropriate physician and helps them schedule their first appointment. “Once patients are diagnosed with cancer, they are extremely concerned and obviously anxious,” Fisher says. “Getting patients in to see a disease-specific oncologist within a 24-hour period is a major advantage.”

Before implementing the Rapid Access Service, about 30% of patients who called to schedule an appointment cancelled beforehand due to the long wait for an appointment, according to Fisher. Since implementation of the service, that number has dropped to less than 5%.

“The faculty must be able to rapidly adjust their schedules, but we have been very successful,” Fisher says.

Saving Women’s Lives with Innovative Technology

F

ox Chase Cancer Center is committed to offering the most advanced technology for diagnosing and treating breast and gynecologic cancers. Recognizing that the best clinical outcomes come with early detection, Fox Chase has a Mobile Screening Unit that is equipped with state-of-the-art digital mammography technology.

The Center’s Mobile Screening Unit — the only one of its kind in the region — makes it easy for women to get their mammograms because it comes right to their community or place of work. “Through our outreach and partnerships with health centers and social groups, we have used this service to benefit about 6,000 patients each year, many of whom are from underserved populations,” says Kathryn Evers, MD, director of mammography at Fox Chase.

Now with the addition of 3D tomosynthesis this past summer, the Center offers more sensitive breast cancer detection than ever before. Because conventional digital mammography produces one image of overlapping tissue, it is sometimes difficult to detect cancers, and patients may be called back for additional images. By contrast, 3D tomosynthesis takes multiple images of the entire breast, allowing

radiologists to see through layers of tissue and examine areas of concern more clearly. “For patients, these advantages translate into a decrease in call backs and fewer unnecessary biopsies or additional tests, which may substantially lower their anxiety,” Evers says.

Breast screening and research recently received a financial boost via a generous donation by the Philadelphia Flyers Wives. In March, the philanthropic organization announced a $400,000 donation to support various women’s health initiatives, including mobile screening, the use of hormones to prevent breast cancer, metastatic breast cancer research, and molecular breast imaging — a technique that improves breast cancer screening by incorporating the use of nuclear medicine with imaging.

Evers adds, “The Flyers Wives’ generous investment will help ensure that we continue to provide the most advanced and comprehensive services for women’s cancers.”

Engagement through Outreach

A

lthough Fox Chase Cancer Center has always endeavored to improve the health of residents within its service area in Philadelphia, recently the Center has also strengthened its community outreach through multiple key offerings.

Evelyn González, the senior director of the Office of Community Outreach, explains that these goals came out of the Community Health Needs Assessment (CHNA), “which identifies priorities that speak to the specific cancer needs in our surrounding community.”

One area of focus is Fox Chase’s cultural competency, with an emphasis on language services because the local Northeast Philadelphia area has thriving Hispanic and Russian communities. The Vietnamese and Chinese communities are also growing.

Within the past year, Fox Chase accomplished the following: • Increased the number of language interpretation phone lines from four to 77.

• Purchased amplifiers for the hearing impaired and a remote video unit to support deaf patients.

• Focused current efforts on increasing on-site dual medical interpreters.

• Offered more training for employees focused specifically on cultural competency to enhance their ability to work effectively with patients of diverse backgrounds.

• Credentialed physicians in languages other than English to facilitate communication with patients.

A second need, González explains, is to increase access to preventive services such as cancer screenings. “Our free, bilingual education programs help our community members understand the importance of cancer screenings. We then help direct them to services at Fox Chase or provide screenings in community settings.”

The third area of focus is to increase the community’s understanding of the importance of research participation and to develop a system-wide approach when coordinating research with the larger Temple University Health System.

With another CHNA on the horizon in 2016, González says, “Our team looks forward to making Fox Chase community benefits even stronger.”

The Study that Launched 50,000 Papers (and Counting)

O

ctober 2016 will mark the 25th anniversary of a landmark study that originated in a laboratory at Fox Chase Cancer Center. The study, called “A retroviral oncogene, akt, encoding a serine-threonine kinase containing an SH2-like region,” is considered the starting point for a plethora of research that significantly impacted the field of cancer.

Fox Chase researchers Philip Tsichlis, MD (now at Tufts University in Boston), Joseph R. Testa, PhD, and Alfonso Bellacosa, MD, PhD, studied the differences between a protein, known as AKT, that originated from human instead of viral DNA. Although that work would take about a day with modern facilities, the comparison of human and viral sequences took months of effort 25 years ago. The work paid off when the researchers obtained the results of their analysis.

The study revealed that although the viral and human genetic sequences coding for the AKT protein were similar, there were some important differences in the viral sequence that made the protein it coded for continuously active, and it gave the cell a better survival ability.

The reason this finding was so significant is that the AKT protein is important in shutting down the process that kills abnormal cancer cells. Because viral-derived AKT is continuously active and the human form is not, the scientists realized there must be an additional factor that activates it. Once activated, AKT appeared to be extremely important in tumor formation.

“When the AKT protein is activated, cancer cells can survive, proliferate faster, and invade more. It was also found that AKT can regulate angiogenesis,” says Testa, a professor in cancer biology at Fox Chase and the Carol & Kenneth E. Weg Chair in Human Genetics. “It turned out that AKT is involved in so many aspects of cancer. It is indeed a central player.”

Testa recently ran a search and found that more than 50,000 papers have been published that include AKT in the title or abstract. The subsequent research has demonstrated that, among other important properties, AKT is overly expressed in many tumor types, including cancers of the prostate, breast, lung, ovaries, and pancreas.

Fox Chase is planning a scientific symposium next year to mark the 25th anniversary of the paper’s publication.

Bellacosa, then a postdoctoral fellow and now a professor in cancer epigenetics, fondly recalls the importance of the work. At the moment of discovery, they were the only ones in the world who knew the origin point of several cancer types.

Bellacosa says, “Sharing the excitement of that discovery creates a very strong emotional bond, and so my friendships with Phil and Joe are very strong to this day.”

Patients’ Immune Systems Help Fight Cancer

M

any oncologists consider immunotherapy the single most important discovery of the 21st century for cancer treatment. Immunotherapy, which uses a patient’s own immune system to help fight the disease, is “changing the way we understand and treat cancer,” says Anthony J. Olszanski, RPh, MD, director of Early Clinical Drug Development and co-director of the Melanoma Program, who works on developing new drugs at Fox Chase Cancer Center.

In a clinical trial, Olszanski and his colleagues are testing a new combination of immunotherapies for patients with advanced melanoma. One of the drugs is pembrolizumab, a so-called checkpoint inhibitor that the FDA approved for melanoma in 2014. Checkpoint inhibitors block a receptor called PD-1, which may allow specialized cells of the immune system, called T-cells, to target and kill melanoma cells. “These checkpoint inhibitors have arguably been the most impressive drugs ever discovered for patients with melanoma,” Olszanski explains.

However, pembrolizumab does not work for all patients. Olszanski and his team are adding another immunotherapy called TVEC (talimogene laherparepvec), an experimental drug, hoping that the combination will be more effective for some patients. TVEC is a scientifically modified form of the common herpes virus that has been altered to specifically target and kill melanoma cells.

A 38-year-old woman with advanced melanoma enrolled in the trial and has had an impressive response to the combination therapy. The disease on the patient’s upper body was first treated with standard surgery to remove the tumor. Subsequent pathology showed that her disease was advanced and had some high-risk features. A recurrence of the disease later resulted in another operation to remove the new growth. One month later, the disease returned and the patient realized that the tumor was growing larger every day.

“We needed to try something different,” Olszanski says. “I explained the clinical trial in detail. She understood that we did not know if the combination was safe or if it would work, and that we would have to follow her very closely. Understanding this, she signed the consent form and we began treatment.”

First, TVEC was injected directly into the melanoma disease site and pembrolizumab was given intravenously. Within three weeks, her disease stopped growing and she had no serious adverse effects. Within two months, there was only a scar at the site of the lesion. Because there was no lesion to inject, TVEC therapy ended. With no new areas of disease, the patient is now being treated with pembrolizumab alone.

Indeed, immunotherapy is showing real promise for certain patients with cancer.

DONOR PROFILE

Neil Hall When Neil Hall received a diagnosis of stage III melanoma, he had a lot of anxieties and questions.

The difference was how his doctors at Fox Chase Cancer Center addressed his concerns. Hall says that his doctors, Jeffrey M. Farma, MD, co-director of the Melanoma Program, and Eric I. Chang, MD, surgical oncologist, cared just as much about him as a person as they did about treating his disease.

“I was impressed with the way they helped me understand my treatment and the associated risks. I felt cared for,” shares Hall.

After surgery to remove a growth, his doctors tested his lymph nodes, which came back cancer free. In response to the good news, Hall donated $50,000 to the Melanoma Program, a multi-disciplinary team of clinicians and scientists with extensive backgrounds in translational research.

Overall, Hall’s experience at Fox Chase taught him to be more sympathetic to others coping with cancer. “As a patient, I developed a very worthwhile understanding and sensitivity of others dealing with the disease.”

Recognizing ‘A Special Energy’

W

hen Richard “Dick” Millham talks about Fox Chase Cancer Center, he can’t help but mention his late wife, Virginia. “My wife was diagnosed with breast cancer in the late 1980s,” says Millham. “She had a mastectomy at another local hospital and was deemed cancer free. But when the cancer metastasized again six years later, I said to her doctor, ‘We’re going to Fox Chase because it’s the only place that can give us hope.’”

Millham knew about Fox Chase, having lived in the Philadelphia area all his life. When he walked through the doors for the first time, he was touched. “I felt a special energy,” he says.

“Every employee was so helpful and courteous.

The care was just outstanding.”

“Ginny,” as Millham affectionately calls his wife, was treated successfully for three years.

Toward the end of her treatment, Virginia needed additional surgery. Afterward, the couple’s insurance company advocate insisted she be sent home, but Virginia’s surgeon felt she needed an extra day to recover. “Our insurance company came back with a ‘No,’” Millham says.

“Any other hospital would have called her a cab and sent her on her way, but her surgeon came to me and said, ‘Dick, I don’t want you to worry.

We’ll help you sort this out.’”

Although Virginia died in 1998, Millham was so grateful for the care she received that he contacted Fox Chase’s Bucks County Chapter of the Board of Associates and sent them a check for $10,000. He joined the chapter a few weeks later and has been supporting Fox Chase ever since.

Between his personal contributions and the money he’s raised from friends, Millham has donated close to $500,000 to Fox Chase. Last year, he set up an endowment for cancer research and has included the Center in his estate plan. Milham has even made the Center a beneficiary to his 401K. Millham’s tireless advocacy for Fox Chase has never let up, even through his own cancer journey when he was diagnosed with Merkel cell carcinoma, a rare form of skin cancer. Through treatment from radiation oncologist Thomas J. Galloway, MD, and surgical oncologist Miriam N. Lango, MD, Millham has been cancer free for more than three years.

These days Millham, now 80 years old, splits his time between working at the insurance agency he founded in Doylestown, Pennsylvania and working the raffle table at Fox Chase fundraisers. He adds, “Anyone can see that Fox Chase is just plain special.”

Honoring Fathers, Raising Hope

B

oth 2013 and 2014 were difficult years for Bill and Sue Federici. In 2013, Sue’s father, Gerard Flaherty, was diagnosed with stage IV lung cancer after an X-ray did not detect any trace of the disease several months earlier. Around the same time, Bill’s father, Richard Federici, was coincidentally also diagnosed with stage IV lung cancer.

In October 2013, Flaherty died at the age of 81, and just six months later, Federici died at 85. Both fathers were happy and healthy until the last three months of their lives. “It was striking how precipitous the fall was for them,” Bill says. “They went from being two healthy men in their eighties to dying in a really short amount of time.”

After their fathers’ deaths, Bill and Sue looked for something positive to come out of their pain — and they also wanted to honor the memory of both men. Although Flaherty and Federici were not treated there, the couple turned to Fox Chase Cancer Center, having become familiar with the Center through local charity events they had attended with West Pharmaceutical Services, where Bill serves as chief financial officer and senior vice president.

West is a leading fundraiser for Fox Chase and has raised more than $1.5 million since 2004.

Clapper, PhD, professor of cancer prevention and research. “When we met with the doctors, we were touched by the passion and energy they brought to the research and detection of cancer,” Bill says. “The studies being conducted at Fox Chase offer hope for early detection.” He adds, “The way the scientists work hand-inhand with the clinicians is amazing.”

The Federicis donated a $1 million fellowship to further lung cancer research at Fox Chase in memory of their fathers. They started the Richard P. Federici and Gerard R. Flaherty Endowed Fellowship for Lung Cancer Research in February 2015, with the hope that more doctoral students will be able to work alongside Fox Chase professionals to find an early detection test and cure for the disease that took their fathers’ lives. Additionally, Bill joined both Fox Chase’s Foundation Board of Directors and the Board of Directors in the fall of 2015.

“We feel blessed by all that’s been given to us by our fathers,” Sue says. “This fellowship is a way for us to give to other cancer patients what neither of our fathers had: an early diagnosis and a few more years of life.”
