laying video games or using your phone is not the root of the mu s c u l o s k e l e t a l condition, but it can flare symptoms.

During the Boston Red Sox’s World Series run in 2018, star left-hand- BY ASHLEY BURLAGE, M.D. ed pitcher David Price took some time off to alleviate pain caused by carpal tunnel syndrome. He blamed the condition on playing an exorbitant amount of hours of Fortnite — an online video game with more than 250 million users worldwide.

Twenty-six-year-old YouTube and Instagram sensation Gabbie Hanna, who has more than 6.5 million followers, had a similar experience. She recently announced that she is suffering from carpal tunnel from using her smartphone too much. Is this possible? Sure is. However, using technology isn’t the root of the issue.

Carpal tunnel’s symptoms of pain, tingling, weakness or numbness in your hand or wrist is caused by compression of the median nerve. For most, the pain worsens over time. So, any repetitive motion, including constant use of your phone, tablet or logging long hours playing video games, can cause a flare up of symptoms if you don’t take breaks. If the pain becomes unbearable, you do have choices.

Carpal tunnel syndrome is an issue for more than three million Americans. And it’s a big issue. The US Bureau of Labor Statistics reported in 2014 that the condition was tied for first among nonfatal illnesses and injuries that caused people to be out from work. The median amount of time off was 32 days.

Generally, older people suffer from carpal tunnel, but anyone can be diagnosed.

Pregnant women are very susceptible because of hormonal changes that cause swelling that increases pressure on the tendons. Rest is the first step in helping to relieve this pain.

A recent study using rats found that rest was just as effective as an experimental drug at reducing discomfort and regaining function after injuries caused by repeated moderate-strain activity, such as carpal tunnel and other common musculoskeletal disorders (tendinitis, tendinosis and low back pain).

Other options

Early diagnosis is integral to effectively treating carpal tunnel. Additional nonmedical approaches include: ■Wearing a splint: This can help relieve symptoms, but is not a long-term solution.

You don’t want to wear the splint forever.

If the pressure persists, it can lead to nerve damage and symptoms may worsen.

■Minimizing activity: This too is not a long-term solution, especially for those who live active lifestyles or are locked into the activity that causes symptoms to flare.

■Cortisone shots: These periodic injections can relieve the pain, but will not fix the overlying issue of the compressed nerve.

Avoid waiting too long if pain or other symptoms persist. If so, surgery may be the best option to elude nerve damage.

Release surgery

When non-medical means fail, carpal tunnel release surgery is the premier procedure to remedy the condition.

The 10-minute outpatient procedure is done under light anesthesia and can be done open or endoscopically. Either way, the surgery detaches the transverse carpal ligament that presses on the median nerve.

I prefer the limited open approach, which makes a smaller incision (two centimeters) than the traditional carpal tunnel release, allowing for expedited recovery.

Relief can happen for people who undergo release surgery immediately or within a few weeks. They have two weeks of light activity with their fingers completely free. Most don’t need physical therapy unless they are struggling with stiffness.

Breast cancer awareness is important all year long

Foods that promote healthy hair and skin

O

ctober marks the beginning of National Breast Cancer Awareness Month, an annual pink ribbon campaign that aims to increase knowledge and awareness of the disease.

Breast cancer is a formidable foe. According to the World Health Organization, an estimated 627,000 women lost their lives to breast cancer in 2018. But women are not helpless in the fight against this invasive disease, as the WHO notes early detection is critical and could potentially save thousands of lives each year.

Breast cancer signs and symptoms

The first sign of breast cancer often is a breast lump or an abnormal mammogram.

Breast cancer stages range from early, curable breast cancer to metastatic breast cancer, with a variety of breast cancer treatments.

A proactive approach is a key component of protecting oneself against breast cancer. While many breast cancer symptoms are invisible and not noticeable without a professional cancer screening, women can keep an eye out for certain signs of breast cancer they might be able to detect on their own.

Monthly self-exams can help women more easily identify changes in their breasts. During such self-exams, women can look for the following signs and symptoms and are advised to report any abnormalities they discover to their physicians immediately.

Changes in how the breast or nipple feels: The National Breast Cancer Foundation Inc. says nipple tenderness or a lump or thickening in or near the breast or underarm could indicate the presence of breast cancer. Some women may notice changes in the skin texture or an enlargement of the pores in the skin of their breast. In many instances, skin texture has been described as being similar to the texture of an orange peel. Lumps in the breast also may indicate breast cancer, though not all lumps are cancerous.

Change in appearance of the breast or nipple: Unexplained changes in the size or shape of the breast; dimpling anywhere on the breast; unexplained swelling or shrinking of the breast, particularly when the shrinking or swelling is exclusive to one side only; and a nipple that is turned slightly inward or inverted are some signs and symptoms of breast cancer that can affect the appearance of the breast or

nipple. It is common for women’s breasts to be asymmetrical, but sudden asymmetry should be brought to the attention of a physician.

Discharge from the nipple: The NBCF notes that any discharge from the nipple, but particularly a clear or bloody discharge, could be a sign of breast cancer. The NBCF also advises women that a milky discharge when they are not breastfeeding is not linked to breast cancer but should be discussed with a physician.

Learning to recognize the signs and symptoms of breast cancer can increase the likelihood of early diagnosis, which greatly improves women’s chances of surviving this disease.

Tips for a comfortable mammogram

Mammograms remain one of the best methods to detecting breast cancers, giving women the opportunity to start treatment early if cancer is detected. In countries with early access to quality screening and treatment, breast cancer survival rates are now greater than 80 percent.

Did you know?

Though women may notice various signs and symptoms that could be indicative of breast cancer, breast pain is generally not one of them. The NBCF notes that there are various harmless causes of breast pain, such as puberty, menstruation and child birth, but that breast pain is not commonly a symptom of cancer. However, in rare instances breast pain may correlate with cancer.

For example, breast tumors may cause pain, but cancerous tumors are not generally reported as painful. Though it may not be linked to breast cancer, breast pain should still be reported to a physician immediately, advises the NBCF.

The organization Mammography Saves Lives says that, since 1990, mammography has helped reduce breast cancer mortality in the United States by 40 percent. Mammograms usually take around 20 minutes. During a traditional mammogram, a woman’s breast is placed between two plates. One plate holds the breast in place, while the other takes images, and the breasts must be compressed to get clear pictures of breast tissue. Some women find the process to be uncomfortable.

Even though mammograms can be essential parts of preventive healthcare, many women avoid them because of pain and other discomfort. However, women should not put off mammograms because they are worried about discomfort. There are many ways to avoid pain during mammograms that can make the entire experience more comfortable.

• Schedule the mammogram for a week after a menstrual period when hormonal swings are less likely to increase breast sensitivity.

• Caffeine can make the breasts more tender.

Reducing caffeine consumption for two weeks before the mammogram can help.

• Keep your feet and trunk facing forward and simply turn your head at the mammogram machine.

• Reduce tension by breathing deeply a few times before the procedure.

• Try a pain reliever before the mammogram.

• Ask the mammography center if it has padding, as cushioning between the breasts and the plates of the mammogram machine can reduce pain.

By taking these steps, women may be more comfortable during mammograms, which can play a vital role in the detection and ultimate treatment of breast cancer.

Millions of people across the globe deal with conditions that affect the skin and hair.

Acne is a skin and hair ailment that, according to the American Academy of Dermatology, is the most common skin condition in the United States.

But what if the foods people eat could affect the health of their hair and skin?

There is evidence that they can. Foods that contain certain nutrients, phytochemicals and enzymes may help people develop healthier hair and skin.

Blueberries: The U.S. Department of Agriculture gave blueberries its top ranking for antioxidant activity. The antioxidants in blueberries neutralize free radicals and protect people from premature aging, which can help the skin look younger and more vibrant.

Eggs: Eggs are great sources of protein and biotin, which are two nutrients that may help promote hair growth. Biotin is essential for the production of a hair protein called keratin.

Fatty fish: Fatty fish, such as salmon, are high in omega-3 fatty acids. Ciara Foy, a Toronto-based holistic nutritionist, says about 3 percent of the hair shaft is made up of omega-3 fatty acids. They’re also found in cell membranes and in the natural oils that keep the scalp and other areas of the skin hydrated.

Red bell peppers: These peppers contain more vitamin C than oranges. According to dermatologist Dr. Julia Carroll with Compass Dermatology, vitamin C is an antioxidant precursor to collagen production, so consuming more vitamin C through foods can neutralize free radicals that could damage skin.

Spinach: Trade lettuce for spinach, which is rich in calcium, iron, magnesium, omega-3 fatty acids, and much more. These nutrients can be the building blocks of healthy skin and hair.

Oysters: Oysters are good sources of zinc, which aids in skin cell renewal and repair.

Coconut: Coconut water is great for hydration and contains potassium, an electrolyte that moves nutrients into the cells.

Coconut oil also can be used to boost hair health as a pre-wash conditioning treatment.

The secret to beautiful skin and hair may be hiding in plain sight at your local supermarket.

Speaking up to defeat depression

D

A survivor shares her story

epression has impacted Kristen Davis’ life since she was a teen.

It has caused her unbearable emotional pain, filled her mind with suicidal thoughts, and blocked her path to personal and professional fulfillment.

Now at age 31, she sees herself as a survivor — overcoming her darkest days to finally achieve the mental stability that has enabled her to live a healthier and productive life. “It takes great strength to conquer mental illness, just like any other debilitating disease,” says Davis.

“People who can learn to manage their illness and rebuild their lives truly are survivors.”

Davis first experienced depressive thoughts in high school. Her struggle with depression worsened during her college years, marked by depressive episodes that included prolonged sadness, anxiety and a feeling of worthlessness.

She reached her lowest point a few years later when entering graduate school.

“When I started graduate school in 2014, the stress of being back in school and other new responsibilities took a toll on me,” says Davis. “I had cycled off an antidepressant I was taking and was vulnerable. My depressive episodes became most intense and frequent. I began to have anxiety attacks and daily suicidal ideation, which led to an incident of self-harm.

I could hardly sleep and was crying all the time.

I felt like I was losing my mind.”

As her depression became insurmountable, Davis entered the psychiatric ward of a Washington, D.C.-area hospital seeking help.

Ultimately, her battle with mental illness caused her to leave graduate school after only a few months.

During the next year and a half, Davis and her family desperately sought a way out of the abyss of mental illness that was crippling her life. She saw over 20 different mental health professionals, tried 15 different medications, and cycled in and out of seven different psychiatric facilities. Nothing succeeded in making her feel better.

What became clear to Davis and her doctors was that her depression was treatment-resistant.

Standard medications would offer her little or no relief. The challenge of treatment resistance is a significant hurdle for doctors in treating patients with mental illness. One in three people with depression will become treatment resistant, according to a study published in the American Journal of Managed Care.

Seemingly out of answers but not out of hope, Davis checked into a clinic in North Carolina.

There, her doctor wanted to gain insight on her individual genetic markers to uncover the biochemical factors impacting her depression.

Using the results of a simple cheek-swab genetic test, Davis’ doctor determined that she had a gene that affected her brain’s ability to use serotonin — often referred to as the “happy” chemical for its role in a person’s happiness and well-being. This genetic result suggested that Davis was more susceptible to stress. Moreover, other genetic markers indicated that her body could not metabolize certain antidepressants effectively.

“My doctor felt that the baseline in my brain without medication may actually be a depressed state,” says Davis. “He prescribed medication that was more likely to work, given my genetic variants, and I felt better within a matter of weeks. My suicidal ideations faded, and I could interact with people again. Of course, all my problems were not immediately solved, but I had the mental stability I needed to rebuild my life. I believe my doctor’s choice to use this genetic test saved my life.”

Understanding how her genes have played a critical role in her battle with depression empowered Davis. “This test helped me gain the proof that having depression was not my fault,” says Davis. “Knowing more about my genes also gives me important knowledge that I can use to make decisions to stay brain healthy for the rest of my life.”

Today, Davis uses her own experiences to advocate on behalf of the millions of other sufferers of mental illness in the U.S. “The primary lesson I learned as I have opened up about my depression is that no one is immune to the effects of mental illness, and almost everyone is searching for answers,” says Davis. “I couldn’t be prouder to use my experience to help others.”

Making sense of doggy dental care

Your vet can evaluate your dog’s mouth, teeth and gums and identify any trouble spots.

Brushing is best. Ideally, that means brushing your dog’s teeth daily, but that’s not realistic for all dog owners. If you’re brushing at home, start slow and make sure it’s a positive experience. First, introduce dog-specific toothpaste with your finger and progress to a toothbrush once your pet becomes comfortable. Your vet can also brush your dog’s teeth if you’re having trouble.

Use crunchy kibbles. Dry, crunchy foods can be helpful. As your dog chews, particles from the dry food scrape against his or her teeth, acting like a toothbrush to help reduce tartar buildup.

Watch for warning signs. Persistent bad breath, red gums and changes in behavior or eating habits can all be signs of dental problems. If you notice any of these signs, make an appointment to see your veterinarian.

A good long chew can help scrape away plaque and dirt, and most dogs are happy to comply. Natural choices include rawhide or a knucklebone. Knucklebones are a softer bone that’s gentler on the teeth. Never give harder items, such as hooves or bones from steak, rib or ham shank as these can fracture teeth.

ou put a lot of thought into keeping your dog healthy: quality food, routine checkups, plenty of fitness. But what about his teeth?

Dental care for dogs is important, but it’s an aspect of care that is frequently overlooked by dog owners. If he has a toothache or sore gums, he’s dealing with pain and stress that you may not even know about. Left untreated, bacteria introduced by the problem can enter the bloodstream and affect his heart, kidneys or liver.

Veterinarians report that an estimated 85 percent of dogs over age 4 are suffering from some form of periodontal disease, a painful oral condition that can lead to tooth loss and infection. The good news? All of these problems are preventable with regular dental cleanings and professional checkups.

Knowing there’s a problem with your pet’s mouth starts by understanding what a healthy dog’s mouth should look like. Teeth should be intact (not jagged or broken) and free of plaque and tartar buildup. Tongues should be moist without any lumps or cuts and gums should be salmon pink, though some dogs naturally have black or black and pink gums.

Making sure you are aware of what your pet’s mouth typically looks like and talking to your vet if you spot any issues can go a long way toward protecting your furry friend.

nyone who has experienced pain understands that it can be debilitating. Pain can make it challenging to manage everyday tasks and can affect relationships and physical and mental health in many ways.

Pain medications were once the norm, but with the rise of the opioid epidemic, it may be wise to take a different approach to pain treatment.

While chiropractic manipulations are most often associated with back aches, stiff necks, slipped discs and whiplash injuries, there are many different benefits — some of which may surprise those unfamiliar with chiropractic care — for this type of care.

Here are four examples: Migraines and headaches: Research shows that spinal manipulation can be an effective way to treat migraines, tension headaches and headaches that begin in the neck. In clinical trials conducted at Macquarie University, 72 percent of migraine sufferers had noticeable or

substantial improvement after a period of chiropractic treatment.

Accident injuries: Chiropractors can assess musculoskeletal anomalies caused by motor vehicle accidents as well as slips and falls. Whiplash is a common issue in

car crashes, and this may be addressed by a chiropractor.

Anxiety and stress: The fight or flight mechanism that is activated when a person experiences stress and anxiety can take its toll on various parts

of the body. Because stress involves nerve impulses, chiropractors may be able to work primarily with the spine where these impulses originate to help release tension and nerve irritation. In addition, adjustments can improve blood circulation, which may help signal the brain to turn off its “rally to action” response to stressful situations.

Fibromyalgia: Fibromyalgia is marked by widespread musculoskeletal pain in the joints, muscles, tendons, and other soft tissues. It can be combined with temporomandibular joint syndrome, or TMJ, as well as irritable bowel syndrome, among other illnesses. Chiropractors can remove subluxations, which are misalignments of the vertebrae in the spine and joints that lead to pain responses. A Florida State University study found that those with fibromyalgia who combined resistance training with chiropractic treatment two times a week saw improvements in their symptoms.

Chiropractic care may be an asset for pain management, as well as a way to improve overall health.

Tricks for Treats:

Braces don’t mean the end of Halloween fun

or kids and candy lovers everywhere, Halloween is heaven. Seemingly endless supplies of chocolate, gummies, lollipops and caramel apples are around every corner, and all you have to do is ask. Pure, sugary bliss.

Of course, too much of anything is never a good idea, and the same is true for sweets — a steady diet of sugar can harm both your dental health and your waistline. Perhaps appropriately, October is National Orthodontic Health Month, a time set aside to raise awareness and promote education about orthodontics.

According to the American Dental Association (ADA), orthodontics is the dental specialty concerned with correcting issues of malocclusion, also known as a bad bite. These include crooked, crowded or misaligned teeth as well as jaw alignment problems. While braces and other forms of orthodontic care are usually associated with kids and teens, it’s become more common for older patients to see an orthodontist, too. In fact, more than a quarter of all orthodontic patients in the United States are adults.

Correcting a patient’s smile is about more than just improving overall appearance. The ADA also notes that orthodontic treatment can also help prevent or treat tooth decay, gum disease, problems with speaking or chewing and jaw problems, to name a few.

For the 4 million Americans that are currently wearing braces, it’s especially important to watch what you’re chewing on. But that doesn’t mean Halloween can’t still be sweet. It’s all about making the right choices, said Dr. David Amram of Coolsmiles Orthodontics (www.coolsmiles. com) in Port Jefferson and Medford.

Amram, who’s worked as an orthodontist for more than 20 years, has treated thousands of patients of all ages. And he’s seen countless patients for emergency appointments because of bent wires or broken brackets. These problems happen throughout the year, but there’s a predictable uptick around the holidays, he said.

“Immediately after Halloween we see many emergencies, when people have to come in without an appointment,” Amram said. “Hard candies like lollipops or Jolly Ranchers can knock braces off because they are so hard. Sticky candies like Laffy Taffy or Twizzlers can pull on the braces until they pop off, or pull or push the wire in ways that cause poking. This also happens with gum and chewy caramel candy bars like Snickers and Twix.”

Not only will these issues be awkward and uncomfortable for patients, but they could also delay the overall progress of orthodontic care.

And really, who wants to wear braces longer than they have to?

Three reasons why organic is the way to go

Amram said that he’ll always be honest with patients about the consequences of eating risky foods with braces, but that he’ll never be upset if they do slip up.

“Honestly, it is an important way to build a relationship of trust with patients and, ultimately, gain compliance with them,” he explained. “I tell my patients I’m happy to see them, even if they need to come in for an extra appointment. Then I’ll sit down with them and ask if they remember the do’s and don’ts of keeping their braces intact. If they don’t, we’ll go over the list again together.”

When it comes to younger patients, peer pressure can often play a role with how well they comply with the requirements of treatment, Amram said. If a child’s friends are all chewing gum together and he or she is offered a piece, it can be tough to say no. But those same friends might have their braces for months longer because of the need for additional care.

So with so many candies on the list to avoid, what’s left? Rejoice, chocoholics and peanut butter lovers — most chocolate candy bars and peanut butter cups are soft enough to enjoy without worry, Amram said. If you’re looking for some crunch with your sweet, try regular M&Ms, Reese’s Pieces or Kit Kats.

Conveniently, fun-size versions of candy bars are usually thinner and typically lack the bulk to do serious damage to braces.

Amram said he likes to give out pretzels or chips as a fun alternative to candy on Halloween. “I buy the little bags of pretzels to

give out. I like them because they are made in a facility that doesn’t use tree nuts, and they’re a kind option for kids in orthodontic treatment. We also have the assorted chocolate mini’s because not everyone can eat pretzels.

I would advise having a variety of options on hand, and always asking kids if they have any food allergies.”

Ultimately, a good phrase to remember is, “When in doubt, trade it out.” It’s easy to swap a chewy candy with a friend who has something you like.

Lastly, don’t forget to brush. “Sweets and treats can cause cavities. Brushing and flossing are more important than ever during this sugar-filled season,” writes Bright Now! Dental, a group of more than 150 dental offices located throughout the country. “Orthodontic patients should be especially mindful to brush and floss after consuming sugary and starchy foods.”

And if there’s anything leftover when October draws to a close, consider a local candy buy back program offered at your dentist or orthodontist. The national Halloween Candy Buy Back initiative began in 2005 when Dr. Chris Kammer, a Wisconsin dentist, wanted to limit overconsumption of sugary treats.

Modern grocery stores are unlike the ones many of today’s adults encountered when they were children. Grocery stores are not only bigger today than they were years ago, but they’re also stocked with items that weren’t available until relatively recently.

Organic fruits and vegetables now take up ample real estate in grocery store produce sections. Many shoppers may wonder if they’re better off choosing organic versus traditional products, and research suggests they are.

1. Organic foods contain less pesticides and toxic materials. A 2014 meta-analysis published in the British Journal of Nutrition found that organically grown crops were less likely than conventionally grown crops to contain detectable levels of pesticides.

2. Organic crops contain more antioxidants than conventional crops.

The same analysis also found that organic crops had significantly higher levels of antioxidants, which promote strong immune systems, than conventionally grown crops. Organic crops were found to contain 69 percent higher levels of flavanones and 51 percent higher levels of anthocyanins than their conventional counterparts. The extra antioxidants in organic foods is nothing to scoff at.

3. Organic products cannot contain synthetic hormones. Conventionally raised animals are sometimes injected with synthetic growth hormones so they will gain weight more quickly and produce more milk. Residue from those substances, which cannot be used in the production of organic meat and dairy products, is believed to contribute to widespread antibiotic resistance, according to the Biodesign Center for Environmental Security at Arizona State University. In addition, some studies have suggested a strong connection between the hormones given to cattle and cancer in humans.

When choosing organic products at the grocery store, consumers may be benefitting their overall health in various ways.

Helping children adopt healthy behaviors

C

hildhood obesity is a global health issue. In fact, childhood obesity is so prevalent that the World Health Organization identifies it as one of the most serious public health challenges of the 21st century. As daunting a task as tackling childhood obesity may seem, it’s a problem that can be solved.

The WHO notes that, in 2016, an estimated 41 million children across the globe were overweight. That’s a troubling statistic, as overweight and obese children are likely to remain so into adulthood, increasing their risk for various diseases, including diabetes and cardiovascular disease.

Parents of children who are overweight or obese may find it difficult to get youngsters to change their lifestyles and adopt healthy behaviors that can help them lose weight and keep the weight off.

Like adults, many children are creatures of habit, particularly when it comes to diet.

And getting kids to change those habits is not always so easy. However, a few simple strategies might be all it takes to help kids lose weight and get on the path to a healthier lifestyle.

Limit sugar intake.

The WHO recommends reducing kids’ sugar intake. Sodas and juices, even fruit juices, are often loaded with sugar. Many children drink these beverages every day, with some even consuming multiple servings per day.

Water is a healthy alternative to soda and juice, and parents can slowly transition their youngsters away from sugary beverages by diluting the beverages with water and only allowing kids to have them on special occasions.

Cook pasta according to package directions.

Drain and rinse with cool water. In large bowl, mix pasta, peppers, oranges, green onions, spinach, carrots and dressing. Toss to coat well.

Top with chow mein noodles. Place in lunchbox with kiwi and edamame.

Switch things up at snack time.

Focus on more than the dinner table.

Many children eat daily snacks, and that’s perfectly normal. Youngsters are growing and, as a result, tend to get hungry between meals. What parents serve at snack time can have positive or negative effects on youngsters’ weights.

The American Academy of Pediatrics recommends parents use snack time as a way to increase kids’ intake of fruits and vegetables. Fruits and vegetables are nutrient-dense, and the WHO says increasing fruit and vegetable intake is a great way to fight childhood obesity.

Getting overweight and obese kids to eat healthy diets is just one aspect of helping them lose weight. Routine physical activity is another essential part of a successful weight loss plan. The WHO recommends that children get at least 60 minutes of moderate- to vigorousintensity physical activity every day.

Parents can promote a physically active lifestyle and limit the time kids spend on devices or playing video games by only allowing access to such distractions after kids have been physically active.

Assemble taco salad with lettuce, taco meat, cheese and tomatoes. Place in lunchbox with guacamole and chips.
