What You Should Know About Dementia and Alzheimer’s Disease Dementia has been called the greatest global challenge for health and social care in the 21st century. Globally, more than 47 million people are living with dementia at a worldwide cost of 818 billion dollars.

Are Alzheimer’s disease and dementia the same thing?

Dr. Palekar: Dementia isn’t a specific disease. It’s a term that describes a wide range of symptoms associated with a decline in memory or other thinking skills severe enough to reduce a person’s ability to perform everyday activities.

Alzheimer’s disease is the most common form of dementia, accounting for 60 to 80 percent of cases. It’s an irreversible, progressive brain disease that slowly destroys memory and thinking skills, and eventually even the ability to carry out the simplest tasks. Alzheimer’s affects over five million people in the United States, nearly 400,000 in New York, and over 50,000 here on Long Island. For each person with Alzheimer’s disease in the United States, there are approximately three family members or other unpaid caregivers, almost 16 million caretakers in all.

What causes Alzheimer’s?

Dr. Palekar: While we still don’t know how the Alzheimer’s disease process begins, it appears that damage to the brain starts a decade or more before problems show up. During this period, while people may be free of symptoms, toxic changes are taking place in the brain. Abnormal deposits of proteins form amyloid plaques and tau tangles throughout the brain, causing once-

healthy neurons to begin working less efficiently. Over time, these neurons lose their ability to function and communicate with each other, and eventually they die.

As Alzheimer’s progresses, the damage spreads to a nearby structure in the brain called the hippocampus, which is essential in forming memories. As more neurons die, affected brain regions begin to shrink. By the final stage of Alzheimer’s, damage is widespread, and brain tissue has shrunk significantly.

Isn’t is a natural consequence of aging?

Dr. Christodoulou: No. While infrequent, mild memory loss may be a part of aging, Alzheimer’s is not. That said, the greatest known risk factor is increasing age, and the majority of people with Alzheimer’s are 65 and older.

However, up to five percent of people with the disease have early onset, which often appears in the 40s or 50s.

What are the most common signs?

Dr. Christodoulou: A typical early sign of Alzheimer’s is a loss of memory for new information that begins to disrupt daily life. However, different people may show different initial symptoms such as: new problems with words in speaking and writing, confusion with time or space, or withdrawing from work or social activities.

Is there a cure?

Dr. Palekar: Alzheimer’s has no cure, but early detection and treatment can help.

Medications may slow the progression of Alzheimer’s symptoms, providing patients and families with the opportunity to plan for the future. There are also programs that can support caregivers to help reduce their burden and stress.

Any advice on prevention?

Dr. Christodoulou: Strategies for overall healthy aging may help keep the brain healthy and reduce the risk of Alzheimer’s. These measures include eating a healthy diet, staying socially active, reducing stress, avoiding tobacco and excess alcohol, treating hearing loss, and exercising both the body and mind.

Aerobic exercise like walking appears particularly important for prevention.

The cost of smartphone addiction

Are our smartphones destroying the fabric of the American family? Think about that question. How many young families with children are allowing their elementary school children to have smartphones with little or no restrictions?

In April, I was at a celebration with

Grandparents and an aunt were also there to celebrate the eighth-grade daughter’s confirmation. By traditional definition, they are a strong, intact family.

After grace was said, I was amazed at what followed. These children are bright and articulate. They are not inhibited to share their opinions. After a few minutes of banter, which I initiated, they immediately became obsessed with their smartphones. The only communication for the remainder of the meal was shared among the adults present.

As I flew back home after the celebratory dinner, I could not help but be distracted by my cellphone observation. I decided I would be more attentive of young people and how and when they use their smartphones. I must admit I was taken back by my observation.

On Tuesdays, I take an early morning train to New York City. I teach in the graduate school of social work at Fordham University.

I was deafened by the silence. Based on observation, most of the passengers were on their smartphones, their tablets and/or their laptops.

It was the rare row of seats in this crowded train where people were actually engaged in conversation!

So I decided to look further into the smartphone issue. I already knew from my experience that smartphones were becoming a problem in college classes, so much so that I had to develop a specific policy on the use of cellphones in my classes.

However, I wanted to know more. When did you get your first smartphone? How many hours a day do you spend on it? Are you permitted to have a cellphone at family dinner?

Would you rather text than speak to someone directly or leave a voicemail?

I was definitely concerned by their responses. I sought input from my classes in a fouryear school and a community college. All who responded were students in the classes I taught at these respective schools. For the most part, their answers were the same.

Most students said they received their first smartphone by late elementary school, early junior high school — that is fifth or sixth grade.

As small children, their use of their smartphone was limited by bedtime. However, by high school most students admitted they were using their smartphones from 10 to 15 hours a day, and in some cases, even more!

Most admitted that they would rather text or leave a voice message instead of talking.

The only split was with those who had family dinners where cellphones were prohibited; however, those not having a family meal said it was not an issue. They were equally divided on how many had a family dinner and how many had not had a family dinner since early elementary school.

These observations could not be seen objectively as conclusive since the survey is very limited in number. However, it does offer us a lot of food for thought. It helps to explain for me as a teacher why college students’ writing skills have deteriorated over the years and their critical thinking skills are almost nonexistent. The human connection seems to be lost.

The next generation seems more grounded in one-dimensional nonhuman connections rather than face-to-face human interaction.

For the sake of our future, we need to go back to simpler times where people were more important than social media posts and human touch with respect and dignity more valuable than a social platform. We must reclaim the fabric of our American family life before it is too late!

P.S. I still write hand-written notes and letters!

The time traveler that saves millions of lives

ne could easily be forgiven for not knowing certain things.

While strolling along the moonlit shores of Riverhead’s Peconic Estuary or, closer to my home, at tiny Cordwood Park, on the back side of Stony Brook Harbor, you might come upon a prehistoric carousel of love. Yet watching the late spring mating circles of horseshoe crabs — at once peculiar and comical — an observer might never know how very significant these odd creatures are. One might not know, as I did not for many a year, that they have been on this Earth for so long that they survived five mass extinctions, an impressive feat for any earthling.

One might also be wholly unaware that people in surgery, those who receive stents or joint replacements, or the large numbers of us who get flu vaccines, take insulin or receive intravenously delivered chemotherapies or antibiotics are safer, free of dangerous endotoxins, thanks to the coppery blue blood of horseshoe crabs.

Really, who knew that one of our saving angels has not feathery wings but leathery hard carapaces, seven pairs of legs and a pointy tail with eyes on its underside. Tooling around the seashores, ocean shallows and estuaries for nearly

450 million years, and unchanged for over 300 million, they have been largely cancer-free and carefree — until recently.

Growing up on Long Island, one saw larger groupings of horseshoe crabs seemingly everywhere. But then their harvesting as bait had dropped measurably in the 1950s and ’60s, and their use as fertilizer had stopped decades before that. And while their local harvest has gone down significantly from the late 1990s, their numbers on Long Island and the waterways of the greater New York region show a continuing decline, according to both the Department of Environmental Conservation and the Atlantic States Marine Fisheries Commission.

However, in other parts of the East Coast, specifically the rich Delaware Bay region, the overall stock remains stable, while in the Southeast (North Carolina through Florida), indications are the numbers of horseshoe crabs have actually increased.

So, what has happened in our neck of the woods, and what can we do to ensure steady populations of these ancient arthropods whose abundant eggs are a great, life-saving food source for migrating birds, and whose special blood, once extracted, saves us? How, in short, do we return the favor?

The reasons for regional differences in stock abundance are many and depend as much on natural cycles as harvesting by fisherman and drug manufacturers (the majority of horseshoe crabs, once their blood has been extracted to produce limulus amoebocyte lysate, or LAL, are kept alive and returned to the waters).

One reason for our local decline is that other states — Maine, New Hampshire, New Jersey and Pennsylvania — have not harvested any horseshoe crabs since 2007. Yet there have been very few harvest moratoriums here in New York, and they are small and temporary.

Horseshoe crabs are preferred by our local fish-

ing fleets as bait for whelk, eel and conch. Apparently, neighboring moratoriums have made our crusty old co-inhabitants more valuable as a bait source here.

What can be done to keep their numbers steady? Increasingly, concerned citizens are encouraging the use of nylon and other mesh bait bags, which require only a tenth of the regular portion of horseshoe crab bait. It’s efficient, and it needs only further promotion. Others are looking to test alternative bait sources.

Scientists at the University of Delaware have developed such an alternative, and some individuals and groups, like Cornell Cooperative Extension in Riverhead, want to do a twoyear test in our local waters. Some are considering breeding season moratoriums during the spring, while allowing the horseshoe crabs to be harvested come summer and fall, in prime fishing season. Others are calling for a full, multiyear halt on bait harvesting. Reporting pilferage of large numbers of horseshoe crabs — sometimes flatbed or small pickup truckfulls— to the NY DEC can be helpful, as they will give out stiff fines to those who are caught.

Then there is this: Spreading the word in articles, classrooms, at eco-fairs, among fishing clubs and at town hall meetings in shore towns that these very old animals are very valuable; to us, in certain medicines and medical procedures. To the migrating wildlife and fish who feast on their larvae. To our local fishermen, a vital industry on Long Island for over 150 years. And, of course, for the horseshoe crabs themselves; their eons-long survival a testimony to adaptation, endurance and whatever spirit resides in such strange and remarkable beings.

Preventing and treating Lyme disease

Ah, summer is upon us. Unfortunately, this means that tick season is getting into full swing.

Thus, it is good timing to talk about Borrelia burgdorferi, better known as the bacterium that causes Lyme disease. This bacterium is from the spirochete class and is typically found in the deer tick, also known as the blacklegged tick.

What do deer ticks look like? They are small and can be as tiny as a pencil tip or the size of a period at the end of a sentence. The CDC.gov site is a great resource for tick images and other information related to Lyme disease.

If you have been bitten by a tick, the first thing you should do is remove it with forceps,

Do not crush or squeeze the tick, for doing so may spread infectious disease (1). In a study, petroleum jelly, fingernail polish, a hot kitchen match and 70 percent isopropyl alcohol all failed to properly remove a tick. The National Institutes of Health recommend not removing a tick with oil (2).

When a tick is removed within 36 to 48 hours, the risk of infection is quite low (3). However, a patient can be given a prophylactic dose of the antibiotic doxycycline, one dose of 200 mg, if the erythema migrans, or bulls-eye rash — a red outer ring and red spot in the center — has not occurred, and it is within 72 hours of tick removal (4). Those who took doxycycline had significantly lower risk of developing the bulls-eye rash and thus Lyme disease; however, treatment with doxycycline did have higher incidence of nausea and vomiting than placebo.

What are the signs and symptoms of Lyme disease? There are three stages of Lyme disease: early stage, where the bacteria are localized; early disseminated disease, where the bacteria have spread throughout the body; and late stage disseminated disease. Symptoms for early localized stage and early disseminated disease include the bulls-eye rash, which occurs in about 80 percent of patients, with or without systemic symptoms of fatigue (54 percent), muscle pain and joint pain (44 percent), headache (42 percent), neck stiffness (35 percent), swollen glands (23 percent) and fever (16 percent) (5).

Early disseminated disease may cause neurological symptoms such as meningitis, cranial neuropathy (Bell’s palsy) and motor or sensory radiculoneuropathy (nerve roots of spinal cord).

Late disseminated disease can cause Lyme arthritis (inflammation in the joints), heart problems, facial paralysis, impaired memory, numbness,

pain and decreased concentration (2).

How do we prevent Lyme? According to the Centers for Disease Control and Prevention, we should wear protective clothing, spray ourselves with insect repellent that includes at least 20 percent DEET and treat our yards (3). Always check your skin and hair for ticks after walking through a woody or tall grassy area. Many of us on Long Island have ticks in the yard, so remember to check your pets; even if treated, they can carry ticks into the house.

Diagnosis of Lyme disease

Many times Lyme disease can be diagnosed within the clinical setting. When it comes to serologic or blood tests, the CDC recommends an ELISA test followed by a confirmatory Western blot test (3). However, testing immediately after being bitten by a tick is not useful, since the test will tend to be negative, regardless of infection or not (4). It takes about one to two weeks for IgM antibodies to appear and two to six weeks for IgG antibodies (5).

These antibodies sometimes remain elevated even after successful treatment with antibiotics.

The cardiac impact

Lyme carditis is a rare complication affecting 1.1 percent of those with disseminated disease, but it can result in sudden cardiac death due to second- or third-degree atrioventricular (AV) node conduction (electrical) block. Among the 1.1 percent who had Lyme carditis, there were five sudden deaths (6). If there are symptoms of chest pain, palpitations, light-headedness, shortness of breath or fainting, then clinicians should suspect Lyme carditis.

Does chronic Lyme disease exist?

There has been a debate about whether there is something called “chronic Lyme” disease. The research, unfortunately, has not shown consistent results that indicate that it exists. In one analysis, the authors note that the definition of chronic Lyme disease is obfuscated and that extended durations of antibiotics do not prevent or alleviate post-Lyme syndromes, according to several prospective trials (7). The authors do admit that there

are prolonged neurologic symptoms in a subset population that may be debilitating even after the treatment of Lyme disease. These authors also suggest that there may be post-Lyme disease syndromes with joint pain, muscle pain, neck and back pain, fatigue and cognitive impairment.

Ultimately, it comes down to the IDSA (Infectious Diseases Society of America) arguing against chronic Lyme but in favor of post-Lyme disease syndromes, while the ILADS (International Lyme and Associated Diseases Society) believes chronic Lyme exists.

Regardless, the lingering effects of Lyme can be debilitating. This may be as a result of systemic inflammation (8). Systemic inflammation and its symptoms can be improved significantly with dietary and other lifestyle modifications.

But to throw one more wrench in the mix, the CDC recommends that physicians look beyond Lyme for other possible diagnoses before diagnosing someone with chronic Lyme disease (9).

Prevention is key to helping stem Lyme disease. If this is not possible, treating prophylactically when pulling off a tick is an important step.

Contact your physician as soon as you notice a tick. If you have a bulls-eye rash and it is early, then treatment for two to three weeks needs to be started right away. If it is prolonged and disseminated, then treatment should be for approximately three to four weeks with antibiotics. If it has affected the central nervous system, then IV antibiotics could be needed.

Like a kid in a candy store

As you reach for a sweet treat this June in honor of National Candy Month, consider how the abundance of candy today is a rather exceptional thing.

For much of human history, sugar was an expensive indulgence reserved for celebratory desserts. Sugary treats were a luxury for the rich. People also used sugar for therapeutic functions, with early candy serving as a form of medicine, including lozenges for coughs or digestive troubles.

Sugar was also used as a preservative; similar to salt, sugar dried fruits and vegetables, preventing spoilage. But all in all, sugar was carefully conserved. In George Washington’s time, the average American consumed only 6 pounds of sugar a year (far less than the 130 or so pounds consumed annually per person today).

The use of sugar swelled dramatically in the 1800s. Suddenly, sugar was everywhere, and with it came new technological advances in candy production. Sugar shipped from slave-powered plantations in the West Indies became more affordable and available with new, steam-powered industrial processes.

These changes were part of the Industrial Revolution, made possible by prized whale oil and its valuable lubricating properties. In 1830, Louisiana had the largest sugar refinery in the world. The invention of the Mason jar in 1858 drove demand for sugar for canning, and in 1876, the Hawaiian Reciprocity Treaty made sugar even more available. People couldn’t get enough of sweetness.

The availability of sugar brought a slew of new inventions to the culinary scene: candy!

Confectioneries sprang up everywhere. The shops’ best customers were children, who spent their earnings on penny candy. Hard candies became very popular.

As Yankee whaling reached its peak, Victorian-era sweets boomed with a succession of creations: the first chocolate bar was made in 1847; chewing gum followed in 1848; marshmallows were invented in 1850 and, in 1880, fudge. People’s breaths were taken away when sweets with soft cream centers were tasted at the Great Exhibition in London in 1851.

Some candies, especially hard ones, were sold as being “wholesome” and even healthy.

Unfortunately, candy was anything but nourishing. Sugar was sometimes adulterated with cheaper plaster of paris or chalk. Other candies were far more toxic.

In 1831, Dr. William O’Shaughnessy toured different confectionery shops in London and had a range of dyed candy chemically analyzed; he found a startling number of sweets colored with lead, mercury, arsenic and copper.

But as ubiquitous as candy was on land, a sweet treat was quite rare at sea, especially on a whaleship. Sugar on board was still a luxury reserved for the captain and officers. The crew had to settle for molasses, which was often infested; one whaler wrote it tasted like “tar.”

Candy only makes brief glimpses in whaling logbooks, or daily records.

On May 22, 1859, William Abbe journaled on the ship Atkins Adams: “Cook & Thompson Steward making molasses candy in galley.”

(Earlier on the voyage, he described molasses kegs as “the haunts of the cockroach.”)

Laura Jernegan, a young daughter who sailed with her father and family on a three-year whaling voyage, wrote in her diary on board the Roman, “Feb 16, 1871. It is quite pleasant today. The hens have laid 50 eggs ...” Then, an exciting thing happened – she passed another whaleship at sea, the Emily Morgan. There was a whaling wife aboard, too! Laura wrote: “Mrs. Dexter [the wife of Captain Benjamin Dexter] sent Prescott [her brother] and I some candy.”

In other cultures, whales still facilitated the treat scene – no sugar needed. Frozen whale blubber was (and is) a traditional treat for the Inuit and Chukchi people. Called muktuk, cubes are cut from whale skin and blubber and conventionally are served raw.

While whaling in our country is a thing of the past, the years of unrestricted whaling reflect how, in essence, people treated the ocean “like a kid in a candy store,” as noted by author Robert Sullivan. In the 20th century, so many whales were caught so quickly and efficiently that soon even whalers themselves were worried about saving the whales.

Today, as we continue to gather resources from the sea, we must ensure the ocean can replenish itself faster than we can sweep its candy off the shelves.

Who will make your medical decisions if you are unable to?

In New York State, any individual over the age of 18 may designate an individual to make medical decisions on his/her behalf by signing a health care proxy and designating a health care agent.

The health care agent is only authorized to act

By signing this document and designating an agent, you avoid any confusion or issues when it comes time for your family to make a medical decision on your behalf as your family and the doctors already know who you want to make those decisions.

A valid health care proxy will allow your health care agent to make medical decisions for you if you cannot with any health care professional, not only decisions while you are in a hospital or nursing home.

Additionally, when signing a health care proxy, it is also very important to sign a second document, called a living will, which states your preferences as they relate to life-sustaining treatment (medical treatments/procedures that, if not provided, will result in the patient’s death).

Examples of life-sustaining treatments include cardiac pulmonary resuscitation (CPR), a feeding tube and ventilator.

A living will is important because, although your health care agent can make most medical decisions on your behalf, a health care agent must know your wishes as they relate to life-sustaining treatment in order to make those specific decisions on your behalf. A correctly executed

living will is “proof positive” of your wishes as they relate to life-sustaining treatment and cannot be questioned by other family members who may disagree.

If you do not have a health care proxy and are admitted to a hospital or nursing home, the Family Health Care Decisions Act enacted by New York State will determine who can make medical decisions on your behalf. This act provides a hierarchical list of people who may make your medical decisions if your doctors determine that you lack the capacity to make your own medical decisions.

The list is: court-appointed guardian, spouse/ domestic partner, a child who is over 18 years old, a parent, a sibling or a close friend. The issue many people may encounter is that most people have more than one child who can act as the person who will make their health care decisions. In this situation, the doctors would have to specify one of the children to make the decisions, which can cause tension and disagreement among the children. Further, the Family Health Care Decisions Act is only applicable to decisions while a patient is in the hospital or a nursing home. Once a patient is discharged, the person designated to make the medical decisions no longer has authority to do so.

In order to be certain the person you want is empowered to make your medical decisions, a health care proxy is the preferable option. It is also wise to sign a living will so your health care agent knows your specific wishes as they relate to life-sustaining treatment. It is best to consult with an estate planning attorney who can advise you on all your options and ensure your documents are valid.

Suffolk Federal awards $30,000 in college scholarships for local students

Suffolk Federal presented a total of $30,000 in college scholarships ($5,000 each) to six local high school seniors who will be attending Suffolk County Community College on June 6.

“Supporting our younger generation to become productive community members by providing financial assistance as they undertake their college experience is an important focus for Suffolk Federal,” said Ralph D. Spencer Jr., president & CEO of Suffolk Federal.

“We are happy to know that the scholarships awarded will help to alleviate some of the student’s financial burden, so they can focus on their future educational and career goals.”

From the $30,000, four of the scholarship award recipients received $5,000 each as part of the Suffolk Federal Scholarship. In addition, one student, Amber Leon, received an additional $5,000 scholarship provided by the LT Michael P. Murphy Memorial Scholarship Foundation, which was

matched by Suffolk Federal for a total of $10,000.

Also, William Clifford received $5,000 as part of the Michael E. Reilly Foundation Memorial Scholarship for Excellence in Fire Science and Emergency Medical Technology, which is awarded to a student enrolled in a Fire Protection Technology program at Suffolk County Community College.

Recipients were judged based upon their academic achievements, extracurricular and community activities as well as the quality of written essays.

Steamed Soft-Shell Clams and Mussels in Garlic and White Wine YIELD: Makes 8 to 10 servings INGREDIENTS: • 8 dozen soft-shell clams, scrubbed • 4 dozen mussels, scrubbed • ½ cup extra virgin olive oil • 1 bay leaf • 6 bruised garlic cloves • 1 cup chopped flat-leaf parsley • 4 sprigs fresh thyme • 1 cup dry white wine • ½ pound unsalted butter, melted DIRECTIONS:

Rinse clams and mussels under cold running water; place in large pot with two cups of water, oil, herbs and wine; steam until shells open, 6 to 8 minutes. Discard any whose shells don’t open. With slotted spoon remove clams and mussels from broth and divide evenly among individual soup bowls; discard herbs. Strain broth through a sieve or cheesecloth and pour into individual bowls or cups.

Serve clams, mussels and broth immediately with melted butter for dipping.

Clam and Garden Vegetable Chowder

YIELD: Makes 10 to 12 servings INGREDIENTS: • 2 quarts freshly shucked clams in liquor • ½ pound diced bacon • 2 tablespoons extra virgin olive oil • 2 large onions, diced • 1 medium green bell pepper, diced • 1 medium zucchini, diced • 1 large celery rib, cleaned and diced • 1 fennel bulb, cleaned and diced • 4 carrots, peeled and diced • 5 medium potatoes, scrubbed and diced • One 28-ounce can diced Italian plum tomatoes with their juice • 1 cup chopped fresh flat-leaf parsley • 2 tablespoons fresh oregano • 2 tablespoons fresh thyme leaves • Sea salt and black pepper, to taste DIRECTIONS:

Drain and reserve liquor from clams; rinse clams in cold water, chop and set aside.

Strain liquor through fine sieve or cheesecloth; set aside.

In a large pot, cook bacon pieces until crisp; remove and drain on paper towels; add olive oil to bacon fat, then onions, pepper, zucchini, celery, fennel, carrots and potatoes; stir to coat thoroughly and cook, stirring frequently until fat and oil are absorbed and vegetables have started to change color and consistency, about 5 to 8 minutes.

Add tomatoes with their juice and enough water to cover veggies; cover and cook over low heat, stirring occasionally, until veggies are tender, about one hour. Add bacon, clams and their liquid, herbs and seasoning; cook until just heated through, about 3 minutes.

Serve immediately with oyster crackers.

We are so lucky. It’s summer and we live on Long Island and if there’s one thing we’re known for it's our miles and miles of beaches skirting the Atlantic Ocean, the Long Island Sound and several bays in between. To spend a day "by the sea, by the beautiful sea," to play in the sand and surf, and then to dine on its bounty is the essence of summer for us.

When we’re exhilarated and exhausted by the salt air and sunshine, nothing makes a more perfect ending to the day than tucking into a seafood dinner. Of course, it must start off with freshly shucked, ice cold clams and oysters, and if a good sea breeze is kicking up, a bowl of chunky chowder keeps everyone happy until the steamers and mussels have relinquished themselves to their garlic and wine bath.

But all these briny appetizers are just a tease leading to the main attraction: lobsters grilled to charred succulence accompanied by Long Island spuds, a bowlful of husk-wrapped corn, another bowl of tender young greens from the garden — all chased down, of course, with a Long Island chardonnay or sauvignon blanc.

Then finally, there is the moment when, content with the conviviality of the gathering, sated with the taste of the sea, we sit back, contemplate the sunset, sip a mug of good strong coffee and find room somehow, somewhere, for fresh Long Island strawberry cobbler crowned by dollops of vanilla ice cream.

It’s been seven years since Will Smith and Tommy Lee Jones teamed up as Agent J and Agent K for the last time in “Men in Black 3.”

This past Friday, Sony Pictures delivered the fourth and final installment of the series, “Men in Black: International,” to theaters nationwide.

Directed by F. Gary Gray (“The Fate of the Furious”) and written by Art Marcum and Matt Holloway, the sci-fi flick follows the London-based team of Men in Black, a secret police force that monitors and maintains order on the comings and goings of extraterrestrials.

While investigating the murder of the alien Vungus the Ugly (Kayvan Novak), top agent H/Henry (Chris Hemsworth) and probationary agent Molly Wright/Agent M (Tessa Thompson) travel around the globe (hence the title) to Paris, New York City, Naples, Marrakesh and the Sahara, to “protect the Earth from the scum of the universe.”

Along the way, they find (and constantly lose) the Tesseract, a magical, glowing, cube-

like object that has the power to control the universe. On top of that, they suspect a mole in the MIB organization, further threatening the fate of planet Earth.

The neuralyzer is back to erase our memory, the comedic lines and sarcasm too.

Every method of transportation has a warpspeed option, from a rocket-powered bike, bullet train and a Lexus, and the weapons of choice are bigger, fancier and more powerful than ever before.

Scarier, stranger and cuter aliens lurk in every corner, in particular the shape-shifting extraterrestrial assassins Les Twins (Laurent and Larry Bourgeois) who seek to possess the Tesseract, and Pawny (Kumail Nanjiani), a pocket-sized, scene-stealing green alien.

Hemsworth and Thompson (of “Thor: Ragnarok” fame) have great chemistry together but any notion of romance between the two is mute. The film also stars Liam Neeson as High T, the head of MIB UK branch; Rafe Spall as Agent C, Agent H’s nemesis; Emma Thompson as Agent O, the head of MIB; and Rebecca Ferguson as Riza Stavros, an alien intergalactic arms dealer who has three arms.

The movie has been receiving more negative reviews than not, even going as far as to say “it’s a case of the blahs” and “you’ll forget it in a flash,” but fans of the original

A graduate of the Massachusetts College of Art and Design, she has shown her jewelry in the Portland Museum in Oregon, the Holter Museum of Art in Montana as well as galleries such as the Young and Constanin Gallery in Vermont, Stones Throw Gallery in Massachusetts and the Carlyn Gallery in Texas. I recently visited Randall at her studio in East Setauket where this metalsmith of over 20 years hand makes every piece of original jewelry.

How did you get started in jewelry making?

I initially enrolled in art school as a fashion design major. On a lark, I took a jewelry class at MassArt and fell in love! I have been making jewelry ever since.

What is your inspiration for the creative process?

The impulse to collect is at the heart of my creative process. I collect all kinds of natural debris like: found animal bones, skulls, beach stones, pine cones, crab claws and shells; found turtle shells, semiprecious stones, sea shells and beads. These found objects are then catalysts for designs. I will either use a material directly or use just a shape, line or texture from something I’ve found in nature.

What else influences your art?

As a little girl, I loved to visit the American Museum of Natural History and the Metropolitan Museum of Art with my family. I was fascinated with medieval weaponry, taxidermy, ancient Egyptian art and Pacific adornment. As a young woman, I had the opportunity to travel through Europe and North Africa, experiencing the art from these cultures firsthand. Later in life, I lived in Texas with my husband and three sons, which instilled a love of Native American and Mexican silver jewelry. Midcentury design and Scandinavian modern design have also influenced my jewelry.

What tools and equipment do you use?

I own lots of tools I have collected over the years, each with a specific purpose. I also modify tools. For example, I grind down the jaws of steel pliers, then polish them to create a smooth surface that won’t mar metal. I use various shaped hammers for forging and chasing, various shaped pliers for bending and shaping and digital calipers for measuring. I use a mini drill press, a flex shaft with assorted attachments, a tumbler, polishing motor and an ultrasonic cleaner as well.

What was your favorite piece that you designed?

A favorite piece in recent memory is currently on view at Studio 268 in Setauket. It’s a large sterling silver and moss agate flower mounted on black canvas, displayed in a shadow box. I made it to illustrate the idea that jewelry is not just a functional, wearable medium; jewelry can also be viewed as “art” displayed and hung on a wall.

What materials do you work with?

I like to work with both traditional sterling and argentium silver because they are relatively soft and easy to forge, yet strong enough to cut easily with a jeweler’s saw. Argentium silver is brighter and whiter than traditional sterling silver and tarnishes at a rate 70 times slower than traditional sterling silver. It is virtually tarnish-free!

Did you ever have a piece that you couldn’t bear to sell?

What else can you tell me about the process?

I hand make or “fabricate” most jewelry in my studio. “Fabricating” includes forging, soldering, stone setting, tumbling and polishing. I also work with a casting and plating company. The caster uses an ancient process known as “lost wax casting” to reproduce silver multiples, which I finish and then use in designs. The plater submerges silver jewelry in a bath that chemically coats the pieces in 24-karat yellow or rose gold to produce “vermeil.” Vermeil jewelry has a thick, durable 24-karat gold finish over sterling silver, at a fraction of the price of solid gold jewelry.

Is there a material that you wish to experiment with in the future?

24-karat gold! 24-karat gold is 100 percent pure gold, as you probably know, not alloyed with any other metals like 10k, 14k or 18k gold.

also like to experiment with a small-scale 3-D printer to produce resin models that could be cast. I would like to figure out how to utilize 3-D printing technology in my work, if that’s viable.

Is there a period of jewelry making that you most admire?

My favorite period is the 1950s and ’60s.

I love the American studio jewelry movement and also modernist Mexican and Scandinavian jewelry from this time period. At midcentury, American universities across the country began offering serious metalsmithing programs. Because these skills were taught in a conceptual, university setting, jewelry began to be seen as contemporary art or miniature sculpture, not just wearable craft.

How do you decide on an individual design?

I make multiple versions of designs, sometimes three, five, even 10 variations of the same piece. After experimenting, I choose the one I like best and then scale back details until the design is distilled to a simple, clean piece. I also take commissions and make one-of-akind commissions at a client’s request.

Are there jewelry makers whom you admire in the past or present?

Some of my “art heroes” include Alexander Calder, Georgia O’Keefe, Andy Goldsworthy, Vivianna Torun Bülow-Hube, Betty Cooke, Art Smith, Coco Chanel and Jill Platner. There are too many to list and I discover new influences every day.

Yes, I made a pendant from sterling silver, horsehair and a cast plastic fishing lure that I found on the beach for our senior thesis show at MassArt. The finished pendant resembled a tiny, abstract broom, almost like a miniature African totem. I loved how it came out and wanted to use it as an inspiration for future work, so I put it in the exhibit with “Not for sale” on it.

Where can we see your jewelry?

My work was recently included in the Setauket Artists Spring Show at Deepwells Mansion. It is currently part of the Small Works Show in Studio 268 where my jewelry will continue to be shown through June on Saturdays and Sundays from noon to 4 p.m.

L

ong Island beaches have become the Island’s internationally known trademark attraction. Long days surfing at Atlantic Ocean beaches, relaxing at the gentle waters of South Bay beaches coupled with gazing at what seems like meticulously painted sunsets at the rocky North Shore beaches have provided storybook summer memories for Long Islanders for generations.

Yet, how many of us have had the opportunity to understand how the beaches have come to be what they are today, and the stories of past residents and visitors who enjoyed them so long ago?

Kristen Nyitray, the director of Special Collections and University Archives, as well as a university archivist at Stony Brook University, takes readers along for the story of the history of beaches in Nassau and Suffolk counties in her book, “Long Island Beaches” or what she describes as “a facet of Long Island’s social and cultural history and lure of picturesque beaches.”

Published by Arcadia Publishing as part of its Postcard History Series, the 128-page paperback book details coastal Long Island history beginning with the Native Americans, who had respect for its beauty and used it to live off the vast resources of the coast, often

engaging in whaling and fishing. Beach areas became desirable for land ownership in the 16th and 17th centuries and were an asset during the 18th century with lighthouses and stations opening up to combat shipwrecks.

Long Island beach destinations became commercialized during the mid- to late 1800s, with hotels, restaurants and attractions popping up in response to increases in transportation efficiency, even becoming a major source of illegal prohibition transfers. Environmental activism took hold by 1924 when Robert Moses worked with the New York State Council of Parks and Long Island State Park Commission to build beaches and parks throughout the island, along with bridges to link the barrier islands of Jones Beach Island and Fire Island to the South Shore coast.

Nyitray organizes her book by county, then shore and community. Black and white photographs, along with vintage postcards, gleaned from local libraries, historical societies, museums and private collections are sprinkled throughout, beautifully display the coastal culture so ingrained on the island.

It begins with a survey of Nassau County North Shore beaches, showcasing historic hotels and the wealth of those who resided on the coast.

For example, Nyitray tells the story of John Pierpoint Morgan Jr., the benefactor of Morgan Memorial Park in Glen Cove in honor of his late wife. It featured seven miles of coast, and as reported by the New York Times in 1926, was one of the first breaks in elitist private estates and chances for the public to access the sound.

Nassau’s South Shore was also populated by hotels and home to the Long Beach Boardwalk and Jones Beach State Park. The Moses-led endeavor at Jones Beach was made accessible to the disabled in 1883 when Strandkorbs, rolling beach chairs, became available. Made of wicker, people were pushed along the boardwalk in them, a major stride in accessibility.

Suffolk’s central beaches consisted of Lake Ronkonkoma and Shelter Island, with the latter being home to the Prospect House Hotel, consisting of a two-story bathing pavilion and a relaxation haven for guests in what is today the Shelter Island Heights Beach Club. The North Shore beaches were home to exclusive communities such as Belle Terre and Greenport.

Albert Einstein even vacationed with his friend David Rothman in Cutchogue, after Einstein visited Mattituck to lease a home for sailing, later renting a home in Nassau Point.

Suffolk South Shore beach history consists largely of Montauk and Fire Island. Nyitray speaks of journalist Margaret Fuller, who tragically drowned with family near Point O’Woods after the ship she was sailing on, The Elizabeth, sank after hitting a sandbar. At the request of poet Ralph Waldo Emerson, Henry David Thoreau traveled there to search for her, but to no avail.

These stories are just a sampling of the anecdotes that Nyitray lays out, and by the end of the book has the reader walk away with an immense sense of pride in being a Long Islander, along with better appreciation for being able to live in a place of such indisputable beauty, history and culture.

Studies say Long Island opioid use higher among lower income households

Long Island’s opioid-related use and poisoning, which nearly doubled from 2015 to 2016, was higher among lower income households in Nassau and Suffolk counties, according to a recent study in the American Journal of Preventive Medicine.

Looking at hospital codes throughout New York to gather specific data about medical problems caused by the overuse or addiction to painkillers, researchers including Fusheng Wang, an assistant professor in the Department of Biomedical Informatics at Stony Brook University, George Leibowitz, a professor in Stony Brook’s School of Social Welfare, and Elinor Schoenfeld, a research professor of preventive medicine at the Renaissance School of Medicine at Stony Brook, explored patterns that reveal details about the epidemic on Long Island.

“We want to know what the population groups are who get addicted or get poisoned and what are the regions we have to pay a lot of attention to,” Wang said. “We try to use lots of information to support these studies.”

The Stony Brook team, which received financial support from the National Science Foundation, explored over 7 years of hospital data from 2010 to 2016 in which seven different codes — all related to opioid problems — were reported.

During those years, the rates of opioid poisoning increased by 250 percent. In their report, the scientists urged a greater understanding and intervening at the community level, focusing on those most at risk.

Indeed, the ZIP codes that showed the greatest percentage of opioid poisoning came from communities with the lowest median home value, the greatest percentage of residents who completed high school and the lowest percentage of residents who achieved education beyond college, according to the study.

In Suffolk County, specifically, the highest quartile of opioid poisoning occurred in communities with lower median income.

Patients with opioid poisoning were typically younger and more often identified themselves as white.

People battling the painkilling affliction in Suffolk County were more likely to use self-pay only and less likely to use Medicare.

In Suffolk County, the patients who had opioid poisoning also were concentrated along the western section, where population densities were higher than in other regions of the county.

The Stony Brook scientists suggested that the data are consistent with information presented by the Centers for Disease Control and Prevention, which has found significant increases in use by women, older adults and non-Hispanic whites.

“The observed trends are consistent with national statistics of higher opioid use among lower-income households,” the authors wrote in their study. Opioid prescribing among Medicare Part D recipients has risen 2.84 percent in the Empire State. The data on Long Island reflected the national trend among states with older residents.

“States with higher median population age consume more opioids per capita, suggesting that older adults consume more opioids,” the study suggested, citing a report last year from the American Journal of Preventive Medicine.

Nationally, between 21 to 29 percent of people prescribed opioids for pain misused them, according to the study, which cited other research. About 4 to 6 percent of people who misuse opioids then transition to heroin. Opioid costs, including treatment and criminal justice, have climbed to about $500 billion, up from $55.7 billion in 2007, according to a 2017 study in the journal Pain Physician.

The findings from the current study on Long Island, the authors suggest, are helping regional efforts to plan for and expand capacity to provide focused and targeted intervention where they are needed most.

Limited trained staff present challenges for the implementation of efforts like evidenced-based psychosocial programs such as the Vermont Hub and Spoke system.

The researchers suggest that the information about communities in need provides a critical first step in addressing provider shortages.

New York State cautioned that findings from this study may underreport the burden of opioid abuse and dependence, according to the study.

To understand the extent of underreporting, the scientists suggest conducting similar studies in other states.

Scientists are increasingly looking to the field of informatics to analyze and interpret large data sets.

The lower cost of computing, coupled with an abundance of available data, allows researchers to ask more detailed and specific questions in a shorter space of time.

Wang said this kind of information about the opioid crisis can provide those engaging in public policy with a specific understanding of the crisis. “People are not [generally] aware of the overall distribution” of opioid cases, Wang said. Each hospital only has its own data, while “we can provide a much

more accurate” analysis, comparing each group.

Gathering the data from the hospitals took considerable time, he said. “We want to get information and push this to local administrations. We want to eventually support wide information for decision-making by the government.”

Wang credited his collaborators Leibowitz and Schoenfeld with making connections with local governments.

He became involved in this project because of contact he made with Stony Brook Hospital in 2016.

Wang is also studying comorbidity: He’d like to know what other presenting symptoms, addictions or problems patients with opioid-related crises have when they visit the hospital. The next stage, he said, is to look at the effectiveness of different types of treatment.

A resident of Lake Grove, Wang believes he made the right decision to join Stony Brook. “I really enjoy my research here,” he said.
