Cooking with cheese

I enjoy using cheese as an ingredient in cooking, and its texture, flavor and color add another dimension to the finished dish.

Cheese can be added in chunks, diced or even shredded to food at the beginning, middle or even end of cooking. You can fry, bake or broil cheese; add it as an ingredient to many of your favorite recipes; or make an incredible grilled cheese sandwich. Some of my hints for cooking with cheese are…

• Bring cheese to room temperature before use in cooking.

• When melting or blending cheese into a recipe, use a low temperature for a short period. Cheese is high in protein and prolonged cooking or cooking over high heat will cause the cheese to separate and result in a stringy, tough, somewhat rubbery product. When used in the oven, never exceed 375 F or the cheese will break down.

• Hard cheeses can tolerate higher temperatures than soft cheese because more of the protein has been broken down into small, less easily coagulated fragments.

• A double boiler keeps the heat moderate, thus avoiding a hard, stringy mess.

• Do not add cheese too soon during cooking; it may curdle or melt away to a stringy mess.

• To prevent cheeses such as Swiss types and mozzarella from becoming stringy during cooking, add a little wine or lemon juice before melting them.

• When stirring cheese, use a wooden spoon and never use a slotted spoon, which will become clogged with cheese.

• Cheese works well with sauces that have been thickened with flour or other starches. To avoid a lumpy cheese sauce, add a little flour, cornstarch or arrowroot at the start of the recipe (before adding the cheese). You can heat the cheese for an extended period and the sauce will remain smooth and creamy.

• Dice, shred or crumble cheese into dishes to hasten its melting time and ensure an even distribution throughout the dish. Grated cheese blends into sauces better than chunks or julienned pieces.

• When melting cheese on top of food, add it near the end of the cooking time. For a brown crispy layer of cheese, add it early in the cooking process.

• The rind of a Parmigiano-Reggiano wheel is edible and adds an incredible flavor to soups, stews, gravies and even pasta. Toss the rind into the simmering food and cook until it softens. Remove, chop into small pieces and return the bits to the pot.

Long Island’s first mobile stroke units Lifesaving service for the community

According to a study in the American Heart Association’s journal Stroke, when a blood vessel supplying the brain is blocked, nearly two million brain cells are lost for each minute that passes, making stroke one of the most timesensitive diagnoses in

That said, Suffolk County residents now have one more reason to look to Stony Brook Medicine for the highest level of care for both ischemic stroke (when a clot blocks the flow of blood to the brain) and hemorrhagic stroke (bleeding within the brain tissue).

This month we are launching Long Island’s first mobile stroke unit program — a revolutionary pre-hospital program designed to provide specialized, lifesaving care to people within the critical moments of stroke before they even get to the hospital.

While new to Long Island, mobile stroke units have successfully reduced stroke disability and have improved survival rates in other major metropolitan areas across the country. Stony Brook Medicine is collaborating with over 40 emergency medical service (EMS)

agencies throughout Suffolk County to provide this lifesaving, time-sensitive care.

Each mobile stroke unit is a mobile emergency room with a full crew of first responders, brain imaging equipment and medications. The units also have telehealth capability to Stony Brook University Hospital, which allows our physicians at the hospital to communicate in real time with the crew and patient, and immediately check for a blocked vessel or bleeding in the brain. This helps to markedly accelerate the time needed to make an accurate stroke diagnosis.

The first responders onboard the mobile stroke unit can then begin administering timesensitive, advanced stroke treatments while the person is en route to the nearest hospital that can provide them with the appropriate level of care.

The units are in operation seven days a week, from 8 a.m. to 8 p.m., which is the window of time when most stroke calls are received in Suffolk County.

One is strategically stationed at a base station located off of the Long Island Expressway at Exit 57. The other, which will be launched soon, will be stationed similarly off of Exit 68. These locations were chosen for easy east-west and north-south access. The team will take calls within a 10-mile radius of each base, which includes about 40 different communities.

What to do about thyroid nodules Most identified incidentally are benign

More than 50 percent of people have thyroid nodules detectable by high-resolution ultrasound (1). Fortunately, most are benign. A small percent, 4 to 6.5 percent, are malignant, with the number varying depending on the study (2). Thyroid nodules are being diagnosed more often incidentally on radiologic exams, such as CT scans of the chest, MRI scans, PET scans and ultrasounds of the carotid arteries in the neck (3).

There is a conundrum of what to do

By David with a thyroid nodule, Dunaief, M.D. especially when it is found incidentally. It depends on the size. If it is over one centimeter, usually it is biopsied by fine needle aspiration (FNA) (4). While most are asymptomatic, if there are symptoms, these might include difficulty swallowing, difficulty breathing, hoarseness, pain in the lower portion of the neck and a goiter (5).

FNA biopsy is becoming more common. In a study evaluating several databases, there was a greater than 100 percent increase in thyroid FNAs performed over a five-year period from 2006 to 2011 (6). This resulted in a 31 percent increase in thyroidectomies, surgeries to remove the thyroid partially or completely.

However, the number of thyroid cancers diagnosed with the surgery did not rise in this same period. Though the number of cancers diagnosed has increased, the mortality rate has remained relatively stable over several decades at about 1,500 patients per year (7). Thyroid nodules in this study were least likely to be cancerous when the initial diagnosis was by incidental radiologic exam.

Treating borderline results

As much as 25 percent of FNA biopsies are indeterminate. We are going to look at two modalities to differentiate between benign and malignant thyroid nodules when FNA results are equivocal: a PET scan and a molecular genetics test. A meta-analysis (a group of six studies) of PET scan results showed that it was least effective in resolving an unclear FNA biopsy. The PET scan was able to rule out patients who did not have malignancies, but did not do a good job of identifying those who did have cancer (8).

On the other hand, a molecular-based test was able to potentially determine whether an indeterminate thyroid nodule by FNA was malignant or benign (9).

Unlike in the PET scan study above, the researchers were able to not only rule out the majority of malignancies but also to rule them in.

It was not perfect, but the percent of negative predictive value (ruled out) was 94 percent, and the positive predictive value (ruled in) was 74 percent. The combination test improved the predictive results of previous molecular tests by 65 to 69 percent. This is important to help decide whether or not the patient needs surgery to remove at least part of the thyroid.

Significance of calcification on ultrasound

Microcalcifications in the nodule can be detected on ultrasound. The significance of this may be that patients with microcalcifications are more likely to have malignant thyroid nodules than those without them, according to a small prospective study involving 170 patients (10). This does not mean necessarily that a patient has malignancy with calcifications, but there is a higher risk.

Good news

As I mentioned above, most thyroid nodules are benign. The results of one study go even further, showing that most asymptomatic benign nodules do not progress in size significantly after five years (11). The factors that did contribute to growth of about 11 percent of the nodules were age (<45 years old had more growth than >60 years old), multiple nodules, greater nodule volume at baseline and being male.

The authors’ suggestion is that, after the follow-up scan, the next ultrasound scan might be five years later instead of three years. However,

they did discover thyroid cancer in 0.3 percent after five years.

Thyroid function may contribute to risk

In considering risk factors, it’s important to note that those who had a normal thyroid stimulating hormone (TSH) were less likely to have a malignant thyroid nodule than those who had a high TSH, implying hypothyroidism. There was an almost 30 percent prevalence of cancer in the nodule if the TSH was greater than >5.5 mU/L (12).

The bottom line is that there is an urgent need for new guidelines regarding thyroid nodules. Fortunately, most nodules are benign and asymptomatic, but the number of cancerous nodules found is growing. Why the death rate remains the same year over year for decades may have to do with the slow rate at which most thyroid cancers progress, especially two of the most common forms, follicular and papillary.

Gap Inc. announced plans last week to close 230 of its namesake brand stores over the next two years as it works to restructure. Gap will close about 130 Gap stores, or more than half of the fleet slated for closure, in 2019.

The retailer will split into two publicly traded companies: Old Navy and a yet-to-be named company that will include the Gap brand, Banana Republic, Intermix and leisure labels Athleta and Hill City.

In a statement Gap said its remaining store fleet will be a “more appropriate foundation” for future growth.

BAM! you discover that you can’t buy a used, off-brand Roomba online and expect it to work for more than a week (it is now a glorified doorstop that makes me look way more successful than I am).

In the chaos of this transition, there are some victories. Both of my apartment’s fire alarms are incredibly sensitive and wail at the slightest inkling that I’m frying up turkey bacon. Using my adulting skills (and The Force) I’ve repurposed my replica Luke Skywalker lightsaber and have placed it in the living room so we can easily shut off the fire alarm, which would otherwise require a ladder. will ever get to being an actual Jedi, but it is better than nothing.

During my ongoing efforts to embrace and tackle adulthood, I made a leap of faith by hosting not just one of my friends but three of them at the same time. In my mind, I was opening a sleepaway camp complete with preplanned activities, snack times and lots of sugar-free popsicles. When opening a sleepaway camp for your friends, be wary ... they’ll probably want to actually do vacation things.

After you move across the country twice, it can be pretty difficult to maintain long-distance friendships. We as people tend to evolve and grow apart when placed far away from one another and/or while going through major life changes. Living in Los Angeles, work tends to get in the way of just about everything else. The extraordinary cost of living makes it hard to find time to take care of yourself let alone be there for others. Work, relationships, dreaded time zones ... all can easily become excuses to disconnect. For me though, the greater the distance the more I begin to see which friendships matter and which don’t.

Best friends are the individuals you can go without seeing for months or even years and pick up right where you left off.

They’re the ones who see your potential when you cannot. They know me better than I know myself. So thank you to my friends, the ones who have guided me on my path and inspired me through their own successes and comebacks. My friends are all pretty unique. Each occupies a special place in my heart. Their combined chemistry can be exhausting sometimes but also wildly entertaining (except if you’re our Uber driver in which case I’m so sorry).

Port Jefferson is just about as small as a small town can be.

What’s most impressive about this community is, despite its smallness, the friendships made during the Port Jeff chapter of my life have proven to be the most lasting and most meaningful.

I’m saying this because at present I’ve got three of my best friends in the world squeezed into my Hollywood apartment occupying an air mattress, my mattress and the couch, respectively. (So sorry to my roommate Andrew.) It’s not quite summer camp, but it sure is fun being back with the gang.

We’re about to head to Venice Beach. My best friend Jon and I have committed to wearing matching cat swimsuits. That should hopefully distract the rest of my friends from the $85 parking ticket they already got and the fact that I only own three blankets and there’s four people here. But hey, at least we’re together!

Celeste Mauro, a Northport resident, has earned art degrees from Adelphi University and Parson’s School of Design. She has been an avid watercolorist for over 30 years, but that didn’t stop her from experimenting with collage and mixed media that have enhanced her work. Mauro was a member of The Firefly Artists Gallery in Northport. Two years ago, she and fellow local artist, Demerise Perricone, launched a new art gallery in Northport: Gallery Sixty Seven. I was able to catch up with this busy lady recently to talk about her art and her new adventure!

What motivates you to start the creative process?

The patterns and motifs found in nature inspire me to create an image that evokes intuitive feelings or sentiments rather than a realistic painting.

You have been known for your work in watercolor. What is it about watercolor that attracts you?

The transparent quality of watercolor allows the work to have an inner glow, almost as though lit from behind. I enjoy working with acrylic paint and mediums that offer translucency. Plus they are the best adhesive for collage.

Can you tell us about your new adventure?

Gallery Sixty Seven, located at 67 Main St. in Northport, is a gallery as well as studio space, which makes it unique. Now as the owner and a Northport resident, I especially love that people from the community feel free to drop by to observe art being created or to browse the works in the gallery. Gallery Sixty Seven has a strong “Northport” feel to it! As local artists, we are inspired by the natural beauty of local sites and our clients appreciate that.

I alternate media and layer paint or printing over collage … then collage over that. The free-form shapes of torn paper add a sense of abstraction; the brushwork adds detail and the printing adds an element of surprise.

How did you choose the artists for your new gallery?

As a small gallery, we can represent but a handful of artists. All of the artists in the gallery have a distinctive style that sets them apart and yet their different styles play beautifully off one another. As lifelong artists, they each have a body of work that exemplifies their unique personal perspective. I also look for professionalism and a cooperative spirit in each artist.

Do you offer workshops?

Many people are curious as to how artwork is created. This section provides the step-bystep process, shown with photos, which takes the mystery out of the process.

Yes! The gallery is looking to expand its offering of workshops since they were so successful last year. The instructors are the gallery artists who are experienced art teachers; our limited studio space allows for lots of personal attention … a winning combination. Can you explain what collage is and how it enhances your work in watercolor and acrylic? Prepping and Steaming an Artichoke YIELD: Makes 4 servings INGREDIENTS: • Juice of one whole lemon • 4 globe artichokes • 4 slices lemon • 4 garlic cloves • 1 fresh bay leaf DIRECTIONS:

Fill a large bowl with cold water and lemon juice; as you prepare the artichokes as follows, place them in the lemon water to keep them from turning brown. With a kitchen scissor, cut off the thorny tips of the outer leaves. Using a serrated knife slice about 1 inch off the tip of the artichoke. Pull off any small leaves at the base near the stem. Cut off an inch or so at the bottom of the stem, and using a vegetable peeler, remove the tough outer skin.

Alternatively all but an inch or so of the stem can be removed and peeled and cooked separately. Gently prying the leaves open, run the artichoke under cold water. Set up a pot large pot with about 3 inches water and a steamer basket.

Eating an artichoke Pull off a leaf; gripping it between your thumb and forefinger, dip it in butter or sauce.

Bite down on leaf and scrape away tender pulp with your front teeth. Discard leaf. Continue with each leaf until you get to the tender inner leaves with the purple tips; eat only the light-colored parts. With a spoon or knife, scrape out and discard the fuzzy inedible choke that covers the heart, which is the best part of all and well worth all that preliminary work. Cut the heart into quarters and dip it into the sauce. Enjoy.

Stuffed Artichokes

YIELD: Makes 4 servings INGREDIENTS: • ½ cup unflavored breadcrumbs • ½ cup grated Parmesan cheese • 2 tablespoons minced fresh garlic • 2 tablespoons chopped fresh flat-leaf parsley • 1/3 cup extra virgin olive oil • Salt and freshly ground pepper, to taste DIRECTIONS:

Preheat oven to 375 F. In a medium bowl combine breadcrumbs, grated cheese, garlic, parsley, 3 tablespoons of the olive oil, salt and pepper. With the heel of your hand, press down on artichokes to force leaves to separate and open a little. Divide breadcrumb mixture into 4 equal portions and stuff each artichoke between leaves and in central cavity; drizzle with remaining olive oil.

Wrap each artichoke in aluminum foil, place in baking dish and bake for 30 minutes; remove foil and bake another 10 to 15 minutes until browned and tender. Eat as in Eating an Artichoke but be sure to scrape away and discard fuzzy choke. Serve with rustic Italian bread and a tomato and mozzarella salad.

If they had a beauty contest for vegetables, I think the artichoke would win. It’s such a pretty veggie, so flower-like and mysterious with its closed leaves and well-hidden heart deep in its center. Actually a member of the thistle family, it is more specifically known as a globe artichoke and unfortunately is as difficult to find as it is to prepare, but well worth the effort ultimately.

A good artichoke feels heavy, its leaves are tightly closed and squeak when you squeeze it. Because it is such a special veggie, it can be steamed and eaten with a simple lemon and butter sauce; on the other hand, because it has such an interesting construction, it lends itself beautifully to a breadcrumb, garlic and olive oil stuffing in between the leaves. Or when in Rome or not in Rome, do as the Romans do and braise it in white wine, garlic, lemon juice and olive oil and season it with a generous sprinkling of fresh parsley.

Place the lemon, garlic and bay leaf in the water and bring it to a boil; stand the artichokes in the steamer basket, cover and simmer for about 30 minutes, until outer leaves can be easily pulled off. Serve hot, warm, at room temperature with melted butter or hollandaise sauce.

SBU researchers link PTSD and dementia risk for 9/11 responders

BY DANIEL DUNAIEF Every year, the country pauses on 9/11, remembering the victims of the terrorist attacks and reflecting on the safety and security of the country. At the same time, a Stony Brook University study continues not only to remember the first responders but also to understand the physical and mental consequences of the work police, firefighters and other first responders performed in the immediate aftermath of the attacks.

Recently, Sean Clouston, an associate professor in the Department of Family, Population & Preventive Medicine at SBU Renaissance School of Medicine, and Ben Luft, the director of the SBU WTC Health and Wellness Program since 2003, published research in which they demonstrated a link between a protein commonly connected with Alzheimer’s disease to post-traumatic stress disorder, or PTSD, in first responders.

In a small preliminary study, the researchers found a difference in the level of the protein between first responders who are battling chronic PTSD and those who aren’t battling the condition. The Stony Brook scientists published their work in the journal Alzheimer’s & Dementia: Diagnosis, Assessment & Disease Monitoring.

The researchers cautioned that the presence of the markers doesn’t necessarily indicate anything about present or future changes in cognitive function.“We don’t know the specificity of the markers,” Luft explained in an email.

loid and don’t develop problems with their thinking. Neurodegenerative diseases without amyloid rarely have severe symptoms, which don’t appear to worsen with time.

“This paper doesn’t look at cognitive symptoms,” Clouston said.

“We do have papers looking at cognitive impairment and other memory-based differences. It wasn’t a part of this paper.”

The newest research is part of an ongoing program in which the university follows 11,000 responders who came to the World Trade Center. The study for this paper involved a smaller subset of this population. This type of research can and does have application to other studies of people who have traumatic experiences, the scientists suggest.

Most traumatic experiences are unique to each person, as people who suffer physical and emotional trauma in combat often confront the aftereffects of head injuries. Among the first responder population who survived the attacks on 9/11, most of them “faired pretty well physically,” Clouston said.

“We didn’t have a lot of head injuries. Understanding PTSD in this crowd is really useful for the literature as a whole because it allows us to focus on the long-term psychiatric fallout of an event without worrying about exposures that are different.”

The scientists had at least some idea of the timing and duration of exposures. This research suggests that it might be helpful to think about the kinds of problems that cognitive impairment can cause, which might involve managing other health-related problems.

Luft added that the population they are studying shows the benefit of immediate care. “One

Sean Clouston and Benjamin Luft thing for sure is that the care of the first responders has to occur very quickly,” he said. “Now that we know the history, the greatest chance you have in mitigating the effect of this type of trauma is to deal with the problem from the get-go.”

First responders have benefited from psychotherapy as well as from various pharmacological treatments. Luft suggested that they might even benefit from having therapists available in the field, where they can receive near instantaneous psychological support.

In addition to the psychological trauma, first responders have had physical effects from their work in the aftermath of the attacks, such as respiratory and gastrointestinal problems, as well as autoimmunity issues.

People have these problems because “of the pro-inflammatory effect of PTSD itself,” said Luft. The researchers believe trauma can affect the immune system and the brain.

According to Clouston, the next step with this work is to replicate it with a larger scale. The experiment was “fairly expensive and untried in this population and novel in general, so we started small,” he explained in an email. The scientists would like to “get a larger range of responders and to examine issues surrounding symptomatology and other possible explanations.”

rival on Long Island, he worked on a collaborative project that was shared between University College London and the University of Victoria.

An expert in aging, he felt like his arrival came at just the right time for the WTC study, as many of the first responders were turning 50. After giving talks about the cognitive and physical effects of aging, he met Luft and the two decided to collaborate within six months of his arrival.

Clouston is focused on whether PTSD caused by the terrorist attacks themselves have caused early brain aging. A self-proclaimed genetics neophyte, he appreciates the opportunity to work with other researchers who have considerably more experience in searching for molecular signatures of trauma.

Clouston said his family has suffered through the trauma of cognitive decline during the aging process. His family’s struggles “definitely bring [the research] home,” reminding him of the “terror that many family members feel when they start noticing problems in their siblings, parents, spouses, etc.”

As for his work on the recent study, he said he is excited about the next steps. “Little is known about the subtypes of amyloid,” he suggested and there’s a “lot more to explore about the role [of this specific type] in the population. I do think it could be really informative about the types of symptoms.”

Men’s baseball clinches third consecutive weekend sweep

For the third consecutive weekend, the Stony Brook baseball team took all three contests en route to a sweep after securing a 2-0 win over Maine on March 31 that continued the Seawolves’ run in first place of the America East.

Standing now at 14-8 overall and 6-0 in league play, the Seawolves got another great starting pitching performance from Bret Clarke (Royersford, Pa.), who went eight scoreless while allowing three hits. Freshman Brad Hipsley (Saratoga Springs) was the deciding factor as he laid down a squeeze bunt that scored senior Brandon Alamo (Artesia, Calif.) to give Stony Brook all it would need on the morning.

“We ran into an outstanding pitching performance today but to the team’s credit we pitched and played defense until we were able to break through with a couple of runs in the eighth. I’m extremely proud of how we’ve started conference play and also how well we played at home all March,” said head coach Matt Senk.

Women’s softball takes down Hartford WEST HARTFORD, CONN.: Led by Melissa Rahrich’s (Brighton, Colo.) strong performance in the circle, the Seawolves took down Hartford on March 31. With the victory, Stony Brook took its first conference series of the season.

Rahrich went all five innings in the circle, blanking the Hawks and only giving up three hits while striking out five. Jordyn Nowakowski (San Diego, Calif.) and Rahrich paced the offense for the Seawolves. Nowakowski hit two solo home runs, and Rahrich drove in four runs.

“We came out with a great approach today and that was important to winning the game and the series. We were productive offensively, pitched tough and worked our way out of the situations when Hartford got runners on. Keeping the momentum on our side for the entire game was key,” said head coach Megan Bryant.

Rahrich picked up her seventh win in the circle while Nowakowski now has four home runs for the season. The Seawolves are 2-1 in conference play after the series win. The team returned home to face Wagner on April 2. Results were not available as of press time.

MOVIE REVIEW

This ‘Dumbo’ doesn’t soar

In viewing Tim Burton’s “Dumbo” it is hard not to compare it to Disney’s animated feature that served as source and inspiration. The delicate and wonderful cartoon ran 65 minutes and was both enchanting and heartbreaking.

Like all of Disney, there is delicacy about this 1941 film that has made it an enduring classic.

The story in both cases is that of the baby elephant, Jumbo Jr., a pachyderm born with giant ears. It is what makes him different that ultimately proves him special. These giant appendages give Jumbo Jr. — crowned Dumbo — the gift of flight. Ultimately, it is a tale of the “other” — a being ostracized for being different and then finding success, and, more importantly, joy in this distinction.

The original film ends with Dumbo’s rise to fame and his reuniting with his mother.

Burton’s version extends the length and the plot to a bloated two hours. The film is stunning to watch with incredible CGI in the creation of the title character. Dumbo is a wholly realized creation with eyes that are mournfully soulful.

The film (in 3-D) is visually satisfying but comes up short on character development.

The story is set just after the end of World War I. Wounded soldier Holt (a brooding but sympathetic Colin Farrell) returns to a failing circus and to his children, Milly and Joe (Nico Parker and Finley Hobbins, in nicely understated performances). He has lost his arm to the war and his wife to influenza. The circus is run by a roguish charlatan, Max Medici (Danny DeVito, doing what he does and does well), and is populated by the expected archetypes — the mermaid, the strongman, the snake charmer, etc. Instead of pursing this world and background lives, Burton opts for broad strokes and frenetic action.

It is the children, and, in particular the scientific Milly, who discover Dumbo’s gift. After the reveal of Dumbo’s talent, the film shifts with the arrival of a villainous entrepreneur V. A. Vandevere (scenerychewing Michael Keaton with an impenetrable and unrecognizable accent). Vandevere fools Medici into signing away his company so that he can headline Dumbo in his Dreamland amusement park. Here, the world becomes even bleaker as it segues into a clumsy indictment of corporate greed. What ensues is often tense and dramatic, but there is a

desolation that pervades, only lifted by the final images of freedom.

While there are plenty of homages to the original (the lullaby “Baby Mine,” the pink elephants are particularly clever and a mouse in a uniform harkens to the antecedent’s sidekick), the film has a very modern point of view, especially on the issue of caging

animals. It is an important message and one that needs to be heard, but rings oddly false in its period setting.
