Shopping for a new home is stressful even when you’re young and healthy and the only limitation is the size of your bank account.

As a senior, many more issues come into play: Do you merely want to be relieved of chores like lawn mowing and home repair?

How would you enjoy a community setting, with meals served and activities planned? Do you need regular assistance with everyday tasks? Or do you and/or your spouse require qualified staff to help with medical issues or debilitating memory loss? What will happen if your needs change during your stay?

Montana 55’s Annual Housing Guide was born out of the latter situation. Two co-workers were faced with a parent who needed an immediate change to their living situation. While coping with their parents’ serious illnesses, they also had to grapple with an unknown world of senior housing options, full of confusing jargon and acronyms.

They would have loved to have had the 3rd Annual Senior Housing Guide you hold in your hands right now. As soon as they found qualified facilities for their loved ones, they set about publishing the state’s first housing guide. Since then, it’s grown into a quarterly magazine. the subdivision,” says Travitz. The Council on Aging donated four acres to the Ravalli County Economic Development Authority, which plays host to the Bitterroot College of the University of Montana.

After a lifetime of ongoing home and yard maintenance, retirees or nearretirees age 55 and older may want to kick back and pump energy into other chore-free activities. In that vein, Travitz is confident that such residents would see their home or home-awayfrom-home dreams come true.

“A senior co-op provides owners

the freedom to live an active lifestyle without the burdens of home maintenance, lawn care and other household duties,” he explains. “The typical co-op owner is looking to downsize from the large home where they have raised their family. Or they’re looking for a second home in another part of the country where they can visit, but they know that while they’re gone their home is being taken care of.”

“We do offer a Day Stay program where seniors can stay with us on an hourly basis for only $10 an hour,” says Amy Peterson, Community Relations director at Canyon Creek. “We also offer respite stays where seniors can stay with us for a short term stay at $100 a day.”

Caregivers who realize that a muchneeded break from daily care keeps them healthy may appreciate the staff’s holistic approach.

Overall, the facility, located at 1785 Majestic Lane, helps those suffering from memory loss, which also makes for happier families.

“There is a great need for caring for those with some form of dementia,” says Peterson.

The Personal Touch Program is three-pronged, as the staff aims to “nurture intact abilities while reducing the adverse effects of impairments, provide a comfortable social and caring environment, and preserve dignity and independence while respecting family concerns,” according to Peterson.

To form the best supportive partnership with each family, the staff welcomes input and guidance from family members in order to best meet the particular needs and preferences of each resident/patient.

To avoid the impersonal atmosphere that may exist at other memory support facilities, the Canyon Creek staff prides itself on maintaining meaningful interactions while fostering strong individual relationships with each resident.

As for atmosphere, Peterson assures that Canyon Creek puts out the welcome mat in a big way.

“We custom designed and built Canyon Creek Memory Care Community with a distinctly residential character,” explains Peterson.

The grounds feature peaceful gardens, activities areas, a beauty and barber shop, a courtyard and a walking path, all in a secure environment.

“Every aspect of our community has been planned to help your loved one feel comfortable and at home,” Peterson adds.

Residents have at their disposal a wide range of activities, such as homemade cooking classes, therapeutic arts and crafts, exercise classes, reminiscing sessions, music therapy and pet therapy – all of which

“The goal for the entire subdivision,” Travitz says, “is to develop a step-upin-care campus, independent living, assisted living and a nursing home, that allows seniors to age in place even though they may need more assistance as they grow older.”

A new continuing care facility in Great Falls is putting the resident back at the center of senior health care.

The Grandview at Benefis, set to open in Great Falls in late summer 2013, creates a resident-centered model of care which provides both medical and social benefits and pushes the envelope of seniors’ health care, said Frank Soltys, executive director of Senior Services at Benefis Health Systems.

Residents can receive assisted living, memory support and skilled nursing services while retaining as much independence as possible in the continuing care retirement community.

Plans for the new campus also include the eventual inclusion of independent, maintenance-free living residences, from which residents can transition into higher levels of care as medical needs dictate.

Although services are largely private pay, fees are similar to elsewhere in Great Falls, said Charlene Nelson, marketing and sales director for the new campus.

Three senior care cottages comprise the skilled nursing portion of campus.

“The outside will look like a house,” Soltys said.

The inside consists of 16 private bedrooms, each with a private bath, and a shared living, kitchen and dining area. The setup allows for more privacy and more personalized care than more traditional models of care, Nelson said.

Older models tend to be institutional, she said, “Whereas, what we’re creating is very residential.“ Unlike if they remained in their private homes, residents don’t have to worry about maintenance in the residences, which are constructed to be as accessible and free of trip-and-fall hazards as possible. so couples can visit their other halves who are in different care areas without worrying about transportation or weather.

Grandview’s assisted living apartments allow for as much independence as possible at every stage by providing three levels of care, Nelson said. Each resident will undergo a level of care assessment before they enter the campus.

Assessment results will be used to craft a personalized service plan, which will be reviewed every six months (or more often as needed) and adjusted as necessary.

“It’s the only place in north-central Montana that has all the levels of care together,” Nelson said, adding that multiple levels of care can reduce stress for people who otherwise might need to move to different facilities as their needs change. At Grandview, “they can age in place, so to speak,”

Nelson said.

The campus also has direct ties to Benefis acute care facilities, something a large percentage of people who were surveyed before the Grandview project began said was important, said Soltys.

In addition to the medical services offered, Grandview’s senior care cottages, assisted living and memory support also provide ample outdoor and socialization opportunities.

Residents can enjoy garden areas, walking paths, a town center area and a spa.

Large garden areas aren’t common, Soltys said. “And I think that’s something that’s important to folks here in Montana,” he said.

Garden areas can be particularly beneficial for memory care residents, Nelson said. All of the plants will be edible and the areas can provide tranquility to residents, she added.

Other activities and programs in memory support are planned to help stave off memory loss.

“So even though they may have trouble remembering yesterday, we’re trying to give them the best today,” Nelson said.

Activities and areas such as a library, computer room and game room will benefit assisted living residents.

“There’s just a lot to do outside their room, and that’s what we’re trying to foster,” said Nelson, stressing that the campus will provide more than just a room.

“Life gets smaller as we get older because there are less things that we can do, so we’re trying to make it as big as possible,” she added.

The aptly-named Aging in Place Loan program is a subset of the Home Improvement Program Loan administered by NeighborWorks in Great Falls, a virtual powerhouse nonprofit that has dutifully, consistently provided affordable housing to lower-income families and revitalized declining neighborhoods since 1980.

While an estimated 25 percent of NeighborWorks loans go to individuals or heads of households age 55 or older, the specific Aging in Place Loan is open to anyone of any age, stresses executive director Sheila Rice. There is no age minimum requirement to apply.

“HIP Loans help senior citizens make the changes to their homes that allow them to stay in their home and live out their years in a familiar place,” says Rice, a longtime community leader and activist.

“Sometimes, just a small change, like adding bathroom grab bars, makes a big difference,” she added.

“Other times, a bathroom on the main floor will allow a senior to stay at home longer. These loans are a great way for NeighborWorks to help seniors stay in their homes, so we offer not only loan funds, but assistance in drawing up work specifications and finding a contractor to do the work.“ The HIP loan is an amortizing loan repayable in affordable monthly payments, which means that the loan, spread out over time, may eventually be written off. Nursing homes and assisted living options are becoming less and less like the stereotype of rigidity of the past.

However, nine out of 10 seniors want to stay in their own homes as long as possible despite fall risks, according to AARP.

Have a conversation about long-term care options, but if seniors are determined to stay home, try to find the resources to enable them to do so, said Laura Booth, caregiver support coordinator at Adult Resource Alliance of Yellowstone County.

Most-used basic home health care options include someone to remind seniors to take medication, or do light cooking and cleaning, Booth said. Sometimes, an in-home care provider can supply transportation or card-playing company, she added.

Often, seniors feel like they are imposing if they

ask family for help, Booth said. If a parent asks you for something, don’t make them feel bad for asking for help, she said. “Be willing to step up and do it.“ Schedule a therapist to do a home evaluation to identify hazards, suggested Mary Dalton, resource specialist program manager with Missoula Aging Services.

Minor home modifications can make a senior’s home livable longer, she said. For example, place risers under couch legs to lessen the effort of lowering oneself and raising oneself from it or add grab bars to the shower to lessen the risk of falling.

Don’t overlook nutritional needs, Dalton said.

Use programs such as Meals on Wheels, or family and friends to maintain a healthy lifestyle, she said.

Depending upon the amount of care needed, this option may end up being more affordable than moving into a facility. Home health care agencies are licensed and usually Medicare certified. To earn Medicare certification, the agency must meet specific federal guidelines and criteria concerning patient care.

Medical help provided by a home health agency may include nursing care, physical therapy, occupational therapy, speech pathology, a medical social worker who assesses emotional needs, and a home health aide. Agencies must adhere to a physician-approved plan that is updated every two months. Most of the agencies in Montana offer all of these services.

Some have all but one.

Nonmedical help may include 24-hour care, hygiene assistance, some housekeeping, meal preparation, medication reminders, rides to appointments and errands and, of course, companionship.

When people begin talking about seeking long-term care outside their homes, they broach one of the most emotional topics for families with aging adults.

“Mom took care of me for 40 years, and now I’m going to expect someone else to take care of her.

Is that fair?” is a common way to feel, said Derik Sapp, local long-term care ombudsman at Cascade and Chouteau County Aging Services Agency. The conversation brings up death and the last chapter of life, which is especially difficult for both parties, said Sapp, who works with seniors and their families to find the best option for care.

The idea of losing independence and moving from their home can be daunting and unappealing to seniors, said Laura Booth, caregiver support coordinator at Adult Resource Alliance of Yellowstone County. Socialization and availability of services, such as the prepared meals and medicine reminders assisted living facilities provide, can mean better quality of life – and more independence, longer, she added.

“I think that’s the biggest fear with most seniors, (that) if they say that they’re vulnerable or if they admit to it that a family member’s just going to put them away in a nursing home,” Booth said.

Include seniors in every aspect of the process so they feel like they are controlling the next part of their lives and where they will spend it, Sapp said.

Empower them to be the decision-maker.

“She needs to live there. We have to look at the fact that what I see in the facility could be the exact opposite of what mom sees,” he said.

Accept that you might not be the best person to have the conversation. Maybe your loved ones listen more to another family member, or trust a doctor or family friend. So find that person and let them start the conversation, suggested Booth.

Take the opportunity to visit friends who are in nursing homes or assisted living facilities, Sapp said.

“In their minds, it doesn’t matter where they’re going. It’s not going to be home,” Sapp said.

“So if there’s a success story, use it for heaven’s sake; use the success story, and that can be your segue,” he said.

The best way to undertake the conversation, regardless of who’s doing it, is to use subtle approaches.

“Maybe ask that parent, ’What would you like? How about if we find some resources and then we can look at them together?’” Booth said.

Don’t assume that aging adults haven’t considered the options for additional help or assisted living.

“I think older adults are very much aware of their decline and their situation,” said Mary Dalton, resource specialist program manager at Missoula Aging Services.

Even if you’ve done research and have information to share, listen to what they say about long-term care, she said. Don’t patronize, and make sure to validate how they’re feeling.

“It’s possible that they already know 28

everything you’re going to talk about,” she added.

One way to bring up the topic is to ask, “How do you see yourself remaining as independent as possible?” Then build off what they say, Dalton said.

Remember, what you want might be different from what they want. Do what’s best for them, instead of what you think is best for them, Dalton said.

If the conversation becomes too emotional, back off and revisit the topic later, she added.

Ignoring the issues, though, isn’t the thing to do. Accidents or rapidly declining health factors can take the decision about where and how people want to spend the last years of their lives out of their control. Because of the risk of health issues, such as strokes

or injuries caused by falls, no time is too early to start the conversation.

Regardless of what communication method you choose, create an ongoing conversation about how your loved ones are feeling and what they need.

Don’t rush the process, Sapp said, adding that you can’t expect people to change their situation in a day or a weekend. When people delay talking about assisted living, an emergency situation, such as a fall that places a person in assisted living, can happen.

Then people tend to react with emotions instead of what’s ideal, Sapp said.

“That’s why starting the process earlier is better off than waiting for an emergency to occur,” he said.

Pat from Great Falls asked this question: Q: My grandmother doesn’t believe in flu shots and she refuses to get one. I have heard that getting a flu shot becomes more important as you get older. Is this true and, if so, how do I convince her to start getting one?

A: You are correct to be concerned for

your grandmother – many people don’t understand how risky the flu virus can be. Statistics show us that approximately 40,000 Americans die each year due to pneumococcal infections, with seniors representing the largest group of fatalities.

Yet, only 58 percent of Americans over 65 receive the influenza vaccine.

protect yourself from the flu by getting your annual flu shot. The flu shot is a harmless vaccine. So you need to ask yourself what you would prefer: spending weeks feeling lousy or getting a quick, painless shot in the arm.

The U.S. Centers for Disease Control and Prevention recommends a seasonal flu shot every year for people 50 or older. People 65 or older have two options – the regular-strength flu shot or Fluzone High-Dose, a vaccine that creates a stronger immune response to combat agerelated weakening of the immune system. Your health care provider can help you decide which dosage is better for you.

As part of the Affordable Care Act’s preventive benefits, people in Medicare and Medicaid can get an annual flu shot at no cost and both flu vaccines are covered. For everyone else, many insurance plans provide coverage for the flu vaccine. It is best to double check on your co-pay or responsibility for payment.

Where to get a flu shot?

Flu shots are available in many locations such as your doctor’s office, work places, supermarket or drugstore.

Because costs may vary it’s a good idea to first check with your insurance provider about coverage. To find where you can get the flu shot, visit http://flucliniclocator.org or call the CDC hotline at 1-800-CDC-INFO for help.

Prevent catching or spreading the flu.

You can catch the flu if you’re around an infected person who coughs or sneezes. You can also pick up flu germs from touching a surface that someone with the flu has touched, such as a telephone or doorknob, then passing the germs from your hand to your nose or mouth. Take these steps to protect your health: • Cover your nose and mouth with a tissue when you cough or sneeze.

• Wash your hands often with soap and water, especially after you cough or sneeze. Alcohol-based hand cleaners are also effective.

• Avoid touching your eyes, nose or mouth.

5. Not having a health care power of attorney and a living will: These documents are even more critical when a couple is unmarried and not otherwise related because the laws of privacy would exclude the other partner from the hospital room or access to their partner’s doctors.

6. Underusing trusts: Too many people falsely believe that healthy, responsible heirs don’t need a trust.

However, every heir can use the benefits of having their inheritance in trust. It provides asset protection as well as protection against divorce of the heir.

7. Failure to have enough liquid assets at death: When you die, there are financial responsibilities that need attention. Debts and taxes need to be paid.

8. Failure to carefully consider property that passes outside of a will: Many of us have significant parts of our wealth in retirement plans, IRAs and life insurance, none of which usually pass under a will. Pay a lot of attention to the beneficiary designations and the payout options under the policy or plan.

9. Assuming that your loved ones will cooperate and work things out after you’re gone: Mark Twain said, “You never really know someone until you share an inheritance with them.”

10. Failure to keep an estate plan up-to-date: Even the best estate plans can get stale if not revisited periodically. You should pull your plan out and read it annually. You should have your estate planner review it every three to five years as well as upon the occurrence of significant events like birth of new children, marriage, divorce, significant changes in wealth or upon significant law changes.

11. Thinking that estate planning is something that you can do yourself: If I had a dollar for every do-it-yourself estate plan that I had to fix after the fact, I’d be a wealthy man. Hint: it’s much more expensive to repair a defective estate plan than it is to prepare one.

12. Too much joint tenancy property: The joint tenancy trumps the estate planning documents.

13. Naming the wrong person to handle your affairs: Too many people simply name an oldest child even though the youngest child is the most capable of the group. Even the best estate plan can crumble due to the selection of the wrong person to be trustee or executor.

14. Equal is not always equitable: The vast majority of people want to divide their estates between their children equally. But equal is not always equitable.

If properly explained to your loved ones in advance, unequal division of an estate often makes more sense.

15. Failure to consider all possible estate planning angles: It makes sense to have an estate planning advisor who can “think outside the box” to tailor an estate plan for your unique situation. What works for someone else too often is foisted upon others, and the result is not good.

16. Failure to consider digital assets like Internet passwords for email and accounts like brokerage accounts: It’s a good idea to place your passwords for your various accounts in a safe place that a loved one can find after you are gone.

17. Failure to do adequate death tax planning: Many spouses want simple “I love you” wills where they leave their entire estates to each other. There are ways to give the spouse what you want them to have and get all of the tax benefits that you can.

18. Failure to cover all reasonably foreseeable future events: Many people simply assume that they will die before their children or their spouse. Will your estate plan work if your child or spouse doesn’t survive you?

19. Failure to maintain adequate records that people can find: Keep a trail that someone coming behind you can find – it will make administration of your estate much easier.

20. Failure to adequately prepare for incapacity: Having a will is not enough. What happens if you are incapacitated but still alive? You need a power of attorney or your loved ones will have to go to court to handle your affairs.

These are my “top 20” errors that people make in estate planning. Vow not to make them yourself. Putting your affairs in order will make your life more serene and enjoyable.

Hospice (HPC) – a coordinated program of home and

inpatient health care that provides or coordinates palliative and supportive care to meet the needs of a terminally ill patient and the patient’s family arising out of physical, psychological, spiritual, social and economic stresses experienced during the final stages of illness and dying and that includes formal bereavement programs as an essential component.

Independent Living (IL) - any housing arrangement

designed exclusively for seniors, generally those aged 55 and over. Housing varies widely, from apartment-style living to freestanding homes. In general, the housing is friendlier to older adults, often being more compact, with easier navigation and assistance in yard maintenance if there is a yard.

a facility that provides skilled nursing care, or intermediate nursing care, to a total of two or more individuals or that provides personal care.

Retirement Home (RTH) – a building or buildings in which separate living accommodations are rented or leased to individuals who use those accommodations as their primary residence.

Respite Care (RC) – service that provides temporary relief periods and other stress-relieving supports for family members who provide continuous care for developmentally disabled relatives at home.

Skilled Nursing Facility (SNF) – the provision of

nursing care services, health-related services, and social services under the supervision of a licensed registered nurse on a 24-hour basis.

Types of Independent Living: • Subsidized senior housing. The U.S. Department of Housing and Urban Development (HUD) subsidizes senior housing complexes for low income seniors. Waiting lists are common, so it is essential to plan ahead for this option.

• Senior apartments. Apartment complexes restricted by age, usually 55+. Rent may include community services such as recreational programs, transportation services, and meals served in a communal dining room.

• Retirement communities. Groups of housing units for those aged 55+. These housing units can be single family homes, duplexes, mobile homes, townhouses, or condominiums. Monthly fees may cover services such as outside maintenance, recreation centers or clubhouses.

• Continuing Care Retirement Communities (CCRCs).

CCRCs offer service and housing packages that allow access to independent living, assisted living, and skilled nursing facilities in one community. If residents begin to need help with activities of daily living, for example, they can transfer to an assisted living or skilled nursing facility on the same site.

Can it help with nursing home bills?

To be eligible for Medicaid in a nursing home, you must:

✘ Meet all non-financial criteria

(be 65 or older, blind or disabled; a U.S. citizen; and have a Social Security number).

✘ Be admitted under a doctor’s order.

✘ Meet the medical criteria for needing nursing home care.

✘ Meet the financial need requirements.

✘ Get a pre-admission screening.

Nursing Home Medicaid Assistance Medicaid helps pay for nursing home care for those who could not otherwise afford care. There are specific “resource limits” that must be met in order to receive Medicaid. Approximately 2/3 of all nursing home residents in Montana use Medicaid. Seniors who suspect that they will need Medicaid should contact their local Office of Public Assistance in order make an appointment with a case manager and start the application process.

The Medicaid Community Services Program The Medicaid Community Services Program pays for personal care, skilled nursing care, home health aides, home dialysis attendants, and hospice care for seniors who are eligible for Medicaid.

The State of Montana operates and manages the Montana Veterans’ Nursing Home in Columbia Falls and oversees a contract for the operation of Eastern Montana Veterans’ Nursing Home in Glendive.

Both facilities are restricted to honorably discharged veterans who have served in the armed forces of the United States, and their spouses. Eligibility for admission is determined without regard to race, color, sex, culture, social origin or condition, political or religious ideas, or ability to pay for the cost of care.

An applicant must meet the following criteria: • be a veteran, spouse, or surviving spouse • be 55 years of age or, if younger than 55, an invalid and/or unable to earn a livelihood • meet medical and physical requirements for intermediate nursing care or domiciliary care 38

• have the mental status necessary to function in a manner conducive to the safety and comfort of residents and staff Residents pay on the basis of their ability. The Veterans’ Administration contributes toward the cost of care for each veteran; spouses do not qualify for this benefit. Citizens Group Challenges Board Statement on Assisted Suicide Montanans Against Assisted Suicide (MAAS) is a grassroots citizens’ group opposed to physician-assisted suicide. Our reasons include that legalization is a recipe for elder abuse, for example, by an adult child wanting an inheritance who suggests, encourages or coerces his father to request and then take the lethal dose. In Oregon, other suicides have increased with legalization of physician-assisted suicide. MAAS is now engaged in a legal challenge against the Montana Board of Medical Examiners’ Position Statement No. 20, which erroneously implies that assisted suicide is legal in Montana.
