Tooth Sensitivity Following a Filling HAVE YOU EVER experienced discomfort after having a newer white filling placed? Most patients today are aware of better choices for their teeth. There are times when a tooth becomes hypersensitive following the placement of a resin filling. Many times the tooth is completely asymptomatic prior to the filling being placed. There are several causes for this condition. As dentists, we are constantly bombarded with new products that try to save us time. When I first learned the process in dental school, we had a three step process using a separate etch, primer and bonding agent. Today manufacturers have combined all three into one bottle. A recent study demonstrated that failure to etch the tooth with phosphoric acid separately reduced the bond strength by 20 percent. Some early thoughts were that the acid caused postoperative sensitivity. This has been disproved. The resin material shrinks as the blue light sets the material. There can be a twoto-three percent contraction of the material that setup internal stresses in the tooth if the resin is not placed in small increments. The newer resins have improved on this polymerization shrinkage. By layering the resin, we greatly reduce this effect. Another factor is failure to close the dentinal tubules that open during the etching process. Open dentinal tubules are the primary reason for tooth sensitivity. Failure to close properly can lead to extreme sensitivity to both temperature and touch. There are products that can be placed on these tubules to seal them closed. Some of the proclaimed whitening toothpastes are very abrasive and open the gum line tubules causing sensitivity. Tooth sensitivity can be quite a nuisance. It is a good idea to discuss this with your dentist prior to having resins placed.

GROWING UP, my parents ran a boys’ boarding school in England. During the holidays, all the students would travel home and my siblings Hugh, Elaine, Paul, Kathryn, Becky and I would be left with the whole house to ourselves. We would take over dorm rooms and invite friends to stay and put on elaborate plays for our parents and their friends. At Christmas, we would each claim a rectory style table in the main dining room that would be ours to hold our gifts. We would all fight for the spot closest to the entrance; my eldest brother Hugh usually won—he is just so competitive! On Christmas day, we would gather in the main hall by the fire to open our gifts. After lunch with all the dishes finished, my father would smoke a cigar. After he was done, we would finally open them, one at a time, and my mother would compose a list so that we could write our thank you notes. As hard as it was to wait, I do think that it taught us patience. In our home now, we take turns. One year we do it my way and wait: not very popular with my husband or son. This year they will be happy, as it’s our year to open gifts first thing in the morning. In this issue, we asked our Enterprising Women of 2013 to share their own holiday memories with our readers. We also meet this month’s Enterprising Woman, Dr. Kimberly Williams, a nephrologist who has recently opened A Place of Our Own to help parents with medically fragile children. Classes for any age include: Tots-N-Tunes music appreciation for 2- to 5-year olds, Music-N-More for 1/3 Square pre-schoolers, small group private piano 5.05” xand 4.35” and choir lessons for 4-year olds to adults, as well as lessons in voice, guitar, piano, violin and drums for all ages. Northlake, the only private Academy with a choir room for practice and performing, now offers a year round Adult Choir that sings in the community. Although Leslie Domingues works many hours per day, she says, “It doesn’t feel like work, because it is my passion. I am truly blessed!”

Of course, I am seven months pregnant; so, my reasons for adding a few pounds are favorable and pretty ironclad. We’ve come to both anticipate and loathe staying active through the chilly temperatures and seasonal decadence. Actors are entrepreneurs; you are the product being packaged, promoted and sold. It takes time. Your pictures will determine whether you even get an initial audition. So, invest in professional headshots from a photographer that is experienced in shooting actors on a regular basis and has an excellent reputation. Once you are thoroughly trained, have some real working acting experience and have progressed as an actor, sign with a reputable agency with which you also feel a good connection. To stay fine-tuned during and throughout auditions, continue your film acting training. This is more important than ever so that you will always put your best foot forward! Search for independent and short films that you can be a part of through colleges and other various resources so that you can build your résumé experience. Finally, network, network, network. Attend casting director seminars, make friends with other actors and fellow classmates. Everyone is a connection to lead you to the next step. Since 1970, the 1/3 Vertical Non-Bleed rate of cesarean deliveries has risen from 2.43” x 8.875” 5 percent to over 30 percent nationwide. With that rise, so rose the rate of VBAC. Unfortunately, over concern for litigation, many hospitals stopped offering VBACs or imposed very restrictive policies limiting access. Many expectant mothers are forced to leave their doctor or even local hospitals in search of a facility and provider who support and encourage VBAC. Some dread the thought of a surgical delivery so much that they avoid pregnancy altogether.

When is It Appropriate to Take an Antibiotic? THE MAIN PURPOSE of an antibiotic is to control or eradicate a potential harmful bacterium, virus, or fungus that invades our otherwise healthy body. Antibiotics are often overly prescribed by health care providers, either due to habitual prescribing habits or through pressure from patients who demand their providers to prescribe an antibiotic for an ailment that brought them to the clinic. Little does the public know that inappropriate use of antibiotics can cause more harm than good. Antibiotics’ side effects include drug reaction, development of multidrug resistant organisms and antibioticassociated colitis.

Talk to your doctor to find out if you are a candidate and what their policy is regarding VBAC. Your health and that of your baby should always be the priority.

THE WRAPPED PRESENTS in my Christmases past extended from under the tree into the outer room. Doll houses, three-wheelers, stuffed animals, tricycles, bicycles, furniture and more took up space. I wrapped everything! I love hearing the sound of paper tearing and the squeals of happiness. Now, electronic devices and gift cards take up much less room under the tree. Ever wonder how small gifts cost more than larger ones? Not all presents are wrapped and matched with bows. Gifts like showing patience and kindness have great value. In therapy, I hear a lot about what people want and what they hope for. The top three: Someone to listen to them; Someone to do things with them; To be appreciated. We can give these gifts. Remove items from your hands, look at the person (patience). Invite someone to go somewhere (kindness). Say something specific about something you find admirable (appreciation). Giving these three things is all about your presence; there just isn’t any substitute for you. When we make our lists and check them twice, maybe we can add patience and kindness. That is the wonder of these holidays— the example set before us in the nativity scene, God giving His presence, which is our hope.

Strong Legs Beat Weak Ears AS WE AGE, problems with balance become increasingly common. Who doesn’t know someone in their 70s who looks like they are teetering on the verge of a fall? The reason for this is usually a combination of factors. However, inner ear problems are a major factor in balance dysfunction and become increasingly common as we age. Inner ear problems in younger age groups can make your life miserable. However, usually younger patients with inner ear balance problems do not fall. On the other hand, for older patients, inner ear balance problems are a common cause for falls. The most important reason why older patients fall and younger patients don’t? Strong legs. Falls in the elderly are a huge cause of morbidity and mortality. Fractures of the long bones in the elderly, especially of the hip, result in prolonged period of immobility and an increased death rate. A recent Finnish study reported that among hip fractures in those over 65 years old, the death rate is 27.3 percent within the first year and 79 percent total mortality for the study duration. In our practice, we see many older patients with balance problems, vertigo and dizziness. Our goal is not just to improve their inner ear balance function, but to improve their overall balance and prevent falls. What we recommend varies from non-medical treatments, medications and sometimes surgical treatments to attain this goal, depending on the individual case. However, one thing that we recommend to all patients is to walk every day in order to strengthen their legs. Strong legs will help keep them from falling, even when the inner ears are giving them problems. Strong legs beat weak ears.

ON THE SURFACE, Dr. Kimberly Williams is a human dynamo, constantly in motion. After beginning each morning with a 5:45 workout regimen, Dr. Williams spends her day treating nephrology patients across the Northshore. With offices in Abita Springs, Madisonville, Hammond and Denham Springs—along with hospital rounds and overseeing the recent opening of A Place of Our Own, a pediatric day health care center for medically fragile children—her swift pace may seem a bit overwhelming to some. However, once she enters an exam or hospital room, everything changes. Dr. Williams spends as much time with her patients as they need. From an early age, Dr. Williams knew she wanted to become a physician. When she was growing up, her parents Drewrome and Patricia Williams would take her and her siblings to volunteer at local hospitals and with organizations like the Red Cross. “I think they tricked us because they wanted us to fall in love with the medical profession” she joked. “It didn’t really work for my brother and sisters, but it sure worked for me!” Originally thinking she would specialize in cardiology, she changed her mind after taking a nephrology rotation the last semester of her intern year. A field of medicine that concentrates on the treatment of kidney problems and renal replacement therapy, Dr. Williams said she knew immediately that nephrology would become her passion. “There is a continuity you have with dialysis patients that I enjoyed. I liked getting to know them; seeing them on a regular basis.” Following her residency, Dr. Williams applied and was accepted into the Nephrology Fellowship Training Program overseen by LSU School of Medicine and Ochsner Hospital. She completed her two-year training in 2006. Because both doctors and patients were displaced during the aftermath of Hurricane Katrina, she took a position at Montgomery Kidney Specialists in Montgomery, AL. As natives of South Louisiana, she and her husband Darian Clark (whom she married earlier in 2006) knew they wanted to return to the area as soon as the New Orleans medical community stabilized. They moved to the Northshore in 2008. During their time in Montgomery, Darian pursued a business venture that has since expanded into the New Orleans area as well. An entrepreneur by nature, Dr. Williams said that within one week of searching for a smoothie shop in Montgomery and finding none, Darian had developed a business plan to start his own. The first

Smoothies & Things opened in 2007. There are now two locations in Montgomery, one in New Orleans and one on the Southeastern campus in Hammond. Dr. Williams explained that Darian’s entrepreneurial spirit inspired her to start her own nephrology practice in 2010. Her offices in Madisonville and Abita Springs are in cooperation with Dr. John Angelo and Dr. Cheryl Brown. The three also run Abita Square and Renal South dialysis units. Her offices in Denham Springs and Hammond are run with the assistance of her Nurse Practitioner Sarah Callaway. “While I divide my time between the offices, I make sure there is always someone on call and available to handle any medical emergencies that arise. This allows me to address the broad need for nephrology care in the area while still providing the personal care I want to give my patients,” she said. Patients are referred to Dr. Williams through their primary care physicians. During the initial evaluation, she assesses kidney function and consults with her patients as to next steps. Because there are different types of kidney disease, underlying causes, and varying degrees of severity, Dr. Williams works with each patient individually to determine the best course of action. Dr. Williams said patients who have kidney dysfunction as a result of over the counter non-steroidal medicines such as ibuprofen, can see improvement once the medicine is discontinued. For patients with advanced kidney disease, however, dialysis may be the only treatment option until a kidney transplant is performed. When that is the case, Dr. Williams schedules a series of educational sessions to prepare her patients—both physically and psychologically. “Part of my job is to show patients that dialysis doesn’t have to be as traumatic as they fear. The internet is filled with negative, and often outdated, stories from dialysis patients. We clear up misinformation, and very clearly go through what they can expect.” “In some cases, patients can even receive dialysis treatments in their own homes. Some continue to work and go to college, adjusting their activities to their dialysis treatment schedule,” she added. “I want all of my dialysis patients to understand that they can still be able to live relatively normal lives.” Dr. Williams is also helping to bring a sense of normalcy to parents of medically fragile children. This past summer, she worked with local medical administrator Tammy Oser to open A Place of Our Own Pediatric Day Health Care Center in Covington. The center serves medical needs children, six-months to five years of age who are not eligible for enrollment in regular day care facilities or pre-schools because they require medical intervention throughout the day. Pediatrician referrals are required for admission. Explaining her reasons for getting involved with A Place of Our Own, Dr. Williams again credited her parents, and her early exposure to community service. “They taught us compassion and the importance of helping people with different needs” she explained. “It was another opportunity to help fill a need in the community.” Dr. Kimberly Williams’ office is located at 397 Highway 21, Suite 601, Arbor Walk Plaza in Madisonville. Annual Gift Tax Exclusion

WHAT IS THE “Annual Gift Tax Exclusion Amount” (AGTE)? This is the amount that anyone is free to give to anyone without having to report the transaction to the IRS. The amount for 2013 (and 2014) is $14,000 per person. There is no limit as to how many different people you are able to make this gift to and absolutely no requirement that the recipient be related to you. The $14,000 AGTE applies to both husband and wife, so a married couple can actually gift $28,000 to each person, per year. If you gift someone more than the AGTE amount, then you will need to file a federal gift tax return (Form 709). However, no gift tax will be due as long as you have not exceeded your “lifetime” gift amount, which is $5.25 million in 2013 ($5.34 million in 2014). This is also known as the “applicable exclusion amount” (AEA) for estate tax purposes. A husband and wife each have their own AEA, so combined they may leave their children (or anyone else) $10.5 million ($10.68 million in 2014) free from any federal estate taxes. However, the IRS keeps a lifetime accounting of the “gifts” made over the AGTE amount (through the Forms 709) and those gift amounts will be deducted from the AEA at death.
Some gifts are exempt from any gift tax requirements so their value is unlimited. This includes: gifts to spouses (if they are U.S. citizens), gifts to charities, and gifts covering someone’s medical and/or tuition expenses as long as it is payable directly to the provider or institution. The AGTE is a “calendar year” gift, which means you may gift $14,000 on December 31, 2013 and then gift another $14,000 the next day, on January 1, 2014. However, the gift must be completed (meaning the check must be cashed) in the calendar year. Lastly, you may not take a deduction on your income taxes for making an annual gift. The benefit is that neither the donor, nor the donee, is required to report the transaction on their tax returns. Your proof of the gift is as simple as your canceled check with a memo note of “annual gift.”

Mother Nature’s Gift of Green Tradition WRITTEN BY ANNA RIBBECK
WE CAN THANK our ancient ancestors for passing down holiday traditions associated with Mother Nature. Starting with the most evident symbol of Christmas—the tree—we go to the most ancient societies and find trees that stayed green used as symbols of eternal life. The original trees were either spruce, pine or fir and the traditional decorations were edible fruits and nuts with candles for light. Today many people still use cut-live trees but they go up earlier and are adorned with glass ornaments, ribbon and electric lights. Here are some tips if you plan to use a live-cut tree this year. Either cut or purchase your tree at least a week before you plan to bring it indoors. Season the tree in a bucket of treated water—a can of sprite added to the bucket works nicely. If you purchase a tree, make sure to have the tree lot remove at least a half inch of trunk from the bottom of the tree before you bring it home to the water bucket. This will help the tree to take a nice gulp of water! Also hose the tree
down to remove dust and pollen that may be attached to the needles. This will help a bit with allergy issues. I suggest using some natural decorations this year. String some cranberries and popcorn to use as garland. Kumquats would make beautiful, fragrant ornaments along with small apples, limes and lemons. Use pretty ribbon and greening pins to secure these to the fruit and hang on the large branches. Oranges stuck with whole cloves also make a wonderful ornament and smell delightful. Of course, I would still go for electric lights in lieu of candles—just safer! After Christmas, your tree or live-cut decorations can be placed outside in the garden for birds and wildlife to enjoy for a while. They love the popcorn-cranberry garlands! This can be a real time of togetherness and learning a little of history at the same time. Thanks again to Mother Nature and her ability to keep the distant past and the present tied firmly together in the most beautiful of all garlands. Happy holidays!

W is for Wanting More WRITTEN BY ALEX PERCY
IN HER MOST RECENT alphabet mystery, W is for Wasted, Sue Grafton brings Kinsey Milhone back to life in all of her minimalist glory. Living alone in a small garage apartment in Santa Teresa, next to her 88-year-old landlord, Milhone is up to her neck in death. The story is set during the summer of 1988 just as the materialistic eighties are waning but nonetheless providing a good backdrop to Milhone’s own moral ethics and the disenfranchised she sets out to help in this latest mystery venture. Milhone learns of the death of a homeless man who has her name and phone number on a scrap of paper and teams with his friends (also homeless) to figure out why he needed a private detective. Once it is revealed that the man left Kinsey his entire $600,000 fortune, the focus shifts to his grown children, who he disinherited and who fight to get at his inheritance. In a subplot, Grafton elaborates on the theme of greed, with the story of a morally corrupt private eye who is trying to swindle an even shadier scientist trying to hold on to a governmental grant. Grafton weaves all the stories together as if the overall novel were a fine woven blanket. However, the introduction of Milhone’s relatives detracts from the overall story. In trying to work Milhone’s personal history into the novel, Grafton creates an awkward plot point that is best left out of the novel given that Milhone herself isn’t much of a family person. Milhone has a better chance of bonding with Ed, the newly acquired Japanese Bobtailed cat, than with family.

Eggplant Risotto Start to finish: 45 minutes Servings: 4 Ingredients: 1 eggplant (may be chopped into 1 inch cubes if preferred) ¾ cup Marc de Champagne vinegar ¾ cup olive oil Bonsecco 1 tablespoon butter 1 cup Arborio rice (or 1 pack risotto melanzane - risotto with eggplant) 4 cups vegetable stock (chicken stock may be substituted)
1/3 cup soft goat cheese 4 tablespoons flaked almonds 20 mixed olives, sliced Variation: Include 3 ounces of chopped ham Preparation: Cut the eggplant into thick, angled slices and place them in a large shallow dish. Mix 8 tablespoons vinegar and 8 tablespoons olive oil for a marinade, pour it over the eggplant and let marinate for about 10 minutes. Melt the butter in a pan. Add the risotto and cook until the grains have a glassy look. Deglaze with 4 tbsp vinegar. Then add 1 cup vegetable stock, bring to a simmer and add the next cup of broth just as the previous one is absorbed by the rice. Repeat until the risotto is cooked. I LOVE EGGPLANT! Commonly known in England as aubergine, it has always been a staple at our holiday dinners. Each year as the holidays roll around I am always in a quandary on how to keep traditions alive, yet wanting to introduce something new to keep my time in the kitchen a bit more enjoyable and interesting. I am blessed that my family is very good at indulging my whims and willing to try whatever I place on the table. So this holiday season my eggplant will be cooked with an added twist. The test for success will be the empty plates and sighs of contentment heard around the table. From my family to yours – have a happy and safe holiday.

AS I REFLECT on my second year as your Parish President, I am grateful to have the opportunity to serve St. Tammany in a leadership role. The vision we are working to bring to life for St. Tammany is underway, and we will continue that vision as we enter 2014. A second location of the Federally Qualified St. Tammany Community Health Center was opened in Covington. This will strengthen public health care as well as enhance mental health services. As St. Tammany’s first ever Economic Development Director, Don Shea is focused on the economic enrichment of our Parish. Wensel Conroy and her team at the newly formed Department of Culture, Recreation and Tourism, have overseen improvements to Camp Salmen Nature Park in Slidell and worked with the
Department of Facilities Management to rehabilitate the St. Tammany Fishing Pier. Our Parish has placed drainage and infrastructure at the forefront, completing several extensive projects intended to make us less vulnerable to weather events and to make our roads more user-friendly. The Lacombe Roundabout was recently completed, giving drivers a congestionfighting tool at a major interchange. The entire staff of St. Tammany Parish government worked diligently in 2013. The goal of St. Tammany Parish government is to continue to serve our citizens through customer service, transparency, accountability and cooperation. We plan to stay on course in 2014. CHRISTMAS! The very word brings joy to our hearts. No matter how we may dread the rush and the long Christmas lists–when Christmas comes there is still the same warm feeling we had as children. For many, that joy is Ponchatoula with store windows decorated, white lights sparkling and illuminated ornaments hanging from every light post. This special holiday season begins with the Ponchatoula City Lighting ceremony when our Mayor, Bob Zabbia, flips the switch illuminating the city just a few minutes after Santa himself arrives on a Ponchatoula fire truck. Thousands come to toast the Advent season and share in the warmth of a small-town celebration. Ponchatoula shops & businesses stay open late and many offer yuletide cheer in the form of
delicious snacks and refreshments. Santa will again visit Ponchatoula mounted on his sleigh in the annual Christmas parade this year themed “Santa’s Workshop.” Floats, as well as local school bands, marching groups and dance teams, all process down Pine Street in their holiday garb throwing candy, trinkets, beads and other treats both kids and adults will enjoy. The pride of parents, grandparents, neighbors and even visitors from far away can be seen on the faces of the participants and viewers alike. This is the time to show your love, generosity, devotion and to partake in the beauty and joy of the season. Share time with family and friends in the glow of Christmas. Come to Ponchatoula and enjoy the warmth that enfolds our hearts and our home.

Delacroix, Partner at Adams and Reese LLP; W.L. Trey Folse III, St. Tammany Parish School Superintendent; Bret G. Kolman, Chief Executive Officer for Lakeview Regional Medical Center; and George “Townsend” Underhill IV, Senior Vice President of Development for Stirling Properties.
John Donahue, President and Chief Executive Officer of DonahueFavret Contractors will be our 2014 chairman. As part of his incoming duties, John has appointed four one-year advisory board members to round out our membership representation in the year ahead. Mr. Frankowitz will conduct research and interviews with the mayor, city attorney, department heads and members of the former Charter committee to consolidate a list of recommendations and issues. Public hearings for input will be held in January 2014.
