Are you a City and County of Swansea resident?

Yes Are you in receipt of any of the following?

Housing Benefit?

Council Tax Reduction?

Income Support?

Income Related Job Seekers Allowance?

Income Related Employment and Support Allowance?

Guaranteed Pension Credit?

Working Tax Credit and have a current NHS tax credit exemption card/certificate?

Child Tax Credit and have a current NHS tax credit exemption card/certificate?

A HC2 certificate under the NHS Low Income Scheme?

Or are you?

A partner of someone claiming one of the above benefits living at the same address and included in their benefit claim?

Under 17 years old and have a parent/guardian who is in receipt of one of the benefits listed above?

Aged between 17 – 19 in full time education (non-advanced) or enrolled on an approved training scheme and included (named) in your parent/guardians benefit claim?

A young adult (under 20) in full time education or enrolled on an approved training course and being supported by Social Services?

How to apply?

Pop along to one of the following venues:

• Active Swansea Leisure Centres – Penlan, Morriston, Penyrheol, Cefn Hengoed, Bishopston (photos will be taken free of charge) • Libraries – Swansea Central, Morriston, Gorseinon, Oystermouth, Townhill • Grand Theatre • Civic Centre And take with you: 1 Completed application form (inside this brochure).

2 Proof that you’re receiving a PTL qualifying benefit, included in the benefit claim or enrolled on approved training scheme such as a recent letter.

3 Separate proof of your City and County of Swansea address (a recent Council Tax/utility bill).

4 One recent passport size photo per person (no photo required if you are enrolling at Active Swansea Leisure Centres or renewing – unless your appearance has changed).

5 £2.00 fee per person.
