What is Urinary Incontinence? Urinary incontinence is estimated to affect 12–17% of US males, with increasing prevalence associated with aging. Stress urinary incontinence (SUI) is defined by the International Continence Society as the complaint of involuntary leakage on effort or exertion, or on sneezing or coughing. Prostate surgery or radiation are the most common cause of male stress urinary incontinence. The good news is there is hope for patients who often are suffering in silence. Surgical therapy should be considered in men with post-prostatectomy stress urinary incontinence (SUI) that persists beyond the first postoperative year, or even earlier in men with
severe symptoms. Other patients with severe urinary incontinence or with neurogenic bladder can also benefit from the treatment. The surgical device aims to prevent involuntary urinary leakage during storage by increasing bladder outlet resistance while allowing unimpeded flow during voluntary urination. The “Gold Standard” surgical treatment, to date, is the artificial urinary sphincter. How the Device Works? The artificial urinary sphincter (AUS) is a surgically implantable device used to restore urinary control in men. Most commonly, the device is implanted in men who have sphincter or valve muscle damage following
prostate surgery. The entire device is implanted inside the body in a brief outpatient surgical procedure lasting approximately one hour. With appropriate evaluation for the proper selection of candidates for the AUS, the long-term results in experienced hands are excellent. One of the leading artificial urinary sphincter implanters and authority in the country, Dr Angelo Gousse has performed more than 500 AUS implants, making him the number one implanter in Florida. Components and Function The AUS consists of three components: the cuff, which goes around the urethra, the pump, which goes inside the scrotum, and the balloon,

which holds the fluid for the device (see figure). The balloon is available in different pressure ranges and is filled with sterile salt water that is very safe even if it leaks. The device works hydraulically, with the cuff around the urethra staying closed at all times. When the person wants to urinate, the pump is squeezed and the cuff opens. Automatically, in 35 minutes, the cuff closes. Technique of Implantation In the Male Insertion of the AUS is performed in a hospital operating room under either a general or a spinal anesthetic. Two small incisions are made: one in the groin area and the other between the scrotum and the rectum. The properly measured size cuff is
placed around the urethra and the tubing from the cuff is passed up to the groin area. Recovery is less than 2 weeks. An Excellent Track Record The AMS 800 has been
proven to be effective in the treatment of male incontinence following prostate surgery, and is considered the gold standard by most urologists. When using this device,

most men are dry, with only minor leaks or dribbles of urine, usually with strenuous exercise or exertion. Most men use one pad or less per day to manage these minor leaks. Although long-term outcome data is currently being accumulated, the procedure appears to be most effective for mild to moderate male urinary incontinence. Are Parents To Blame For Their Child’s Obesity? Most parents don’t let their children blame others if the child is doing something that is the child’s fault. They tell their children to own up to their mistakes and find a way to fix them. When it comes to the childhood obesity epidemic in the United States, parents should take some of their own advice and realize that they need to fix the mistakes they are making in guiding their children’s nutrition, says Dr. Joseph Galati (www.drjoegalati.com) author of Eating Yourself Sick: How to Stop Obesity, Fatty Liver, and Diabetes from Killing You and Your Family. He understands that parents want the best for their children in all aspects of their lives but he says too many parents do not take nutrition seriously when they allow their children to make poor food choices. “Obesity - with all its tremendous complications - will directly decrease the very oppor-
tunities they are working so hard to provide for their children,“ he says. The root causes of obesity are poor food choices, excessive snacking and large food portions. “Parents who serve their children fast food and don’t prepare home-cooked meals are fostering bad health that can last a lifetime.” Through a combination of poor eating habits and little exercise, Galati says today’s parents are setting up their children to live their lives as unfit, obese and unhealthy. As a result, today’s generation will probably have a shorter lifespan than their parents. The problems start early, he says. A child who is obese at age 2 has a 50 percent chance of being obese as an adult. But Dr. Galati says all is not lost. He offers several tips for parents who want to stop the trend of both adult and childhood obesity. Reinstitute the family dinner.
Several studies have shown that regular family dinners at least a few times a week have many benefits for children, including improving their eating habits, grades, family relationships and overall health, Galati says. Children who eat with their families also display fewer risky behaviors and have a lower risk of obesity. Fight for home economics classes in high school. The high school home economics class taught the basics of food nutrition and cooking. Galati says the elimination of these classes across the country is likely one of the reasons many children don’t know where their food comes from. “We have raised an entire generation that is disconnected from the kitchen,” he says. Don’t underrate obesity’s impact on health. Obesity is the root cause for 13 different cancers, cardiovascular disease, fatty liver and cirrhosis, diabetes, obstructive sleep apnea, erectile
dysfunction and many more. Obesity has more than doubled worldwide since 1980. “It should be taken more seriously,” Galati says. Improve your health IQ. It’s a cheerful and affectionate breed. But the Xoloitzcuintli also can be stubborn and headstrong. The dog and the president could end up quarreling right in the middle of a Rose Garden signing ceremony, making for bad optics. Portuguese Water Dog. This breed is hypoallergenic, something the notoriously germaphobic president could appreciate because such dogs don’t shower you with dust and dander, and there is less dog hair all over the furniture and floors, Rotonda says. Why it’s a longshot: The president’s immediate predecessor owned Portuguese water dogs, which could be a deal breaker. GoldenDoodle. A GoldenDoodle is technically a mutt, but it’s also a designer breed that America loves, Rotonda says, so if Trump wants to go the populist route this is the answer. On a more practical note, Rotonda says, GoldenDoodles are another hypoallergenic breed, but minus the recent Pennsylvania Avenue history of the Portuguese water dog. “Just playing with or petting a dog can go a long way toward relieving all the strain and tension that come with the presidency,” Rotonda says. “Plus, unlike the rest of the world, a dog won’t judge you no matter what you say.”

What Does Mailbox Money Have To Do With Retirement? Remember when you played Monopoly as a kid and itched for your turn to be the banker, dealing out all that cash to your friends who were investing in properties such as Park Place and Tennessee Avenue? These days, some people are figuring out how to be the bank in real life to provide themselves with more stable and predictable retirement income. Here’s how they do it: As an alternative to a 401(k) account or investing in the stock market, they become private lenders for people who are investing in real estate. And just as borrowers send monthly payments to a bank or mortgage company to repay the loan, these private lenders receive a monthly check in the mail as repayment with interest – mailbox money. “Have you ever watched those TV shows where people are flipping houses?” asks Jadon Newman, CEO of Noble Capital (www.Noble capital.com), a financial advisory firm. “Private lending is how a lot of flippers are able to flip houses. They end up making a profit, but so do the people who loan them
the money.” The concept of private lenders fronting the capital for a real estate investment is called hard-money lending, and the loans are short-term – typically 12 to 18 months. For some people, it’s a viable alternative to other means of investing. “Real estate investment provides more certainty and predictability than many other forms of investment, especially the stock market,” says Newman, whose company matches private lenders with real estate investors. “Nothing against the stock market, but there’s a lot of risk there.” Newman says a few advantages to “being the bank” and becoming a private lender for real estate investors include: Low risk. Certainly, real estate has its risks, but those can be greatly mitigated if the projects are structured the right way. Newman says that when his company works with investors, the loans are written at 70 percent loanto-value or lower, giving the investors 30 percent protective equity to insulate them against market volatility. “Since the investment is backed by a
tangible asset, there is less speculation and, therefore, greater security,” Newman says. Steady cash flow. With fewer companies offering pensions, one problem many retirees encounter is they don’t have that monthly mailbox money coming in. Investors who act as private lenders do receive a check each month. Potential for good returns. Borrowers seeking hard money loans often do so because they can’t get conventional loans, or because the ability to obtain money is much quicker. As a result, hard money lenders can charge higher interest rates than what the rates would be for a conventional loan. For example, Newman says the investors he works with usually get a 6 to 8 percent return. “It’s a predictable, reliable return,” he says. “The biggest worry people have in retirement is that they are going to run out of money,” Newman says. “That’s why it’s important to explore different ways to get a good return on your money, and a lot of people are finding that ‘being the bank’ puts them on the right side of the equation.”

7 Tips for Fathers Raising Daughters In Today’s World As Father’s Day (June 17) rolls around, we are mindful that it wasn’t that long ago when a father’s job was to attend his daughter’s pretend tea parties and make her a pretty dollhouse. But today’s fathers must navigate a new world and avoid gender specific roles. “The fathers of today’s girls need to be engaged, creative and flexible,” says Sheri D. Engler, author of The Pearls of Wisdom: A Fairy Tale Guide to Life’s Magic Secrets for All Ages (www.thepearlsof wisdombook.com. All proceeds go to the Angel Whispers Foundation, which was organized to empower young females.) She says today’s dads should encourage girls at every opportunity. “Women are breaking down many barriers, but there is still a lot of work to be done,” Engler says. “Today’s fathers play a critical role in their daughter’s selfimage and can be instrumental in encouraging them to challenge the status quo.”

Treat women with respect. Your daughter is watching how you treat women, and it will have a lifelong impact on her in ways you can’t imagine. You must be cognizant of your attitude and behavior around women at all times. This includes not saying negative things about her mother if you are divorced. See yourself as a role model of the kind of man you would want your daughter to marry one day, because that frequently influences how women choose their partners.
Introduce her to books with girl heroes. There are plenty of books to choose from, so when reading to her let her enjoy a variety of adventures including books with strong female leaders like The Pearls of Wisdom or other books that were written to empower girls. This will send the message that you believe in her. Or perhaps help her write a story of her own imagining herself as the hero or lead character. If nothing else this will give you a valuable view of how she sees herself.
Give her compliments about things other than her beauty. It is OK to tell your daughter she’s pretty, but that should not be the only compliment you give her. Compliment her intelligence, her resourcefulness, her imagination, her many skills, her hard work, and her strength. Honestly tell her the unique things you love about her, the things that make her a good and special person.
Share music with her. Play your favorite music and tell her why you like it and let her do the same. Take her to concerts. If she wants to play an instrument, help her learn how. Music, and the arts in general, can be very bonding experiences.
Teach her the magic of self confidence. Teach her about the power that comes from believing in herself and believing she can achieve greatness in the world, while understanding that there are many girls who simply want to be a “mommy” like theirs , which is the most

6 Ways Women Can Empower Themselves And Inspire Others Women have faced many challenges throughout history, and the list is long of those who overcame adversity and became influential, inspirational figures. Often, though, a young girl doesn’t have to read about famous females to find her heroes. Her mother’s actions while overcoming challenges and taking care of the family can influence and even help transform that daughter’s thinking well into her adult life. It is a celebration - and like Veteran’s Day and the recently passed was Memorial Day we give thanks for our freedom, but also celebrate our country. As the holiday certainly brings back memories of our childhood - it is a time to create new memories, with friends and family. While Aventura resident Colin O’Malley doesn’t look forward to the hurricane season - he does very much embrace the summer and the Fourth of July. “What we have done for the past four years is to host a party for two days at our house,” O’Malley said. “We have a pool, barbecue and a place to go in case of rain or too much sun. At night, it’s all about fireworks. From our house, you can see some local events - as well as those from miles and miles away.”
IT’S ABOUT THE FOOD What the holiday comes down to is fun. Whether you grill chicken wings in the backyard - or take your burgers to the beach - it’s all about food, family and fun. Speaking of burgers, here is a recipe to try. You will impress everyone around the grill. CLASSIC BURGER • 1 lb. lean ground beef • 3 Tbsp. Tomato Ketchup • 2 tsp. Worcestershire Sauce • 1 tsp. Garlic Powder • 1 tsp. Onion Powder • 1/2 tsp. Pure Ground Black Pepper • 4 slices cheese • 4 hamburger rolls Mix ground beef, ketchup, Worcestershire Sauce, garlic powder, onion powder and black pepper in medium bowl until well blended. Shape into 4 patties. Grill over medium heat 4 to 6 minutes per side or until burgers are cooked through (internal temperature of 160°F).

With June 21 officially being the day where we recognize that it is indeed summer, you can feel it in the warm air, nightly thunderstorms and weekend outings. What makes this area of the country so special during the summer, are the events - and nobody can put on a show like South Floridians. From Miami to Fort Lauderdale and everywhere in between, summer signals beaches, pools, barbecues, and of course, the Fourth of July. When it comes to this holiday, we just cannot use that one day. What we - as residents of Miami-Dade and Broward have become is an event area. We take the Fourth of July and turn it into a weekend if it happens to fall on a Friday, Saturday, Sunday for Monday. If it’s in the middle of the week, like this year - happening on Wednesday - we use the entire week to celebrate America and the great summer ahead. “I love this time of year,” Alison Morris
WHERE THE FIREWORKS ARE Because we are on the coastline, and the beach is a tremendous destination to watch fireworks in the evening, mostly every seaside community has a display or a celebration. LIFESTYLE 4 Skills Your Child Needs To Know Before Entering Kindergarten Kindergarten is supposed to be fun and educational, and allow children an opportunity to blossom because it is usually the first time the tykes are away from their parents for any significant amount of time. And while most kindergartens don’t exactly have GPA requirements, for a child to succeed there are a few milestones that should be reached before they toddle through the door, says Alise McGregor, founder of Little Newtons (www.littlenewtons.com), an early education center with locations in Minnesota and Illinois. “New parents especially can have a hard time understanding that there are actually things their child should know prior to kindergarten,” McGregor
says. “But if they take time to make sure their child knows some basics, it will make kindergarten a better experience.” McGregor says the typical things a child should know prior to entering kindergarten are: Basic shapes. The child should have learned basic shapes like the square, circle, rectangle and triangle in preschool. If not, parents should teach them. In kindergarten, they will probably be introduced to the hexagon, star, heart and octagon. The letters of the child’s name. They should at least be able to recognize the letters of their name. In kindergarten, they will learn their upper-
case letters and begin lowercase letters. They will also begin to learn how to write freehand without tracing. Numbers 1 to 10. Although some children will be able to count to 100 by the time they reach kindergarten, all should be
able to at least be able to count to 10. Numbers will be used starting on the first day of kindergarten so the children need to at least understand and recognize numbers. Social skills - The child should be able to follow directions, be able to be
separated from the caregiver and use the restroom independently. Children who scream and cry when they are separated from caregivers disrupt the rest of the class. Teachers simply do not have the time to supervise individual students who cannot inde-
pendently use the restroom. McGregor says that it is important for children to get off on the right foot early in school. By teaching children these skills prior to attending kindergarten it should make it easier for them to succeed.

5 Ways A Healthy Gut Makes A Healthy Brain The idiom “trust your gut” means relying on intuition, rather than thoughtful, deep analysis, to make a decision. But research shows there is actually a tangible connection between gut health and brain health, and that linkage can affect emotions and cognitive processing. Research conducted at the California Institute of Technology by Elaine Hsiao showed how unhealthy or healthy microorganisms in the stomach can influence behaviors differently. Another study, led by Kirsten Tillisch at UCLA, suggested probiotics can have a positive effect on behavior, mental outlook and brain function. “Scientists have now determined that humans have two brains; the second one resides in the gut and is called the enteric nervous system,” says Richard Purvis, author of Recalibrate: Six Secrets To Resetting Your Age and CEO of Skin Moderne Inc. (www.skinmoderne.com). “It has more neurons than the spinal column or central nervous system. Understanding the relationship helps to clarify why the process of taking care of the gut and the brain within it also helps improve the health of the brain in your
head.” Given Americans’ notoriously poor eating habits, Purvis says gut health has never been more important. A Tufts University study estimates that over 318,000 deaths a year – or nearly half of American deaths caused by heart disease, stroke and diabetes – were hastened by unhealthy eating. “Processed foods and sugar are among the biggest culprits for promoting the growth of bad bacteria in the gut,” Purvis says. “You can greatly improve your gut health – and by extension your brain health – by being kinder to it on a daily basis.” Purvis recommends four nutritional tips and a nature trip - that benefit your gut and your brain: Daily servings of cultured, fermented probiotic-rich foods. “Sauerkraut, kimchi, kombucha pickled veggies, yogurt, and kefir encourage the growth of good bacteria,” Purvis says. “By ingesting healthy, probiotic-rich foods, you are guaranteed colony-forming units of bacteria, plus
food sources are much cheaper than supplements.” Prebiotic foods. Non-digestible shortchain fatty acids help your good bacteria flourish, says Purvis. These are found in artichokes, garlic, leeks, dandelion greens, beans, oats, onions and asparagus. A diet that keeps blood sugar balanced. “This also keeps gut bacteria balanced,” Purvis says. “A diet high in rich sources of fiber, especially derived from whole fruits and vegetables, feeds the good gut bacteria and produces the right balance of those short-chain fatty acids to keep the gut lining in check.” Gluten reduction. Reducing gluten, or avoiding it altogether, Purvis says, will further improve gut health as well as healthy brain physiology. He agrees with medical professionals who say gluten can interfere with the absorption of nutrients, hurting digestion and sometimes leading to “leaky gut,” or damaged intestine walls. Getting outside and into nature. “You need to connect with more microorganisms – the more, the merrier,” Purvis says. “Their purpose is to perform lifesustaining functions. Move outside, do some gardening, plant flowers, mow the
lawn, take a walk in the woods. Do things that connect you and your immune system with all the microorganisms in the soil.” “Lifestyle choice is considered by most the culprit contributing to our unhealthy bacteria,” Purvis says. “So you have a choice, and the one you make with your diet will affect your whole body, and not least of all, your brain.” About Richard Purvis: Richard Purvis is the CEO of Skin Moderne Inc. Tips for talking about Alzheimer’s with a family member (BPT) - It’s a conversation no family wants to have, but one that can be vitally important - talking to a loved one about memory loss or cognitive decline. Mary Foss of Eagan, Minnesota, said she knew it was time to broach this sensitive topic with her husband Darrell when she noticed he was having difficulty doing the home improvement projects he had enjoyed all his life. “Darrell would spend hours building and fixing things around the house - it was his hobby,” Mary said. “But it got to a point where he was having trouble doing even routine tasks. He would get frustrated and soon began doing less and less. I knew something was wrong, but dismissed it for a while.” As Darrell’s retreat from activities he once loved grew, so did Mary’s concern. Eventually, Mary and the couple’s adult daughter, Michele, decided to share their concerns. “We just told him that something is not right and you need to see a doctor and figure out what it is,” Foss said. “I thought maybe he had a mini-stroke or a brain tumor. We were shocked when the diag-
nosis came back as mild cognitive impairment, possibly Alzheimer’s.” The Fosses’ story is not uncommon. Talking about memory or cognition problems with a family member can be daunting for many families. Denial, fear and difficulty initiating conversations about this sensitive subject are common barriers. “Alzheimer’s disease is challenging, but talking about it doesn’t have to be,” said Ruth Drew, director of Information and Support Services at the Alzheimer’s Association. “Family members are typically the first to notice when something is not quite right and it’s important to discuss these concerns and follow up with your doctor.” During Alzheimer’s & Brain Awareness Month the Alzheimer’s Association is offering these six tips to help families facilitate conversations about Alzheimer’s and other dementias: 1. Have the conversation as early as possible - Addressing memory or cognition problems early offers an oppor-
tunity to identify the cause and take action before a crisis situation occurs. 2. Think about who’s best suited to initiate the conversation - If there is a family member, close friend or trusted adviser who holds sway, include them in the conversation. 3. Practice conversation starters Be thoughtful in your approach. For example, consider an open-ended question such as, “I’ve noticed a few changes in your behavior lately, and I wanted to see if you’ve noticed these changes as well?” 4. Offer your support and companionship - Let your family member or friend know that you’re willing to accompany them to the doctor and offer your continuous support throughout the diagnosis process. 5. Anticipate gaps in selfawareness - Someone experiencing cognitive decline may be unaware of problems. Be prepared to navigate con-
fusion, denial and withdrawal, as people may not want to acknowledge their mental functioning is declining. 6. Recognize the conversation may not go as planned - A family member may not be open to discussion. They may get angry, upset or simply refuse to engage. Unless it’s a crisis situation, don’t force the conversation. Take a step back, regroup and revisit the subject in a week or two. While receiving the diagnosis was difficult, Mary Foss said it also provided a path forward. “As difficult as it was to receive Darrell’s diagnosis, it provided an answer to what we’re facing,” Foss said. “We took a few days to grieve, but then we focused our attention on getting educated about the disease and what we can do to fight it. Heart disease. Several studies have shown a link between periodontitis and heart disease. “The same bacteria causing periodontitis symptoms like inflammation, bleeding, and bone loss around teeth can travel through the bloodstream to the arteries,” Katz says. “Bacteria can latch onto the walls of the arteries and cause small blood clots, increasing the risk of restricted blood flow to the heart.” Stroke. The findings from a study titled “Impacts Of Periodontitis On Nonfatal Ischemic Stroke” showed that patients who suffered a stroke also had evidence of an oral infection. “Research has indicated that gum disease is nearly equal to high blood pressure as a source of causing strokes,” Katz says. Cancer. “Bacteria swells the gums, and it can cause similar reactions to
other tissues,” Katz says. A study published in Cancer Research found that some of the same types of bacteria that trigger periodontal disease may also be linked to a higher risk of esophageal cancer. Another investigation, in the Annals of Oncology, found that men with an advanced form of periodontitis were 45 percent more likely to get diagnosed with cancer. “More proof you are simply endangering yourself while leaving bad gums unattended, allowing bacteria to spread,” Katz says. Erectile dysfunction. Research has suggested there’s a connection between systemic inflammation – the kind that could be caused by that traveling bacteria in your mouth – and increased risk of developing impotence. In research from Taiwan, men with erectile dysfunction were 79 percent more
likely to have been diagnosed with chronic periodontal disease. Pr os t ate-specific antigen (PSA). When the prostate becomes inflamed or infected, PSA levels increase, notes the American Academy of Periodontology. “The AAP states that men with indicators of periodontal disease tend to have higher levels of PSA, as well as more inflammation of the prostate,” Katz says. That can lead to a condition known as prostatitis, which can be manifest in painful irritation, difficult ejaculation, and urination urgency. “It’s simple: Maintaining healthy gums increases your chances of a healthy body,” Katz says. “Poor oral hygiene causes infection of the gums. It can send toxins into the bloodstream. It’s proven good oral hygiene is the most important preventative health measures one can take.”

REAL ESTATE 3 Things Retirees Should Consider When Deciding Whether To Rent Or Own A Home Home ownership has long held an honored position as an integral part of the American dream. But when retirement time comes, rethinking that dream could be in order. Sometimes renting a house or apartment is the better bet both financially and in terms of the retiree’s changing lifestyle and health, says Eric Kearney (www.erickearney advisor.com), an investment advisor for Retirement Wealth. “When people plan for retirement, they focus on things like how much they have saved, how much Social Security will pay, and whether they have a pension,” Kearney says. “But as you get older, you also need to think about such issues as whether you can keep mowing the lawn or handling other day-today chores that homeownership requires. If you must hire someone to do them for you, how much will that eat into what may already be a tight monthly budget?” The truth: There’s no answer that will fit everyone’s situation, he says. So retirees, or those approaching retirement, should weigh their personal pros and cons. “There’s a lot to think about,” he says. “Should you sell the house you raised your family in and downsize to something more suitable for just the two of you? If you’re planning to move to somewhere else in the country to enjoy your retirement, is it more prudent to buy in that new location, or is leasing the way to go to give you more flexibility if it doesn’t work out?”
Some things retirees should think about as they ponder the own vs. rent question include: Maintenance issues. When you own a home, every leaky faucet, electrical problem or faulty appliance is yours to handle as best you can. If you can do it yourself, great; but often, these household repairs mean calling in a professional at a sometimes exorbitant cost. When you rent, it’s up to the landlord or the property management company to take care of the repairs. Mobility. Selling a house can be a long and complicated process, and you never know what the market might be like when the time arrives. Whereas breaking a lease is much simpler. “If your children are scattered all over the country, you may want to move closer to one of them,” Kearney says. “Also, if your health takes a turn for the worse, selling a home can be a significant burden on your family.” The inheritance. For many people, a house is the most valuable asset in their estate and they might want to leave it to their children in the will. Once again, it’s a matter of weighing the pros and cons. “Having a home to pass down to the children is a noble gesture, but it is not always feasible,” Kearney says. “Before considering whether owning or renting is the right option,” Kearney says, “it’s essential to review all the intricacies of your situation and decide based on your finances and your overall health and well-being.”
