Drawbacks Of Hiring Your Own Aide By Terry Adler, CEO, Always Classic Care
Why should you go through an agency when you can hire your own aide and pay less per hour? Remember the age-old adage “You get what you pay for”? You may pay less for your aide hourly, but when you hire independently, do you really know what you are getting yourself into? These are just a few of the complaints that I have heard from patients who contacted us after they had bad experiences with private aides whom they had hired on their own: • Things seemed to disappear too often • She didn’t listen to what I wanted • She got so bossy that I felt like I was the employee • She kept telling me her problems and I felt that I needed to help her with extra money • She kept coming late and leaving early • She was on her cell phone all the time • I liked her, but she had no car and was nervous going to unfamiliar addresses, even to the doctor • She did not cook the way I like and would not listen • She didn’t buy the brands or cuts of meat that I wanted • She was nice, but not too bright • She didn’t dress appropriately and when we went out, I was embarassed With our agency, as with any other legitimate agency, we do not stand for any complaints, not any. • Our aides are professionally trained with all
documentation and references carefully checked • All aides and nurses have been screened by the FBI with confirmation that they have no criminal records • A registered nurse opens each case to assess the exact needs of the new client • The client and/or family is interviewed to determine their specifications and expectations • After the assessment and interview, staff carefully matches an aide to the personality and needs of the client • If for any reason the match is not perfect at any time, staff will make changes immediately You definitely can hire whomever you like. However, you should consider the following items carefully: • How well do you know this person? Can you trust him or her? Are you sure the aide does not have a police record? How do you know? • Is the aide insured to cover any liability if you should become injured while in their care? By hiring through an agency, you can be certain that liability covering the aide is in place. • Does your aide have a legal right to work in this country? That translates to a Green Card, Permanent Residency or actual citizenship. • Will you be paying your share of their Federal taxes? Our agency does. We report and we pay, as good citizens should.

Many people are unaware of their responsibility to IRS when hiring a household employee, or of the aide’s legitimate status to work in the US. However, frequent news reports feature innocent homeowners who get into trouble simply by not reporting as an employer to the Federal government. Our aides are our employees and we report their earnings and our share to the IRS. Avoid heartache or trouble for you or your loved one. Always consider hiring through a professional agency. Not only do you get careful matching, supervision, and an easy way to make changes that are best for you, but you also get a strong element of protection by ensuring that your aide is legitimate to work, has no criminal record, and has a degree of liability protection as is required by the home health agency. These are all reasons for the slightly higher cost. Protect yourself by preventing problems. An agency is your safest way to go. Even the home health aides know that, and although they accept statewide standard rates that may be lower than what they could get from the client privately, reputable aides know that by working through reputable agencies, they also are protected and can be steadily employed with respect. Hope this has been helpful. With my 20+ years of experience as a cardiac and home health nurse, I will be happy to answer any questions you may have regarding home health. GRATEFUL FOR HER ‘NEW’ VISION Braverman Eye Center Patient Tsion ‘CeCe’ Etsubneh Can See Perfectly
JANUARY 2019 • AVENTURA DIGEST • PG 8
Hollywood - She may be just 26 years old, but Tsion ‘CeCe’ Etsubneh cannot express how happy she is with her vision. With a recent Lasik Surgery done at the state-of-the-art Braverman Eye Center in Hollywood, “CeCe” wanted to brag a little bit about her life changes. For the past 23 years, she has suffered with poor vision, having to wear glasses and contacts along the way. Because youngsters are subjected to being made fun of by other students, school was never much fun. “When you are a kid, all you know is that a majority of the other children are not wearing glasses and not being made fun of all the time,” she explained. “When you are that young, there is no understanding or compassion. Your first instinct is to make fun of something that is not familiar to you.” Children all over the world, who suffer with vision problems endure a lot of tough times while growing up, and CeCe, who wore contacts for years, was no different. It was always a battle of emotions - and often fighting something she had little control of at the time. From trips to Hawaii to scuba diving to other things that wearing corrective lenses would hold her back from, CeCe was tired of the restrictions - and decided to do something about it. Years ago, her stepfather had the pleasure of crossing paths with one of the best, Dr. Stanley Braverman, who recommended him for surgery. The experience was such a positive one, that when CeCe’s high myopia (nearsightedness) got worse, there was no other choice, but to turn to Dr. Braverman for help. No matter what age, when it has to do with our eyesight, it is always that fear of the unknown. It’s only natural to feel apprehensive when it comes to our eyes. “I cannot tell you how amazing the entire experience was,” CeCe explained. “Everything about Dr. Braverman and his staff was first class and truly something that changed my life. Many my age do not have this done. Not so early in their life.” From the initial meeting with Dr. Braverman - to a sit-down with Rosemary Hartford (Refractive Surgery Coordinator) going over the entire procedure - CeCe became a different person. After watching her stepfather have Lasik done, she was sold. “The procedure was quick and I never felt anything,” she recalled. “The best part

Because of Dr. Stanley Braverman and the staff at Braverman Eye Center, the 26-year-old is seeing better than ever. of it was a few days later, I was ready to do everything that I had done before - and much more.” Having the surgery done - especially at such a young age - you tend to notice how things change around you. Something so common as getting out of the house in the morning, reading signs and billboards and just objects around you. “Anyone who is ever faced with this decision - like I was - do your research and see what is best for you,” she pointed out. “I fully recommend Dr. Braverman and his staff. They are professional all the way. For CeCe Etsubneh, a lifetime of rollercoaster emotions have been put to rest with a visit to Hollywood and the Braverman Eye Center.
Z-LASIK SURGERY What To Expect With Z-lasik Surgery All Laser Z-Lasik surgery is performed on a patient’s cornea, which provides most of the focusing power when light enters the eye. Topical anesthetic drops are applied to the eye. The surgeon first uses a special Ziemer LDV femtosecond laser to create a thin layer of corneal tissue that can be folded back (a corneal flap). Then, another type of laser (an Allegretto Eye-Q excimer laser) is used to reshape the underlying cornea, painlessly. After the excimer laser treatment, the corneal flap is repositioned and bonds back into place naturally. The corneal reshaping improves the focusing power of the cornea. Although no one can promise
patients “perfect” vision, most Laser Vision Correction patients with mild to moderate prescriptions with or without astigmatism, do achieve 20/20 or better vision. About Dr. Stanley Braverman Dr. Stanley Braverman is a Voluntary Assistant Clinical Professor at the University of Miami School of Medicine’s Anne Bates Leach Eye Hospital/Bascom Palmer Eye Institute. He is also an Adjunct Associate Clinical Professor at the Nova Southeastern School of Optometry and the official LASIK and Cataract Surgeon for the Florida Panthers. Women’s Self Defense Course Can Make A Difference By Larry Blustein . Aventura Digest
Last April, Keli O’Riley was leaving a restaurant in North Miami, after a meal with some friends, when a man approached her, demanding her money, rings and all her valuables. As she cooperated with her assailant like anyone will tell you to do, O’Riley was then punched repeatedly and kicked in the stomach, head and legs. There was no way she could defend herself. She emerged battered, bruised and her confidence was shattered beyond comprehension. “He took my life away, right there in that parking lot,” she recalled. “It took me months to ever go out of the house. I was
reduced to rubble. It was without a doubt the most horrifying and humiliating thing, and it will stay with me for the rest of my life.” It was in the weeks that followed when O’Riley began to think and figure out any way she could to have avoided what followed the robbery. She played things over and over again - the entire 5 minutes that indeed changed her life. “I felt that if there was a way I could have defended myself, things would have been different,” she said. “Even after he punched me that first time - if I knew anything - but I didn’t.” What happened to O’Riley is an ongoing thing, and never knowing when and where it can
happen, the only thing for sure is to be prepared and trained for something so vicious, and yes, life altering. If you want to defend yourself and feel empowered, the City of Aventura and the Aventura Police Department want all ladies to join for a 3-day women’s self-defense course. On Tuesdays Feb. 5, 12 and 19 from 7 p.m. - 8:30 p.m. at the Community Recreation Center (3375 N.E. 188th Street), you will learn awareness, reaction training, strikes and focus of power. “As someone who has been in law enforcement since the age of 22, I can tell you that coming across women who could have avoided being attacked, self
defense training is a huge benefit,” Detective Randy Thornton pointed out. “A kick to the groin, a quick strike to the throat. Anything that will disable. We don’t want women to get in brawls. Just do enough to get away from the situation.” It is a depressing but true fact that a lot of women feel afraid when going anywhere alone these days. Of course, two men can be extremely unequally matched when it comes to strength and size, but the sad truth is that the majority of women can put up little fight against the majority of men purely because of our biological make-up. And that’s a scary thought.

“A couple of years ago, as 19year-old girl was on her way home from one of the self defense classes we hosted, she was attacked,” Thornton explained. “As she walked down a quiet, residential street in the dark, she suddenly felt a large hand on her shoulder and a man grabbing her from behind. Instinctively, her training kicked in - she turned round, kneed him in the groin and ran off.” Thornton says that it isn’t about winning a fight, it’s about doing enough to get away. “You hear a lot of people who teach self defense give you their take on what is the best way too defend yourself,” Thornton pointed out. To me, protecting your vital organs by keeping your arms close to your inside, and having your opponent’s arms on the outside is sound. Also, use your elbows to protect the side of your face if someone strikes. Keep your elbow up and pointed toward your attacker.” Thornton also suggests that if you’re attacked on the ground and feel overpowered, don’t struggle. Fake compliance, especially if the person has a weapon. When they’re relaxed, and you see an opening that’s when you attack to get away. “I highly recommend these courses they are hosting in Aventura,” Thornton said. “It will save your life - no doubt about that.” In person registration is accepted at the Community Center, Founders Park (3105 NE 190th Street) and Waterways Park (3301 NE 213 Street). Also, visit cityofaventura.com - or contact the Community Recreation Center at 305-4663883. Must be 18 or older to take part in the course.

Slow Burn Theatre Company Announces 2019/2020 Season Celebrating the enduring stories and beloved characters that have charmed audiences across genres, the Broward Center for the Performing Arts and the Carbonell Award-winning Slow Burn Theatre Company have announced the 2019/2020 Slow Burn Theatre season in the Amaturo Theater. The season features: Shrek The Musical (Oct. 25–Nov. 10); A Christmas Story, The Musical (Dec. 13–29); Groundhog Day The Musical (Jan. 31–Feb. 16); Ragtime (March 20– April 5); and Footloose The Musical (June 5–21). “These five musicals earned a total of 28 Tony Award nominations for their Broadway debuts which underscores the success of their acclaimed adaptations,” said Slow Burn Theatre Artistic Director Patrick Fitzwater
who will direct and choreograph the productions. “People first fell in love with these characters in books and films and now can enjoy another deep connection that is created by seeing a live performance.” Based on the Oscarwinning DreamWorks Animation motion picture and the book by William Steig, Tony Award-winning Shrek The Musical features all new songs from Jeanine Tesori (Thoroughly Modern Millie; Caroline, or Change) and a hilarious book by David LindsayAbaire. It’s happily from start to ever-after for audiences in this funny fairy tale world of adventure and romance when an ogre and his sidekick donkey set off to rescue a temperamental princess from her tower. Throw in a short-tempered bad guy, a
cookie with an attitude and over a dozen other fairy tale misfits and you’ve got an irreverent production that’s fun for the whole family. This production is presented through special arrangement with Music Theatre International (MTI). Slow Burn Theatre stages A Christmas Story, The Musical, from the songwriting team behind the Tony Award-winning musical Dear Evan Hansen and the Academy Awardwinning film La La Land (Benj Pasek and Justin Paul) with a book by Joseph Robinette. A boy’s Christmas present dream of an official Red Ryder® Carbine-Action 200-Shot Range Model Air Rifle is challenged by his nightmare life of humiliating pink bunny pajamas, demented department store Santas, gaudy leg

lamps and a triple-dogdare to lick a freezing flagpole. The musical is based on the motion picture A Christmas Story distributed by Warner Bros., which was written by Jean Shepherd, Leigh Brown and Bob Clark upon In God We Trust: All Others Pay Cash written by Jean Shepherd. The production is produced with permission of Warner Bros. Theatre Ventures and Dalfie Entertainment. Slow Burn Theatre presents the comedic Karma that is Groundhog Day The Musical, 2017’s Olivier Award Winner for Best Musical on London’s West End. A jaded metropolitan weatherman is stuck on assignment in rural Pennsylvania for the annual groundhog sighting only to discover there is no way out and he is stuck there reliving the same day again and again. Always ready with a oneliner or clever quip about the all-too cheery people of Punxsutawney, he is about to discover during this extraordinary day that maybe the joke may be on him. Groundhog Day The Musical features a book by original co-screenwriter Dan Rubin and music and lyrics by Tony Awardwinner Tim Minchin (Matilda the Musical) and is presented through special arrangement with Music Theatre International (MTI). Written by the awardwinning composer/lyricist team of Stephen Flaherty and Lynn Ahrens (Once on This Island, Seussical and Lucky Stiff ) and noted playwright Terrence McNally, and based on E.L. Doctorow’s distinguished novel, Ragtime is the winner of Tony Awards for Best Score, Book and Orchestrations. Ragtime weaves an American tapestry of historical and fictitious characters that tracks three families in
New York at the start of the 20th Century. The lives of wealthy men and women, immigrants, African Americans, jazz musicians, union organizers and early movie makers intertwine as they pursue life, love and oftenillusive justice. This production is presented through special arrangement with Music Theatre International (MTI). For its summer show, Slow Burn Theatre invites audiences to “kick off their Sunday shoes” with Footloose the Musical featuring the hit song from the best-selling film soundtrack. When a big city boy tries to adjust to high school life after moving to a small town, he faces a few missteps after the daughter of a reverend, who just happens to have banned dancing, falls for him and the locals decide to trip him up. This throwback to teen romance, angst and, of course, the desire to dance celebrates the wisdom of listening to young people and guiding them with a warm heart and an open mind. Footloose the Musical has music by Tom Snow and lyrics by Dean Pitchford with additional music by Eric Carmen, Sammy Hagar, Kenny Loggins and Jim Steinman. The stage adaptation by Dean Pitchford and Walter Bobbie is based on the original screenplay by Dean Pitchford. Slow Burn Theatre will also once again stage two shows in the Abdo New River Room at the Broward Center and those titles will be announced in March 2019. Five show subscriptions packages (starting at $196) and four-show subscrip-
tions (starting at $166) will be available to renewing subscribers beginning January 21 and to new subscribers on March 25. Single tickets will go on
sale in June. Buy tickets online at BrowardCenter.org or Ticketmaster.com; by phone at 954.462.0222 or the Broward Center’s AutoNation Box Office. Local Podiatrist Saves Limbs Being Destroyed By Non-healing Wounds By Marylynne Newmark, Medical Writer
Dr. Charlton Adler, three - time Board Certified podiatrist with offices in North Miami Beach, has been credited with saving the limbs of two patients diagnosed with necrotic fasciitis, a bacterial infection that quickly spreads from foot to leg, destroying healthy tissue and transforming it into gangrene. Both of these cases were misdiagnosed by other physicians who were recommending amputation. “Because of my background in VA medicine dealing with resistant nonhealing battle injuries, this type of diagnosis is very familiar to me,” says Dr. Adler, “After careful assessment, I am able to address these wounds aggressively, consequently saving the limbs.” According to Dr. Adler, this type of bacteria multiplies very quickly, destroying flesh and rapidly infecting the blood. This is the pattern of gangrene. “One case was so severe,” said Dr.
Adler, “that the attending physician recommended immediate amputation.” After explaining his previous successes with various debriding techniques, Dr. Adler took over the case, and the leg was saved. “Another patient lives in the Virgin Islands,” says Dr. Adler. “She had already had a biopsy and was told they needed to amputate quickly, but she heard about my team. By the time she flew up to Miami,” continued Dr. Adler, “ the infection had spread halfway up her leg. We treated her aggressively and were able to save it” . Dr. Adler added “Now, whenever I visit St. Thomas with my family,” he smiled, “ I am treated like an outright hero.” Dr. Adler stated that he uses various debridement techniques, as well as specialized product and other modalities unique to each particular wound. Nonhealing wounds can be related to diabetes, circulatory problems, traumatic
injuries and pressure lesions. As a podiatrist, Dr. Adler’s specialty is limited to wounds below the knee. “However, infection is a symptom of what is going on in the body,” he states. “My studies include the person as a whole, including his or her lifestyle, environment, diet and hydration, even mind, body and spirit.” Dr. Adler’s treatment includes counseling in areas beyond the immediate injury. “For example, nutrition is a major factor in the healing process,” is on staff and has surgical privileges. Exercise: Good For Your Body, But Bad For Your Teeth? Sometimes, what begins with good intentions has unintended, negative consequences. Take working out, for example. People do it to strengthen their bodies and improve their overall health, but they also can damage their dental health in the process. Numerous studies and dental professionals have found that vigorous exercise can harm teeth, gums and the jaw without proper techniques or equipment being used. The problems often start with jaw-clenching or teeth-grinding. Both are common during highintensity workouts or certain demanding sports. Someone who engages in such strenuous activities on a regular basis may have developed clenching or grinding habits, and over time they will feel the
results. “Weightlifters or others exerting maximum effort often clench their jaw, and the cumulative effect can be fractures, chips, or holes in their teeth,” says Dr. Shab Krish, author of Restore Your Rest: Solutions for TMJ and Sleep Disorders (www.krish.com). “The constant stress of lifting can also damage your jaw joints - a potentially very big problem.” Other oral health issues can surface as a result of exercise, and Dr. Krish gives tips on how to prevent them while working out vigorously: Wear an oral appliance. This puts a thin barrier between the upper and lower teeth - far better than leaving them unprotected. Dr. Krish suggests a custom-made appliance one aligning and support-
ing the jaw - by a dentist for optimal effectiveness and comfort. “You need the kind of oral appliance that not only protects your teeth, but your jaw, facial and neck muscles as well,” Dr. Krish says. Breathe through the nose. A study in the Scandinavian Journal of Medicine & Science in Sports found that mouthbreathing during exercise dries out the mouth. The result is a reduction of saliva - which protects the teeth - and that creates an environment for bacteria, leading to more tooth decay. “Nose-breathing can improve your airflow and relax your jaw and neck muscles, which reduces clenching,” Dr. Krish says. “It also has physiological advantages increasing your lung absorption capacity and helping lower blood pres-
sure. Ease up on sports drinks. Drink water for the healthiest hydration, Dr. Krish stresses. “Sports drinks refuel the body with electrolytes, but they also tear up your teeth by eroding enamel and causing cavities,” she says. A study in the clinical journal of the Academy of General Dentistry found that excessive acid in sports drinks can damage teeth after just five days of consumption. Natural coconut water without additives and bottled or tap water with lemon are healthy alternatives. “We all know exercise is great for us,” Dr. Krish says. “Dental damage while exercising is kind of the untold other side of
the story, and the challenge is to get the utmost out of physical activity

New treatment helps prevent migraines
JANUARY 2019 • AVENTURA DIGEST • PG 22
(BPT) Migraine is a disabling neurological disease that affects more than 36 million people in the United States.1 About 40 percent of people living with migraine may be appropriate candidates for preventive treatment, yet the majority of them are untreated.2 AJOVYTM (fremanezumab-vfrm) injection was recently approved by the U.S. Food and Drug Administration (FDA) for the preventive treatment of migraine in adults. AJOVY is the first in a new class of monoclonal antibodies targeting calcitonin gene related peptide (CGRP) and is the only treatment available with both quarterly (675 mg) and monthly (225 mg) subcutaneous dosing options.3 In clinical trials, AJOVY demonstrated significantly more migraine-free days compared to placebo. The most common adverse reactions in clinical trials (≥5% and greater than placebo) were injection site reactions.3 Many patients avoid scheduling activities because they fear having to cancel due to migraine attacks.4 “When I have a migraine attack, I can’t take my children to play dates, parties or other activities, help them with homework or even make their lunches,”
said Kristi Hanson, who experiences migraine attacks three to four times a week. “My kids get so disappointed when I can’t do things for and with them when I have a migraine attack.” Kristi enrolled in the clinical trial for AJOVY and experienced fewer migraine days per month. AJOVY may cause allergic reactions, including itching, rash, and hives that can happen within hours and up to 1 month after receiving AJOVY. Call your healthcare provider or get emergency medical help right away if you have any symptoms of an allergic reaction: swelling of your face, mouth, tongue, throat, or if you have trouble breathing. Talk to your doctor about stopping AJOVY if you have an allergic reaction. “Because of my migraine attacks, it’s difficult for me to say ‘yes’ to friends or family members for parties or even weekend getaways because I’m afraid that I will end up spending the weekend in bed rather than spending time with them,” said Kristi. “When I have fewer migraine days, I can keep my social plans and get to spend more time with my family, which is important to me.” AJOVY is available through retail and specialty pharmacies, and there is a
savings offer for AJOVY at AJOVY.com. Commercially insured patients may pay as little as $0 on prescriptions for AJOVY until December 31, 2019.* Patients can learn more about AJOVY at AJOVY.com. *Limitations apply. Please note, this offer is not available for patients eligible for Medicare, Medicaid, or any other public payer coverage. See full Terms and Conditions for eligibility and restrictions. APPROVED USE AJOVY™ is a prescription medicine used to prevent migraine in adults. IMPORTANT SAFETY INFORMATION Do not use AJOVY if you are allergic to AJOVY or any of the ingredients in AJOVY. Tell your healthcare provider about all the medicines you take, and if you are
pregnant, planning to become pregnant or are breastfeeding. Common side effects of AJOVY include injection site reactions. Tell your healthcare provider if you have any side effect that bothers you or that does not go away. These are not all the possible side effects of AJOVY. For more information, ask your healthcare provider or pharmacist. Call your doctor for medical advice about side effects. You are encouraged to report side effects to the FDA at 1 800FDA1088. Insurance Myths Cleared Up (BPT) - There are several myths and misconceptions that exist when it comes to insurance. For example, true or false? It costs more to insure red cars. The answer is false. Car color has no impact on insurance premiums; however, factors like your driving history, annual mileage driven, the cost of your vehicle and other things will affect how much you’ll pay for insurance. Insurance is one of the most useful investments you can make to help safe-
guard your belongings and financial assets, but it’s important to know what you’re purchasing. Here are four things you should know about auto, homeowners or renters insurance policies to ensure adequate protection in the event of an unforeseen or unexpected loss. Myth: Auto insurance covers all vehiclerelated issues Most states require drivers to carry auto liability insurance. This coverage provides financial
protection if you’re found to be legally responsible for causing injury to another person or their property. It doesn’t cover damage to your vehicle, however, or items stolen from your vehicle, or damage from a storm or vandalism. Collision insurance covers you while your vehicle is in motion and you crash into another vehicle or with a fixed object, like a pothole, guardrail or light post. This coverage is typically required by a lender when

you finance a vehicle. Comprehensive insurance is coverage that protects your vehicle when it is damaged while not in motion. Examples of this could include vandalism, theft or an act of Mother Nature like a hailstorm or fire. “When financing a vehicle, the lender may require you to carry this coverage until it is paid off. If your vehicle is older, however, and it has been completely paid off then comprehensive insurance may not be the right coverage because a vehicle’s value decreases with age. Consider whether the savings of not having comprehensive insurance are enough to offset the risk of potentially having a large repair or replacement bill,” says Kevin Quinn, Vice President of Auto Claims at Mercury Insurance. Misconception: Your auto insurance policy provides coverage whenever you are driving your vehicle A personal auto insurance policy protects drivers in case they’re involved in a collision. However,
there are certain situations where more coverage is needed. “With the advent of the gig-economy, many people are turning to ridehailing jobs, where they can be their own boss and set their own hours,” says Quinn. “Without the proper insurance, however, they’re putting themselves and others at risk if they get into a collision, because your personal auto policy doesn’t cover you when you use your vehicle for commercial purposes. This includes driving for Uber or Lyft, or even pizza delivery. “Mercury provides ridehailing insurance, but if it isn’t currently available in your area or you’re using your vehicle to make money in another capacity like delivering pizzas, you’ll want to purchase a commercial auto policy.” Insurance is a sound investment, but can be complicated, so it’s always a good idea to speak to an insurance agent. These highly trained professionals can provide expert advice and guidance to help ensure you are adequately protected.
