UNDERSTANDING HOME HEALTH By Terry Adler, CEO, Always Classic Care
Thank you for your great response to this column. These are a few of some recent questions: I’m not able to manage my apartment or drive my car anymore. Will my long term care insurance cover someone to help me? Your long term care insurance covers personal care needed at home only. That means that your policy states that it will cover a home health aide who can help the insured person shower, dress, prepare light meals, do laundry, and straighten up bathroom and kitchen after using them for the care of the patient. The aide can also shop for the insured. Aides are professionals who have been trained with a required amount of state-approved hours in a legitimate training school to provide safe and efficient care with a degree of ethics. They are aware of the services they are allowed to provide under insurance coverage. Services requested other than the aforementioned needs to be agreed upon via a private arrangement between you and your aide. As an agency, we cannot and do not get involved in these arrangements when insurance is involved. But I don’t need anyone to help me with my shower or getting dressed. I also can put light meals together or pop frozen dinners or restaurant leftovers in the microwave. What I
need is someone to drive me shopping, for errands and to doctor appointments. I also need someone to clean my apartment, because I can’t really do it anymore. So, what can I do? Those are considered private services. At Always Classic Care, we can provide them for you at a reasonable out-of-pocket cost, much as you arranged before you qualified for coverage from your long term care insurance. If you explain your needs to me, I can make wonderful recommendations for you. We can provide housecleaners and drivers who provide those services for a living, but are screened as well as our aides prior to our recommending them. Why should I go through an agency if I have to hire someone? I can use someone my neighbor uses. You definitely can hire whomever you like. However, you should consider the following items: • How well do you know this person? Can you trust him or her? Are you sure the aide does not have a police record? All of our recommendations have had to be subjected to an FBI investigative screening. • Is the aide insured to cover any liability if you should become injured while in their care? Are you sure? • Will you be paying your share of their Federal taxes? We do. Many people are unaware of their responsibility

HAPPILY EVER AFTER For South Florida Husband & Wife Physicians, Seeing Was Believing at The Braverman Eye Center For Dr. Tsun Yee Law and his wife, Dr. Chau Nguyen, having better than average eyesight is not only important, but life saving. Both physicians who work in Fort Lauderdale have had issues through the years with their vision. Knowing how important it is to what they do, when things were not “perfect” - it was time to look into correcting something that was indeed paramount. Dr. Law’s story is truly something that everyone should listen to. Here is someone who grew up poor - where getting glasses or corrective lenses - put a strain financially on the family. In school, he can recall having to always be in the front row. He did what he could to see, but without corrective lenses, it was tough. Dr. “Shawn” - as he is affectionately called by his associates - was looking to address his vision issues when one of his mentors, Dr. Martin Roche, who just happens to work with his wife, started to speak highly of Dr. Braverman and his many, many success stories. When someone you respect recommends someone - it is a sure bet that things will work out. “As a physician, sometimes you need to be the patient,” Dr. Law explained. “Because I am a surgeon at Holy Cross and have numerous patients, you have to put yourself in another professional’s hands.” That belief brought he and his wife to Hollywood to address the vision problems that both had been suffering with - Dr. Law, a bit worse. damage that many tend to ignore. UV rays can harm the skin through chronic exposure and intermittent sunburns. Minimizing exposure to sunlight, including tanning beds, can help you protect yourself from skin cancer. Scheduling annual checkups with a plastic surgeon can be a good way to monitor suspiciouslooking moles or diagnose skin cancer early. Performing self-examination can also be helpful. When detected early, skin cancer has a more than favorable cure rate. If you do have a skin cancer, both Dr. Amunategui and Dr. Muñoz suggest that you get to your plastic surgeon for treatment. “There are three types of skin cancer,”
Dr. Muñoz said. “Basal Cell Carcinoma, Squamous Cell Carcinoma and Melanoma.” Basal cell carcinoma (BCC) is a nonmelanocytic skin cancer that arises from basal cells. The prognosis for patients with BCC is excellent, but if the disease is allowed to progress, it can cause significant damage. Squamous cell carcinoma (SCC) is usually found on areas of the body damaged by UV rays from the sun or tanning beds. Sun-exposed skin includes the head, neck, ears, lips, arms and legs. Melanoma is a form of skin cancer that arises when pigment-producing cells— known as melanocytes—mutate and become cancerous. “We have watched what the sun can do and how extensive the surgeries are to correct the damage,” Dr. Muñoz explained. “What this all comes down to is prevention and making sure that whenever you go out into the sun, there is total protection.” There are several things to follow when protecting yourself from the UV Rays remember that sunscreen alone isn’t enough. Seek the shade, especially between 10 AM and 4 PM. Cover up with clothing, including a broad-brimmed hat and UV-blocking sunglasses. Examine your skin head-to-toe every month. See a plastic surgeon at least once a year for a professional skin exam. PREVENTS FACIAL AGING When it comes to their face, people will do anything to prevent facial aging. was so intense and she was tired of waiting for the politicians, who have dragged their feet to a point where many were filing law suits. “Medical marijuana was passed by Florida voters nearly three years ago and that should have been that,” Tyler said. “But what happened were those who were against it from the beginning, continued to find ways where the decision could be overturned. That is not what this country was based on.” Because medical marijuana can make no claims, voters who have never used it, had NO valid way of knowing whether the herb was working or not. That is something today that still exists, and most definitely will make it tougher to decriminalize it. “The education that people don’t have
the use of marijuana for a variety of illnesses and afflictions, providing some relief to long-suffering individuals. Marijuana remains illegal under federal law and the Federal government doesn’t recognize any medical value for it. However, a medical marijuana card enables the recipient to avoid civil and criminal penalties at the state level, depending upon the state they live in. Local Podiatrist Saves Limbs Being Destroyed By Non-healing Wounds By Marylynne Newmark, Medical Writer
Dr. Charlton Adler, three - time Board Certified podiatrist with offices in North Miami Beach, has been credited with saving the limbs of two patients diagnosed with necrotic fasciitis, a bacterial infection that quickly spreads from foot to leg, destroying healthy tissue and transforming it into gangrene. Both of these cases were misdiagnosed by other physicians who were recommending amputation. “Because of my background in VA medicine dealing with resistant nonhealing battle injuries, this type of diagnosis is very familiar to me,” says Dr. Adler, “After careful assessment, I am able to address these wounds aggressively, consequently saving the limbs.” According to Dr. Adler, this type of bacteria multiplies very quickly, destroying flesh and rapidly infecting the blood. This is the pattern of gangrene. “One case was so severe,” said Dr.
Adler, “that the attending physician recommended immediate amputation.” After explaining his previous successes with various debriding techniques, Dr. Adler took over the case, and the leg was saved. “Another patient lives in the Virgin Islands,” says Dr. Adler. “She had already had a biopsy and was told they needed to amputate quickly, but she heard about my team. By the time she flew up to Miami,” continued Dr. Adler, “ the infection had spread halfway up her leg. We treated her aggressively and were able to save it” . Dr. Adler added “Now, whenever I visit St. Thomas with my family,” he smiled, “ I am treated like an outright hero.” Dr. Adler stated that he uses various debridement techniques, as well as specialized product and other modalities unique to each particular wound. Nonhealing wounds can be related to diabetes, circulatory problems, traumatic
injuries and pressure lesions. As a podiatrist, Dr. Adler’s specialty is limited to wounds below the knee. “However, infection is a symptom of what is going on in the body,” he states. “My studies include the person as a whole, including his or her lifestyle, environment, diet and hydration, even mind, body and spirit.” Dr. Adler’s treatment includes counseling in areas beyond the immediate injury. “For example, nutrition is a major factor in the healing process,” is on staff and has surgical privileges. Teeth-Whitening Products Could Cause Bigger Problems Than Yellow Teeth Are you thinking that your teeth don’t look nearly as white as the movie stars you see? If so, perhaps you’re thinking about purchasing one of those over-the-counter teeth- whitening products. But buyer beware: Cosmetic teeth-whitening is a $3.2 billion-dollar industry that is not regulated by the Food And Drug Administration because it is considered a cosmetic procedure. The sheer number and types of whitening prod-
ucts can be overwhelming. They include whitening toothpastes, gels, whitening strips, whitening pens, whitening mouth rinses and even paint on bleach. “It is best to consult your dentist before purchasing over-the-counter whitening products,” says Dr. Anita Myers, a dentist and author of the book Stunning Smiles: A Dental Guide To Improve the Way You Eat, Smile & Live (https://dranitamyers.com ). “Some of these products
can lead to thinning enamel, gum irritation and recession, and make teeth overly sensitive.” Some whitening strips contain chlorine dioxide, which can destroy the enamel on teeth. Chlorine dioxide is the same acid that is used to disinfect swimming pools. It whitens teeth by eating away at the surface of the enamel. “All stains are the same,” Dr. Myers says. “Some are mostly on the surface of the teeth and
come from consuming lots of dark-colored liquids like coffee, tea and red wine, as well as foods with vibrant yellow spices like turmeric. The tar and nicotine from cigarettes are also huge culprits. “Often, surface stains can be diminished by routine brushing, flossing and biannual professional cleaning in the dentist’s office. Unfortunately, there are deeper stains you just can’t avoid by being careful about what you eat and drink. And some issues may remain after whitening. Porcelain or composite dental crowns and bondings will not lighten up. So if you change the color of the teeth around them, you might wind up with an uneven smile or
need to replace old fillings and crowns.” If you are going to use an over-the-counter whitening product, Dr. Myers has these recommendations: Don’t leave the strips or gels on longer than recommended. This could lead to sore gums and other dental problems. Avoid acidic beverages. After you whiten, avoid soda, sports drinks, or other acidic beverages for a couple of hours to protect your teeth. Pregnant women or nursing mothers should delay applications. Although no studies have shown this to be a problem, it is best to delay
using those products, Dr. Myers says. Monitor Gums. If you use gel-filled trays, which you wear over your teeth like a mouth guard, it may bother your gums if they don’t fit well. “Stop using the product if you start having this problem,” Dr. Myers says. “If you like the gel-filled trays, your dentist may be able to make one that better fits your teeth.” “If you are in doubt about what to choose as a teeth-whitening product over the counter, look for the The American Dental Association seal of acceptance,” says Dr. Myers. “At least you will know that the product has been evaluated by an independent panel of qualified dentists.”
Bothered By Chronic Pain? Its Source May Surprise You

Pain that lingers or worsens is usually a signal to see a doctor. Medical professionals, however, don’t always diagnose the problem correctly in the initial visit. Chronic pain sufferers, in particular, may want to get a second opinion. Researchers at Johns Hopkins have reported that 40 to 80 percent of chronicpain patients are misdiagnosed. Being misdiagnosed can lead to ineffective treatments and perhaps surgery the patient did not need. One way to save time, money and worry – and most importantly, to receive the right treatment - is to find the source of the pain, says Dr. Victor Romano (www.romanomd.com), an orthopedic surgeon and author of Finding The Source: Maximizing Your Results – With and Without Orthopaedic Surgery. “You treat the source, not the symptoms,” Dr. Romano says. (www.romanomd.com). “Finding the source of pain can help cure a problem using non-surgical means, and it can change someone’s life. “It comes down to peeling back the layers to figure out what is contributing to the pain. Finding and treating the source of an injury helps determine whether surgery is warranted at that time.” Dr. Romano uses a patient evaluation tool called Autonomic Motor Nerve Reflex
Testing (AMNRT) to identify the source of symptoms. Sometimes patients are surprised to learn the origin of their pain. “The body compensates for injuries,” Dr. Romano says. “For instance, a patient may have shoulder pain because they are adapting their stance to accommodate a back injury. Pain in one area of the body is often associated with an injury on the other side.” Dr Romano also points out, “Obstructed breathing - for instance, if you have a cold, allergies, or a deviated septum - will contribute to weakness and pain in your back, shoulders, and hips. Believe it or not, improving your breathing - with nasal sprays or rinses and allergy medication, or fixing your crooked nose - will help improve your strength and relieve your pain.” Dr. Romano lists five common physical ailments that produce chronic pain, some possible sources for the pain, and nonsurgical remedies to relieve it: Tennis elbow. “Why are you using more force on that elbow for gripping or lifting?” Dr. Romano asks. “Testing could reveal the hand is weak from carpal tunnel syndrome. Fix your carpal tunnel problem and your elbow pain will begin to heal on its own.” For therapy, he
suggests self-manipulation of your wrists, grabbing the back of the hand while resting a forearm on your stomach, then pulling on the wrist and bending it downward. Slumping shoulder. “Shoulder weakness can be associated with neck or back problems and obstructive breathing disorder,” Dr. Romano says. “Lower back stretches and a pelvic stabilizing program can help.” Runner’s knee. The medical term is patellofemoral pain syndrome, felt as vague pain around the kneecap from prolonged sitting or more strenuous activities like stair climbing. “The injury is thought to come from overuse, excess weight, or arthritis,” Dr. Romano says, “but the true source for, say, right knee pain is often a weakness in the right hip flexor. Physical therapy, stretching the outside structures and strengthening the inside muscles are often better solutions than surgery.”
Sciatica. This affects about 40 percent of the population at some point. It’s characterized by pain in the lower back going down the leg. There can be several non-obvious sources, such as obstructed breathing and its associated back problems or a tilted pelvis. Dr. Romano suggests a variety of stretches in yoga, along with rotational and hip flexion stretches. Achilles tendinitis. “ Sometimes the source can be foot maladies in the same or opposite foot such as peroneal neuropathy and Morton’s neuroma,” he says. “The Peroneal nerve ends on the top of the foot; Morton’s is the nerve branch between the third and fourth toe. Stretching and strengthening exercises for the calf muscles can help heal it.” “You’re going to keep on getting hurt and have pain,” Dr. Romano says, “unless you find and fix the source of the problem.”

With a cash-out refi, homeowners face trading their lower interest rate for a higher one. Less convenient than other loan products: The application and approval process for a cash-out is anything but efficient, thanks to time-consuming activities like property appraisals and in-person closings. In all, the loan process can take anywhere
from 30-60 days. Additional fees: Borrowers often don’t realize that cash-out refis come with closing fees for such things as appraisals, title searches and credit
reports, adding another layer of cost to the loan. A smarter solution that can potentially spare borrowers thousands in interest cost, according to Harrington, is a home equity loan. Instead of starting over with a new mortgage, you’d simply
take out a separate loan against the equity in your property. This option lets you keep your mortgage interest rate. To make things more clear, here’s a comparison of how the two loans could affect a homeowner like you. Let’s say you took out a $175,000 mortgage six years ago at 3.625 percent interest. After making monthly payments of $798, your balance is $153,365. Now you’re looking to do some renovations and pay off some credit card debt, and you need to borrow $75,000. With your home valued at $300,000, there’s more than enough equity. With a cash-out re-fi loan, you’d “reset” your mortgage balance at $228,365 with an interest rate of, say, 5.75 percent interest. That brings your monthly payment to $1,333, but in 30 years, when the mortgage is paid off, total interest comes to $287,225 (that’s the interest you paid on

your original mortgage and the interest you’ll pay with the refinanced loan). With a $75,000 home equity loan, you may receive a higher rate, but it applies to a much smaller loan amount. If you secured a home equity loan at 9.0 percent APR, your monthly payment for your mortgage and equity loan combined would be slightly higher at $1,559. However, the term of your equity loan is 15 years, and your mortgage is still on track to being paid off in 24 years. In all, your total interest payments come to $174,238 (original mortgage plus home equity loan). Bottom line: In this scenario, a home equity loan comes out as the better financial decision, because not only are you finished paying six years earlier, you would save $112,987 in interest alone. Another upside to using solutions other than cash-out refis is that there are now convenient and fast solutions that let borrowers access their equity with ease. However, if you’re considering a cash-out refi, you may be unaware of some of the pitfalls, or you may not know about the alternative solutions that might work in your financial favor.
