Patients once diagnosed with locally advanced Pancreatic and Liver Cancers were given little hope and often offered limited treatment options all with certain short life expectancies despite the treatment they were receiving. However, a new procedure called Irreversible Electroporation, also known as IRE or NANO-KNIFE for short, has changed all that. Patients are seeing a doubling and tripling of their life expectancies based on recently published reports. Though currently used in patients with known locally advanced diseases it has now found its place in the operating room and used when patients are “unexpectedly” found to have more advanced disease than pre-operative imaging shows. Dr. Robert Donoway, a surgical oncologist trained at Sloan- Kettering, is the only surgeon performing these procedures. During the procedure, in the operating room, after it has been determined that the patient’s tumor is nonresectable, fine needle probes are placed through the tumor using ultrasound guidance to ensure there is no injury to critical structures such as the Bile Ducts and Portal Vein. High voltage current up to 3000 volts is delivered through the probes in 90 microsecond pulses synchronized with the patient’s heart rhythm to avoid any cardiac problems. The procedure lasts between 1 and 2 hours using imaging to follow the ablative process. After the procedure is completed, the probes are removed and the patient is closed and taken to ICU step down for a 24-hour observation period. Most patients are discharged in 2 or 3 days following surgery and because there is no major surgery to recover from, they are able to start postoperative neo-adjuvant type chemotherapy and radiation sooner. After 3 to 6 months of combination treatment patients are reevaluated and restaged. A number have been converted to Resection candidates, which is still deemed the standard of care. The preliminary results have been very encouraging and have more than doubled many patients life expectancies. Clinical trials are ongoing to use new novel agents including monoclonal antibodies and vaccines as the next line of treatment in this highly selected and heavily pre-treated group. Well, it happened again. The family of a former patient of our agency called with a horror story of how the patient was taken advantage of by an aide that the client hired privately. Mrs. R. had been our patient throughout the three years that her long term care insurance paid for aide services, and she was very happy with the aides we had assigned to her. However, once she was faced with paying for the aide out-of-pocket, her friends convinced her that she could pay less money for help that she could hire herself, rather than work through an agency. At first, Mrs. R was very happy. The aide she hired was kind and attentive. She helped shower and dress Mrs R, helping pick out her clothes, accessories, even her jewelry. The aide liked to listen to Mrs. R’s stories about her trips and the lovely momentos she bought back. Mrs. R also

Mrs. R told her daughter that she was very happy with this aide. Then, all of a sudden, small items began to disappear. For every man, woman and child who is of the Jewish faith and takes part in the traditionally holidays, this is certainly your festive time of year. From the start of Rosh Hashanah (at sundown on September 13th), to the first day of Sukkot on September 28th, this is a month that will be filled with joy, food and a time to reflect on where we all are here in 2015. With so much going on in our world these days - from wars to economic and cultural problems, this year’s High Holidays will be a time to understand where we are in a society that watches so many horrific events unfold right before their very eyes. Aside from the great foods and the family events, this is a time of year for Jews all over the world to sit back and talk with friends and family members about the state of our world and how individuals can do more to make things better for their communities. “This is a holiday season that is often one that you enjoy, get together with friends and family to talk about things in our society,” Aventura resident Dina Miller said. “You go to temple to pray for a better life. You pray for people getting along. But this year is much different. Since there is so many negatives going on in this world, the focus could be much different.” As Jews all over the world will use this opportunity - during the holiest of months - to convey the message of peace and understanding - it is going spill over to Aventura, Sunny Isles Beach and other surrounding areas where a still vibrant Jewish community heads out to continue to learn and understand the religion. Kevin Reece moved to Sunny Isles Beach last year at this time and was impressed by the many cultures that make up the Jewish community. He attended a service that was provided at one of the local synagogues. He was then invited to a Rosh Hashanah dinner at a member’s home.
“It was my first year after coming from Chicago,” Reece said. “Back home, we weren’t year round Jews - as far as going to temple, but did observe the holidays. When I came here, that was what I was looking for.” As the holidays begin on Sunday, September 13th - with Rosh Hashanah, there are so many who do not know much about the religion and how these high holidays are here today. ROSH HASHANAH Rosh Hashanah is the celebration of the Jewish New Year. It’s a very important holiday on the Jewish calendar. It is the first of what we call the High Holidays (or High Holy Days), a ten-day period that ends with Yom Kippur—the holiest day of the Jewish year. On Rosh Hashanah, Jews from all over the world celebrate God’s creation of the world. Rosh Hashanah is two days long, and it usually occurs during the month of September. How is Rosh Hashanah Celebrated? During Rosh Hashanah, Jewish people ask God for forgiveness for the things we’ve done wrong during the past year. We also remind ourselves not to repeat these mistakes in the coming year. In this way, Rosh Hashanah is an opportunity to improve ourselves. It’s a holiday that helps us to become better people, and that’s a beautiful thing. Jews from all over the world celebrate Rosh Hashanah in different ways. Holiday traditions can be different depending on where you’re from and how your family celebrates. A special prayer service is held at synagogue. The shofar, a special instrument made from the horn
of a kosher animal (usually a ram), is blown during the Rosh Hashanah service. Tzedakah, or giving charity to people in need, is also part of the holiday. Good deeds are done and charity is given in the hopes that God will seal our names in the “Book of Life,” which brings the promise of a happy year to come. What kinds of foods are eaten on Rosh Hashanah? Food is an important part of Rosh Hashanah. Many special foods are included in a traditional Rosh Hashanah meal as blessings. Sweet foods are eaten to symbolize our hope for a “sweet new year.” We enjoy “new fruit,” a fruit that has recently come into season but we have not yet had the opportunity to
enjoy this year (often a pomegranate). The head of a fish is sometimes served, to remind us to be “like the head and not the tail”—so we’ll be leaders, not followers. The fish also symbolizes the translation of Rosh Hashanah, which means “Head of the Year” in Hebrew. A pretty, symbolic bread called challah is baked, sweetened with raisins and braided into a round shape.
Apples are dipped in honey, again symbolizing sweetness. All of these traditions are important, because they help to connect us to the deeper meaning of the Rosh Hashanah holiday.
YOM KIPPUR Yom Kippur, which dates from biblical times, is referenced in three separate passages in the
als and sacrifices that purified the Temple from the defilement that had attached to it as a result of the sins of the Israelite people. This defilement had caused God’s presence to depart from their midst. There also was another aspect to the day: atonement, the spiritual cleansing of the people themselves. Their role was to serve as an attentive and expectant audience outside the Temple precincts, awaiting the hoped-for successful outcome of the high priest’s service. Their role, according to the Torah, was to abstain from work and to practice “self-denial.” With the Temple’s destruction, the second aspect of Yom Kippur, focused on atonement, came to predominate. The atonement we now perform is
Torah. The Torah refers to Yom Kippur as Shabbat Shabbaton, “a Sabbath of complete rest,” while the Talmud denotes Yom Kippur simply as Yoma, “The Day.” The Torah portrays Yom Kippur primarily as a cultic festival, a day centered almost exclusively upon the Temple in Jerusalem. It was on this day that the kohen gadol, the high priest, performed the complicated ritu-
turned inward; it is an act of selfpurification in which we purge our own lives from the stain of our misdeeds. Like the Israelites during the Temple period, we continue to fast, understanding this self-denial as a cleansing of our soul, an act of self-discipline, and a sign that on this day we rise above our most basic biological necessities to focus our attention on matters of the spirit.

How men and women experience retirement differently (BPT) - What’s retirement like? The answer may depend upon whether you’re a man or a woman. Women report more positive experiences in retirement than men, yet also say they are more likely to experience negative emotions, especially stress, according to a study conducted by MassMutual. The study’s findings have implications for retirement planning, both financially and emotionally. “Women often experience high levels of stress before retirement as they juggle both professional and family responsibilities,” says Elaine Sarsynski, executive vice president of MassMutual Retirement Services. “We are learning that the stress women feel often carries over into retirement, but we’re also hearing that women often make more of their retirement opportunities and
experiences than men.” The study surveyed 905 retirees within 15 years after retirement and 912 pre-retirees within 15 years before retirement. The responses for both groups were evenly split between men and women and respondents had a minimum of $50,000 in retirement savings. One in five women (20 percent) is at least moderately stressed in retirement compared to 15 percent of men, the study finds. Women also are more likely to feel frustrated, sad, lonely and nervous. Before retirement, women are much more likely than men to report being stressed. The study finds that 49 percent of women pre-retirees say they are at least moderately stressed compared to 38 percent of men preretirees. Though whether you are a man or a woman,
saving for retirement contributes to positive emotions and experiences in retirement. Those who have a 401(k) or other retirement savings plans were more likely to report experiencing positive emotions as compared to those who did not. Of the retirement plan participants, 74 percent say they are “extremely” or “quite a bit” happy compared to 68 percent who did not participate in a retirement plan and 70 percent of retirement plan participants say they are “extremely” or “quite a bit” relaxed compared to 61 percent who did not have a plan. “Overall, positive emotions for both men and women tend to increase and negative emotions tend to decrease in retirement,” says Mathew Greenwald, president of Greenwald & Associates, whose firm conducted the study on behalf of
MassMutual. “People also tend to exaggerate expectations for retirement, especially women, partly because it’s difficult for many people to envision what life in retirement will actually be like.” In the years before retirement, women have markedly higher expectations than men for enjoying the social aspects of retirement and somewhat lower expectations for their financial fortunes. During retirement, women report having less free time than men and yet are more likely to have new experiences, find more time for friends, and enjoy more opportunities. Men’s expectations tend to be more in line with their eventual experiences, which are typically less fulfilling than women, according to the study.
Sarsynski recommends that pre-retirees take steps to help themselves gain a more concrete understanding of what their lives may be like once they retire. “Spend time with retirees to gain insights and better prepare both financially and emotionally for
retirement,” Sarsynski says. “We also suggest that pre-retirees track all of their purchases, especially when on vacation or participating in activities they enjoy. That will help them better understand how costly, and therefore how feasible, the lifestyle they envision will actually be.”

(BPT) - As the fall and winter months approach, you’re probably planning to take preventative health measures recommended by your doctor: get the flu shot; wash your hands more often and sneeze into your elbow instead of your hands. But one thing that makes you more susceptible to getting sick is being stressed and run down. Despite living in a world where everyone is constantly onthe-go, you can’t avoid the fact that proper sleep is a key element when it comes to overall health. Sleep is your body’s time to recuperate, so the less quality sleep you get, the less time your body has to do what it needs to do to keep you healthy. The Centers for Disease Control and Prevention (CDC) recommends adults get seven-to-eight hours of sleep each night, and that teens and children get nine hours or more. Unfortunately, a quarter of the American population reports that they get inadequate sleep, which the CDC has
deemed a public health epidemic. However, changing a few simple things about your daily routine or the room in which you sleep can greatly improve the quality of the sleep you get and, in turn, help improve your overall health. Stick to a sleep schedule. Make sure that you go to bed and get up at roughly the same time each day, even on weekends. Having a consistent sleep pattern helps regulate your body’s sleep-wake cycle, helping you to sleep better at night. If you have trouble falling asleep within the first 20 minutes or so of going to bed, that’s okay - simply go into another room and do something relaxing until you feel tired. Agonizing over falling asleep will only make it tougher for you to get the rest you need. Get cozy. This means making sure you have comfortable bedding. In a U.S.-based survey conducted by the American Down and Feather Council (ADFC), 70 percent of consumers surveyed, who use down and feather prod-
ucts, agreed that they get a better night’s sleep with down and feather bedding. Furthermore, 55 percent agreed that there are health benefits to sleeping with down and feather products, because they: • Regulate your body temperature Down and feather is a natural material that wicks away moisture and perspiration, allowing it to quickly evaporate rather than trapping it. This reduces wakefulness due to being too hot or too cool. • Provide proper support - Down and feather pillows conform to the body more naturally than many synthetic fibers or foam pillows. • Help tame allergens - Down and feather products are tightly woven and provide a clean, natural and moisture-free environment, so they aren’t conducive to dust mites, which are a source of allergies
for nearly 20 million Americans. Assess your sleeping conditions. Your bedroom should be cool - between 60 and 67 degrees - and should be dark and quiet. Consider using room-darkening shades, and make sure to ‘power down’ before you go to sleep. This means turning off TVs, computers or other sources of blue light that may hurt your sleep. To help block-out extra noise, consider using a fan or ‘white noise’ machines to help you fall asleep and stay asleep. There are plenty of reasons to love the fall and winter months, and you’ll certainly appreciate these cooler months even more if you’re getting the adequate amount of sleep that you need to stay healthy. Dr. Adler Using Laser In Battle Against Toe Fungus By Marylynne Newmark, Medical Writer
According to the American Podiatric Medical Association, approximately 40% of patients visiting a podiatrist have complaints or signs of toe fungus. It may begin with tenderness or pain in the toe, developing into an unsightly thick, darkened nail, and it can spread from toe to toe. In fact, the fungus can get so out of control that it can infect the foot and ankle. Topical treatment such as Lamisil cream usually is the first course of therapy for a mild or moderate case. However, long-term use or oral antifungal medication is not recommended due to the toxic side effects Lamisil can have on the liver. This is a special risk if a patient is taking other medications, including excessive use of Tylenol that can manifest the same hepatic damage. Over the past few years, Dr. Charlton Adler has been following closely the application and reported results of laser therapy on fungus toenails and other podiatric and wound situations. He is pleased to announce that he now is able to add this service as a therapeutic option in his office in North Miami Beach.
“The introduction of laser in treating toe fungus has had a major impact on attacking this uncomfortable and unsightly problem,” says Dr. Adler. He further explains that Onychomycosis, the medical term for fungal infection of the nail, is often ignored because it can be present for years without causing pain. “When the fungus imbeds itself under the nail,” he adds, “ the nail surface can become darker in color and the area can develop an odor. The fungus lives on debris under the nail, and white marks can appear on the nail plate.” Approved for the treatment of nail fungus five years ago by the Food and Drug Administration, the special appeal of lasers is that it selectively heats and destroys harmful fungi while sparing healthy surrounding skin. Because of the intensity of the heat generated by laser rays, Enrique Mechulon a specially trained and certified laser technician working with Dr. Adler, says that both he and the patient wear laser protective glasses and take several rest periods during the 40-minute process. “I have already witnessed many suc-
cess stories where patients have spent years and thousands of dollars to get rid of their nail fungus,” says Mechulon. “Depending on the patient’s general health and nail problem, it can take anywhere from one to nine treatments, but the results are there with minimum risk to the body,” he adds. “I wanted to offer this therapy for quite a while,” says Dr. Adler. “However, I am cautious in my approach, and I needed more proof on studies and reports.” Dr Adler adds that research shows that 80% of all toe fungus has been healed using laser therapy. “This is in contrast to topical ointments that only work 50% of the time,” says Adler, “and with topical treatment, the infection usually returns.”

If you’re up for a painting challenge, an accent wall is a great way to incorporate color into a space. Choose your favorite color from a throw pillow or wall-art piece in the
room to pull the look together. Varying textures. A room tends to get boring when it focuses on only one texture, so it’s important to create a sense of balance. Try varying the fabric types on your sofa and curtains. Add in some metal or wooden accents to draw the eye around the room. You can even play with lighting to reflect off certain objects and create visual interest. Antique feature. Choose an antique or unique piece of décor to be the inspiration for the room. One-room living: How to live large in a small space and side table. Multitasking solutions. If you’re always multitasking, your furniture can do the same. Place an ottoman with a hidden storage compartment or flip-top serving tray beside a comfy chair.
