CITY NEWS New itineraries, ships and offerings draw more cruise travelers to European rivers (BPT) - Europe continues to attract the attention of American travelers, and travel on river cruises is one of the best ways to see many of the continent’s sights and explore much of its history. In response to strong demand, cruise lines are offering many new and expanded services to accommodate this growing travel interest: Expanding to new rivers.

Many river cruise lines are bolstering their cruise portfolios by expanding to rivers where they haven’t before offered itineraries. Such is the case with Tauck, a leader in premium quality guided travel for 88 years, which is adding two new cruises on the Seine River in 2014. “Cruising the Seine Plus Versailles, Paris &

London,” 14 days, begins with an included two-night stay in London at the landmark Savoy, complete with guided sightseeing. Guests travel next to France for a nine-day cruise along the Seine, before enjoying an indepth exploration of Paris and a two-night stay in Versailles at the Trianon Palace Versailles, a Waldorf Astoria property.

Near-Far Multifocal PresbyLasik: Correction for two kinds of vision problems New procedure can correct both Distance and Near Vision

For years, the miracle of Lasik surgery for vision correction was almost always an either-or procedure. Patients who wanted to throw away their glasses or contact lenses but were old enough to require reading glasses on top of their distance correction, had to choose one kind of correction or the other for each eye. Now, a remarkable new procedure called PresbyLasik allows most patients to eliminate both problems.

Jose Berrios, 64, and his wife, Alma Mazar-Berrios, 62, both qualified for the PresbyLasik procedure and are thrilled with the results.

Jose, a Miami-Dade transit driver, began to require reading glasses years ago. Then over

time, he started to have trouble seeing at a distance as well, and needed either bifocals or two different pairs of glasses. “I was going for a vision test every two years, and the prescription would always change,” he recalls. “I was really tired of it.”

One day, a coworker told Jose about her daughter, who had had successful Lasik surgery by Dr. Stanley Braverman.

Intrigued, he decided to pay a visit to Dr. Braverman at the Braverman Eye Center in Hallandale Beach. “I hoped I’d be able to quit wearing my glasses,” he says.

night, Jose needs to be able to see clearly, because the safety of his passengers can depend on it.

When he’s not working, he

“Dr. Braverman gave me a complete eye exam,” Jose recalls.

“It’s nice not to have to wear glasses anymore,” he says. “I enjoy being able to see again.”

Jose was so pleased with his results that Alma decided to see if she might qualify for PresbyLasik. A month after her husband had the procedure, she, too, visited the Braverman Eye Center. She was doubtful about her candidacy, however, for a couple of reasons.

Her prescription was much stronger and more complex than Jose’s. “I wore trifocals,” she says. “I work a lot at the computer and I was having problems. I couldn’t see TV without glasses--everything was a blur -and I couldn’t see signs when I was driving.”

The other reason Alma wondered whether the procedure could be successful for her: “I’d been told in years past that I wasn’t a candidate for Lasik,” she says. “I didn’t think I’d ever be able to go without glasses.”

But Jose kept urging her to try again. And at her visit to the Braverman Eye Center, Alma discovered that technology had changed since she last inquired about Laser Vision Correction Surgery. During the consultation, she learned that she was

Thank you for your great response to this column.

These are a few of some recent questions: I’m not able to manage my apartment or drive my car anymore. Will my long term care insurance cover someone to help me?

Your long term care insurance covers personal care needed at home only. That means that your policy states that it will cover a home health aide who can help the insured person shower, dress, prepare light meals, do laundry, and straighten up bathroom and kitchen after using them for the care of the patient. The aide can also shop for the insured. Aides are professionals who have been trained with a required amount of state-approved hours in a legitimate training school to provide safe and efficient care with a degree of ethics. They are aware of the services they are allowed to provide under insurance coverage. Services requested other than the aforementioned needs to be agreed upon via a private arrangement between you and your aide. As an agency, we cannot and do not get involved in these arrangements when insurance is involved.

But I don’t need anyone to help me with my shower or getting dressed. I also can put light meals together or pop frozen dinners or restaurant leftovers in the microwave. What I

need is someone to drive me shopping, for errands and to doctor appointments. I also need someone to clean my apartment, because I can’t really do it anymore. So, what can I do?

Those are considered private services. At Always Classic Care, we can provide them for you at a reasonable out-of-pocket cost, much as you arranged before you qualified for coverage from your long term care insurance. If you explain your needs to me, I can make wonderful recommendations for you. We can provide housecleaners and drivers who provide those services for a living, but are screened as well as our aides prior to our recommending them.

Why should I go through an agency if I have to hire someone? I can use someone my neighbor uses.

You definitely can hire whomever you like. However, you should consider the following items: • How well do you know this person? Can you trust him or her? Are you sure the aide does not have a police record? All of our recommendations have had to be subjected to an FBI investigative screening.

• Is the aide insured to cover any liability if you should become injured while in their care? Are you sure?

• Will you be paying your share of their Federal taxes?

Just as it was for Jose, PresbyLasik was successful for Alma. Now she is also one year out from the PresbyLasik procedure. Whether they are at work, out for a drive, watching a movie or walking their dogs (two German shepherds and Australian sheepdog), they do it without the inconvenience of glasses or contacts. “I’m very happy not to have to wear glasses,” Alma says. “It took me awhile to figure out I didn’t need them anymore. WHAT TO EXPECT WITH Z-LASIK SURGERY All Laser Lasik surgery is performed on a patient’s cornea, which provides most of the focusing power when light enters the eye. Topical anesthetic drops are applied to the eye. The surgeon first uses a special Ziemer LDV femtosecond laser to create a thin layer of corneal tissue that can be folded back (a corneal flap). Then, another type of laser (an Allegretto Eye-Q excimer laser) is used to reshape the underlying cornea, painlessly. After the excimer laser treatment, the corneal flap is repositioned and bonds back into place naturally. The corneal

FOOD FIGHT! Anthony Bourdain cuts against the grain when it comes to travel and the world food scene

He is a product of the 1970s - when drug usage and sexual expression was the order of the day. Any day. What Anthony Bourdain learned about a life, growing up under the bright lights of New York City would pave a way for one of the most unusual and intriguing careers.

Bourdain is a wealth of information, wrapped between two marriages and fatherhood that began in his early 50s.

His extensive travel and knowledge of food gives him the right to scoff at the Rachael Rays of the world, who made 30 minute meals a trend.

What Bourdain has become in his many years as a trained chef and world traveler is someone who has formed his opinions by doing and not from listening to what others have to tell him.

He is no Andrew Zimmern, but does not profess to be. He is a foodie in every elegant sense of the word, leaving his often snobbish self open for much criticism.

However, he is known for consuming exotic local specialty dishes; consuming sheep testicles in Morocco, ant eggs in Puebla, Mexico, a raw seal eyeball as part of a traditional Inuit seal hunt, and a whole cobra – beating heart, blood, bile, and meat – in Vietnam. According to Bourdain, the most disgusting thing he has ever eaten is a Chicken McNugget, though he has also declared that the

unwashed warthog rectum he ate in Namibia and the fermented shark he ate in Iceland are among "the worst meals of [his] life."

TRAVELING MAN Bourdain, who had parlayed his sarcasm and truth-telling demeanor into a million dollar empire is so far from the mainstream cooking shows you have on the air. His trips to China, Japan, New Zealand and other major international destinations revolve around fine dining and often fine cultural dining.

Trips to the Phillipines, Australia and other destinations worldwide have paved the way for this New York City-born to Gladys and Pierre Bourdain. His paternal grandparents were French; his paternal grandfather emigrated from Arcachon to New York following World War I. Bourdain's father's family was Catholic, and his mother's family was Jewish; however, he was raised without any religion.

After graduating from the DwightEnglewood School in 1973, he attended Vassar College before dropping out after two years. He graduated from the Culinary Institute of America in 1978 and his career in food started to take off.

Bourdain's love of food was kindled in France, when he tried his first oyster on an oyster fisherman's boat as a youth, while on a family vacation. He worked in the seafood restaurants of Provincetown, Massachusetts, which sparked his decision to pursue cooking as a career.

LIFE IN FOOD Bourdain's Kitchen Confidential program led to an offer from the Food Network to host his own food and worldtravel show, A Cook's Tour, which premiered in January 2002. In July 2005, he premiered a new, somewhat similar television series, Anthony Bourdain: No Reservations, on the Travel Channel. As a further result of the immense popularity of Kitchen Confidential, the Fox sitcom Kitchen Confidential aired in 2005, in which the character "Jack Bourdain" is based loosely on the biography and persona of Anthony Bourdain.

THE LUCKY 13 Recently, Bourdain was asked to select his top 10 restaurants worldwide. In his unusual fashion, he came up with 13.

Bourdain's top 13 with a brief about each: 1) St. John (London) If I had to die with half a bite of anything hanging out of my mouth, it would probably be the roast bone marrow in Fergus Henderson's plain-white dining room at St. John.

2) elBulli (Girona, Spain) It's the hardest reservation in the world. And everything they say is true: It's an adventure, a challenge, a delicious and always fun acid trip to the farthest reaches of creativity.

3) The French Laundry (Napa Valley, California): Here is a place where meals range from Charcoal-Grilled Japanese Wagyu, Alaskan King Crab “Tempura,” Garden Radishes, Brokaw Avocado, Young Fennel and “Mousseline Béarnaise” and much more.

4) Per Se (New York City) The best sitdown, multicourse, white-tablecloth meal of my life was at the French Laundry. And subsequent meals at Per Se, also run by chef Thomas Keller, were no less wonderful.

5) Sin Huat Eating House (Singapore) It's grimy looking, the service can be less than warm, the beer is served in a bottle (often with ice), and the tables sit halfway into the streets of Geylang, Singapore's red-light district.

6) Le Bernardin (New York City) This is the best fish joint...anywhere. And it's relevant and fun, despite its formal service and fine-dining ambience. The grand tasting menu is a stripped-down thing of relatively austere beauty.

7) Salumi (Seattle) It's a sandwich shop with a couple of tables, and a true momand-pop—even if they're the mom and pop of famous chef Mario Batali.

Anything cured, anything braised, any of the limited hot specials.

8) Russ & Daughters (New York City): "Russ & Daughters occupies that rare and tiny place on the mountaintop reserved for those who are not just the oldest and the last — but also the best." The very smoked salmon, herring, caviar and so

much more.

9) Katz's Delicatessen (New York City) Russ & Daughters started as a pushcart nearly a century ago, and it now serves some of the last traditional Eastern European Jewish-style herring and smoked belly lox, sable, and sturgeon.

And since you're close, walk down a few doors to Katz's to remind yourself how pastrami is done right. This is what New Yorkers do better than anybody else. And here's where they do it.

10) Etxebarri (Axpe, Spain) Victor Arguinzoniz grills unlikely ingredients over homemade charcoal: baby eels, imperial beluga caviar, oysters. (The fresh chorizo and prawns work, too.)

Theoretically you can't grill a lot of this stuff, but a handcrafted series of pulleys that raise and lower each item makes it possible. Eat here, and no one is eating better.

11) Sukiyabashi Jiro (Tokyo) The best sushi on earth? Maybe. Jiro Ono is more than 80 years old, and he's been doing old-school Edo-style sushi his whole life.

Every piece of fish is served at precisely the right temperature and the rice and seaweed alone are blackout good. Ono will ruin sushi for you from anywhere else.

12) Hot Doug's (Chicago) This place convinced me the Chicago red hot is, in fact, superior to the New York hot dog. And it's home to two great innovations in American gastronomy: the "foie gras dog" and the weekends-only practice of cooking French fries in duck fat. It's proof that food doesn't have to be expensive to be great.

13) Oklahoma Joe's Barbecue (Kansas City, Kansas) People may disagree on who has the best BBQ. Here, the brisket (particularly the burnt ends), pulled pork, and ribs are all of a quality that meet the high standards even of Kansas City natives. It's the best BBQ in Kansas City, which makes it the best BBQ in the world.

SOUTH FLORIDA LAYOVER In one of Bourdain's newest ventures, The Layover, he had the chance recently to land in Miami - sampling some of the great eateries and meeting many of the food characters that we have on our streets. From a ride on fishing charter boat to see what the Atlantic Ocean is providing this region of the country, to eating at Garcia's Seafood, Colombian foods and even sampling some street food at The Grill Master Café to order a frita cubana, and ginger brussels sprouts from one food truck that boasts delicious Asian fusion. In a world where many aren't quite satisfied how their life is moving along, it's refreshing to know - at the age of 57 - Bourdain is living it for all of us who have long been chained to that ho-hum 9-5 rut!

The musical Chicago is based on a play of the same name written by reporter Maurice Dallas Watkins, who was assigned to cover the 1924 trials of murderesses Beulah Annan and Belva Gaertner for the Chicago Tribune. Annan, the model for the character of Roxie Hart, was only 23 when she was accused of the murder of Harry Kalstedt. She was found “not guilty.” Velma Kelly, based on the Gaertner, was a cabaret singer and society divorcee, when the body of Walter Law was found slumped over the steering wheel of Gaertner’s abandoned car. Police Officers testified they saw a woman getting into the car shortly before hearing gunshots. They later found an automatic pistol and a bottle of gin on the floor of her car. She was acquitted. The two Lawyers were the model for Billy Flynn, a slick criminal attorney, the character in Chicago portrayed by award-winning actor John O’Hurley.

Murder, Greed, Corruption, Violence, Exploitation, Adultery and Treachery, set amidst the razzledazzle of the 1920s is what the story of “Chicago” is all about.

NORTH MIAMI BEACH - The most common cause of heel pain here in the United States is a condition called plantar fasciitis. Athletes are affected, as well as soldiers, sales people on their feet over long periods of time and those who are overweight. It can affect one foot or both.

What is plantar fasciitis?

Plantar fascia is the flat band of ligament that connects our heel bones to our toes. The plantar fasciia supports the arches of our feet. If it becomes strained by walking, running or standing for long periods of time, especially on hard surfaces, it can become weak, swollen and irritated, resulting in severe heel pain.

People with flat feet or high arches are prone to this condition. Pronation (rolling of the feet inward when walking) is also a predictor of plantar fasciitis. Those with tight Achilles tendons or calf muscles are at risk for this, as well as people who are

overweight, or wear shoes that don’t fit well or are worn out.

Pain is usually felt in the heel and foot with the first step out of bed in the morning or after sitting for a long period of time. It is also likely to be felt after standing for a long period of time or when climbing stairs.

How is plantar fasciitis diagnosed?

After an indepth assessment of your medical history that would include past injuries and illnesses as well as current conditions under treatment, your podiatrist will assess how you stand and walk, where the pain is, what time of day you experience the most pain, and your level of physical activity. He or she may even xray your foot to rule out a stress fracture.

HEALTH AMA Designates Obesity as a Disease: What Does This Mean for Americans?

- Obesity levels continue to increase at alarming rates. Approximately one third of American adults are affected by obesity and another one third are affected by overweight.

Despite the fact that roughly 45 million Americans diet each year, the prevalence of obesity in this country has more than doubled among adults in the last 30 years. Without successful intervention, it’s estimated that half of the U.S. adult population could be impacted by obesity by 2030, presenting major consequences to our nation’s health.

To help combat this, the American Medical Association (AMA), recently voted to adopt policy recognizing obesity as a disease, requiring a range of medical interventions to advance treatment and prevention.

“Changing the way we label obesity from a major public health problem to a chronic disease may help encourage more proactive discussions between physicians and patients,” says Holly Lofton, M.D., Director of the Medical Weight Management Program at New York University Langone Medical Center. “Patients struggling with obesity, who are motivated to lose weight but have been unsuccessful with diet and exercise alone, should discuss a range of medical interventions with their physician including prescription weight loss medications, to help them lose weight and maintain it.”

Being a proactive patient goes a long way in the fight against breast cancer (BPT) - The grim reality is that one in eight women will be diagnosed with breast cancer during her lifetime. But women don’t need to sit back and wait for breast cancer to happen.

“Women can become proactive in their own health care to reduce their risks where possible and to increase their chances of early detection if breast cancer strikes,” says Jacqueline Ross, PhD., a registered nurse and senior clinical analyst in the Department of Patient Safety, The Doctors Company.

Breast cancer is second only to lung cancer in causing cancer deaths among women, with 220,000 newly diagnosed cases and 40,000 deaths each year in the United States. Fortunately, death rates from breast cancer have been declining since the 1990s due to early detection, screening and increased awareness.

Women can be proactive by increasing their knowledge of the risks of breast cancer. The majority of women with breast cancer have no direct family history of breast cancer.

The chance of getting breast cancer increases with age. Two-thirds of

women diagnosed with breast cancer are ages 50 and older. Some other risk factors related to breast cancer include radiation exposure, never having been pregnant, having the first child after the age of 35, beginning menopause after 55, never having breast fed, obesity, drinking more than one alcoholic beverage a day and having dense breast tissue, which can mask the presence of a cancerous tumor.

As with any risk factor, some of these can be controlled, but many cannot.

For example, hereditary factors cannot be controlled. A woman who has a sister, mother or daughter who had breast cancer - especially if cancer was in both breasts, was premenopausal or occurred in more than one first-degree relative - is two or three times more likely to develop breast cancer. If a woman has this history, she should consider genetic counseling.

Women can also be proactive by taking steps to help prevent adverse events in the diagnosis and treatment of breast cancer.

Some 92 percent of breast cancer malpractice cases involved a delayed or missed diagnosis, according to six years of data on breast cancer claims from The Doctors Company, the nation’s leading physician-owned medical malpractice insurer. Both patients and physicians have a responsibility to take action to prevent adverse events. Patients can be proactive by communicating with their physicians and then adhering to their instructions. following are other steps patients can take to help prevent adverse events: • Discuss with your physician when and how often to get screened.

Screening recommendations vary.

The American Cancer Society and the Susan G. Komen Foundation recommend that women over 40 get annual mammograms, whereas the U.S. Preventative Task Force recommends screening mammograms should begin at 50 and younger patients should discuss with their physicians when to initiate screening mammography.

• Discuss with your physician whether to get a digital or traditional mammogram. A study in the New England Journal of Medicine compared traditional mammograms to digital mammograms. The digital mammogram is stored in a computer, can be manipulated better for visibility and clarity, has a lower average radiation dosage, but is more costly.

The findings showed that digital mammograms were superior to traditional mammograms for three groups of women: those younger than 50, those with dense breasts (a risk

factor in breast cancer), and those who were premenopausal or who were in their first year of menopause.

• Work closely with your physician on developing a comprehensive health history. -Many risk factors for breast cancer are known.

Share any family history of cancer with your provider.

• Discuss with your physician how to do a selfbreast exam. Often sudden changes can be discovered in-between annual exams.

Let your physician know immediately if you notice any changes.

• If diagnosed with breast cancer, follow all your physician’s instructions for follow-up appointments and medications.

“While women can do nothing about the strongest risk factor for breast cancer - age - there is still a lot they can do to lessen other risks and increase their chances of successful treatment if diagnosed,” says Ross. “They can know the risk factors, get screened, be in touch with their bodies, make healthy lifestyle choices, communicate clearly with their physicians, and follow their doctor’s instructions.”

For under water homeowners, qualifying for refinancing is virtually impossible without HARP. When you refinance through HARP, you’ll likely end up with a lower monthly payment, a shorter-loan term or you can even refinance an adjustable rate mortgage into a 30-year fixed rate mortgage. You can invest those savings in other areas, such as home improvements, retirement savings or college funds.

Home improvements, in particular, can be a great investment because they can increase your home’s value, even as you continue to pay down how much you owe on it.

With mortgage interest rates still historically low but likely to rise - and the changes to HARP, it’s a good time for underwater homeowners to consider applying for the refinancing program. More homeowners will qualify under the new provisions, and some who were previously declined for the program may now be able to qualify for it.

Key HARP changes include: • No underwater limits - Borrowers will now be able to-refinanceregardless of how far their homes have fallen in value.

Previous loan-to-value limits were set at 125 percent. For example, if your home value was $100,000, your mortgage couldn’t exceed $125,000 in order for you to qualify for HARP. That limit has been lifted, so now you may qualify
