This one is the most frequently asked questions I am asked: Why should I go through an agency when I can hire my own aide and pay less per hour? Remember the age-old adage “You get what you pay for”? You may pay less for your aide hourly, but when you hire independently, do you really know what you are getting yourself into? These are just a few of the complaints that I have heard from patients who contacted us: • Things seem to disappear too often • She didn’t listen to what I wanted • She got so bossy that I felt like I was the employee • With her problems, I felt that I needed to help her with extra money • She kept coming late and leaving early • She was on her cell phone all the time • She was nervous driving to unfamiliar addresses, even to the doctor • She did not cook the way I like and would not listen to my suggestions • She didn’t buy the brands or cuts of meat that I wanted • She didn’t dress appropriately and when we went out, I was embarassed. With our agency, as with any other legitimate agency, we do not stand for any complaints, NOT ANY.

Many people are unaware of their responsibility to the IRS when hiring a household employee, or of the aide’s legitimate status to work in the US. Our aides are our employees and we report their earnings and our share to the IRS. Protect yourself by preventing problems. An agency is your safest way to go. While doing your own taxes can seem like a way to save, the opposite may prove true—sure, you’re not paying a professional for their advice, but, if that means missing out on deductions, you might be spending more in the long run. When it comes to filing your taxes this year, it may be time to get a tax professional involved. It is an investment that could even end up saving you money! Using Internet Software / Programs There is an exhaustive litany of tax forms and tax preparation software on the Internet. These solutions can be helpful and efficient at times but, as an overwhelming number of users are starting to discover, these programs are only as effective as those who are inputting the information. When dealing with online filing, it will be tougher for you to make the right decisions if you are not trained to do so. Returns are not a one-size-fits all endeavor. While evasion is illegal, avoidance is legal and this approach is what the ‘smart’ people have figured out. ‘Smart’ people also know if they don’t know something, they need to find someone who can fill the gaps in their expertise. Tax laws are constantly changing and evolving. These transformations are very tough for the average person to keep up with. Not only does the IRS keep closing loopholes but also, as Congress keeps passing new laws, additional loopholes are created. “By not using a tax professional, you are never going to know all the things that can save you money,” said Arun Sareen, President of Sareen & Associates in Hollywood. “Believe me, there are so many things that happened from a tax perspective in 2014 that sometimes it makes my head spin, and I do this for a living. But, I stay on top of those changes, because knowing those things allows me to be that much more helpful to my clients.” Last Minute Tax Changes Many changes were made to the tax laws in the last month of 2014. These modifications make it very difficult for individuals to keep up with all the changes that have been made and the
savings they can get this tax season. Additionally, many taxpayers will have to cope with the tax consequences of the Health Care Act (Obamacare), and that is also uncharted territory. On Dec. 3, 2014, the House passed legislation extending a lot of tax breaks. The Senate then passed the Tax Prevention Act on Dec. 16, which the President approved soon thereafter. It was out of the ordinary that two weeks before the end of the year, tax breaks were extended– retroactive to Jan. 1, 2014 In light of the last-minute changes, there was a great deal of confusion as to what could and couldn’t be deducted. From state and local general sales tax to itemized deductions, tuition and fees, if you do not use a professional to guide you this year, you could be missing out on a lot of money. “This is really going to be a tough year for so many, and not relying on someone who continues to keep up on the changes may cost you,” Sareen explained. “Because our government told us that we were not going to be able to get tax breaks, and then two weeks before the end of the year changed their mind, it sent so many scrambling to start sifting through receipts.” The Tax Increase Prevention Act of 2014 restores tax breaks that expired at the end of 2013. Tax breaks that are relevant to 1040 filers are:
• The deduction for educator’s classroom expenses. • The tuition and fees deduction. • The tax credit for nonbusiness energy property (insulation, windows, doors, etc.). • The exclusion from tax for debt canceled on a principal residence. • The deduction for mortgage insurance premiums. • The deduction for state and local sales tax in lieu of the deduction for state and local income tax. • Charitable IRA distributions. HR 5771 also revises the maximum amount that employers can reimburse employees for mass transit and commuter highway vehicle benefits. The benefit amount for 2014 had been set at $130 per month, but HR 5771 revises the benefit amount to be $250 per month. Other Benefits Among those who will benefit from the retroactive extension are teachers who buy classroom supplies, mass-transit commuters, residents of states with no income tax, parents with kids in college, some homeowners and some retirees with IRAs. Deduction for teachers’ expenses: This measure lets school teachers deduct up to $250 for the costs of classroom supplies that they buy with their own money. It’s available to all teachers, whether they

No glasses for the first time in 50 years

Dave Forchin can look back at his childhood in Jamaica now and laugh. But it wasn’t funny when he couldn’t see what the teacher was writing. The 62-year-old South Florida resident remembers it like it was yesterday, attending primary school at the age of 12 and not being able to see. “That was when I went to the eye doctor and found out that I needed glasses to see and be able to do better in school,” Forchin recalled. For the next 50 years, I had to wear glasses to see since I was very near sighted.” Using computers much of his working life in banking, Forchin never had a serious problem with his vision - until three years ago when a visit to his family optometrist, Dr. David Popper at Eyelooks in Miramar, revealed cataracts. In addition, Dr Popper determined that Mr. Forchin would not be able to drive unless he underwent cataract surgery and that’s where things started to get better. Dr. Popper’s discovery meant a trip to meet Dr. Stanley Braverman and his well respected team at the Braverman Eye Center in Hallandale Beach. While he was apprehensive at first, the moment he met everyone, his fears were put aside. “Any time you have any surgery with your eyes, anyone would wonder what everything was going to be like,” Forchin said. “That’s why when I met Dr. Braverman and we talked over the procedure, I left the office with a great feeling.” As time got closer to the point where Forchin wouldn’t be able to see without the procedure, he looked forward to finally not having to rely on glasses and seeing perfect for the first time in over 50 years. “I remember talking about having to have both eyes done,” Forchin explained. “We sched-
uled the first one (right eye) back on December 12th of last year. Things went so well, we scheduled the cataract surgery on the left eye on January 20th.” In the weeks and months following the surgery, Forchin admits that anyone needing to have this done, they should go ahead and get it scheduled. His life has never been better - and not having to wear glasses is something that he can simply not explain. From the first visit and consultation to the actual surgeries, Forchin simply cannot speak highly enough about Dr. Braverman, the staff and the experience. “It’s simply the most amazing thing that I have ever done,” Forchin said. “No pain, a day to recover and seeing better every day. What more could I have hoped for?” From that 12-year-old who started wearing glasses - to an adult that had to wear thick lenses to see, Dave Forchin is enjoying life more than ever! WHAT ARE CATARACTS A cataract is formed when the natural lens of the eye, responsible for focusing light and producing sharp images, becomes cloudy and hardens, resulting in a loss of visual function. The lens of the eye is clear at birth but is one of the first parts of the body to show the effects of aging. A cataract is painless and usually develops gradually over several months or years. The onset of a cataract in one or both eyes may cause decreased night vision, impaired depth perception, and increased color distortion. Cataracts are the leading cause of vision loss among adults age 55 and older. In fact, poor vision from cataracts affects 60 percent of all adults over age 60. However, cataracts also can result from injury, heredity, or medications, so they

Understanding that no two conditions or patients are the same, Dr. Braverman analyzes the patient’s symptoms, performs a comprehensive eye exam, and may test for glare disability and contrast sensitivity before making a diagnosis. The final decision to proceed with the surgery belongs to the patient. Your optometrist or Dr. Braverman may prescribe glasses, medication or surgery depending on your symptoms, complaints and age. Usually surgery will not be recommended until glasses no longer work to make you visually happy. In the case of clear lensectomy surgery, the patient cosmetically wants to get rid of their
glasses, but they are out of the LASIK treatment range. CATARACT SURGERY: What to expect Thanks to new advancements, this is a great time to have cataract surgery. Your eye is like a camera. It has photographic paper(the retina), and a camera lens. Two excellent options exist for cataract surgery. The choice is yours. Traditional Cataract Surgery is the removal of your cloudy natural camera lens, which is usually dissolved using high frequency ultrasound through a small (less than 3 millimeter) opening. If your eye qualifies, the second option, Laser Assisted Cataract Surgery can now be done using state of the art Femtosecond Laser technology to assist with
parts of the cataract surgical procedure, including softening up the cloudy camera lens. This reduces the amount of high frequency ultrasound needed. At one time, Fort Lauderdale, Daytona Beach and anywhere in Miami was almost the perfect spring getaway for college students and the younger crowd, looking for a great time and warm sunshine. Times have changed through the years, and while many of the south Florida communities embrace the younger crowds, many are going after the families, and that’s been a winning combination. What family wouldn’t want to get out of the snow and the daily temperatures that never get above 20 degrees? This is a destination that has taken Disney and other themed parks out of the equation when it comes to a spring break that has it all. At the center of this spring break madness is Aventura, Hallandale Beach, Hollywood and just about any seaside community that offers ocean, shopping, dining, and did we say, water? The deduction can be a boon for itemizers in Florida. Residents don’t pay an income tax but pay sales taxes, either at the state or local levels. Deduction Of Mortgage Insurance In addition to mortgage interest, private mortgage insurance is sometimes deductible. Don’t mistake private mortgage insurance, or PMI, for homeowner’s insurance that protects against a fire or other loss. PMI comes into play with lower-income homeowners who often can’t afford a big down payment, and instead pay a small monthly fee as insurance against default. The idea is to protect the lender against being stuck with a big loan with zero equity in the home, as well as to allow those without huge nest eggs to buy a property with minimal down payments. If you make a private mortgage insurance payment, in most cases this is deductible. Also worth noting is that local and state property taxes can also be itemized on federal tax returns. Particularly for lower-income Americans, there may be special property tax benefits available based on your community. Now, Unfortunately, A Word About Fraud Last year, the IRS “suspected and stopped” 3.6 million returns showing refunds, and that number is expected to rise again this year. In tax refund fraud, the perpetrators file a tax using someone else’s name and Social Security number, but with a new address or a different DDA (direct deposit account, i.e. checking or savings or stored value card) account to collect a refund, and then, very quickly, they’re gone. Victims usually don’t figure out the scam until they file their legitimate return. That’s when they typically find out that another return has already

It can takes months before all this is ironed out and the taxpayer gets his or her rightful refund. The bad news: Florida is the worst state in the country for this kind of tax refund fraud. If you’re already a previous victim of identity theft, the IRS will provide you with a six-digit “identity protection” PIN. All you have to do is ask. The IRS will send you a form that is essentially an affidavit, which you’ll return with various forms of identification. The IRS will then send you the PIN, which you will use when filing your return. If a return is filed using your information, but without the PIN, it will be rejected by the IRS. If you file your returns here in from Florida, you are eligible for the “identity protection” PIN program described above, even if you haven’t previously been victimized by this particular type of fraud. The IRS is running a pilot program that you can participate in that will allow you to use the PIN for your return. There are things you can do to protect your health but it’s also good to know where germs linger. Some of their hangouts may surprise you. According to WebMD, there are likely more germs in your kitchen sink than in your toilet. So, what can you do to reduce your risk of exposure to germs? Here are a few ideas to help reduce the risks and keep everyone healthy. • Wash your hands. Your hands come into contact with each of the surfaces mentioned above and they can move viruses and bacteria to your nose, eyes and mouth. This means it’s important to keep them clean. It may seem like a long time to be washing them, but the 20second rule is for real. Hand sanitizers are useful but not a substitute for good hand washing. • Sanitize commonly used items. Treat A Skin Tear With Respect By Marylynne Newmark, Medical Writer
As we age and lose collagen in the subdermal layers of our skin, we notice how the skin’s outer layer becomes crepelike and tears easily when brushing against a sharp edge. This was the recent case with a patient of mine, who had experienced a skin tear in a rehab just before I met her. Through lack of proper treatment at the rehab and minimal wound care ordered at home, her healing and circulation became compromised. When I became aware of her pain ten days later, and saw the state of her wound for the first time, I referred her to Dr. Charlton Adler, wound care specialist at both Aventura Hospital and the Memorial System, and accompanied her to her appointment. Dr. Adler pointed out how the edges of the skin tear had rolled under and new subdermal tissue was beginning to fill in the small cavity left by the tear and showed signs of entrapping the borders of the tear by growing over it. After anesthetizing the site, he gently began softening and restretching the rolled over skin, debrided the new dermal tissue that would have been excess, and surgically
placed the original skin flap over its natural area. He secured the flap with steristrips, covered with Tagaderm and left everything in place for a week to heal undisturbed. “It’s such a shame,” said Dr. Adler, “If the skin flap would have been placed on its site in the very beginning, the patient would not have been subjected to this pain, and it would have healed by now.” Wounds, even as superficial as a skin tear, should get professional attention when it comes to the elderly or with diabetics who heal poorly. The body’s healing capacity slows down with age and is complicated with diabetes. A simple skin tear can become infected or compromise circulation, as it did with this patient. Wounds should be cleaned and treated with an antiinfective cream or spray and covered to avoid contamination. Dressings should be changed daily unless specialty stay-in-place products are used after aseptic conditions are employed. And if the wound does not show signs of healing in a few days, please see a wound care specialist like Dr. Adler. Early professional attention can avoid
