Drawbacks Of Hiring Your Own Aide By Terry Adler, CEO, Always Classic Care - Aventura Digest

This one is the most frequently asked questions I am asked: Why should I go through an agency when I can hire my own aide and pay less per hour? Remember the age-old adage “You get what you pay for”? You may pay less for your aide hourly, but when you hire independently, do you really know what you are getting yourself into? These are just a few of the complaints that I have heard from patients who contacted us: • Things seem to disappear too often • She didn’t listen to what I wanted • She got so bossy that I felt like I was the employee • With her problems, I felt that I needed to help her with extra money • She kept coming late and leaving early • She was on her cell phone all the time • She was nervous driving to unfamiliar addresses, even to the doctor • She did not cook the way I like and would not listen to my suggestions • She didn’t buy the brands or cuts of meat that I wanted • She didn’t dress appropriately and when we went out, I was embarassed. With our agency, as with any other legitimate agency, we do not stand for any complaints, NOT ANY.

• Our aides are professionally trained with documentation and references carefully checked • All aides and nurses have been screened by the FBI with confirmation that they have no criminal records • A registered nurse opens each case to assess the exact needs of the new client • The client and/or family is interviewed for specifications and expectations • Staff carefully matches an aide to the personality and needs of the client • If the match is not perfect, staff will make changes immediately. You definitely can hire whomever you like. However, you should consider the following carefully: • How well do you know this person? Can you trust him or her? Are you sure the aide does not have a police record? How do you know? • Is the aide insured to cover any liability if you should become injured while in their care? By hiring through an agency, you can be certain that liability covering the aide is in place. • Does your aide have a legal right to work in this country? That translates to a Green Card, Permanent Residency or actual citizenship. • Will you be paying your share of their Federal taxes? Our agency does. We report and we pay, as good citizens should.

Many people are unaware of their responsibility to the IRS when hiring a household employee, or of the aide’s legitimate status to work in the US. Our aides are our employees and we report their earnings and our share to the IRS. Protect yourself by preventing problems. An agency is your safest way to go. Even the home health aides know that, and although they accept statewide standard rates that may be lower than what they could get from the client privately, reputable aides know that by working through reputable agencies, they also are protected and can be steadily employed with respect. Near-Far Multifocal PresbyLasik: Correction for two kinds of vision problems

For years, the miracle of Lasik surgery for vision correction was almost always an either-or procedure. Patients who wanted to throw away their glasses or contact lenses but were old enough to require reading glasses on top of their distance correction, had to choose one kind of correction or the other for each eye. Now, a remarkable new procedure called PresbyLasik allows most patients to eliminate both problems. Jose Berrios, 65, and his wife, Alma Mazar-Berrios, 63, both qualified for the PresbyLasik procedure and are thrilled with the results. Jose, a Miami-Dade transit driver, began to require reading glasses years ago. Then over time, he started to have trouble seeing at a distance as well, and needed either bifocals or two different pairs of glasses. “I was going for a vision test every two years, and the prescription would always change,” he recalls. “I was really tired of it.” One day, a co-worker told Jose about her daughter, who had had successful Lasik surgery by Dr. Stanley Braverman. Intrigued, he decided to pay a visit to Dr. Braverman at the Braverman Eye Center in Hallandale Beach. “I hoped I’d be able to quit wearing my glasses,” he says. As a bus driver driving at night, Jose needs to be able to see clearly, because the safety of his passengers can depend on it. When he’s not working, he enjoys going to the firing range, a hobby that also requires keen distance vision. And when he relaxes at the couple’s home in Hialeah, he likes to surf the Internet, for which he needs clear close vision. He didn’t really expect to be able to have both problems corrected, but he was happily surprised. “Dr. Braverman gave me a complete eye exam,” Jose recalls. “Then he told me about the (PresbyLasik) procedure and said I’d be a good candidate.” Jose had the painless procedure and since then, he can see clearly both for distance and for reading. He is now two years out from the procedure. “It’s nice not to have to wear glasses anymore,” he says. “I enjoy being able to see again.” Jose was so pleased with his results that Alma decided to see if she might qualify for PresbyLasik. A month after her husband had the procedure, she, too, visited the Braverman Eye Center. She was doubtful about her candidacy, however,

Her prescription was much stronger and more complex than Jose’s. “I wore trifocals,” she says. “I work a lot at the computer and I was having problems. I couldn’t see TV without glasses--everything was a blur -- and I couldn’t see signs when I was driving.” The other reason Alma wondered whether the procedure could be successful for her: “I’d been told in years past that I wasn’t a candidate for Lasik,” she says. “I didn’t think I’d ever be able to go without glasses.” But Jose kept urging her to try again. And at her visit to the Braverman Eye Center, Alma discovered that technology had changed since she last inquired about Laser Vision Correction Surgery. During the consultation, she learned that she was indeed a candidate for PresbyLasik. Alma decided to go ahead. “My hus-

band was so happy with his outcome that he convinced me to have the procedure,” she says. Just as it was for Jose, PresbyLasik was successful for Alma. Now she is also one year out from the PresbyLasik procedure. Whether they are at work, out for a drive, watching a movie or walking their dogs (two German shepherds and Australian sheepdog), they do it without the inconvenience of glasses or contacts. “I’m very happy not to have to wear glasses,” Alma says. “It took me awhile to figure out I didn’t need them anymore. I kept putting my finger on my nose to push them up. I feel very grateful. Both of us have recommended Dr. Braverman to friends.” Dr. Braverman started doing the PresbyLasik procedure over three years ago. It was introduced into the United States approximately seven years ago, having originally been done using the

Allegretto Wavelight femtosecond laser by an Italian Eye Surgeon, Dr. Roberto Pinelli. This is the same Laser that Dr. Braverman uses. Dr. Braverman said “this technique was developed by taking what Eye Surgeons already knew about how Lasik changes the shape of the front of the eye to eliminate glasses, and just modifying the techniques to be able to reduce or eliminate the need for distance glasses, as well as reading glasses.” What To Expect With Z-lasik Surgery All Laser Lasik surgery is performed on a patient’s cornea, which provides most of the focusing power when light enters the eye. Topical anesthetic drops are applied to the eye. The surgeon first uses a special Ziemer LDV femtosecond laser to create a thin layer of corneal tissue that can be

folded back (a corneal flap). Then, another type of laser (an Allegretto EyeQ excimer laser) is used to reshape the underlying cornea, painlessly. After the excimer laser treatment, the corneal flap is repositioned and bonds back into place naturally. The corneal reshaping improves the focusing power of the cornea. Although no one can promise patients “perfect” vision, most Laser Vision Correction patients with mild to moderate prescriptions with or without astigmatism, do achieve 20/20 or better vision. In the 13 years that the South Beach Wine & Food Festival has been attracting some of the top chefs from across the globe, thousands of culinary groupies have flocked to this part of the country - getting away from the cold weather and taking in some of the best that this popular event has to offer. While numerous changes have been made - including the name that has been switched for the 14th year to the Food Network & Cooking Channel South Beach Wine & Food Festival presented by FOOD & WINE - this has become a destination that now brings in some of the best this world has to offer. This year’s Festival consists of more than 75 events during the four-day, star-studded gourmet gathering. Taking place February 19–22, proceeds from the Festival benefit the Chaplin School of Hospitality & Tourism Management’s Wine Spectator Restaurant Management Laboratory and Southern Wine & Spirits Beverage Management Center at Florida International University. “Having been to something like 12 of these events, I made sure that I purchased tickets early this time around,” Gabrielle Elias of Aventura said. “I know that some tickets are very hard to get, so I worked on those first. German shepherds to gerbils: How to choose the best pet for your family (BPT) - Whether a loyal pal like Lassie is what you’re looking for, or a Garfield-like couch potato is more your speed, a pet can make a wonderful addition to your family. But there is more to choosing a new pet than simply falling in love with a cute face. Making an informed and well-researched decision can lead to a happy home life for both you and your pet. “One of the most important things to know is that all pets are awesome. They have the potential to be great family members,” says Tracey Kitching, veterinary technology instructor at Argosy University, Twin Cities. “It all boils down to what you want the animal for, the

demands of the animal, and the husbandry and expenses associated with that animal.” “The priority is the family needs to decide the type of time they have available for the animal,” says Paula Lind, department director of Veterinary Technology at Argosy University, Twin Cities. “The family also needs to look at its capability to provide the appropriate exercise, living quarters and temperament. Sometimes people are mismatched with the animals they choose, and shelters are filled with young dogs that did nothing wrong. They were just a poor match for their owners.” The energy of the ani-

mal is an important factor to consider. “For any species, understanding how much energy that animal has is important,” says Kitching. For example, a border collie wouldn’t be good for people working 16 hours a day, while great Danes tend to make great apartment dogs since they generally lay around a lot. Your family must be able to meet the energy needs of the animal. Lind says that if you work long hours, a cat would be a better choice than a dog, but it’s important to remember that cats also need companionship. For families that aren’t home much, a fish, hamster, gerbil, rat or ferret would be better choices, says Lind. Puppies will

require exercise and obedience training, which will require more time than an older dog or other types of animals. Temperament is another key area, especially when looking at breeds of dogs. People often look at puppies as the ideal dog for their small children, but Lind says this isn’t the case. She wouldn’t recommend a puppy for children younger than 5, and adds that the puppy cowering in the back of the litter is the worst to pick for children. The cost of owning a pet goes beyond the initial purchase of the animal. Kitching says an animal like a hamster will require an initial purchase of a cage, wheel and food,

while a dog will require a collar, leash, obedience lessons, a kennel and food, among other things. The amount an animal eats will also greatly affect the cost of ownership. Before deciding on any pet, research is key. “The biggest thing is you have to do your research and think about the needs of the animal. It’s not about you, it’s about the animal,” says Lind. “It’s going to need your time,

love and attention.” For dogs, talk with breeders, people who own that breed, and research breeds on sites like the American Kennel Club website, which contains info on all breeds of dogs. If you’re adopting an animal, Lind says many rescue groups and shelters do an excellent job of assessing temperament. These shelters will know if an animal is good with children and other animals.

But, you may be overlooking one major health necessity that is as close as your kitchen faucet water. It’s vital all year long, even during winter months when you might think hydration is less important. “Staying hydrated is a very important component of staying healthy,” says physician assistant Tricia A. Howard, a faculty member at South University, Savannah College of Health Professions . “Most people know they need to stay hydrated when they are doing vigorous exercise or in very hot weather, but they don’t realize the importance of making sure they are getting enough water every day.” Why water is important Sixty percent of your body weight is made up of water. Since water carries nutrients to cells in your body, and flushes toxins from vital organs, Howard says it is important to replace what you lose daily. “Our bodies are always losing water even when we breathe we are losing small amounts,” she says. “So, it is important to know how much water you need daily and to make a point of drinking it.”

Your daily intake Howard says men need three liters, or 13 cups, of water a day. Women should drink 2.2 liters or nine cups daily. “If you drink water with each meal, and at least one glass between each meal, you will be very close to what you need to drink every day,” Howard suggests. And there is good news if you want some variety. Howard says beverages like milk and even coffee can take the place of some of the water that you need to drink daily. “Drinks that are high in water, and low in calories, are acceptable substitutions for water,” Howard explains. “But watch out for sugary, high-calorie drinks that will cause weight gain.” Beauty benefits Howard says the benefits of staying properly hydrated every day go beyond good health. Staying hydrated also approves the appearance of your skin. “Drinking a glass of water is one of the easiest things any of us can do,” Howard says. “It is important to how your body works on the inside, and how it looks on the outside. Adding this simple step to your daily routine is well worth the many benefits.”

Fort Lauderdale’s long awaited stage production of “The Lion King,” will run at the Broward Center from January 7, to February 1, 2015 in the Au-Rene Theatre. It tells of the birth of a young lion prince cub, born in Africa, named Simba, and his struggles to accept the responsibilities of adulthood and his destined role as king, much to the dislike of his uncle Scar, who now becomes second in line to the throne. Scar plots with the hyenas to do away with King Mufasa and Prince Simba, in order to make himself the king. When Mufasa is killed, Simba is convinced by Scar, he alone is to blame, there-fore Simba flees the kingdom. After years in exile, he is compelled to return to over throw his uncle, the imposter, and claim the kingdom as his own, thus completing the “Circle of Life.” With Scar and the hyenas gone from the top of Pride Rock, Simba looks down happily at his kingdom, with his family Nala, Timon, and Pumbaa as he and Pride Rock are acknowledged and restored to their former glory. Based on the real life story of the Newsboy Strike of 1899, this Disney musical tells the story of Jack Kelly, a rebellious newsboy who dreams of a life as an artist living away from the big city. After publishing giant Joseph Pulitzer raises all newspaper prices, at the newsboys’ expense, Kelly and his fellow newsies take action. With help from a beautiful female reporter, Katherine Plumber, all of New York City soon recognizes the power of the little man. After many trials and problem tribulations, Jack and Katherine join the newsies in printing their own paper, that tells the outcome of the strike. Jack makes a deal with Pulitzer. The strike ends and all ends well. It is a fast -paced new musical with awesome choreography and catchy tunes written by eight-time Oscar Winner Alan Menken, who also wrote the title song for “Beauty and The Beast,” and “The Little Mermaid.” This a a great family musical, the entire family will enjoy including an added love story between Jack Kelly and the new character Katherine Plumber. They are held to a “fiduciary” standard of care, which means they have a legal duty to place the interests of their clients first. Brokers, or registered representatives, are regulated by the Financial Industry Regulatory Agency (FINRA) and are generally not considered investment advisors by federal regulators. They are held to a lower “suitability” standard, although FINRA views that as including a “best interest” of the client standard. While more than half of U.S. investors use a professional financial advisor (a catch-all phrase that some might see as including financial planners, accountants, brokers, RIAs and even insurance salespeople), four in 10 don’t know which standards govern their advisor, according to a 2013 survey conducted by TD Ameritrade Institutional. Yet it is a key distinction. “It’s not uncommon for investors to think a broker is the same as a registered investment advisor,” says Skip Schweiss, managing director of advisor advocacy and industry affairs for TD Ameritrade Institutional. “Even savvy investors may not know the difference. But different types of advisors deliver different types of services and are held to different professional standards. Before you invest with any advisor, it’s important to understand what their credentials mean, and under what accountability standards they operate.” Fiduciary vs. suitability There are two basic standards for financial advisors: fiduciary and suitability. Of these two standards, Schweiss says, the fiduciary standard for RIAs is the higher one. Under the fiduciary standard, RIAs are bound by a 74-year-old federal act - and regulated by the SEC and state regulators. While both RIAs and brokers are required to act in their client’s best financial inter-

ests, RIAs are required to put their client’s interest first, even above their own or their company’s interests. Brokers (also known as registered representatives) operate under the suitability standard. They are required to have reasonable grounds for believing that securities recommendations are suitable based on information provided by the customer regarding other security holdings, financial and tax status, and investment objectives, and other information that would reasonably need to be considered. “Financial advice can come from practically anywhere these days,” Schweiss says. “Some of it will be good, some will be spurious. When you’re investing your money, it’s important to do your homework and not only research the individual or firm who is helping to manage your money, but the laws and standards to which they are held accountable.” Schweiss also offers these tips for choosing an advisor: • Know who regulates the advisor you’re considering. The regulatory body he or she answers to can clarify what standards he or she operates under. • Ask how he or she is compensated. By companies selling investment products? Fees paid by clients? Commissions? Compensation type may give you an idea of any potential conflicts of interest between what’s best for your money and what puts the most money in the advisor’s pocket.

• Learn whether the advisor has been disciplined in the past for dishonesty or inappropriate behavior. You can find background information on financial advisors through the websites of the SEC, FINRA, National Association of Personal Financial Advisors, Certified Planning Board of Standards and the Financial Planning Association. Information may also be available through your state securities agency. • Ask how frequently your advisor’s firm conducts audits, and if your portfolio assets will be handled by a thirdparty custodian. A custodian will provide a monthly statement of your assets to both you and your advisor. “Information is an investor’s greatest asset,” Schweiss says. “While investing always carries an array of different risks, selecting the right advisor and knowing how he or she operates can help ensure your investments are in alignment with your plans and risk tolerance. Treat A Skin Tear With Respect By Marylynne Newmark, Medical Writer

As we age and lose collagen in the subdermal layers of our skin, we notice how the skin’s outer layer becomes crepelike and tears easily when brushing against a sharp edge. This was the recent case with a patient of mine, who had experienced a skin tear in a rehab just before I met her. Through lack of proper treatment at the rehab and minimal wound care ordered at home, her healing and circulation became compromised. When I became aware of her pain ten days later, and saw the state of her wound for the first time, I referred her to Dr. Charlton Adler, wound care specialist at both Aventura Hospital and the Memorial System, and accompanied her to her appointment. Dr. Adler pointed out how the edges of the skin tear had rolled under and new subdermal tissue was beginning to fill in the small cavity left by the tear and showed signs of entrapping the borders of the tear by growing over it. After anesthetizing the site, he gently began softening and restretching the rolled over skin, debrided the new dermal tissue that would have been excess, and surgically

placed the original skin flap over its natural area. He secured the flap with steristrips, covered with Tagaderm and left everything in place for a week to heal undisturbed. “It’s such a shame,” said Dr. Adler, “If the skin flap would have been placed on its site in the very beginning, the patient would not have been subjected to this pain, and it would have healed by now.” Wounds, even as superficial as a skin tear, should get professional attention when it comes to the elderly or with diabetics who heal poorly. The body’s healing capacity slows down with age and is complicated with diabetes. A simple skin tear can become infected or compromise circulation, as it did with this patient. Wounds should be cleaned and treated with an antiinfective cream or spray and covered to avoid contamination. Dressings should be changed daily unless specialty stay-in-place products are used after aseptic conditions are employed. And if the wound does not show signs of healing in a few days, please see a wound care specialist like Dr. Adler. Early professional attention can avoid

unnecessary pain, compromised healing, and a threat to one’s health. Dr. Adler performs vascular testing right in his office. He prescribes medications and exercise to improve circulation. (BPT) - A visit to the pharmacy can be about more than just picking up prescriptions or supplies for the medicine cabinet. It can actually be an important, and convenient, time to ask your pharmacist key questions that could impact your health. Dr. Andria Fetterman, a faculty member in South University Savannah’s School of Pharmacy, encourages people to use their pharmacist as a resource for information about everything from prescription medication to lifestyle changes. Fetterman says there are five key questions you should frequently ask your pharmacist. What can I expect from this medication? Fetterman says whether you’re picking up an over-the-counter remedy or a prescription, you should feel free to ask your pharmacist what to expect from the medication. “Many medications have potential side effects that you should be aware of before you take them,” advises Fetterman. “It may be something as mild as a little stomach discomfort or drowsiness, but knowing what to expect will help you handle any potential side effects. Also, knowing what to expect could keep you from blaming any new, unrelated symptoms on your medication.” You should also ask how long it will take for your medication to make you feel better. Fetterman says most antibiotics should have you feeling better in three to five days, but an anti-depressant may need to be in your system for three weeks before you begin to feel its effects. If a prescription medicine doesn’t seem to be working in the proper time frame, you should let your doctor know. Remember that sudden discontinuation of some medications, without proper professional advice, can be costly to your health. How long should I take an over the counter medication before I call my doctor? Over-the-counter medicines can provide relief for a number of ailments, but sometimes you need to see a doctor either for a prescription or a correct diagnosis of your symptoms. Fetterman says your pharmacist can help guide you when you’re not sure if it is time to see a doctor. “A pharmacist can give you advice on how quickly you should see symptom relief with an over-the-counter medication,” says Fetterman. “Many times, people will treat themselves too long with over-the-counter medicines or will switch from one product to another looking for

relief, and many times don’t go to a physician when they need to.” How do I manage my condition? “When someone leaves the doctor’s office after a diagnosis, they may be scared or shell-shocked,” Fetterman explains. “There may be questions about medication or daily management that they didn’t think to ask. That’s where a pharmacist can help.” Fetterman uses Type 2 diabetes and high blood pressure as examples. “A pharmacist can counsel you about weight loss, healthy eating and ways to promote cardiovascular activity. With proper disease management, some patients can actually reduce or eliminate their need for prescription medication altogether.” Do these medicines mix? Fetterman says it is important to ask a pharmacist about whether any new medication will mix well with other things you are already taking. “Dietary supplements, herbal supplements and over-the-counter medicines can interact or interfere with prescription medications. When you are having a prescription filled, your pharmacist should ask you about all other medications that you’re taking. If you’re on prescription medication, check with the pharmacist before beginning any new over-thecounter supplements or medication to make sure it is safe.” What should I do about my persistent, minor symptoms? Fetterman says you should never hesitate to ask a pharmacist about a persistent, minor symptom that might be bothering you. “If you’re having a non-specific or persistent symptom that you think is minor, you should still ask your pharmacist about it,” says Fetterman. “A pharmacist can recommend an over-the-counter medicine that may relieve symptoms, but can also advise you to see a doctor about your symptom rather than ignoring it. “A pharmacist can be a great source of health information.
