For the past 86 years, the authorities in Hallandale Beach have been attempting to apprehend the notorious "Outhouse Gang" that shows up in different locations with their graffiti-covered outhouse. This year, they even posed for this photo at the Hallandale Beach City Hall. Members have played an annual game of cat and mouse with police since their first appearance on Halloween 1929. Since then, the crew has left an outhouse covered with political and other messages outside of the city post office.

Since going through the real estate bust, many plans that were approved over the years are not going to be built. We are now seeing another resurgence of development interests that may or

may not be built. i believe it is time to look at this issue again. in the next articles, i will be covering some history of our city and traffic that i believe is critical to understand what the process is and where we can make a difference when it comes to traffic. understanding what can or cannot be done to address traffic in our

communities is often misunderstood. The first issue is understanding that traffic has been an issue since the horse drawn buggy. As a fact, due to accidents and grid lock, traffic management was born. There needed to be order at intersections. Beginning in Rome, with so much commerce, flow of people and carts to the city

history, shows the creation of roads and control of intersections. in the late 1800’s, the first universal illuminated stop signs were created in London to control traffic over their famous bridge. in the late 1800’s to the early 1900’s, uS cities started using police to manage traffic and shortly after that, the first government regulations were cre-

The Saturday Evening Post named this occurrence as the “traffic jam.” So you can see this is nothing new and it is not going to end, it is how it is managed that is the key. in today’s society, the government layers have become much more complex. it is not just about one city putting in a stop sign or light. Each road and mode of transit is managed as one large system. Our city does have city roads. These are mainly within our neighborhoods. There are county roads, state roads and interstate roads. The state agency that regulate roads is the Department of Transportation. They distribute money to counties, other planning agencies who maintain and expand our transportation system. you also have the Federal Aviation Agency and the Army Corps who work with ports. The county takes this money and then spends it on transportation. They control the road construction to an extent, manage the road system, operate the bus system, port and airport. in our

county, we have a Metropolitan Planning Organization that does the long range strategic planning and approves the five year work plan. The MPO is comprised of both county and city officials. i serve on the MPO representing this area of the county. The MPO meets monthly to review transportation issues in our communities. The staff oversees the projects and works directly with the county and cities on the cities issues. Through technical members from our cities, these needs are addressed. Our city roads are still required to meet government standards on how they are built and controlled. The county regulates and enforces these state standards. They also control stop signs, street signs and traffic calming devices. We cannot just place these devices without meeting the requirements. next week’s article: Our roads and our plans.

Mayor Peter Bober and the City of Hollywood will host the 5th annual Veterans Barbecue on Nov. 11

On Wednesday, november 11, Mayor Peter Bober and the City of Hollywood will host the 5th annual veterans Barbecue, honoring men and women who are currently serving or have served in the united States Armed Forces. Mayor Peter Bober will work the grill and members of the Hollywood City Commission, along with volunteers, will serve a free barbecue lunch to Hollywood military personnel, veterans and their families. The celebration is from noon to 4 p.m. at Ty Park, Corporate Pavilion #12 at 3300 n. Park Road in Hollywood.

The menu will feature traditional barbecue fare including, barbecue chicken, hamburgers, baked beans, coleslaw, corn on the cob, fruit salad, cookies and brownies, and an assortment of beverages. Once again, Jaxson’s ice Cream will be scooping up its world famous ice cream. in 2011, Mayor Bober wanted to create an opportunity during the veterans Day holiday to celebrate and salute our veterans and military personnel who have served our country. Later that year, Hollywood hosted the first veterans Barbecue as part of veterans

Day celebrations. “i wanted to do something to make our City’s veterans and military personnel feel truly special over the veterans Day holiday,” says Mayor Bober. “i would always see lines of people waiting to get into local parks on veterans Day, and it occurred to me, it’s not that there are too

many barbecues on veterans Day, it’s that there aren’t enough barbecues that specifically celebrate veterans. We’ve changed that in Hollywood and each year the event gets bigger and better.” in addition to great food and company, the City of Hollywood veterans Barbecue will

feature music by DiXon, a banner plane fly-over with a message of thanks to veterans and a wall of honor. This year, in honor of the City’s 90th anniversary later this month, a traveling photo wall of Historic Hollywood will be on display with images dating back to WWii. Advance reserva-

A holistic approach to orofacial pain management is important, and the social, cultural, psychological and cognitive context of each patient needs to be considered in the process of diagnostic formulation, as well as in the development of a pain management plan according to the biopsychosocial model. Recognition of psychological comorbidities will assist in diagnosis and management planning. Orofacial pain may be defined as pain localized to the region above the neck, in front of the ears and below the orbitomeatal line, as well as pain within the oral cavity. it includes pain of dental origin and temporomandibular disorders, and thus is widely prevalent in the community. up to a quarter of the population reports orofacial pain, and up to 11% of this is chronic pain. Patients with orofacial pain present to a variety of clinicians, including headache physicians, dentists, maxillofacial surgeons, otolaryngologists, neurologists, chronic pain clinics, psychiatrists, and allied health professionals such as physiotherapists and psychologists. Orofacial pain is associated with significant morbidity and high levels of health care utilization. There are few causes for dental pain; however, because of significant neural convergence in the jaws and face, it may be referred, poorly localized, or misdiagnosed. The 4 major causes of dental pain are pulpitis, cracked tooth syndrome, dental abscess, and dentine sensitivity. These are often acute conditions, but because

they are common, they may coexist with other chronic pains. Both the dental pulp and periodontal ligament contain nociceptors. nociceptive output in these areas is triggered by changes in pressure and the effect of inflammatory mediators.

PuLPiTiS Pulpitis is the term used to describe pain because of inflammation of the dental pulp, and it is usually due to dental caries. inflammation of the pulp leads to accumulation of extracellular fluid, inflammatory mediator release, and vasodilatation, which causes an elevation of pressure within the pulp chamber, which is a non-compliant space. The pressure increases further as venous stasis and eventually pulp necrosis occur, with release of inflammatory mediators and necrotic cell contents. Elevated pressure and inflammatory chemicals activate nociceptors in the pulp chamber causing pain. Reversible pulpitis is defined as a transient pain in response to specific stimuli (hot, cold, sweet), which occurs when the pulp is inflamed. These symptoms resolve when the cause of the inflammation is treated. The pain

of reversible pulpitis may be described as fleeting, shooting, stabbing, or sensitive. irreversible pulpitis is characterized by spontaneous pain, which may be worsened by or persist following the removal of a stimulus such as heat or cold. it is an indicator of incipient pulpal necrosis. The pain of irreversible pulpitis is often described as persistent, throbbing, dull, or aching. it may be worsened by physical activity and head movement. Pulpal pain is often poorly localized as the inflammation is restricted to the pulp chamber and is thus not affecting proprioceptive nerve fibers, which are located in the periodontal ligament. it is common for patients to be unable to localize the exact source of the pain. Pulpal pain may respond to simple or opioid-based analgesics, but the pain of irreversible pulpitis will not resolve until pulpal necrosis has occurred or the pulpal tissue has been mechanically removed (by endodontic treatment). if pulpal inflammation and infection reaches the base of the pulp chamber, an area known as the apex or root tip, may extrude through the apical foramen into the periodontal space.

CRACKED TOOTH SynDROME Cracked tooth syndrome occurs when a crack has occurred in the dental hard tissues and reaches the pulp chamber. The crack is

usually not visible to the naked eye. Pain because of cracked tooth syndrome is classically intermittent, provoked on biting or releasing biting on a hard object, and is notoriously difficult to diagnose. it may be described as sharp or sensitive, and is usually related to mastication. The tooth may also become sensitive to hot and cold stimuli. it is thought that the pain is due to fluid shifts within the dentine tubules, which are generated due to pressure differences as the crack opens and closes during mastication. it can be extremely difficult to diagnose.

DEnTinE SEnSiTiviTy Pain because of dentine sensitivity is classically stimulated by exposure to cold, heat, sweet foods/drinks, and mechanical trauma such as toothbrushing. The sensation is due to the movement of fluid in dentinal tubules in response to osmotic or temperature-related effects. Dentinal tubules contain the processes

of cells residing in the dental pulp (odontoblasts), and fluid movement appears to trigger nociceptive output by mechanisms that are as yet unclear. Gingival recession can lead to exposure of the endings of dentine tubules, as can loss of enamel on the crown of the tooth. Dentinal sensitivity is described as very rapid, fleeting, shooting pain, or sensitivity, and is always in response to an identifiable stimulus.

FEMALE uROLOGy: A urologic Sub-Specialty

urogynecology or Female urology is now officially known as ″The subspecialty of Female Pelvic Medicine and Reconstructive Surgery – FPMRS ″. Designations such as FPMRS obtained approval for board certification from the American Board of Medical Specialties in 2012, and in June 2013 practitioners began sitting for a mandatory board certification exam in the subspecialty for the first time. Dr Angelo Gousse is one of the first Board Sub -Certified in Female Pelvic Medicine and Reconstructive Surgery in South Florida. The urologic conditions and treatment options for women vary significantly, because women have different anatomic and hormonal milieu than men. Over the past ten years there have been significant advances in FPMRS. These advances include understanding of female urologic disease processes and management options. Furthermore, several guidelines for management have been developed as a result of numerous evidencebased studies to guide the clinician. Common urologic conditions which fall within the domain of Female urology include: recurrent uTis, overactive bladder, pelvic organ prolapse, urinary incontinence, urethral syndrome, urinary fistula, interstitial cystitis (Bladder Pain Syndrome), urethral diverticula, female urethra stricture, female sexual dysfunction.

CHROniC uRinARy TRACT inFECTiOnS (uTiS) Many women suffer from recurring urinary tract infections also known as chronic uTis. Recurrent uTis are defined as having at least two infections in six months or three infections in one year. This condition is typically caused by gram negative bacteria. Patients with febrile uTis are considered cases of complicated uTis and should undergo an upper urinary tract study to evaluate the kidneys and be prescribed a more aggressive course of antimicrobial agents.

PELviC ORGAn PROLAPSE Pelvic organ prolapse (POP) is a condition associated with laxity and defects of the muscles, ligaments and skin surrounding the vagina.

These anatomical weaknesses cause pelvic organs such as the uterus, rectum, bladder, urethra, small bowel or vagina to prolapse out of their normal anatomic position. Some of the most common POP pathologies include: Cystocele (Bladder prolapse), rectocele (prolapse of the rectal wall/posterior compartment), enterocele (weakness of the cul de sac associated with bowel and peritoneal contents herniation), and vaginal prolapse. Symptoms may include pelvic pressure and discomfort, sexual discomfort, and problems urinating or defecating. numerous transvaginal, abdominal, laparoscopic, or even robotic techniques have been developed to treat POP. Recent controversies and FDA warnings in the uS have diminished the usage of synthetic mesh in the over the past 3 years. usage of biologic allograft or native tissues has become more popular. Some patients prefer a non-surgical approach such as the usage of pessaries.

uRinARy FiSTuLA urinary fistula is an abnormal opening within a urinary tract organ or an abnormal connection between a urinary tract organ and another organ. There are several types of urinary fistulas that are commonly encountered in Female urology. They include: vesicovaginal fistula ,urethrovaginal, ureterovaginal fistula. The most common type of urinary fistula results from injury to the bladder during pelvic surgery, such as a hysterectomy or unattended vaginal deliveries (obstetric fistulas). it is important to note that abdominal hysterectomy is more commonly associated with vesico-vaginal fistula compared to vaginal hysterectomy. The treatment of urinary fistula is usually surgical. Transvaginal approaches can be as successful as abdominal approaches while offering the benefit of being less invasive and a quicker patient recovery.

uRinARy inCOnTinEnCE urinary incontinence, or the involuntary leakage of urine is one of the most common problems treated in female urology. it is important for the clinician to realize that there are several different types of urinary incontinence, each with a different cause and potential treatment options. These include stress incontinence, urge incontinence, overflow incontinence, mixed incontinence (simultaneous stress and urge incontinence), total incontinence and functional incontinence. improper diagnosis of the type of urinary incontinence is probably the most common reason for treatment failure. A detailed history and physical examination is of paramount importance in the proper diagnosis and sub typing of urinary incontinence. in select cases, urodynamic testing or cystoscopy may be useful. A neurologic history and physical examination should be obtained in order to rule out neurogenic bladder. Patients with “pure” stress urinary incontinence report activity–related urinary leakage which interferes with their quality of life. Any increase in abdominal pressure such as coughing, sneezing, laughing, exercising, jumping, running, or event sexual activity may

precipitate urinary leakage. Stress incontinence may or may not co-exist with pelvic organ prolapse. in most cases urethral hypermobility is noted with rotational descent of the urethra upon straining. The need for treatment is largely dictated by the degree that it affects the quality of life of the patient . Pad usage and pad weight are sometimes used to gauge disease severity The current most popular surgical treatment to treat stress urinary incontinence is the sub-urethral sling. Transurethral bulking agent is another less successful and shorter duration treatment option. The suburethral sling can be performed using a synthetic mesh or biologic allograft, autologous rectus fascia. Abdominal procedures such as Burch urethropexy are effective but remain more invasive than the transvaginal sling procedures. Currently, there is no approved medication for stress urinary incontinence. There is no evidence that estrogen therapy is effective in the management of stress urinary incontinence. Patients with urge urinary incontinence report involuntary urinary leakage with bladder spasm and strong urges to void. urge incontinence is one of the symptoms associated with overactive bladderThe treatment algorithm of urge is very different from that of

SPOTLIGHT ON STAGE By Eleanor Hannan The Barber of Seville is considered one of the greatest comic operas ever composed. This clever production sets the action on a movie backlot in Seville, in a large warehouse that contains all the props and costumes of a movie studio. The year is 1940, Spanish movie star, Rosina, is kept captive by Bartolo, her agent, guardian, uncle and owner of the studio, who is out for the inheritance she received from her parents. He attempts to get both the beautiful girl and her wealth by getting her to marry him. Count Almaviva, a nobleman, and aristrocrat in Spain, conceals the fact he is really wealthy, and has been in love with her on screen and off. He constantly serenades her and now follows her to the studio. Add to this mix, a talented barber hired for the day’s shooting, throw in a chorus of movie extras, and you’re in for some backstage antics fueled by love and passion. Figaro, the

studio stylist who helps Rosina dress, encourages her to choose Almaviva who finally reveals his true identity and proclaims his love for her. Happiness is celebrated by all at the end of the day. Oliver Wasow will exhibit a series of “Studio Portraits” of individuals or groups posed in front of painted backdrops. These are the first photos Wasow has done that have included people, and are described by the artist as “romantic and sincere, though not necessarily comfortably so.” Wasow is a fine art photographer living and working in Rhinebeck, ny. His work has been included in benchmark exhibitions such as ‘Manipulated Photography Before Photoshop,’ at The Metropolitan Museum of Art, ‘image World,’ at the Whitney Museum of Art in nyC, and ‘The Photography of invention,’ at the national Gallery of Art in Washington,

DC. He exhibited at the Center previously with the 2011 exhibition Artist unknown / The Free World. The works in Gustavo Oviedo’s exhibition are a product of Oviedo’s intensive study of the terrain between Miami and Key West. Focusing on nautical landmarks, Oviedo has embarked on a thorough examination of local sites by gathering historical accounts in addition to visiting and documenting them personally. His meta-displays collapse official historical and geological research with colorful memorabilia such as antique prints, maps, tourist souvenirs and lighthouse replicas. Gustavo Oviedo was born in Paris in 1981. He was raised between Colombia, venezuela, Mexico and now lives and works in Miami. Levitation and Gravitas is an exploration of flying, floating and falling, and the interconnectedness of these human desires. in this site-specific work, Michael namkung creates a 3dimensional sound

piece that channels an apparition of his body floating down the stairs, and utilizes video to sustain his body in flight. namkung is a multidisciplinary artist whose work draws from the language of sports training and athletic performance to explore the sensory experiences of drawing under physical strain. On Thursday, January 14, at 6:30 p.m. nina Surel, along with Michelle Weinberg, artistic director for nina Surel: Sailing to Byzantium, will give a talk and tour of her exhibition. Saturday, January 16, 2016, from 1-3 pm, nina Surel will present a Green Screen Workshop for students in

grades 5-12. Students will collaborate with one another and with the artist as they create a set, and use props, costumes and makeup to shoot a mini-video work using a green screen. Green screens allow film and video artists to place a new backdrop behind the actors and objects in any scene. This technique is used by nina Surel in the production of the videos she will exhibit. This workshop involves working both in front of and behind the camera. no previous video or film experience is required. The workshop is $20 per student; maximum 20 students. The City of Hallandale Beach will conduct a public participation meeting for the 42nd year CDBG program application at 6:00 P.M. on Thursday, november 12, 2015 at the Austin Hepburn Center, which is located at 750 n.W. 8th Avenue, Hallandale Beach, Florida. The City Commission will consider the 42nd year CDBG grant application in the Commission Chambers, located at 400 South Federal Highway, during the regular City Commission meeting to be held on Wednesday, December 2nd, 2015 at 6:00 P.M. The 42nd year CDBG public improvement project may include but not limited to: retrofitting/upgrade of drainage, concrete construction/reconstruction, street resurfacing, pedestrian lighting, asphalt pavement construction/reconstruction, regarding and sodding of swale areas, landscaping and street trees. An additional application may include funding to continue after school and/or after care programs within the City. Persons interested in expressing their preferences and comments regarding these projects should attend the above referenced public hearing.
