Diabetes and dental care: A guide to achieving a healthy mouth

From Page 1A ––––––––––––––––––– teria. When starches and sugars in food and beverages interact with these bacteria, a sticky film known as plaque forms on your teeth. The acids in plaque attack the surfaces of your teeth (enamel and dentin). This can lead to cavities.

The higher your blood sugar level, the greater the supply of sugars and starches — and the more acid wearing away at your teeth.

• Early gum disease (gingivitis). Diabetes reduces your ability to fight bacteria. If you don't remove plaque with regular brushing and flossing, it'll harden under your gumline into a substance called tartar (calculus).

The longer plaque and tartar remain on your teeth, the more they irritate the gingiva — the part of your gums around the base of your teeth. In time, your gums become swollen and bleed easily.

This is gingivitis.

• Advanced gum disease (periodontitis). Left untreated, gingivitis can lead to a more serious infection called periodontitis, which destroys the soft tissue and bone that support your teeth. Eventually, periodontitis causes your gums and jawbone to pull away from your teeth, which in turn causes your teeth to loosen and possibly fall out.

Periodontitis tends to be more severe among people who have diabetes because diabetes lowers the ability to resist infection and slows healing. An infection such as periodontitis may also cause your blood sugar level to rise, which in turn makes your diabetes more difficult to control. We all know that these problems can happen to anyone, that's why we all go to our dentist on a regular basis. Plaque builds up on all of our teeth. However, high blood glucose levels help germs to build up on your teeth and gums, making these problems worse so that you could actually loose your teeth.

What does this look like if it's happening to you?

The first signs are red, sore, and bleeding gums.

This can lead to periodontitis, which is an infection in the gums and the bone that holds the teeth in place. Pockets form between the teeth, which fill with germs and pus. Sometimes when I write these articles, I think that diabetics are singled out for many diseases, but with gum disease about 85% of all adults develop it. 10% have lost all of their teeth to it. It is difficult to get people to brush and floss

as often as they should.

Many people do not go to the dentist when their gums bleed when they brush or floss. This bleeding is not normal. It needs to be addressed. If the plaque is not brushed and flossed away it hardens into tartar and collects under the gum line.

To make matters worse, more plaque forms over the tartar, so you can imagine how the problem can escalate. Although tartar is inert, it harbors surface bacteria and worsens the disease below the gum line. As the process accelerates, more and more tissue becomes diseased and more plaque accumulates. Once the bone begins to be destroyed, you know the rest; the teeth loosen and false teeth are around the corner.

The prospects for fighting periodontal disease are excellent, as there are many things that someone with diabetes can do to stop the process or correct the disease once it starts.

The first goal is easy.

1. Good Blood Glucose Control

The degree to which a person is in control appears to have a direct relationship to the severity of periodontal disease.

This is clearly a very good thing to know, especially on those mornings when you think you are running too late to brush and floss. You can run out the door or, like many of us, make an appointment with the periodontist of your choice.

It has been found that in people with good control, gingival tissues usually react in a fairly normal manner. In people with diabetes who have poor blood glucose control, bone loss in periodontal disease is particularly severe, says Dr. Bobadilla.

2. Oral Hygiene Make sure you schedule regular dental checkups. For a diabetic this may mean every six months, or if you have periodontal disease more often until you get it under control. Check-ups every three months may ensure problems can be arrested with relatively minor procedures, so you don’t undergo periodontal surgery.

Brush and floss the proper way. If you are not sure, the hygienist can help you out. The hygienist is the person who regularly cleans your teeth at the dentist's office.

Now that you've read this article, you know that brushing and flossing are important to remove bacteria before it causes damage. If necessary, your dentist may recommend mouthwashes using chemicals that destroy plaque-forming bacteria and neutralize their toxins.

Bluesten Park project is ready to take shape

Road From Page 1A –––––––––––––––––––––––––––––––––––––––––––––––––––––––––––––––––––– this never-ending bumper-to-bumper line of congestion has certainly tested the patience of those who live in the community, and those who use the roads for their commute.

While Ruiz has been living in this part of the city for 11 years, it was back in 2013 when the talk began about street widening and progress in a portion of West Hollywood that has been neglected since the 1990s.

From one-lane traffic at rush hour to turn signals not letting enough cars through, many have opted for different routes while this area is being overhauled, and that continues to hamper business.

“I have been very tolerant of the entire process - with an understanding that things will change on this side of town,” Ruiz said. “I know that there are so many positives happening, but I leave for work at 8:25 in the morning and come home at 5:30 in the evening. I have no shortcuts and must sit in the long lines in order to get home.”

Ruiz and others figured two years ago when things started to take shape that the beautification will not only enhance the area, bringing in more people to spend money, but also figured, as a property owner, that she could see a spike in the value of what she works hard to pay each month.

Gamarra Lopez is another resident who has much more at stake. Not only does she live in West Hollywood, but she also works in the area - and has personally watched business pretty much drop off because of the road construction.

“My bosses have laid three people off because of the lack of businesses,” Lopez said. “They know that I really need this position, so they have kept me, but with access to so many businesses along the 441 corridor being limited or in some cases hidden - we are hoping each and every day that things improve.”

As much of a deterrent as the construction has been over the past year, things are really getting crazy, especially at night because of the pattern of the traffic being changed. That, many insist, has been very dangerous - with things that are harder to see at night.

The tradeoff for the inconvenience will be a better and more progressive 441 corridor. Major anchor stores such as Walmart and Burlington Coat Factory will bring other national chains that aid in any growth of a community’s infrastructure.

Lopez has been a truly believer in the many projects that are taking place in her community. She and many who live in her neighborhood have bought into the fact that no matter how much construction and traffic they have to put up with, it will certainly not be forever.

“I am not asking this place to be Aventura or the beach,” Lopez said. “I just want to have a community that I can be proud of and able to get around in and take advantage of all the new businesses - as well as the ones who have survived the past 10 years.”

Concurrently, the CMAR Kaufman and Lynn Construction will be evaluating the costs of what is programed to be built. For example, we know that residents want a pool at this park that will accommodate training and school competitions. They will go over the costs of construction and maintenance versus the demand to decide size.

We also know that residents want an interactive water park for children.

This too will be evaluated. There are two baseball fields, soccer field, tennis, basketball and buildings that need to be laid out.

While ACAI is creating the constructability review, KL will be evaluating the cost and efficeincy of the design with more of a focus on the bricks and mortar costs. In addition to the value of engineering, they will be making recommendations. They are focused on reducing operational, energy (LEED) and life cycle costs while not sacrificing the design. KL has also by law developed a community benefit program. They are focused on local workforce fairs, local vendors and subcontractors. This too will be when our business owners and residents can partner as suppliers and workers.

ACAI has a final date of design and permitting with the city in April 2017. The CMAR is targeted to begin the site and civil permitting by December 2016, this way they can get a head start with an award of Phase I of the CMAR in February 2017 and then Phase II of vertical construction with a completion of the project in December 2018. It is a very tight time line, but with the team all working together they have figured the phasing to shave time off without jeopardizing quality or the design.

Preparing for 20172020. Yes, you read that right, our commission has developed a strategic plan that is revisited every three years. The

current plan expires this September. Our strategic plan takes in consideration 9 goals that have been established in collaboration with the resident’s commission and staff. We have continued to perform citizen and business surveys in alternating years to ensure that we are staying on track to address the needs of our community.

On January 6th, we approved the hiring of Trisect to facilitate the creation of our strategic plan. This team brings a unique perspective, since they are not just facilitators, they have a background from both government and private industry. The two principals are Glenda Hood and Shelly Lauten. Ms. Hood is a former Secretary of the State of Florida, Mayor of Orlando and was instrumental in developing an economic plan and a cultural art plan for our state when she was in office. Ms. Lauten has worked for Disney and the Palmer Golf Industry.

She was known for working with the Central

Florida Regional Plan.

After doing background research on previous plans and surveys, they will be meeting with the commission one on one and then at a retreat.

After, there will be two community meetings with focus groups of 2530 people to review the survey results. The final plan will be presented to the commission in April.

Growing up in Mississippi and later moving to New Hampshire, among other places, Barber, 51, had worn glasses since the age of 6. As he got older, his eyes never got any better - and while he could see things close, it was nearly impossible to focus in on any object far away.

Like any teenager growing up, Barber did wear contacts, but as he would soon find out, having dry eyes would certainly cause many additional problems - even when he switched from the hard lenses to the softer variety.

When he moved to south Florida, things got to a point where his condition of not being able to see things far away became serious, and as his Hollywood Optometrist Justine Chen suggested, maybe Lasik was the answer.

She recommended Dr Stanley Braverman, with over 20 years of Lasik experience. Emmett began to start doing research.

“There were like four doctors I looked at,” he recalled. “Two of them said that my vision was so poor, they didn’t want to do it. That was the toughest part because it was my vision we were dealing with. Plus, you never got to meet with the doctor - just his assistants. That wasn’t very encouraging.”

But that all changed when Barber saw Dr. Stanley Braverman in Hallandale Beach. He knew from the moment he walked into the office that this was the place for him. He felt at ease and very comfortable.

“First of all, 20 minutes after I got to the office, they took me inside and the first person that met with me was Dr. Braverman,” he explained. “I can tell you that in five minutes he not only told me that he could get me seeing better than ever before, but he took me through every step of the process.”

Barber knew that Dr. Braverman was the “dream maker” who could shed him of glasses and everything that caused his poor eyesight. In September, Barber went in for a Bladeless Lasik - Monovision - one eye for distance and one for seeing near.

The experience was something that some three months later he is still talking about, and as someone who uses his eyes for drawing and painting, the transformation has indeed been life-altering, to say the least.

“Right after the surgery, I could see and had no pain,” Barber recalled.

“They sent me home with a covering so I wouldn’t touch the eyes, instructions and some Tylenol, which I never had to take one at all. It was so amazing, and still is.”

Throughout the entire process, Barber talked about how everything - from follow-up appointments, to questions he had - was included in one cost.

That he insists is something that other eye centers do not offer. In addition, he spoke about how the entire office was supportive and encouraging from the first time he visited them.

“I really have to say that the job that Dr. Braverman’s Refractive Surgery Coordinator Rosemary Hartford did throughout the entire process, was something that I cannot put into words,” he pointed out. “She was there every step of the way and answering any questions or concerns that I had.

That I will never forget.”

So, as he looks toward the future, Barber’s vision is perfect, wears no glasses, doesn’t suffer from dry eye syndrome and keeping his weight off and feeling better than ever!

WHAT TO EXPECT WITH Z-LASIK SURGERY All Laser Lasik surgery is performed on a patient’s cornea, which provides most of the focusing power when light enters the eye. Topical anesthetic drops are applied to the eye. The surgeon first uses a special Ziemer LDV femtosecond laser to create a thin layer of corneal tissue that can be folded back (a corneal flap). MLK weekend will be filled with plenty of activities Celebrating the legacy and memory of Dr. Martin Luther King Jr., the City of Hallandale Beach and Community Civic Association kick off a weekend of activities Saturday culminating Monday with the annual Festival at Hallandale High School, 720 NW 9th Ave. The activities include a scholarship breakfast, parade, Dr. King Speaker Series, featuring Ken Brown, basketball tournament and the festival.

The weekend begins at 9 a.m. Saturday with the annual Dr. Martin Luther King Jr. Scholarship Breakfast at the Cultural Community Center, 410 SE Third St. This is the first year the event will also honor long-time community leader Angelean Glass. In the past quarter-century, the event has raised money for more than 110 college scholarships to Hallandale High School students. Tickets are $20 in advance or at the door. The annual Dr. King Parade has been extended two blocks this year because of the construction at O.B. Johnson Park. The parade will step off on Monday morning at 10 a.m. at B.F. James Park, head south along NW 2nd Ave., to Hallandale Beach Boulevard, then west to NW 8th Avenue (Dr. King Jr Avenue), turn north to Foster Road, then west to Hallandale High School. Bands, floats and community groups will parade along the route during an approximately 90-minute event.

Motorists should be aware that westbound Hallandale Beach Boulevard will be closed to all traffic from 10 a.m. to approximately 11:30 a.m. Drivers should use Pembroke Road to avoid congestion.

Inspirational and motivational speaker Ken Brown will bring his message of hope and success to the community at the annual speaker series at 12:30 p.m. Monday in the Hallandale High School Gymnasium. Mr. Brown was born in poverty to child parents, ages 14 and 13. Through education and perseverance, Mr. Brown became a business leader, author and inspirational leader.

At the same time, the annual Dr. King Festival kicks off on the Hallandale High School athletic complex with music, rides and activities. Scheduled to perform are: Reacxion Band, London Young, Lady Elaine & One Love, Kevin Cameron and more.

Also, for the first time, the City will host a three-on-three Peaceful Players Basketball Tournament in the Hallandale High School Gymnasium, beginning at 1 p.m.

By Eleanor Hannan The main object of the play is about marriage, a union between two individuals identically empowered who equally divide all responsibilities such as bringing up children together, sharing successes in their lives and careers, relying on each other's ability to comfort one another in time of need and to make certain neither partner takes advantage of the other.

Wouldn’t this be the ideal marriage?

The two characters in the play, Catherine and Gwen were best friends during graduate school. Catherine went on to become a star academic in feminist history theory, leaving Gwen behind with her ex-boyfriend Don, with plans to marry. Twenty years later, both women wonder about roads they didn’t take.

When Catherine returns home to visit her ailing mother, she takes the occasion to get in touch with Gwen and Don to heal or stir up old conflicts.

Things come to a head between Gwen, Catherine and Don and all three seem to get their wish, at least temporarily. Then things fall apart. Certainly this is not progress. Rapture, Blister, Burn is a bril-

Dr. Ron.., my best friend constantly bickers with his wife. They both seem miserable in their marriage, but refuse to divorce. Wouldn’t marital therapy help a situation like this?

Possibly yes.., if both sides are willing to take responsibility, and work on change…The reasons why couples enter marital/couples counseling can greatly vary.

Whether it be infidelity, financial problems, addictions, disrespect, abuse… or just that sometimes people fall out of love, one thing is

usually for certain.., conflicts are an inherent part of relationships, and need to be resolved in a healthy, responsible manner. Often unresolved conflict involves a habitual repetition of bickering and defending one’s position…. Many couples I have seen in therapy will tell you I often ask this simple question; “Would you rather win an argument, or win a marriage?”

Below is a typical dialogue of those who bicker and defend without successful conflict resolution... Let’s look at

Dr. Adler Using Laser In Battle Against Toe Fungus

According to the American Podiatric Medical Association, approximately 40% of patients visiting a podiatrist have complaints or signs of toe fungus. It may begin with tenderness or pain in the toe, developing into an unsightly thick, darkened nail, and it can spread from toe to toe. In fact, the fungus can get so out of control that it can infect the foot and ankle.

Topical treatment such as Lamisil cream usually is the first course of therapy for a mild or moderate case. However, long-term use or oral antifungal medication is not recommended due to the toxic side effects Lamisil can have on the liver.

This is a special risk if a patient is taking other medications, including excessive use of Tylenol that can manifest the same hepatic damage.

Over the past few years, Dr. Charlton Adler has been following closely the application and reported results of laser therapy on fungus toenails and other podiatric and wound situations.

He is pleased to announce that he now is able to add this service as a therapeutic option in his office in North Miami Beach.

“The introduction of laser in treating toe fungus has had a major impact on attacking this uncomfortable and unsightly problem,” says Dr. Adler. He further explains that Onychomycosis, the medical term for fungal infection of the nail, is often ignored because it can be present for years without causing pain. “When the fungus imbeds itself under the nail,” he adds, “ the nail surface can become darker in color and the area can develop an odor. The fungus lives on debris under the nail, and white marks can appear on the nail plate.”

Approved for the treatment of nail fungus five years ago by the Food and Drug Administration, the special appeal of lasers is that it selectively heats and destroys harmful fungi while sparing healthy surrounding skin. Because of the intensity of the heat generated by laser rays, Enrique Mechulon a specially trained and certified laser technician working with Dr. Adler, says that both he and the patient wear laser protective glasses and take several rest periods during the 40-minute process.

“I have already witnessed many success stories where patients have spent years and thousands of dollars to get rid of their nail fungus,” says Mechulon. “Depending on the patient’s general health and nail problem, it can take anywhere from one to nine treatments, but the results are there with minimum risk to the body,” he adds.

“I have wanted to offer this therapy for quite a while,” says Dr. Adler. “However, I am cautious in my approach, and I needed more proof on studies and reports.” Dr Adler adds that research shows that 80% of all toe fungus has been healed using laser therapy. “This is in contrast to topical ointments that only work 50% of the time,” says Adler, “and with topical treatment, the infection usually returns.”

Tip: To make fresh bread crumbs, pulse day-old bread in a food processor until it resembles coarse crumbs; store in airtight container in the freezer for up to one month.

RISKS OF HIRING AN AIDE PRIVATELY

Well, it happened again. The family of a former patient of our agency called with a horror story of how the patient was taken advantage of by an aide that the client hired privately.

Mrs. R. had been our patient throughout the three years that her long term care insurance paid for aide services, and she was very happy with the aides we had assigned to her. However, once she was faced with paying for the aide outof-pocket, her friends convinced her that she could pay less money for help that she could hire herself, rather than work through an agency.

At first, Mrs. R was very happy. The aide she hired was kind and attentive. She helped shower and dress Mrs R, helping pick out her clothes, accessories, even her jewelry. The aide liked to listen to Mrs. R’s stories about her trips and the lovely momentos she bought back. Mrs. R also liked to talk about her designer clothes and how much she spent on her wardrobe. The aide drove Mrs R around in her late husband’s Lexus, taking her shopping, to her doctors, and to the beauty salon. And when they returned home and Mrs. R reclined in her lounge chair for a little nap, the aide prepared dinner and straightened up the house. Mrs. R told her daughter that she was very happy with this aide.

Then, all of a sudden, small items began to disappear. At first, it was costume jewelry and some twenty dollar bills that Mrs R kept for house cash. Then, it was a diamond ring that Mrs R did not to wear regularly and had put away in a secret place for safe keeping. That upset Mrs. R. She became suspicious. She questioned the aide several times, even saying that if the aide needed money, she should just ask her. The aide became defensive, even sharp and nasty. That’s when Mrs. R called her daughter. The daughter flew down, helped her mom look for everything and questioned the aide. The aide said she was insulted, and quit.

The family could not accuse her without proof, but felt that was a lesson well learned.

That’s when the daughter called us.

Three comfort foods for cooler January evenings FRIED CHICKEN For the Brine ½ cup kosher salt 1 whole chicken, cut into 10 pieces 4 cups buttermilk For the Dredge 4 cups all-purpose flour 1 ¾ cups cornstarch 1 cup Maseca (instant Mexican corn flour) or masa harna 2 tablespoons kosher salt 1 tablespoon paprika 1 tablespoon onion powder 1 tablespoon garlic powder 1 tablespoon ground coriander 1 tablespoon celery salt 1 ½ teaspoons freshly ground black pepper Vegetable oil, for frying BRINE THE CHICKEN: In a large bowl, whisk the salt with 8 cups cold water. Add the chicken, cover, and refrigerate overnight or up to 24 hours. Drain and discard the brine, pour the buttermilk over the chicken, cover, and refrigerate overnight or up to 24 hours.

MAKE THE DREDGE: In a large bowl, whisk together the flour, cornstarch, Maseca, salt, paprika, onion powder,

garlic powder, coriander, celery salt, and pepper.

FRY THE CHICKEN: Fill a large pot halfway with oil and heat to 350°F. Set a wire rack on top of a rimmed baking sheet and set aside. Remove the chicken from the buttermilk (don't rinse it!) and dredge the chicken in the flour mixture.

Working in batches, fry the chicken until the internal temperature of the white pieces reaches 160°F, and the dark pieces reach 165°F, on a deep-fry thermometer (13 to 14 minutes for breasts and 16 to 17 minutes for thighs). Rest the chicken on the rack and serve with your favorite biscuits.

MEATBALLS & GARLIC BREAD 3 hoagie rolls 1 lb. ground beef chuck 1 large egg 3 tsp. minced garlic Coarse salt ground pepper ¾ tsp. dried oregano 2 tbsp. extra-virgin olive oil 1 tsp. extra-virgin olive oil 1 can crushed tomatoes 1 can whole peeled tomatoes sugar

Preheat oven to 400 degrees. Tear 1 roll into pieces; pulse in a food processor until fine crumbs form. In a large bowl, toss crumbs with 1/3 cup water. Add beef, egg, 1 teaspoon garlic, 2 teaspoons salt, 1/4 teaspoon pepper, and 1/2 teaspoon oregano; mix just until combined. Gently form mixture into 12 meatballs.

In a large Dutch oven or heavy pot, heat 1 teaspoon oil over mediumhigh; swirl to coat. Add meatballs and cook, turning occasionally, until browned, about 7 minutes.

Add crushed and whole tomatoes with juice (breaking up tomatoes), 1 teaspoon garlic,

teaspoon oregano, and pinch of sugar. Season with salt and pepper.

Bring sauce to a boil; reduce to a rapid simmer and cook until meatballs are cooked through, about 15 minutes.

Meanwhile, split 2 rolls and place, cut side up, on a rimmed baking sheet. Brush with 2 tablespoons oil and sprinkle with 1 teaspoon garlic; season with salt and pepper. Bake until golden, about 10 minutes. Serve meatballs with garlic bread.

STEAK AND MUSHROOM POTPIES 1 sheet frozen puff pastry 2 tbsp. olive oil 1 lb. beef fillet

¾ lb. mixed mushrooms 1 large onion 1 tbsp. chopped fresh rosemary 1 tbsp. tomato paste 2 clove garlic 2 tbsp. flour 1½ c. reduced-sodium beef stock or broth 1 c. frozen peas and carrots ½ tsp. kosher salt ½ tsp. Freshly ground pepper 1 large egg Heat oven to 400 degrees F. Place 4 deep crocks or ramekins (1 1/4-cup capacity) on a baking sheet. Unfold pastry on a lightly floured surface; roll out seams in dough with a rolling pin. Cut dough into 4 pieces; trim each

piece to fit over tops of crocks with an additional 1/2-inch on each side.

Place dough on a waxed-paper-lined plate and refrigerate.

Heat 1 tablespoon of the oil in a large nonstick skillet over high heat until oil is shimmering.

Sauté beef, in 2 batches, 1 1/2 minutes per batch, until lightly browned.

Transfer to a plate. Add remaining 1 tablespoon oil to skillet with mushrooms and onions; sauté over medium-high heat 4 minutes until mushrooms are softened and onions are translucent.

Stir in rosemary, tomato paste, and garlic, then sprinkle flour over mixture; stir for 1 minute.

Add stock, bring to a boil, and cook until thickened, about 3 minutes. Stir in peas and carrots, salt, and pepper, then beef cubes.

Spoon mixture into crocks. Brush outside edges of crocks lightly with egg wash. Place pastry cut-outs over top of crocks and press dough overhang against outside edges of crocks to adhere. Cut vent holes in centers; brush pastry with egg wash. Bake 24 minutes or until pastry is golden and puffed.
