To get access into a memory care centre, residents must first be assessed.
Arrange a needs assessment A GP can make a referral to an approved Needs Assessment and Service Coordination (NASC) provider, otherwise contact your local District Health Board to request an assessment. Residential Care Subsidy Contact Work and Income for advice about eligibility (0800 999 727). They will look at assets and income, and based on these, Work and Income will work out how much they will contribute towards the care cost. We can provide a copy of the Ministry of Health booklet, which explains what families need to know. Resident Admission Agreement If it’s decided secure dementia care is required, we’ll discuss which apartments are available. Before moving in, a Resident Admission Agreement will need to be signed. This legal document will set out both Summerset’s and the resident’s responsibilities and the conditions on which residents live with us. Occupation Rights Agreement An Occupation Right Agreement (ORA) will need to be signed and discussed with your lawyer. An ORA details and sets out the terms and conditions of the memory care apartment and complies with the Retirement Villages
Act 2003. Enduring Power of Attorney (EPOA) Before moving in, residents need to appoint an Enduring Power of Attorney (EPOA). An EPOA means the resident has authorised someone they have entrusted to make decisions for them. We can only share ongoing health information and make decisions with the authorised representative (EPOA) of the resident.
