GUIDE
Stonehill will offer a new Medicare program for individuals living with dementia beginning July 1. The GUIDE (Guiding an Improved Dementia Experience) Program will provide support and case management for individuals with a dementia diagnosis who are living at home. The goals of the program are to improve the quality of life for both people with dementia and their unpaid caregivers and to help people with dementia to remain safely in their homes for longer by preventing or delaying the need for nursing home care. Stonehill is one of only three providers to offer the program in Iowa.
Guiding an Improved Dementia Experience (GUIDE) Model
What is GUIDE?
The GUIDE model program addresses nine care domains: comprehensive assessment, care plans, 24/7 access to a helpline, ongoing monitoring support, care coordination and transitional care management, referral and coordination of services and supports, medication management and reconciliation, caregiver education and support, and respite services. Program participants will work with Stonehill’s GUIDE Care Navigator, Beth LaGreek, who will help coordinate, educate, and provide support in the nine care domains as they progress through the dementia journey.

To be eligible for the program, individuals must meet the following criteria: a beneficiary has a diagnosis of dementia confirmed by a clinician, is living at home or in an independent or assisted living community, has Medicare as their primary payer, is enrolled in Medicare Parts A and B and not enrolled in a Medicare Advantage plan, and is not enrolled in Medicare hospice benefit. The program is fully covered by Medicare and is available at no cost to eligible participants.
The goals of GUIDE are to:
Improve the quality of life for people living with dementia
Enhance support for caregivers of people living with dementia
Help people living with dementia stay in their homes and communities longer
Program services include:
Comprehensive Assessment & Care Plan
Get an assessment to identify your individual health needs and to build a care plan that is tailored to provide the services you need.
Caregiver Support
A relative or unpaid nonrelative who helps as a caregiver can receive education and support such as direct communication with a care navigator when they need it.
GUIDE Respite Services
GUIDE Respite Services can be provided, up to an annual cap, so caregivers may take a break when they need to. Support comes from local in- home respite providers, adult day centers and nursing homes.
Coordination & Support
Get connected to community- based services like meals and transportation. Care teams will also work together to coordinate clinical and support services
24/7 Access
Care navigators help you get care and 24/7 access to a care team member or helpline to ask questions or get support.
