Our network of affiliate organizations provides a comprehensive array of integrated programs and services for people experiencing mental and physical challenges, as well as the issues associated with aging.

MENTAL HEALTH

ASSOCIATES FOR BEHAVIORAL HEALTHCARE Associates for Behavioral Healthcare is an outpatient service option for those looking for therapy, counseling, mental health and psychiatric services for adults, children and families. We are staffed with psychiatrists, nurse practitioners, physician’s assistants and therapists. We annually provide more than 21,000 outpatient therapy and psychiatric visits. Associates for Behavioral Healthcare has been providing care in the area for more than 20 years. Our diverse group of professionals has expertise in nearly all aspects of counseling, mental health and psychiatric treatment. Most insurances are accepted. MENTAL HEALTH ABBE MENTAL HEALTH CENTER Abbe Mental Health Center is a state-accredited community mental health center serving individuals of all ages. Services include outpatient psychiatric evaluations and medication management; individual, family and group therapy; specialized services for children and families and intensive community based services for adults. Professionally licensed staff are trained in a variety of best practice models of treatment to meet the diverse needs of individuals. Specialized community-based services such as: Community Support, Home-based Mental Health Nursing, Assertive Community Treatment Teams, Day Habilitation programming, Peer Support and Club 520 Recovery Center are available for individuals that need comprehensive treatment and support to recover from a serious mental illness.

Same Day Access Services Abbe Mental Health Center offers individuals with immediate needs Same Day Access (SDA) services. Hours vary by location. Cedar Rapids Main Office Walk-in hours: Monday through Friday...........8:30 - 11:30 a.m. Monday and Tuesday............................ 2 – 4 p.m. Wednesday .........................................2 – 5:30 p.m. Individuals are seen in the order they arrive. An initial mental health assessment is made by a licensed mental health professional who will provide treatment recommendations. The copay for SDA depends on insurance coverage. We accept most insurances.

Child and Adolescent Services We offer an array of service options to suit the diverse needs of children, adolescents and their families. Psychiatric Evaluations and Medication Management services are provided for those individuals that need medications to treat their condition. Our staff consists of psychiatrists, nurse practitioners and physician assistants who are licensed to prescribe and monitor medications.

MENTAL HEALTH Individual, Family and Group Therapy services are provided for children, adolescents and/or families who need assistance with issues that may affect their mental health. Our outpatient staff are licensed mental health professionals trained in Best Practices such as Parent Child Interactive Therapy (PCIT), recovery-based and trauma informed practices. PCIT is an evidence-based therapy designed for parents/caregivers and their young children who are experiencing behavioral and/or emotional difficulties. The main focus of PCIT is changing negative parent/ caregiver – child interaction patterns into positive behaviors and interactions. School Based Mental Health Services are provided to students in designated schools. Staff works with children and families to identify areas of need and help them access appropriate services.

3 Adult Outpatient Services We also offer many treatment options for adults. Psychiatric Evaluations and Medication Management services are provided for individuals that need medications to treat their condition. Individual, Family and Group Therapy services are provided for adults and/or families who need assistance with depression, anxiety, trauma or other life stressors impacting their mental health. Best practice models include EMDR, CBT and other trauma informed practices. Eye Movement Desensitization and Reprocessing (EMDR) is a therapy treatment designed to alleviate the distress associated with traumatic memories. Cognitive Behavior Therapy (CBT) is an option for those seeking an integrative mental health option. The relationships between behavior, thoughts and feelings are examined. Elderly Waiver – Mental Health Outreach therapy services provided to eligible seniors in their homes to provide treatment and support to reduce the likelihood of requiring care in a medical institution.

These services are available for individuals who may be experiencing a mental health crisis. A licensed mental health professional and physician are on call after hours.

MENTAL HEALTH Additional Services Integrated Health Home Abbe offers an Integrated Health Home (specialized medical home) for care coordination of behavioral health and physical health care needs for individuals with a serious mental illness (SMI) Assertive Community Treatment (ACT) provides individualized care delivered by a multi-disciplinary team. The team includes a psychiatrist, nurses, licensed social workers, vocational specialists, substance abuse specialists and case managers. The team provides a high intensity of outreach services to individuals struggling to recover from a serious mental illness such as Schizophrenia or Bipolar Disorder.

Community Support offers support to individuals in a natural setting such as their home or places within the community. A wide variety of assistance is available including assistance with accessing community resources, ensuring basic needs of food/housing/ medical care are met, developing natural supports, support and advocacy, education and employment planning or crisis intervention as needed. Home-Based Psychiatric Nursing services are provided by registered nurses that assist with medication administration, medication set up, coordination with labs/pharmacies/physicians and health and wellness education. Emphasis is on improving the understanding of medications and establishing a routine of taking medications to reach maximum benefit. AGING SERVICES Aging services seeks to enhance the lives of older adults by helping them maintain as much independence as possible. Each year, approximately 3,000 older adults are matched with the resources needed to ensure their continued independence. The first step to continued independence for you or your caregiver may be speaking with Case Management. A case manager will match your needs with community resources, assess and coordinate options and develop an individual care plan that will allow for ongoing support. Additionally, a case manager will be able to assist with Elder Abuse Intervention. Older adults can be especially vulnerable to financial exploitation or self neglect. Our elder abuse specialist can help intervene and find necessary supports and services.

Respite Program The Respite Program is designed to alleviate some of the stress that the primary caregiver may experience. This is an in-home care option. A respite provider can provide temporary relief by completing minor tasks the caregiver would otherwise coordinate on their own. Some of these tasks may include meal prep, providing companionship, light housekeeping, personal care and offering medication reminders.

Providers are screened in compliance with the State of Iowa’s criminal records investigation check to ensure the safety of everyone. After the family’s needs are assessed, they are matched with a compatible provider. Hourly rates and schedules are determined by the family and the provider. Financial assistance may be available for eligible families.

Home Repair Services Small home repairs can help maintain and improve the patient’s home function and safety. A combination of contractors and volunteers may assist with the following needs: • Minor home repairs

• Lawn mowing

• Snow removal

• Seasonal yard care

• Home adaptations (wheelchair ramps, grab bars, handrails, etc.) These services are available to anyone who meets the eligibility guidelines.

Adult Day Health Centers

Volunteers are able to help patients remain in their homes by:

These CARF accredited centers provide services for individuals age 18 and older who may require support, supervision and structure away from home to remain independent. Our staff is committed to assisting participants experiencing Alzheimer’s disease and related dementias, vision loss, depression, brain injuries, developmental disabilities, impairments resulting from stroke, and other conditions, achieve maximum levels of independence.

• Scheduled transportation (medical appointments, grocery stores/ food banks and farmer’s markets) • Home and phone visits to home-bound patients • Well-being checks • Holiday meal deliveries

Witwer Healthy Aging Program Witwer provides active lifestyle programs that support and nurture the strengths of older adults enabling them to maximize their well-being. Programs are offered at multiple sites across Linn County and include Lifelong Learning Programs, health promotion and wellness education and leisure and social activities. We welcome self-referrals, as well as those by local agencies and family members.

RESIDENTIAL TREATMENT SERVICES We offer two residential treatment centers Penn Center, in Delhi and Chatham Oaks in Iowa City. Both are licensed by the Iowa Department of Inspections & Appeals. These facilities provide short and long-term residential treatment services for adults with mental health and medical conditions, intellectual disabilities and elderly who require 24-hour treatment and supervision. Our programs help individuals gain the necessary skills to return to live in the community of their choice. Services include: • • • •

Psychiatric services include on-site psychiatrist and 24-hour on call consultation Medical services include nursing, on-site (Penn Center), off-site (Chatham Oaks) medical physician, podiatrist, physical and occupational therapy Social services and nursing services Skill attainment and community integration activities

PENN CENTER COMMUNITY-BASED SERVICES Daily Home-Based Habilitation Services are provided to individuals with a primary mental health diagnosis. Housing is provided and facilitates a supportive environment for individuals to fully integrate into the community. Services can be supported up to 24 hours based on individual needs. Services are offered in Delaware, Linn, Jones, Benton and Johnson Counties.

Hourly Home-Based Habilitation Services are provided to individuals in their own homes/apartments. This model of service is permanent, supported housing with emphasis placed on empowering individuals to take charge of their own recovery by learning the skills to manage their illness, live independently and develop natural supports. Both daily and home-based habilitation provide services and life skill development opportunities including: • • • • • •

Coping skills/symptom management Illness education Medication education Community integration Daily living skills management Supporting individuals in their recovery process

Home and Community-Based Services (HCBS) to individuals with intellectual disability or brain injury are offered both hourly and daily depending upon need. These services offer skill building, support and supervision to individuals to assist them in living and working in the community of their choice. Services are offered in Delaware, Linn, Jones and Benton Counties. Community-based services offices are located in: Manchester and Cedar Rapids.

RESIDENTIAL TREATMENT SERVICES Day Habilitation activities and environments are designed to foster the acquisition of skills, appropriate behavior, greater independence and personal choice. Services assist the individual to attain or maintain the client’s highest functional level. Services are offered in Manchester and Iowa City.

Crisis Stabilization provides therapeutic support services in a 24-hour community-based program. On-site staff provide opportunities that include crisis counseling, medication management and housing for a short term stay (3-5 days). Services located in Cedar Rapids. Services include: • • • • •
