The excitement is ‘building’ at St Andrew’s following the Development Assessment Commission’s approval of our planned major upgrade. This upgrade, referred to as the Eastern Clinical Development (ECD) will provide St Andrew’s with improved and expanded clinical facilities. We anticipate our new building and car park will be ready for operation in November 2017.

Details of this very exciting project are on page 16.

It is the staff at St Andrew’s that sets us apart and I am delighted at the number of stories that feature some of the members of our team. In particular the recent additions to our Nursing Division page 10 with the appointments of Tracey Clark and Adrian Saunders to the two newly restructured Associate Director of Nursing positions.

In addition we also welcome Tina Donaldson to the role of Quality & Risk Manager. I know that Tracey, Adrian and Tina will provide excellent support to Jacquie Wiley and the entire Nursing team and I wish them all a long and happy stay at St Andrew’s.

The other staff stories like ‘Home Made’ nurses on page 5, the regular features of Executive Update (Sharon Kendall), page 4 and ‘My’ St Andrew’s Rob Doherty, page 13 highlight the talent and culture of our wonderful organisation.

I would like to take this opportunity to thank Susy Daw, one our Board members who took on the role of MC again for our most recent Breast Cancer Day Luncheon held on October 7. The event was a huge success with more than 250 people in attendance.

We are very proud of the comprehensive work we do in breast cancer and the article on page 6 describes the various options for patients requiring a mastectomy. Another interesting clinical article related to breast cancer is on page 8 where Professor Martin Borg highlights the cutting edge technology for radiotherapy treatment using the Deep Inspiration Breath Hold.

Working closely with our colorectal surgeons to utilise the ‘fast track’ bowel surgery approach is proving to be beneficial for many of our patients.

The article on page 7 identifies the advantages of such an approach and in particular for our elderly patients.

More recently we have been thrilled to host a number of international visitors that have all come to St Andrew’s to have a close look at our organisation and in particular our commitment to patient safety and quality. These visits have been well received and they have all been so impressed with the sense of team work and the passionate culture we have.

EXECUTIVE SHARON UPDATE KENDALL “Living the Dream” is Sharon Kendall’s mantra for her ‘second tour’ at St Andrew’s Hospital. Sharon is well known to many of the team at St Andrew’s, having worked here for 10 years before leaving and returning in March this year to the Director of Business Performance and Development role.

Her remit sees her portfolio providing strategic direction and advice to St Andrew’s Hospital on all high level discussions and decision making relating to health insurance funding, business development opportunities and performance improvement.

With a key focus to also undertake special assignments and projects and report against the hospital’s strategic operational plan, Sharon’s role is both diverse and exciting.

Having more than 25 years experience in the SA Healthcare industry, Sharon has held a number of senior roles in nursing, finance, ICT and administration. She has a diverse range of experience of both public and private sectors having a keen interest and expertise in health funding models and clinical costing. Sharon is passionate about quality and safety and how data can support the monitoring of performance and drive efficiency.

She is also a surveyor with the Australian Council of Healthcare Standards, and holds a BA and an MBA.

Sharon’s passion for culture, quality and safety, are cornerstones of St Andrew’s Hospital, and along with striving for a life-work balance, were the primary drivers for her return.

“It’s great being part of an organisation with a long history in health and one that makes a difference to the SA community that we serve. To be back working with a fantastic team is both an honour and a privilege.”

If asked to describe the qualities that make a great nurse, most people would use words such as “professional”, “responsive”, “caring”, “competent”, “knowledgeable” and “dedicated”.

We think all of those traits apply to our nurses at St Andrew’s, and we are proud of the role we play in helping them develop their skills and knowledge and transfer it into everyday situations.

Our new nurses don’t live-in like they used to in the ‘60s and ‘70s (St’ Andrew’s opened its first Nurse Training School in 1961 and two years later a Nurses Home was built to house up to 50 trainees), but the program is intensive and comprehensive, running for 52 weeks over a seven-day roster.

The St Andrew’s Hospital Graduate Nurse program is unique and has been extremely successful. We have welcomed 164 new nurses into the program since its inception in 2001, and three in four of them have gone on to work at the Hospital.

Significantly, 70% are still with us, helping to ensure our high standards of care are maintained.

The graduates have the chance to work across a number of clinical areas (except Emergency Service), including urology, general surgery, haematology and oncology, plastics and theatre.

They are also invited to participate in special learning opportunities provided by the Hospital, including study days, in-service sessions and tailored workshops. Without fail, they finish the program with a high level of competence and confidence as Registered Nurses.

This year we received more than 480 applications from students for our 2016 intake, and we interviewed 126 of them to fill the 10 available places.

It’s a rigorous process, but we think that’s important.

The Clinical Manager in our Cardiac Unit, Lisa Walter, is a great supporter of the program having worked with many of the graduates. “The program facilitates the consolidation of knowledge and transition from a

university student to Registered Nurse in a supportive and nurturing environment that focuses on skill development and career progression,” she said.

‘HOME MADE’ NURSES Aasha Dyson has fond memories of being part of the program during her graduate year in 2011, when she worked on the second floor (colorectal, urology and general surgery) and in our Cardiac Unit in its very first year. Aasha had planned to be a community nurse following her graduate year, but three years later she is passionate about hospital nursing and one of the valued Registered Nurses in the Cardiac Unit.

Aasha remembers her interview for the Graduate Nurse Program and being impressed by the rotations offered, the study days and how St Andrew’s seemed like a great place to work.

“I was actually offered another graduate nurse role with another hospital but chose St Andrew’s, which in hindsight was a great decision,” she said. The working environment here is amazing. Everyone is keen to share knowledge, they’re friendly and happy and the rosters are flexible.

Transitioning from a student nurse to a Registered Nurse is a big deal.

The responsibility of patient care is a little daunting and St Andrew’s certainly helped me through it.”

ST ANDREW’S BREAST CLINIC PROVIDING THE BEST OUTCOMES FOR ALL PATIENTS its surgeons offer a diverse range of options.

The traditional approach is an implantbased reconstruction, usually using a silicone implant. Depending on the technique used, this surgery can be complicated and certain procedures may take up to six hours. An alternative is a reconstruction which uses a combination of the patient’s own tissue (usually removed from the back) and an implant.

St Andrew’s has one of Australia’s most comprehensive cancer care facilities and the services of our multi-disciplinary Breast Clinic – a partnership between the Hospital and Dr Jones and Partners – are available to all, whether or not they have private health insurance.

During their consultation, patients are reviewed by a specialist and have appropriate imaging taken.

Breast Reconstruction If patients require a mastectomy, they may elect to have reconstructive breast surgery and St Andrew’s and

A third option is a technique known as TRAM (Transverse Rectus Abdominis Muscle) Flap surgery, which uses the patient’s own tissue to complete the entire reconstruction. The skin, fat paddles and musculature are removed from the abdomen and used to mould a new, life-like breast.

There are several options in TRAM Flap surgery, but of particular note is DIEP (Deep Inferior Epigastric Perforator) Flap surgery which, unlike the other approaches, does not remove the abdominal muscle and thus preserves lower muscle function. Amazingly, the skin and fat paddles are used to form a new breast but the underlying muscle remains intact, ensuring normal muscle function post operatively. This surgery

also preserves the muscle fibres, along with the blood supply, which are reconstructed with microsurgery.

DIEP Flap surgery also takes less time because a CT Scan prior to the operation identifies the specific location of the largest blood vessels, allowing the surgeon to know exactly where to operate.

Highlighting our international reputation in cancer care, St Andrew’s is also a test site for the National Breast Device Registry, which is part of a collaboration involving 10 other countries. The Registry tracks the success of breast implants and techniques over time and measures the performance of the device, the doctor and the hospital.

Dr Rod Cooter, who is based at Waverley House Plastic Surgery Centre on the St Andrew’s grounds, is the Registry’s lead specialist for plastic surgery.

‘FAST TRACK’ BOWEL SURGERY St Andrew’s is helping change the way bowel surgery is performed. Working with surgeons from Colorectal SA, we are utilising the “Fast Track” approach to this surgery, resulting in shorter hospital stays, faster recovery and fewer complications.

The “Fast Track” methodology uses new pre-operative, surgical and post-operative techniques, and it begins before the patient even arrives in hospital.

Conventionally, patients drink up to four litres of bowel preparation to completely clean out the bowel prior to surgery. This can sometimes result in dehydration and an electrolyte imbalance, which can significantly impact on recovery time (and is usually unpleasant). Instead, “Fast Track” uses a small enema, and that is often all that is required. Patients are significantly more comfortable pre-operatively and recover much more quickly.

Supplement drinks are used to help prepare patients nutritionally, with the type of supplement dependent on the reason for the surgery. Cancer patients receive a five-day supply of a nutrition support drink to have preoperatively, while non cancer patients receive two days of a carbohydrate drink.

In the operating theatre, the surgeons use a laparoscopic approach, which reduces the length of stay in hospital from as many as 12 days to as few as five. Patients are more mobile and in most cases are walking the same day.

Similarly, fluids and food are introduced much earlier with “Fast Track”. Patients are usually eating within 24 hours, compared with up to three days for open bowel surgery.

One of the other major benefits of laparoscopic surgery is that the incisions are small, so patients are likely to require less analgesia and can be out of bed a lot sooner, reducing the risk of complications related to bowel motility and bedrest.

The positive results achieved with “Fast Track” are especially critical for elderly patients, who are more likely to have existing health issues which can compromise any surgery. For example, dehydration which may result from the bowel preparation drinks used before traditional surgery can be very serious for an older person.

CUTTING EDGE BREAST CANCER TECHNOLOGY AT ADELAIDE RADIOTHERAPY CENTRE Where taking a deep breath can help your heart.

At Adelaide Radiotherapy Centre (ARC) women with a left sided breast cancer diagnosis now have access to new radiotherapy technology known as Deep Inspiration Breath Hold (DIBH).

Deep inspiration breath hold is a treatment technique for breast cancer patients that minimises radiation exposure to the heart. ARC is the only South Australian Radiotherapy service provider who is able to offer this technique to patients. ARC’s Radiation Oncologists led by Associate Professor Martin Borg are now employing this technique in suitable patients with left-sided breast cancers across all four treatment sites, including their St Andrew’s site in the city.

The method requires patients with left-sided breast cancer to hold their breath while treatment is administered - hence the name Deep Inspiration Breath Hold.

Associate Professor Borg explains that we are now capable of synchronizing the delivery of radiation with the patient’s breathing using our breath control unit. This enables us to limit the amount of tissue beyond the breast that receives radiation.

“DIBH enables us to reduce cardiac radiation dose in patients referred to us with left-sided breast cancers. Not only is this a significant clinical achievement but it also has significant benefits in reducing patient anxiety, comforted

by the knowledge that their heart is receiving the least amount of radiation dose achievable.” Associate Professor Borg said.

“There’s also a learning curve to figuring out the process of setting up patients on a daily basis and making sure the setup is accurate. At ARC we have specially trained staff who work through a systematic process partnering with patients to do just that.

It has taken time to build up that kind of expertise and we are very pleased with the results so far.”

ADELAIDE RADIOTHERAPY

At St Andrew’s we are committed to maintaining the highest standards and we monitor 541 separate Performance Indicators every month. Of these, 95 are non-clinical, covering such important areas as Work Health & Safety, energy and waste management, and patient feedback.

Every six months we are required to report a number of these Indicators to the Australian Council of Healthcare Standards (ACHS), and we are pleased to report that in a majority of cases we perform at a rate better than the aggregate private hospital rate.

The Performance Indicators are monitored using a traffic light system, and currently 106 of the 451 are classified as “green” (better than the available benchmark) and only two as “red” (requiring attention).

The majority are “amber”, meaning we are happy with our performance but always looking for ways to improve.

The overall aim, of course, is to maintain patient safety and provide the best care possible in line with our vision that “St Andrew’s will be the best private hospital in Australia with a culture where relationships matter”.

Of particular note is the falls rate at St Andrew’s, which since January 2013 has consistently been better than the 20th Centile set by the ACHS. The 20th Centile represents 80% of hospital results reported to ACHS above this line. This is an outstanding result achieved thanks to significant work by the Falls Working Group and all nursing staff.

PERFORMANCE INDICATORS The Nursing Division continually reviews all falls and the strategies used to minimise the number of incidents.

St Andrew’s patients are risk assessed using a variety of tools and strategies, with the process beginning prior to admission.

Equipment and strategies are organised to meet individual patient needs. The Falls Committee reviews all falls and aligns practice to the National Standards, with the Falls Coordinator providing education and input at point of care with nursing staff.

NEW NURSING DIVISION STRUCTUREPLANNING FOR THE FUTURE

Our Nursing Division has a new structure to better reflect current and likely future needs. Of particular note is the decision to appoint two Associate Directors of Nursing (ADON) to coordinate the operational management of defined clinical areas.

Tracey Clark has been appointed ADON to oversee the Procedural Areas, which include theatres, our Angiography areas, and the procedure rooms where we undertake Endoscopies, Colonoscopies and Bronchoscopies.

Tracey has extensive theatre experience and was previously the ADON at Pindara Hospital in Queensland, where she oversaw an expansion from four procedural areas to 17.

The new ADON for Ward Areas is Adrian Saunders, who has held a number of pivotal senior management positions within the Nursing Division over the past 20+ years. Adrian is responsible for all inpatient clinical areas, including specialty areas, as well as the offsite Chemotherapy service.

We are sad to lose another of our long-standing and valued personnel, with Robyn Ware resigning to move overseas, but are delighted to welcome Tina Donaldson to take on her important role as Quality & Risk Manager. Tina joined us from Burnside Hospital. In line with contemporary practice, the Quality Coordinator role now encompasses organisationwide risk.

The Nursing Division has a number of exciting projects getting under way, including the review of our Clinical Handover and a review of all inpatient documentation.

FOUNDATION IT HAS BEEN ANOTHER BUSY QUARTER FOR THE FOUNDATION! I am delighted to report that we have been successful in raising the $40,000 required to purchase a new Cold Cap Machine for the Chemotherapy Suite.

Thank you to everyone who donated – your support is greatly appreciated.

This piece of equipment will assist in reducing hair loss for chemotherapy patients and will make a significant difference to many people.

In August we held our second SALA Exhibition and I would like to thank our exhibiting artists, Dr Randall Sach, Dr John Tomich and Pippy Mount, for their commitment and stunning work!

Thank you also to those that attended opening night and the exhibition itself.

And a very special thank you to the Volunteers who helped to make the event the success that it was.

Next year we will be celebrating the Hospital’s 80th birthday and the Foundation team is looking forward to being involved in planning various events.

When I caught up with Rob Doherty to write this edition of My St Andrew’s, he was quick to let me know that he was only supposed to be at the Hospital for two weeks as a labour hire contractor when he started with us in 2008. He quickly became part of the St Andrew’s family, however.

In addition to the day-to-day maintenance “rescue requests”, Rob has seen a lot over his seven years on South Terrace, including the redevelopment of the Coffee Shop, the building of the new theatre block, a rebuild of the hospital kitchen after a fire in 2012 and, most recently, the construction of the St Andrew’s Medical Centre and the rebuilding of Theatre 7 to house the much anticipated da Vinci XI Robot. When I asked how he felt about the pending Eastern Clinical Development, he said: “Mate! I can’t wait! It’s great to see the Hospital going forward”.

As anyone who has worked in a large organisation will attest, the Maintenance Department plays a significant role in the smooth operation of the business. St Andrew’s is no

different. Our 12-strong team has a variety of trades under its collective belt, including two electricians, two trades assistants and two biomedical engineers. Rob is our resident“chippy”.

It goes without saying that a good team is critical to job satisfaction – and Rob says the maintenance team is brilliant.

“Nothing is too hard, we’ll give anything a go and then there’s the jocularity!”

With over 200 patient rooms, the operating theatres, a Coffee Shop, Day Patient Suite, Angiography Suite, historic Chapel and the much-loved, heritage-listed Waverley House – not to mention all the administration areas – the maintenance team has plenty to keep it busy. But wait, there’s more! Most people don’t realise it also looks after the maintenance needs of a number of properties along South Terrace, many of which are heritage listed and require a constant watchful eye.

I know all at St Andrew’s will echo my thanks to Rob and the team for their hard work, good humour and “can do” attitude. (And by the way, can I speak to you about building a shelving unit for our new office?)

CANCER SEMINAR The new education facilities on the third floor of the St Andrew’s Medical Centre received high praise from participants after a recent seminar.

On 21 October, Adelaide Oncology & Haematology, in partnership with St Andrew’s, presented a successful interactive education session on the treatment of common malignancies to 49 GPs.

Associate Professor Ken Pittman, a medical oncologist, presented on the history and treatment of renal cancer and provided a range of case studies in relation to treatment options and the management of complex side effects.

GPs play an important role in the journey of cancer patients from the early period of diagnosis to the later period of terminal care.

He was followed by Dr Harminder Takhar, who discussed the history and treatment of prostatic cancer, including palliative care options.

Dr Takhar has qualifications in both medicine and pharmacy.

The intent of these sessions was to give an overview of these common cancers, current treatments, oncology emergencies and treatment related toxicities.

A broad knowledge of cancer treatments provided at these sessions is designed to provide GPs with the knowledge to identify and deal with side effects and toxicities.

Two other sessions in the series were scheduled for November, looking at lung and breast cancer.

It was not only the quality of the presentations and the venue that caught the eye at the first seminar. Some of the GPs present commented that the outstanding catering was among the best they had ever been offered.

BUILDING A BETTER

ST ANDREW’S would be accompanied by extensive landscaping with native plants, energy efficient lighting systems and high levels of fresh air.

We have also held positive discussions with neighbours to ensure they are aware of what is being proposed.”

The State Government’s Development Assessment Commission (DAC) has approved plans for a proposed $45 million expansion.

The proposed development, which will be entirely self-funded, will involve construction of a new day surgery facility containing two operating theatres, three procedure rooms, two angiography suites with the associated recovery beds, a 28 bed ward, a staff gym and a three-level carpark, two of which will be underground.

The day surgery unit will be linked to the hospital’s existing first floor theatres and the Intensive Care Unit.

CEO, Mr Stephen Walker said the DAC application followed extensive discussions with local residents, the Design Review Panel, Heritage SA and the Adelaide City Council.

The build, on the corner of Gilles Street and St John Lane, is expected to take about 18 months with up to 100 construction workers on site during the most intensive periods.

“The project will consolidate our position as one of Australia’s premier private hospitals.”

“We have a South Australian project design team and where possible, will be using South Australian construction firms with South Australian subcontractors,” Mr Walker said.

“Once finished, the development will boost hospital bed numbers from 201 to 217 with all patients having private rooms. It will also increase the number of operating theatres from nine to 11 and provide an additional 100 car parks on site.”

Earlier this year, St Andrew’s opened a new floor at the St Andrew’s Medical Centre on Hutt Street.

The $4 million development features a new function centre and houses the St Andrew’s Hospital Foundation as well as the Hospital’s human resources, information technology and finance operations.

Mr Walker said the new development was planned on land currently used as a staff carpark. The building is being designed to be as environmentally sustainable as practicable and

FROM THE ARCHIVES WAVERLEY HOUSE – IS IT, OR ISN’T IT?

There’s much conjecture around the spelling of our beloved Waverley House. Is it Waverley or Waverly?

If the walls could talk, they could explain to us why there was a change in the spelling at some point in history.

We are sure they could also share some interesting stories from the last 155 years – including cries of shock at the time when it was almost demolished!

Waverley House was one of the first mansions in Adelaide’s south-east corner. It was built in the early 1860s as a private residence for prominent businessman and pastoralist William Sanders. In 1946, the house was acquired by the Legacy Club of Adelaide to be used as a children’s hostel. Eighteen years later it was

bought by the Board of St Andrew’s Presbyterian Hospital.

The Hospital’s intention was to use the building as a temporary nurses’ home, but by the 1970s it was little used and the decision was taken to demolish it. Fortunately, however, a demolition submission was rejected, and the house was refurbished and subsequently used for some years as the St Andrew’s Hospital Nurses’ Training School.

Between 1988 and 1989 the coachhouse at the rear of the building was

converted into the existing St Andrew’s Hospital Chapel. Waverley House remains part of the Hospital property and is preserved as a heritage building. Over recent years it has been enhanced by the restoration of a community garden through the Secateurs Gardening group.

Waverley House, its historic wall and gigantic pine tree form an important part of the South Terrace Heritage Precinct.

It is also home to a number of specialists and is the Hospital’s Board Room. We still haven’t been able to determine why the spelling of Waverley House changed.

COMMUNITY STUDENTS FROM HYOGO UNIVERSITY, JAPAN VISIT ST ANDREW’S Twelve Japanese nursing students and a lecturer from the Hyogo University of Health Sciences in Kobe visited St Andrew’s in August during a threeweek cultural and health information study tour of South Australia. The visit was part of a reciprocal tour experience which also saw nursing students from the University of Adelaide travel to

Kobe to learn more about Japanese society, different nursing practices and alternative approaches to contemporary health care.

The St Andrew’s tour was a highlight of the Japanese students’ stay and their only opportunity to visit a private hospital in Australia.

ST ANDREW’S HOSPITAL PRIZE IN HEALTH ADMINISTRATION Carolyn Manhire received the St Andrew’s Hospital Prize in Health Administration, which is awarded to the student in the Graduate Certificate in Health Administration at Flinders University who achieved the highest Grade Point Average (GPA) throughout the course. Carolyn works in the Rural & Remote Mental Health Unit, Country Health SA Local Health Network. She was awarded her prize at the ACHSM SA state conference on Friday 31 July 2015.

VISITORS TO ST ANDREW’S We have had the pleasure of hosting a number of Hong Kong Hospital Managers at St Andrew’s recently.

On Monday 7 September Professor Joseph Lui, Clinical Director Hong Kong University Health System, stopped off in Adelaide, (enroute to New Zealand) for the day to have a look at our safety and quality system and to talk about clinical costing. CEO Stephen Walker also took the opportunity to take Prof Lui as a VIP guest of the University of Adelaide to the public lecture by the Hon Julia Gillard.

On Monday and Tuesday, 29 and 29 September, we welcomed nine managers from the Hong Kong East Cluster Hospital Authority who attended a two day development program that we prepared for them at St Andrew’s.

It was an absolute pleasure to have them here and we look forward to a long and mutually beneficial relationship.

Extremely friendly staff, very clean environment, the staff explained everything to me clearly.

The nursing staff were excellent and it was so nice to be able to chat with the staff.

I felt really comfortable at St Andrew’s.
