What orthopedic and sports medicine needs does our community have?

Geoffrey Doner, MD

Front Range Orthopaedics, a Centura Health Clinic, combines an awardwinning, hospital-based orthopedic program with well-respected orthopedic surgeons. Its goal is to provide patients with the ultimate experience and excellent outcomes when undergoing orthopedic surgery. How did you get your start in orthopedics and sports medicine?

Growing up, I was an athlete, so it was a natural fit when I started looking into medicine. I feel I can relate with athletes, amateur and professional, and help them get back to the activities they love after an injury knocks them down.

I received a Bachelor’s in biology and communications at the University of Colorado in Boulder, where I also attended medical school, followed by a five-year orthopedic surgery residency in New York and a year-long sports medicine fellowship in Ohio, specializing in shoulder and knee reconstruction. I’m thankful for all of my experiences, especially for my communications studies. I believe it has helped me in learning to listen to my patients and give them both the empathy and clear answers they need to take steps towards health and recovery.

Colorado Springs in a unique place to practice sports medicine. We have a lot of military folks in town — I call them my warrior athletes. We care for many of the injuries they sustain, including quite a few shoulder, knee and hip injuries. Then, out east, we have the farming population. We treat people from the rural communities for a variety of injuries — everything from farm equipment accidents to run-ins with barnyard animals. I just reconstructed an ACL that was damaged when the patient was kicked by a horse.

The Springs is also full of outdoors enthusiasts; I see a lot of ankle fractures and broken wrists — definitely some Incline injuries. We’ve also got the skiing and snowboarding population and mountain bikers, so really quite a variety of sports injuries, in all shapes and sizes of people. I’ve done everything from a hip labral repair in a 14-year-old ballerina to treating a torn rotator cuff in a 70-year-old skier.

What are some of the most common ailments you treat? How can people work to ensure health in these areas?

About half of my patients are average, active people and the other half are professional or student athletes. I see quite a bit of rotator cuff tendinitis in all ages, which typically results from overuse of the shoulder in general labor or over exercise. While it is fairly common and easy to treat, it can be avoided by strengthening the rotator cuff through specific exercises, particularly important for athletes.

Meniscal tears — a twisting injury to the knee — are also fairly common, as is tendinitis — a painful tear — in the hip. I also see quite a few labral tears, which are twisting injuries in the hip joint. However, only about

ten percent of patients I see require surgery; many can be treated with physical therapy to help the body naturally repair itself. There are plenty of non-operative and preventative treatments people can add to their routines, including proper diet and moderation of exercise as they age.

What are some recent developments in your field?

How has this improved treatment and recovery for your patients?

In the last ten years, we have really increased in our understanding and knowledge of hip arthroscopy, which is a minimally invasive surgical procedure on a joint. Historically, when a patient had a hip injury, we didn’t have the technology to go in and arthroscopically repair it; now, we have a mini, high-definition camera that we can insert into the area and use to guide the surgeon. Twenty years ago, we had to make a five-inch incision, and now we just need to make three, small holes; it’s much easier on the patient. The cameras have also continued to improve, just like your TV at home. Now, when you look at a standard definition screen, you notice a difference.

Joint replacements have also become more anatomic, functioning more and more like the body’s original joints and hopefully lasting patients longer through their lives. Both of these improvements have made surgical recovery a much easier process.

Anything we have done and can do to make treatment less invasive helps patients recover more quickly.

Our physical therapists have also learned quite a bit in recent years about getting post-operative patients into therapy immediately, versus waiting for them to wear and get cut

out of a cast. We now emphasize the importance of motion earlier, which has significantly cut the average time in therapy.

Have you seen any partnerships develop locally in the world of orthopedics and sports medicine?

On April 1, Front Range Orthopedics teamed up with Penrose-St. Francis Health Services, part of the Centura Health team, to lead the way in local orthopedics and sports medicine.

We operate the Orthopedic Center for Excellence five days a week on the sixth floor of St. Francis Hospital for patients who’ve had total joint replacements, including hip, knee, shoulder and ankle. It’s truly a onestop-shop: patients can see a doctor, get physical therapy, have X-rays done, and more. This facility allows us to be more available to patients on the north and east ends of town. We’ve already seen patient outcomes improve over the summer as care is now more seamless and immediately available.

Front Range Orthopaedics is also growing, with locations from Monument to Woodland Park — we are continually branching out to bring quality, orthopedic care to the entire Pikes Peak region. We even have doctors who travel to provide care to underserved areas, including monthly trips to La Jara, Colo., outside of Alamosa.

We also have valuable partnerships with local athletic teams, including the Colorado Springs Switchbacks, F.C. We attend games at Sand Creek Stadium and provide medical care for the players; it’s been great working with professional athletes. I also serve as the team doctor for the Falcon High School football team. ✚

Genetic counseling provides patients knowledge of history, future

2013 was a big year for breast cancer genes.

In May, actress and director Angelina Jolie revealed she had opted for a double mastectomy as a preventative measure after discovering she was a carrier of a mutation in the breast cancer (BRCA) 1 gene, drawing widespread attention to use genetic testing in detecting hormonal cancer risk.

In June, the Supreme Court ruled that naturally occurring breast cancer genes cannot be patented. This paved the way for more labs to jump into the genetic testing scene, driving down the cost for patients, but also complicating the testers’ abilities to interpret the lab results.

Kate Crow, genetic counselor at Hereditary Cancer Service in Pueblo, part of Southern Colorado Centura and Penrose-St. Francis Health Centers, has worked in genetics for 18 years and has seen many changes in the field during her career.

“They had just discovered the BRCA1 and BRCA2 genes when I was

negative test result. “It’s very important to understand your test results before going to a surgeon,” Crow said.

WHO SHOULD GET TESTED

in training,” Crow said. “Now we have testing for 20 – 40 breast cancer genes.”

HOW IT WORKS

For now, the BRCA1 and BRCA2 gene mutations make up the majority of inherited breast cancers. BRCA1 and BRCA2 are tumor suppressor genes; when they work correctly they keep you from developing breast and ovarian cancer — or breast and prostate cancers for men. Those with mutated

genes are at risk for developing cancer.

“If we find patients with one of these two genes, we can talk about profound treatment options like a risk-reducing mastectomy or removing the ovaries,” Crow said. “Surgery isn’t the only option, some people prefer to just watch it very carefully.”

The process starts when the patient receives a family history form several weeks prior to their appointment. This allows time to contact relatives for medical history.

“It’s important to keep in mind the men in your history, too,” Crow said.

Ahead of the curve: Colorado Dermatology Institute invests in medical education professionals, students

The road to improved medical education in our community is being paved by a group of dedicated dermatologists in Briargate.

Colorado Dermatology Institute (CDI), located at 8580 Scarborough Drive, Suite 225, recently developed an affiliation with the University of Colorado School of Medicine’s Department of Dermatology, allowing for collaboration between the only two dermatology residency programs in the state.

“They graciously came to me and asked if we would share resources,” said Reagan Anderson, DO, dermatologist at Colorado Dermatology Institute. “This means our residents now have access to university dermatology and some of the most complex cases in the state, so it really helps round out their education. Plus, we’re not a hospital — we’re a community clinic — so it offers residents access to another environment.”

There are added benefits for CDI patients, too — including the addition of Susan Schroeder, MD, the newest dermatologist at Colorado Dermatology Institute. Dr. Schroeder is also

an assistant professor of dermatology at the University of Colorado Denver Anschutz Medical Campus. “Dr. Schroeder brings university connections and offers us an easier route to get the care our patients need, especially when it comes to rare, genetic skin conditions and large cancers that require a multi-specialty approach,” Dr. Anderson said.

Dr. Anderson specializes in Mohs Micrographic Surgery for the treatment of skin cancer. Mohs is the most proven and effective method for removing both surface skin cancer and any remnants that exist below the surface of the epidermis. “It’s a highly precise procedure, so it also allows for the best cosmetic results because you cut out less

skin surrounding the tumor,” he said.

“It’s generally a safer, more convenient and less expensive procedure.” Dr. Anderson is one of 40 Mohs surgeons in the U.S. to have earned the American Osteopathic Board of Dermatology Certificate of Added Qualification in Mohs Micrographic Surgery.

In addition to in-house surgical expertise, CDI ensures a high quality of customer service for its patients from the moment they walk through the door. “This is something we want the residents to see and learn, too,” Dr. Anderson said. CDI wants to see its new collegiate affiliation set the standard for future residency programs in Colorado Springs across medical disciplines. “I hope this will be a bridge

for the osteopathic and allopathic communities; we can learn from each other, and it can be mutually beneficial for all our residents,” Dr. Anderson said.

“I can’t tell you how excited we are to see this take off and grow into something that builds our community and develops future dermatologists in our state. I’d love to see more clinics step up to the plate to institute residency programs and make our city a champion for medical education and care.” ✚

CU School of Medicine students experience poverty-health link

As Colorado Springs prepares to train the next generation of physicians from the University of Colorado School of Medicine (CUSOM), community stakeholders engaged with 22 medical students from August 11–13 in the Poverty Immersion in Colorado Springs (PICOS).

Colorado Springs will welcome its first cohort of medical students from the CUSOM Colorado Springs Branch (CSB) in April 2016 as they begin their third-year clinical rotations. PICOS served as an immersion experience for students to learn more about the community and about people who have limited resources. “This experience got me thinking more critically about the world around me,” said Erica Martinez, a second-year CUSOM medical student.

PICOS was designed to help participants understand demographics and healthcare disparities in Colorado Springs, define health equity and analyze social determinants of health effects

on health outcomes.

Thanks to the generous support of Community Health Partnership, Kaiser Permanente, and other organizations, participants also walk away from the program able to obtain and utilize basic resources while experiencing life in poverty, identify attitudes toward poverty and those with limited resources, and recognize the relationship between adverse childhood experiences and health outcomes.

“Overall, I was really impressed with PICOS,” Martinez said. “It was not a superficial experience, it made me dig deep and really think about the perspectives and everyday lives of those who are under-resourced.”

Representatives of Pikes Peak United Way and El Paso County Public Health educated PICOS participants on the community’s demographics and health indicators and how poverty contributes to poor health outcomes. To develop empathy towards resource-limited community members, participants

An appointment with a genetic counselor lasts about 60 – 90 minutes, during which time the counselor records the family history and estimates potential cancer risks.

Patients are encouraged to ask questions, and counselors assess how patients feel about their history, potential risk and whether they would like to be tested.

“Most opt to be tested and know their

risks,” Crow said. “I would say just one out of every 20 – 30 patients isn’t ready to know; it makes them too nervous.”

Two-thirds of the people who see Crow for genetic testing have cancer and want to know if they might be susceptible in the future; the other third are unaffected people who might have a family history. Crow recommends anyone with a family history of breast, thyroid, ovarian or prostate cancers receive testing, or if

you’ve had a family member develop cancer before age 50.

Those who choose to be tested can submit a specimen at the time of the initial appointment; it takes about two to three weeks to get results back from the lab. Then, the genetic counselor discusses the results with the patient, ensuring they understand all its implications and their options.

While the future of genetic testing is a much-debated topic, Crow is hopeful

that it will continue to help her do her job: saving lives.

“It’s the closest thing to a cure. Isn’t that exciting?” ✚
