Freeborn County Community Health Worker
Freeborn County Public Health is hiring a Community Health Worker of the Karen culture to build a better relationship with the Karen population living in Freeborn County. The Community Health Worker will be responsible for assisting Public Health in implementing health education programs for underserved populations. This position will help our underserved population gain access to needed medical, social, educational, vocational and other services related to individual’s whole health.
Responsibilities may involve implementing case management support activities under direction of staff working with clients, performing client intake, providing informal counseling and information about program resources to clients, and providing recommendations to Public Health team on cultural implications and responsiveness for health education programs.
Minimum qualifications of education and experience:
High school diploma or GED
Bilingual proficiency in English and Karen. Community Health Certification or ability to obtain Certification within two years of hire.
The position will be a full-time position. The salary range is $22.77 - $30.37. Candidates must have a valid Driver’s License and reliable transportation.

The Home and Community Based Services (HCBS) RN/PHN/Social Worker position is responsible to determine level of care for individuals needing an institutional level of care and assisting persons with long term or chronic care needs in making care decisions and selecting support and service options that meet their needs and reflect their preferences. Collaborates to promote service accessibility and provide comprehensive coordination of services to meet the needs of clients across the continuum of social services, public health, medical and other community services. This position is responsible for implementing the required Local Public Health Activities that are organized according to the following areas: assuring adequate local public health infrastructure; promoting healthy communities and healthy behavior; preventing the spread of infectious disease; protecting against environmental health hazards; preparing for and responding to disasters and assisting communities in recovery; and assuring the quality and accessibility of health services. Pay range $34.074 – $47.70. Prior public health experience and bilingual abilities desirable but not required. The Public Health Office Support Specialist position is responsible for performing a variety of diverse and complex clerical tasks that require attention to precision and accuracy. This position is responsible for providing exceptional customer service to people walking into the Department or calling into Public Health. This position provides office support for the nurses and social workers when needed by making packets, typing, faxing, in the operation of copy machine and anything else as needed. This position is responsible for Accounts Payable and Employee Reimbursements. The Public Health Office Support Specialist must maintain HIPPA while doing daily tasks.
