What to Do When You Hear the Life-Changing Words, “You Have Cancer”

As a medical oncologist, I understand the toll a cancer diagnosis takes on patients and their families and I know how overwhelming this news can be. Patients receive a lot of information while dealing with a range of emotions, including shock, fear and uncertainty.
Here’s what I want patients and anyone impacted by cancer to know about finding the best care. As a breast cancer specialist, I’ve seen thousands of women – and many men – survive and thrive after breast cancer. It’s a one-step-at-atime process that starts with getting the right information and making crucial decisions at the right time.
FIND THE RIGHT PLACE FOR CARE
Many people diagnosed with cancer have a sense of urgency and want to jump right in and start treatment. However, in most cases, there is time to research and, most importantly, get a second opinion. It’s crucial to find a physician who specializes in your kind of cancer and has access to the latest treatments and therapies.
GET A SECOND OPINION
Getting a second opinion for something as life-changing as cancer is important. The purpose is to have another cancer specialist review your test results, give an opinion about your diagnosis, and recommend treatment options. In some cases, a second opinion will confirm your doctor’s diagnosis and plan. Or, you may gain more details about your type of cancer. For some, a second opinion can lead to new treatment options; getting the right diagnosis and treatment plan is important.
A second opinion can alleviate a lot of concern and second-guessing. It’s best to get a consultation from a specialist who focuses on your kind of cancer. With this information, you can feel confident about what at first may feel like an overwhelming treatment plan. The specialist who provides the second opinion can work in partnership with your primary oncologist to coordinate the best treatment plan for you.
KNOW YOUR KIND OF CANCER
I can’t emphasize enough that knowledge is power with breast and any other kind of cancer. Not all cancers are alike. Before selecting the treatment plan that’s right for you, you’ll want to know everything about your specific cancer. Your doctor will explain staging or the degree to which your cancer has advanced and spread to other parts of the body. Staging is measured on a scale of one to four, with four being the most advanced disease.
Breast cancer, for example, has several subtypes that have their own molecular drivers. Some tumors are
driven by hormones, specifically estrogen or progesterone.
Cancers that are sensitive to hormones — estrogen receptor (ER) positive or progesterone receptorpositive (PR) — typically grow more slowly and respond better to certain treatments. For example, if your tumor is ER-positive, treatment with anti-estrogen hormone therapy may block cancer cell growth.
It’s also important to select a medical center where you can participate in clinical trials. My organization, City of Hope, is currently conducting more than 500 clinical trials, speeding pioneering treatments to the people who need them today.
I also recommend that patients and families take advantage of personalized supportive care services. Our teams of social workers, psychiatrists and others partner with patients and their families to address the many physical and emotional issues that can arise during and after treatment.
Finally, try to be positive, ensure you have a good support system, and hold onto hope; I believe hope is essential.

Medicare Annual Enrollment is Happening Now

The Medicare annual enrollment season is now in full swing. If you’re Currently on Medicare then this enrollment season is made just for you. Between October 15th and December 7th is the time to change to a different insurance carrier that offers a Medicare Advantage plan or a Medicare drug plan.
The insurance companies are able to make changes every year to their plan benefits, co-pays and premiums. Once these changes are approved by Medicare, they can release the new plan information October 1st and that is when a Medicare beneficiary can start researching their options if they think they may want to make a change. Any change made becomes effective January 1st
Many people, for some reason, take the task of looking for a new plan on by themselves. Most often I find that these are the people that when they first enrolled in a Medicare Advantage plan they enrolled directly with the company and didn't have a personal broker to explain options and benefits and establish a relationship with. Other times during the Medicare annual enrollment period (AEP) I come across people who started with a broker when they first went onto Medicare, but then that person left their Medicare insurance career or ended up just not being a good fit for the Medicare beneficiary to feel comfortable working with year to year. The problem with trying to see if your current plan is the right plan for you without the help of a Medicare insurance professional is that you don't know what to look for because you “don't know what you don't know”. And, when you use a broker to help you find the right fit for you, their services are free! Now that’s a silver lining to off-set the “dark cloud” of being inundated with Medicare plan materials in your mailbox.
If you're looking to change your Medicare Advantage plan you need to be careful because not all Medicare Advantage plans participate with all of the medical networks in Orange County. If you switch to a plan because you think they have a better hearing aid benefit for example, and you don't double check to make sure that your preferred doctors and hospitals are in network you may end up on a plan that doesn't allow you access to those providers and hospitals. You also need to be sure to pick the right plan that offers the lowest co-pays on your medications.
Medicare drug plans also change annually. There can be changes to the deductible, premium, co-pays, networks, and formulary (the drugs that are covered by the plan).
People who have a Medicare drug plan are those that use their Original Medicare card as their primary insurance and then usually purchase a Medicare supplement plan to cover their share of the expenses that Medicare doesn’t pay. Often during AEP I get calls from people in this situation looking to see if a move to a Medicare Advantage plan could help them with their RX expenses. (And for sure it would because there aren’t RX premiums OR deductibles on the majority of the Medicare Advantage plans offered in Orange County.) Their Medicare supplement plan premiums also tend to increase annually, and not having to pay that premium every month can really help them save a lot of money.

In truth, many dermatologic treatments are technically a form of very controlled skin damage. “For over 20 years, yoga has been a major life tool for me, bringing me much freedom, peace and joy,” she explains. “I want to share that with everyone.”
She loves to guide and observe the transformation of all her students with this ancient practice of connecting mind, body and soul.

It is with a grateful heart and tremendous gratitude that I am writing to you - my loyal customers, friends, vendors and colleagues - to share my vision for the continued growth and future of Brio Tuscany Grille. I would also like to take this opportunity to definitively put to rest the rumors I have been hearing about my imminent retirement and relocation to Florida.
While my wife, Jill and I do own a vacation house in Florida, our home and our hearts have always been here in Dana Point so the idea of leaving Southern California simply couldn’t be further from the truth.
As a restaurateur and business owner, I have been truly blessed in having brought on board a team of the highest caliber - in their dedication, skill, enthusiasm and integrity. Each staff member is my extended family and I would like to make special mention of some of the individuals who have been by my side for the better part of two decades. Brio Tuscany Grille would not be where it is today without people like Peppe (General Manager), Dining Room Supervisor (Claudio), Executive Chef (Eudocio), Musician (Justin), Sous Chef (Candido) and many more!
So as for retirement, I have to admit that I do feel “semi-retired” some days - but that has everything to do with the fact that I have surrounded myself with such a loyal and highly competent team. While I continue to oversee the operation, I am able to leave much of the day-to-day managerial responsibilities in their very capable hands. As a result, I am able to enjoy spending time with my family and friends - pleasures I certainly hope to continue when I do eventually retire.
For all intents and purposes, Peppe and Eudocio, in particular, have always been my true “partners” on this journey - their insight and contribution to developing the wonderful relationships and culinary traditions helped establish Brio into what I hope and believe is an iconic Dana Point dining destination. I am excited to announce that as my partners in this ongoing endeavor, Peppe and Eudocio are committed to taking on an even bigger role as we continue our collaboration to further innovate and expand our offerings.

The last time we showcased Ortega Animal Care Center in South County Magazine, Dr. Fluent and Dr. Brehmer were excited to begin offering the highest quality veterinary care and scope of offerings to South Orange County’s animal residents.
“Our main focus was growing the hospital by expanding the hours and making ourselves available 7 days a week,” says Dr. Lynlee Fluent. “We’ve done quite a bit more than that in the past few years.”
In fact, since they opened six years ago, staff at the Ortega Animal Care Center has doubled, and not only are their in-house services available 7 days a week, they offer a 24-hour pet hotel staff, pick up, and delivery. Front to back administrative changes include digital dental radiography and a comprehensive in-house laboratory and full-service pet pharmacy. So why all the upgrades?
“We’re constantly looking for new ways to make pet care as convenient and accessible for pet owners as possible, while keeping our prices

Pets of the Month
Three year old Prince wasn’t always this handsome. When Prince first arrived at the shelter, he was covered in sores and missing most of his fur due to a massive flea infestation. After a little bit of care and a whole lot of love, he transformed into the fine-looking feline he was always meant to be. Add to that a sweet and outgoing personality and there’s no way you can call Prince anything but charming.
This sweet little pup is Honey, a 5 month old terrier mix. She is a bundle of energy and is always ready to run and play. When she isn’t chasing after toys, her favorite place to be is in your lap. Honey gets along well with the other small dogs and would make a great addition to the family.
Bear is a 2 year old neutered male Chihuahua mix. He has a very sweet personality and loves to be held. Bear gets along well with the other small dogs he has met and wouldn’t mind another dog in the home. He would be a great addition to a loving family.
Six month old Rosita is now available for adoption! She has a sweet personality and is curious about everything around her. Rosita loves playing with the other kittens, especially her best friend Midnight. They love to cuddle and play on the cat wheel together.

Hide and Seek Observations on How People Store Their Planning Papers

Every estate attorney has heard the story recited in her conference room: “We know Mom and Dad did a trust; we’ve looked everywhere, but we can’t find where they kept it. And their lawyer died before they did.”
Estate attorneys hearing these sad stanzas respond as best they can, but not with much optimism. Staff members write and call entities that had any contact with the trust—banks, insurance companies, credit unions, stock brokers, notaries, or real estate agents among others. The firm will likely contact the landlord where the parents’ trust attorney kept offices prior to passing. Some attorneys check with local lawyer contacts in the Estate Planning Bar, or with the surviving spouse of the deceased attorney, to find the name, if any, of anyone who may have acquired the late lawyer’s firm after he or she left.
Regrettably, most efforts are unlikely to lead to the trust. One of the concerns contacted may produce a copy of the trust, but without the original trust, the trust administration will probably require some involvement with the Probate Court to allow the as-
sets to pass to the parents’ family. This may mean that the terms of the trust get displaced in favor the Probate Code’s list of beneficiaries, including anyone that Mom and Dad intended to disinherit.
Sound practice should include both advising children of Mom and Dad secured their estate documents, and how to gain access to those documents. Simply put: assure that each of the successor trustees listed in the trust instrument has instructions on where to find and how to reach the estate documents. When a parent wants to discuss the terms of a trust with an adult son or daughter, she ex-
pects to be able to readily retrieve the book or the packet from a place that is reasonably reached, but not a spot on the book shelf convenient for visiting aunts and uncles to peruse if they are sleeping on the sofa.
Many people use their safety deposit boxes to store their trusts. These locations are certainly secure, but they do not meet the test of “accessible.” There are times when a family needs to get to estate documents when the boxes are not available. And when a family member dies, many banks and security companies have strict requirements about the access to safety deposit boxes, in some cases even including the presence of a law enforcement official at the time of the opening. This interferes with both the convenience and the privacy which families expect from their family trusts.
A locked file cabinet in the family den provides a good alternative for the home of the trust: it is accessible but secure, central but free from wondering eyes. Two steps complete this location as a good alternative for the planning document—first, reminders for Junior or Juniorette that the trust is in the cabinet, and second, a fixed, permanent and communicated location for the key to the file cabinet, one that provides access to the drawer when the time comes for the next generation to put the trust to its intended use.
Every family is free to decide which is more important, security or access. But whichever variable weighs more heavily for any household, every parent should assure that his or her successors know where the documents are kept, and how to get hold of them. Remember: courts are free, sometimes required, to hold that a trust which is missing is a trust which is revoked, which means…Probate Time.

A year later, when the concrete moves again and damages their brand new floor coating, the devastated homeowner will learn that this damage is not covered by the typical Epoxy warranty. “We are the only company that I know of that uses a ‘structural crack repair’ technique to fix cracks. The process is so effective we can offer our customers a warranty that can cover future damage from cracks!” says Paul.
After the surface prep is completed, a very effective moisture control primer is applied to the concrete. “This is extremely important” says Paul because for many homes built in Socal before 2005, elevated moisture vapor levels in the garage concrete can have a detrimental impact on floor coatings. Paul does a fast and non-destructive moisture test for every one of the pre-2005 homes he visits and develops a unique installation plan for each home. For newer homes, Paul and his crew can use a basic primer that dries a bit faster allowing installation of a garage floor in as little as one day!
The color chips that give the Perkins floor its legendary appearance are sandwiched between this primer and finish coat. Perkins carries the widest selection of colors and sizes in the market and uses a ‘full chip broadcast’, not a partial sprinkling of chips like many of its competitors. “The full chip broadcast gives the floor a consistent appearance and makes the floor much more durable and helps hide dirt better than the partial broadcast finish” says Paul.
Finally, Perkins installs the SpartaFlex polyaspartic finish coat and the finished garage comes to life. “Epoxy & SpartaFlex floor coatings are virtually identical in appearance when they are first installed” says Paul. “However, when you will really begin to appreciate your SpartaFlex floor is a few months down the road when you start to see problems develop with your Epoxy floor. If there was a better material on the market, I would be selling it, but there isn’t and that is why we love selling SpartaFlex.”

When I was a child, I remember my parents moving our family into a new house, with pristine, white carpets. Beautiful to look at, but completely impractical when you consider that two young children would soon be tromping about.
As expected, my brother and I managed to ruin a swath of the floor covering in spectacular fashion … and in relatively short order. Enough so that it was replaced with a more manageable shade. There’s a theory that Malcom Gladwell popularized, noting that anyone can become an absolute expert in their field, as long as they spend 10,000 hours practicing.
More recent studies in psychology note that while this theory doesn’t necessarily tell the whole truth about being world-class, the act of “deliberate practice” certainly makes a difference. John Christensen has been refinishing furniture and creating custom cabinetry since the early 80’s. If you even throw two hours a day for that span of time at the wall, you’ve hit 8,000 hours. 4 hours a day? Well, suffice it to say, John’s an expert at custom, handbuilt cabinetry.
In high school, a friend who graduated before John and went into the field of antique furniture would toss work John’s way, so he began to hone his craft. Work experience and a couple credits later, John was hooked.
