One of the best exchanges in the film happens roughly three-quarters of the way through, as Kathleen Kelly and Joe Fox are chatting. HELPING SENIORS RECOGNIZE STROKE WARNING SIGNS AND PREVENTION

Strokes occur when blood flow to the brain is interrupted, leading to damaged brain cells. This interruption can result from a blockage in an artery (ischemic stroke) or the rupture of a blood vessel (hemorrhagic stroke). Strokes can have devastating consequences, including paralysis, cognitive impairment, and even death.

One of the most critical aspects of stroke awareness is recognizing the warning signs and acting quickly. Common symptoms of a stroke include:

• Sudden numbness or weakness in the face, arm, or leg, especially on one side of the body

• Confusion or trouble speaking or understanding speech

• Difficulty walking, dizziness, or loss of balance or coordination

• Severe headache with no known cause

Prevention is key when it comes to reducing the risk of strokes, particularly for seniors who may be more susceptible to cardiovascular risk factors. Preventable measures seniors can take include:

• Manage Hypertension (High Blood Pressure): High blood pressure is a leading risk factor for strokes. Partner with a health care provider to address hypertension through lifestyle modifications, adopting a heart-healthy diet, engaging in regular exercise and, if necessary, adhering to prescribed medications.

• Control Diabetes: Strive to manage blood sugar levels through diet, exercise, medication adherence, and regular monitoring of blood glucose levels.

• Maintain a Healthy Diet: Focus on consuming a variety of vegetables, fruits, whole grains, lean proteins, and healthy fats while limiting sodium, saturated fats, and processed foods.

• Stay Active: Regular physical activity is beneficial for cardiovascular health and can help lower the risk of strokes. Aim to engage in moderate-intensity exercise, such as walking, gardening, or Tai Chi for at least 150 minutes per week.

• Quit Smoking: Smoking significantly increases the risk of strokes and other cardiovascular diseases. Those who smoke should seek resources for smoking cessation to improve their overall health.

In writing this editorial, I recalled a friend once asking me to explain the premise of a book called “All Creatures Great and Small.”

How Can I Tell If I Have Skin Cancer?

There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.

Look especially for change of any kind. Do not ignore a suspicious spot simply because it does not hurt. Skin cancers may be painless, but dangerous all the same. If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.

ACTINIC KERATOSIS

An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered a precancer because if left alone, it could develop into a skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed sun, such as the backs of the hands, forearms, face, neck, and scalp.

SQUAMOUS CELL CARCINOMA

Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly and some forms are tender to touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year and is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.

WHAT PATIENTS NEED TO KNOW

1. Everyone needs to perform a self-skin exam once a month.

2. Melanoma can appear anywhere on the skin, including the palms/soles, under your nails, and places...for most, have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check, it could save your life.

3. What to look for: ABCDEs, and an ugly duckling

A - Asymmetry: If I draw a line down the middle, do the sides match?

B - Border: Is there a nice crisp border, or does the mole melt into the surrounding skin?

C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?

D - Diameter: Any mole larger than 6mm, ie the head of a pencil eraser, has a higher risk of becoming melanoma.

E - Evolution: Has it changed?

F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?

4. Everyone needs a full body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

Health Savings Accounts (H.S.A) and Medicare

If you have a H.S.A. when you sign up for Medicare, you can’t contribute to your H.S.A once your Medicare coverage begins. If you contribute to your H.S.A after your Medicare coverage starts, you may have to pay a tax penalty. To avoid a tax penalty, you should stop contributing to your H.S.A at least six months before you apply for Medicare.

Now, here is a really interesting piece of information regarding how your H.S.A can reimburse you for your Medicare premiums paid. Yep, even if your Medicare premiums are automatically deducted from your Social Security check, you can take tax-free withdrawals from a health savings account to reimburse yourself for those premiums. You can use your H.S.A money tax-free to pay for Medicare Part B, Medicare Part D and Medicare Advantage plan premiums once you’re 65. (But not premiums for Medicare supplement policies) in addition to paying for other out-of-pocket qualified medical and dental expenses. (Now including over the counter medications and sunscreen!)

There’s no time limit for withdrawing money from a health savings account to pay for these expenses. So, if you’ve had money in a H.S.A for several years and didn’t realize you could withdraw the money tax-free for Medicare premiums, you could reimburse yourself for all those premiums at any time. (Just make sure to keep your receipts for expenses).

Sanctuary by the Sea in La Jolla The Lodge at Torrey Pines

Ever since my now teenage kids were littles, I have relished the opportunity to create “Mom & Me” time with each individually - small outings to spend one-on-one. Local library events, stops for frozen yogurt, a movie matinee (anything Marvel with my son and every Disney princess with my daughter). These days the dates look more like sushi lunches and strolls through the shopping mall, but I recently managed to arrange a bit more elaborate adventure.

As our son quickly approaches his senior year in high school, I took advantage of spring break to arrange an extra special treat in one of our family’s favorite day trip destinations - La Jolla - with an overnight stay at The Lodge at Torrey Pines.

OPEN AIR (AND INDOOR) ENDEAVORS

Neither of us happen to be super outdoorsy, but our stay provided the perfect blend of indoor luxury and outdoor relaxation. Guests can borrow e-bikes to tool around the property and La Jolla at large, play an (in our case extremely competitive) game of croquet on the lawn, take a dip in the heated pool (with underwa-

ter speakers!), or practice skills on the putting green.

My son had fun educating his mother on the correct form for weighlifting in the Lodge’s state-of-the-art fitness facility featuring TechnoGym equipment. For my part, my daily exercise routine leans more toward brisk walking, so I was thrilled for the opportunity to traverse the meandering paths and rugged terrain of the Torrey Pines State Reserve when signing up for a morning a guided tour. Working toward my 10,000 steps that day was defined by rare Torrey pine trees, towering sandstone cliffs, sweeping ocean views, fragrant coastal sage

scrub and chaparral and abundant birdlife in the salt marshes below.

In addition to the abundant activities and dining on delicious woodfired dishes at The Grill, there was plenty of quality time to learn about things that interest my son, his plans for the future and all the excitement to come down the road. And we couldn’t have asked for a better backdrop of understated elegance and coastal charm.

Found sick and injured during a storm, Little Foot’s first few months of life have definitely been a trial. After recuperating in foster care, he is now back and ready for adoption. Little Foot has a quiet demeanor and can be a tad shy until he gets to know you, but he doesn’t mind being petted and likes the company of other cats. He would do well in a quiet home where he can finally start to settle in.

Sourdough Sam is bundle of silly fun. One year old, he is full of happy energy and just loves to run and play. He likes outdoor activities and would likely make a great jogging or hiking buddy. Sam is looking for an active adopter that can keep up with him and continue with his training.

California Gun Services Safe Gun Storage, Ownership and Disposal

After my grandfather passed away, I moved in with my grandma, doing chores around the house and helping to maintain the garden.

One day, I was looking for my grandfather’s whetstone, to sharpen an old pair of shears, and I stumbled across his Colt Python 357 Magnum. I thought it might be a relic from one of the movies he worked on in the 50’s, but according to my grandma, it was just “one of those things your grandpa just insisted on having.”

Jeff McCabe has gotten more of his fair share of calls from people like myself, who accidentally come across family firearms in their home and don’t have the first idea of what to do with them. The owner of California Gun Services (CGS), Jeff is in the business of safe gun storage, ownership and disposal. The first step? Following California gun laws. I had zero clue that by driving the Colt to the Army Navy to try and turn it in, I’d be breaking a couple laws. According to Jeff there are laws restricting gifting or selling a gun, receiving a gun, borrowing a gun, transporting a gun, even storing guns. New laws come out every year, and it is almost impossible for anyone, even aficionados to keep up with them. Jeff and his team work closely with local law enforcement to ensure the process is easy and safe.

My grandma was afraid the grandkids might find it and would “just feel safer without it.” A sentiment shared by Jeff’s friends, family and fellow church parishionersmany of whom have sought his assistance over the years to ensure the proper storage or disposal of firearms they found themselves in possession of upon the passing of a loved one.

A BUSINESS IS BORN

What initially started as a favor for a friend has grown into a bonafide side business as word has spread about Jeff’s particular skill set and confidence in handling firearms. As the owner of the Orange-based One Way Painting, Jeff often encountered situations whereby he would come across a firearm when moving furniture around for a client prior to starting a painting project.

“Many times, the firearm belonged to my client’s father, grandfather or late husband and she never touched it because she was afraid of guns, but didn’t know how to get rid of it safely,” he says. “So many times these guns are loaded and just stuck in a nightstand or stored under the bed.”

Over the years, Jeff has encountered a variety of different circumstances where individuals seek his assistance in disposing of unwanted firearms. All situations call for professionalism and expertise, but some also require a more delicate approach. One such case was a recent call by a family whose patriarch was once a responsible gun owner, but was now suffering the early signs of dementia. “His family wanted the firearms removed for safety reasons, but the gentleman lawfully owns the guns so it’s important to have a heightened sensitivity so as not to create upset,” says Jeff.

In addition to lay people looking for help, Jeff and his team also assist estate and probate attorneys representing

clients who are surviving family members in possession of unwanted firearms to navigate the process of safe gun disposal.

Raised in a family that had firearms for hunting and sporting, Jeff was taught early on how to handle, be respectful and always be safe when handling firearms. Jeff adds that “ … we as Americans have a great privilege to be able to own a firearm, but with that comes an even greater responsibility to not only protect ourselves legally by complying with Federal and State laws, but to protect ourselves, our neighbors and most importantly, our children from all possible dangers associated with firearms.”

San Clemente resident Rhonda Murphy couldn’t agree more. “There are many things to deal with when a loved one dies. In 2010, when cleaning out my husband’s closet, I came across several of his guns. I didn’t want to keep them and was unsure of what to do with them. My daughters didn’t want the guns and I didn’t want to deal with the hassle of storing them as my grandkids are often at my house. A friend suggested I call Jeff. He was very nice, easy to talk with, trustworthy and knowledgeable. He came to my house, checked all the guns to make sure they were unloaded and removed them. Jeff also told me that one of the guns was a collector’s item and was able to sell it for me. One less thing to have to worry about. I can’t recommend Jeff enough.”

As part of their auditing procedures, California Gun Services gives clients the option of a Voluntary Registration Form (in the case where the weapon was inherited). Many of Jeff’s clients are like my grandma who just prefers that the gun is destroyed. Jeff holds a Federal Firearm License and as such, is able to take possession of firearms without a Gun Transfer. He can also inspect firearms to ensure they are still in working order, and even tell you if the gun, or guns, you have is California Compliant.

“Most people are unaware that firearms that were purchased legally from a gun shop just a year ago may now be illegal in California,” says Jeff. “If we find a firearm that is out of compliance with the law, you’ll receive solid advice on how to make it compliant, sell it out of state, or potentially dispose of it. Our process is approved and certified by local law enforcement, it’s legal, safe and adheres to all state and federal firearms laws.

California Gun Services’ processes are approved and certified by local law enforcement and adheres to all state and federal firearms laws for the following services:

GUN AUDIT

Jeff will come to you and inspect your firearms to make sure they are in proper working order and that they are California Compliant. If your firearm is not compliant with the California Law, you will receive solid advice on how to make it compliant or possibly dispose or sell it out of state.

GUN DISPOSAL

If a firearm is found to be unsafe to use, cannot be repaired and has no historical value. California Gun Services will permanently destroy it by torching the receiver or frame into three parts or cutting it with a saw into five parts. Jeff will document this with photos and log it for the Alcohol, Tobacco and Firearms and email you the proof of destruction.

GUN TRANSFERS

Private Party Transfers happen between two individuals and will need to take place at a Federal Firearm Licensed Dealer. Since this needs to be done at a Gun Shop, Jeff cannot offer this as an in-home service. If you have inherited or legally obtain a firearm, he can provide you with a Voluntary Registration form or you can download it from the California Department of Justice which is NOT required to do at a gun shop. California Gun Services has a Federal Firearm License and can legally come to you and take possession of a firearm without a Gun Transfer.

GUN PURCHASES OF UNWANTED FIREARMS OR COLLECTIONS

California Gun Services will come to you and appraise a single gun or an entire collection of any size. You will receive an explanation of how guns are legally sold and transferred. California Gun Services can then make you a fair market value offer and/or explain how a gun can be put up for sale on consignment.

AMMUNITION AUDIT

California Gun Services will make sure you are using the right ammunition for the firearms that you have and that it is legal in the State of California. Jeff will inspect the ammunition to make sure that it has been stored correctly, is in good condition and will ensure that the ammunition is being stored properly, using the right type of container and stored in the right location in your home.

Q Does white really go with fish/seafood and red with red meat or is there some overlap?

A When in doubt the white with seafood and red with red meat is a good rule. But of course, there’s definitely some overlap here when getting into the details. There are some fantastic lighter bodied red wines out there that can easily be paired with fish and other seafoods. Some of my favorites are Pinot Noir, Beaujolais, and Grenache, each of them have a light body with very light tannins, won’t overpower the delicate fish. And for red meats you can easily pour a full bodied white wine that has high acidity, think California Chardonnays, white Rhone blends, Rieslings and Chenin Blancs. All of these have nice body that can stand up to high fat red meats, and nice acidity that can shine through some of the more robust seasonings and marinades that are used to cook them.

Q Does the shape/size of the wine glass make a difference?

A This is one of those topics that I wish people took more seriously! In short, yes, the shape and size of the glass makes a huge difference! Wine glasses are designed with very specific shapes and sizes to help deliver wine aromas to your nose. If search “red wine glass” and “white wine glass” you’ll notice red wine glasses are much bigger and more bowl shaped, that’s because there are vapors coming off the wine as the alcohol slowly evaporates, those vapors are filled with compounds that help you taste the wine. White wines a typically chilled and won’t give off as much aromas, therefore the glass doesn’t need to be as large. Another reason for the smaller shape for whites is they don’t need need to aerate like reds do, less surface area reduces aeration and keeps the wine cooler. Reds on the other hand love to aerate and

therefore need all the surface area to slowly oxidize (in a good way). The bowl-shaped narrower opening at the top of the glass does two things that are both very important: 1) It helps trap the aromas into a smaller area to help you small them. Think about how much more difficult it would be to smell wine in a cereal bowl with an 8 inch opening than a wine glass with a 2 inch opening. 2) It helps keep the wine in the glass when you swirl the heck out of it, which you should do! I love swirling wine, it introduces oxygen (good oxidation) into the wine and gets all those great aromas swirling around the glass for you to smell!

Q Should I decant wine before I drink it?

A good sommelier is a helper, a guide to finding the best wine for your food and experience. For this month’s installment, we again approached South County-based sommelier Micah Sampson for a bit of education about the wine/food pairings, whether the size and shape of the glass makes a difference and whether wine should be decanted before its served. A Decanting can do wonders for your wine, but it isn’t always necessary. You should have a nice decanter at home if you’re serious about enjoying some great red wines (don’t worry about decanting white wines), and that decanter should have a nice wide bottom that creates a ton of surface area when the wine is poured in, and a narrow top that keeps too much air from getting introduced and makes it easy to pour a glass. The types of red wines you’re decanting are usually full bodied reds that usually have a some age on them (like 5-10+ years), not those cheap red blends you’re buying for $8.99 at the market. The idea behind decanting is you’re introducing oxygen into the wine and a chemical change is happening, a lot of those volatile compounds (bad aromas) are evaporating off and the harsher tannins are softening up a bit, which allows you to enjoy the wine more. So, if you have a 2010 Barolo or a 2015 Napa Cabernet, let those bad boys decant for 30-60 minutes, maybe take some small sips and smells during that time so you can see how the wine changes.

How to Thrive at Home: The Art of Choosing Incontinence Products for Bladder Leaks

Whether aging in place at home, or residing in a senior living community, older adults living with incontinence face unique health challenges.

The risk of pressure sores, UTI’s, and falls are all unfortunately common problems that occur in these individuals. However, seniors can greatly reduce their risk of these challenges, and subsequently, reduce their risk of ending up in the hospital by choosing high quality incontinence products. Below are two features to look for in an incontinence product.

1. Fully Breathable Products.

Fully breathable products that allow the skin to breathe will reduce the risk of pressure sores.

Without breathability, the person wearing the product may get skin irritations and skin breakdowns. Not only can these skin irritations become very painful, but the worry, and ancillary negative effects (such as possible hospital admission, decreased quality of life, time, and money) are staggering.

In addition to wearing a fully breathable product, choosing skin care products and washcloths that protect

elderly thinning skin are equally as important. Transparent barrier creams and “no water needed” cleansing creams, for example, can aid in reducing risk of irritation.

2. Super Absorbent Products.

Getting a good night’s sleep is important. Super absorbent products are key in helping older adults who have bladder leaks sleep through the night. “Restorative sleep”, the term meaning 7-9 hours of uninterrupted sleep that allows a person to go through the 4-5 sleep cycles, has many proven benefits, including cell regeneration, tissue healing, hormone regulation, and being more alert in the morning.

If a person is wearing a product that keeps them dry longer, and is made from soft comfortable materials, the likelihood of them staying in bed at night is greater. If they stay in bed, they reduce their risk of falling during the night, whereas others wearing a less absorbent product may wake up in the night, wet and uncomfortable. In turn, that person gets up out of bed groggy and thus a fall may happen. Falls are a leading contributor to hospital admissions. Choosing high quality products helps reduce the risk!

Super absorbent products may also reduce the risk of urinary tract infections in older adults if they are wearing a high-quality product with a double absorbent core that locks

in urine while decreasing bacterial growth. A double absorbent core helps to wick moisture away from the skin quickly, thus keeping people dry and comfortable, while also reducing urine odor.

If you or a loved one are challenged with incontinence, it is recommended that you inquire about high quality products from a specialized provider, such as Mountain View Services. If you have a H.S.A. when you sign up for Medicare, you can’t contribute to your H.S.A once your Medicare coverage begins. If you contribute to your H.S.A after your Medicare coverage starts, you may have to pay a tax penalty.

The US tax code prohibits having two tax favored accounts that offset the same benefit. You cannot contribute to an H.S.A once you start Medicare, as both include tax favored components for healthcare benefits. Stop contributing to your H.S.A the month you go on Medicare.

Now, here is an interesting piece of information regarding how your H.S.A can reimburse you for your Medicare premiums paid. Yep, even if your Medicare premiums are automatically deducted from your Social Security check, you can take tax-free withdrawals from a health savings account to reimburse yourself for them. After you turn 65, you can use your H.S.A money tax-free to pay for Medicare Part B, Medicare Part D and Medicare Advantage plan premiums. (but not premiums for Medicare supplement policies) in addition to paying for other outof-pocket qualified medical, dental and vision expenses. (now including over the counter medications and sunscreen!)

There’s no time limit for withdrawing money from a health savings account to pay for these expenses. So, if you’ve had money in a H.S.A for several years and didn’t realize you could withdraw the money taxfree for Medicare premiums, you could reimburse yourself for all of those premiums at any time. (Just make sure to keep your receipts for expenses).

If you’re one of those higher income earners who plan on working past 65 but are concerned about having to pay more for Medicare Part B and Part D because of how much you make, opening a Health Savings account now and contributing the maximum amount annually makes a lot of sense. In 2024 an individual with selfonly coverage can contribute up to

$4150 and if they’re 55 or older they can make a catch-up contribution up to $1000 over their annual limit.

If you’re in your 50’s or early 60’s now, this is a great strategy to pay for those Medicare Part B and D premiums that will be higher for you then for those falling under the standard baseline income for Part B; which in 2024 is $103,000 for an individual and $206,000 for joint filers. This higher premium is referred to as “IRMAA” (Income Related Monthly Adjusted Amount) and it impacts those making more annually than the amounts noted in this paragraph. I’m utilizing this option for when I go onto Medicare, so I’m fully funding my H.S.A now for down the road when I turn 65.
