When I was younger, my Mom loved to read me the book “If You Give A Mouse A Cookie.” The plot essentially revolves around an ever-growing list of to-do’s that had to be accomplished, all set into motion by the simple act of giving a mouse a cookie. How Can I Tell If I Have Skin Cancer?

There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.

ook especially for change of any kind. Do not ignore a suspicious spot simply because it does not hurt. Skin cancers may be painless, but dangerous all the same. If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.

WHAT PATIENTS NEED TO KNOW 1. Everyone needs to perform a self-skin exam once a month.

ACTINIC KERATOSIS

An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered a precancer because if left alone, it could develop into a skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed sun, such as the backs of the hands, forearms, face, neck, and scalp.

BASAL CELL CARCINOMA

Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases of diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal, or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.

2. Melanoma can appear anywhere on the skin, including the palms/soles, under your nails, and places...for most, have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check, it could save your life.

3. What to look for: ABCDEs, and an ugly duckling A - Asymmetry: If I draw a line down the middle, do the sides match?

B - Border: Is there a nice crisp border, or does the mole melt into the surrounding skin?

C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?

D - Diameter: Any mole larger than 6mm, ie the head of a pencil eraser, has a higher risk of becoming melanoma.

SQUAMOUS CELL CARCINOMA

Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly and some forms are tender to touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour.

Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.

MELANOMA

Melanoma is a very dangerous form of skin cancer. Melanoma is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year and is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.

E - Evolution: Has it changed?

F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?

4. Everyone needs a full body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

A good sommelier is a helper, a guide to finding the best wine for your food and experience. For this month’s installment, we again approached South County-based sommelier Micah Sampson for a bit of education about the wine/food pairings, whether the size and shape of the glass makes a difference and whether wine should be decanted before its served.

Q Does white really go with

fish/seafood and red with red meat or is there some overlap?

A When in doubt the white with

seafood and red with red meat is a good rule. But of course, there’s definitely some overlap here when getting into the details. There are some fantastic lighter bodied red wines out there that can easily be paired with fish and other seafoods. Some of my favorites are Pinot Noir, Beaujolais, and Grenache, each of them have a light body with very light tannins, won’t overpower the delicate fish.

And for red meats you can easily pour a full bodied white wine that has high acidity, think California Chardonnays, white Rhone blends, Rieslings and Chenin Blancs. All of these have nice body that can stand up to high fat red meats, and nice acidity that can shine through some of the more robust seasonings and marinades that are used to cook them.

Q Does the shape/size of the

wine glass make a difference?

A This is one of those topics that I wish people took more seriously!

In short, yes, the shape and size of the glass makes a huge difference!

Wine glasses are designed with very specific shapes and sizes to help deliver wine aromas to your nose. If search “red wine glass” and “white wine glass” you’ll notice red wine glasses are much bigger and more bowl shaped, that’s because there are vapors coming off the wine as the alcohol slowly evaporates, those vapors are filled with compounds that help you taste the wine. White wines a typically chilled and won’t give off as much aromas, therefore the glass doesn’t need to be as large. Another reason for the smaller shape for whites is they don’t need need to aerate like reds do, less surface area reduces aeration and keeps the wine cooler. Reds on the other hand love to aerate and

therefore need all the surface area to slowly oxidize (in a good way). The bowl-shaped narrower opening at the top of the glass does two things that are both very important: 1) It helps trap the aromas into a smaller area to help you small them. Think about how much more difficult it would be to smell wine in a cereal bowl with an 8 inch opening than a wine glass with a 2 inch opening. 2) It helps keep the wine in the glass when you swirl the heck out of it, which you should do! I love swirling wine, it introduces oxygen (good oxidation) into the wine and gets all those great aromas swirling around the glass for you to smell!

Q Should I decant wine before

I drink it?

A Decanting can do wonders for

your wine, but it isn’t always necessary. You should have a nice decanter at home if you’re serious about enjoying some great red wines (don’t worry about decanting white wines), and that decanter should have a nice wide bottom that creates a ton of surface area when the wine is poured in, and a narrow top that keeps too much air from getting introduced and makes it easy to pour a glass. The types of red wines you’re decanting are usually full bodied reds that usually have a some age on them (like 5-10+ years), not those cheap red blends you’re buying for $8.99 at the market. The idea behind decanting is you’re introducing oxygen into the wine and a chemical change is happening, a lot of those volatile compounds (bad aromas) are evaporating off and the harsher tannins are softening up a bit, which allows you to enjoy the wine more. So, if you have a 2010 Barolo or a 2015 Napa Cabernet, let those bad boys decant for 30-60 minutes, maybe take some small sips and smells during that time so you can see how the wine changes.

uick, quality cuisine. Inventive dishes and experiential dining in an informal, welcoming setting. What’s not to love? Two of my recent favorite finds are a pair of diverse - but equally delicious - fast casual restaurants: The Crack Shack and Pepper Lunch.

The Crack Shack stands at the forefront of the chicken craze with the culmination of a meticulously prepared menu, thoughtfully sourced from local boutique farms and bakeries, pristine local ingredients, laid-back vibe and “unexpected level of hospitality that brings a certain je ne sais ‘squawk’ for sumptuous delight.”

Housed in a cool 17th Street locale in Costa Mesa, The Crack Shack’s bright, open space is outfitted with casual banquette seating and communal tables. A vibrant open area visible from every table overlooks a cornhole game which on the evening I visited kept a group of youngsters entertained while their parents noshed on dishes that start with locally raised, free-living chickens without antibiotics.

A full bar adds to the lively ambiance, as does the quirky messaging on the walls and humorous chicken imagery throughout.

The menu boasts a variety of yummy sandwiches, salads, bowls and sweets, but

My son and I lean into seafood specialties so we had our work cut out for us in deciding between the Crispy Skin Salmon with buttered jasmine rice, Vera Cruz Pacific Sea Bass (enhanced with cilantro rice, capers, tomatoes, Spanish olives and citrus butter) and chamomile-dusted seared Diver Scallops. More of a land lover? Rumorosa serves up a tender braised beef short rib and a grilled-

to-your-liking 14-oz boneless ribeye with loaded fingerling potatoes.

ou need to enroll in a Medicare drug plan when you first become eligible for Medicare.

Even if you don’t take drugs now, you should consider joining a Medicare drug plan or a Medicare Advantage Plan with drug coverage to avoid a penalty. You may be able to find a plan that meets your needs with little to no monthly premiums. Don’t make the mistake of thinking you can avoid paying the monthly premium for drug coverage by using one of the alternative prescription drug purchasing options that you can download the app to your phone and show the pharmacist the coupon code when you pick up your prescription, or purchase your prescriptions on one of those online formats. Don’t get me wrong. Certainly, use those if they get you your medications for less out of pocket than your Medicare drug plan will. However, use it in conjunction with your Medicare drug plan.

If you go 63 days or more in a row without Medicare drug coverage or other creditable prescription drug coverage, you will have to pay a penalty if you sign up for Medicare drug coverage later. The exception is someone that’s on Medi-Cal. (Low income) The cost of the late enrollment penalty depends on how long you didn’t have creditable prescription drug coverage. Currently, the late enrollment penalty is calculated by multiplying 1% of the “national base premium” ($36.78 in 2025) by the number of full, uncovered months that you were eligible but didn’t enroll in Medicare drug coverage and went without other creditable prescription drug coverage. The “national base premium” increases each year, meaning the penalty amount also increases each year. After you enroll in Medicare drug coverage, the plan will tell you if you owe a penalty.

If you do owe a penalty, that amount will be added to your monthly plan premium.

ur readers may already know this via the family-friendly travel destinations we’ve featured over the years, but when our son was six (and our daughter 5-yearsold), we embarked on an albeit ambitious plan to visit all 50 states as a family by the time he graduated from high school. Now a senior with only

Amway Grand a few short months to go, we finally crossed the last locale off the list with a long weekend in Grand Rapids, Michigan.

I had worried that that a March visit in the region’s waning winter might not be the ideal time to visit, but with our (self-imposed) deadline looming, we decided to seek out activities and attractions amidst potential snow showers. Turns out, the weather only added to the experience of our quest to “try something new for the first time.”

and charm of the Lumber Baron Bar (think premium drinks and appetizers, a crackling fireplace and leather club chairs).

We loved whiling away an hour or two one evening at Woodrow’s Duckpin Bowling where we tried our hand for the first time at the “sport.”

While there are many similarities to traditional 10-pin bowling, duckpin is played with smaller balls that lack finger holes and the pins are shorter, squattier and lighter, which makes it challenging to achieve a strike. If your teen is anything like our particularly competitive son, it’ll make for an even more entertaining experience.

Situated on the banks of the Grand River, this Western Michigan city might be lesser traveled that some of more well-known metropolises across Lake Michigan (Chicago and Milwaukee). But as the state’s second-largest city, Grand Rapids proved to be the ideal crossroads of cultural attraction, family-friendly entertainment, destination dining and accommodations of distinction.

With regard to the latter, the iconic Amway Grand Plaza proved to the perfect “home base” – blending history, modernity, sweeping river views, entertainment and opulent accommodation. The riverfront property boasts what is arguably one of the most ornate hotel lobbies I’ve ever set foot in showcases a soaring gilt ceiling – the largest goldleaf installation in the nation – and a trio of massive, showstopping Czechoslovakian crystal chandeliers. Between the historic Pantlind Wing and the Glass Tower, this premier AAA Four-Diamond hotel offers 656 guest rooms ranging from historic to contemporary, modest to magnificent. We booked a pair of guestrooms, both spacious spaces defined by plush bedding, high-tech lighting, and much to the delight of our teens - universal smartphone charging stations, 50-inch flat screens and free high-speed WiFi. Its location Grand can’tAmway be beat – literally at the heart of downtown within easy walking dis-

tance (and Skywalk access) to downtown shopping, dining and many of the attractions we wanted to visit.

Home to nine onsite restaurants and bars (more on this in a minute), an indoor pool, onsite spa, state-of-the-art fitness center, four rooftop pickleball courts and duckpin bowling, Having said that, you could almost spend the entire weekend in at the Amway Grand Plaza. The Kitchen Counter by Wolfgang Puck serves freshly baked and made-to-order breakfast made mornings easy, the onsite Starbucks was a favorite for my daughter just like it is at home and my husband and I enjoyed settling into the warmth

AREA ATTRACTION Named after the Grand River that wends its way through the vibrant downtown, Grand Rapids’ original occupants were the Hopewell Indians. More than a century later, it had evolved into “America’s Furniture Capital” due to a lumber boom that lured craftsmen, carpenters and woodworkers. While the city is a modern metropolis in its own right, recognized as No. 8 on the list of America’s Best Towns to Visit (CNN 2024), Grand Rapids embraces its past, which made for a fun and educational itinerary that included stops at a pair of popular museums. paired with delicious house made focaccia. Popular entrees range from a traditional Rigatoni Bolognese and Black Spaghetti (crab, spinach, Calabrian chili) to a Bistro Burger grilled to your liking and topped with caramelized onions and a tender filet mignon accompanied by brown butter mashed potatoes.

of the nation’s oldest – offers an array of permanent and traveling exhibits inspired by science (The Nature of Color exhibition allows visitors to experience the vibrant world of color from their mind-bending physics to the feelings they evoke), history and culture. The Gerald R. Ford Presidential Museum showcases tons of memorabilia from the native son’s presidency and boasts a replica of the Oval Office.

Nature and art lover? Immerse yourself in one of the world’s finest horticultural display gardens and sculpture parks. Named the “Best Sculpture Park in the United States by USA Today in 2023 and 2024, the renowned indoor/outdoor Frederik Meijer Gardens and Sculpture Park showcases over 200 captivating sculptures that are a blend of art and nature. Significant works of art (including more than 50 major works by masters Auguste Rodin, Henry Moore and Barbara Hepworth) are set within a variety of natural settings connected by waterways, meandering paths, lawns, meadows and quiet walkways.

The Downtown Market was a great place to grab lunch and people watch.

Essentially a super fun food hall mixed with a farmer’s market, the market features a collective of 19 artisan food merchants (think butchers, bakers and ice cream makers) and restaurants 56

open seven days a week. Stalls are filled with specialty grocery items like fresh baked bread, wine, seafood and spices.

On the topic of dining out, I highly recommend Bistro Bella Vita. A big hit with our family, this downtown eatery offers selection of French and Italian cuisine is crafted from scratch using fresh, sustainably sourced ingredients.

For starters, the kids devoured the Rustica pizza piled high with Italian sausage and pepperoni, while I savored a warm bowl of butternut squash bisque

Keeping the theme of competitive “sport” going, it was adults vs. kids in collective scores playing “challenging and fun for all ages” indoor 9-hole mini golf courses at Big Mini Putt Club. Also in the heart of downtown, the venue is primarily walk-ins only (happy hour pricing Mon-Fri is $7 per person). In addition to mini golf, there are a variety of free games such as Nintendo 64, darts and giant Jenga and our family’s favorite coin-operated amusement games – Air Hockey and Skee-Ball (where mom likes to believe she’s the reigning champion).

As I hope to carve out time for future family bonding adventures beyond our borders, we will remember Grand Rapids as an ideal destination of abundant attraction and beautiful accommodations.

Obtaining and Maintaining Medicare Creditable Drug Coverage

You need to enroll in a Medicare drug plan when you first become eligible for Medicare.

E

ven if you don’t take drugs now, you should consider joining a Medicare drug plan or a Medicare Advantage Plan with drug coverage to avoid a penalty. You may be able to find a plan that meets your needs with little to no monthly premium. Don’t make the mistake of thinking you can avoid paying the monthly premium for drug coverage by using one of the alternative prescription drug purchasing options that you can download the app to your phone and show the pharmacist the coupon code when you pick up your prescription, or purchase your prescriptions on one of those online formats. Don’t get me wrong. Certainly, use those if they get you your medications for less out of pocket than your Medicare drug plan will.

However, use it in conjunction with your Medicare drug plan.

If you go 63 days or more in a row without Medicare drug coverage or other creditable prescription drug coverage (such as from an employer plan), you most likely will have to pay a penalty if you sign up for Medicare drug coverage later.

The cost of the late enrollment penalty depends on how long you didn’t have creditable prescription drug coverage. Currently, the late enrollment penalty is calculated by multiplying 1% of the “national base premium” ($36.78 in 2025) by the number of full, uncovered months that you were eligible but didn’t enroll in Medicare drug coverage and went without other creditable prescription drug coverage. The “national base premium” typically increases each year, meaning the penalty amount will also increase each year. After you enroll in Medicare drug coverage, the 62

plan will tell you if you owe a penalty.

If you do owe a penalty, that amount will be added to your monthly plan premium. The penalty is lifelong.

Medicare drug plans (Medicare drug plans and Medicare Advantage Plans with prescription drug coverage) have their own list of what drugs are covered, called a formulary. Plans include both brand-name prescription drugs and generic drug coverage. The formulary includes at least 2 drugs in the most commonly prescribed categories. This helps make sure that people with different medical conditions can get the prescription drugs they need.

Medicare drug coverage typically places drugs into different levels,

called “tiers,” on their formularies.

Drugs in each tier have a different cost. For example, a drug in a lower tier will generally cost you less than a drug in a higher tier. Medicare drug coverage has 5 tiers: Tier 1- preferred generic; Tier 2- standard generic; Tier 3- preferred brand name; Tier 4- nonpreferred brand name; Tier 5-Specialty Tier. The best way to find out what plan is best for you is to work with a broker who helps you create a Medicare.gov account where you can input your medications, pharmacy preference and then search for the lowest priced plan given your current medications. This is something you can do for yourself, but I find that my clients prefer me to help them get started with the process.
