Thank you for the warm welcome to this amazing community. Now seven months on the job, I am getting to know a few more people; I’m finding my way around this beautiful landscape more easily; and I have gained a great appreciation for the remarkable people here at Skagit Regional Health.

As a newcomer, I have had a chance to observe and listen to our dedicated staff, providers and volunteers who care so very much about providing excellence in patient care. They have a passion for what they do and they bring it to work every day. Every smile.

Every hot meal. Every comforting word. Every expert procedure.

Every interaction. Every clean patient room. These all add up to an outstanding patient experience.

While paying attention to the details, we have also faced our challenges.

It takes time for new leadership to settle in.

We experienced a very busy flu season at both our hospitals. Our 19 clinics have been busy as well and we are working hard to recruit additional providers to improve patient access.

We are facing financial shortfalls that resulted in a staffing reduction. We are still getting accustomed to the new Electronic Health Record.

We are moving forward and have reached a point where we are ready to advance several strategic initiatives to meet the needs of our community. This is the exciting part.

One of the most amazing steps is the construction of a new imaging center – a project that is a partnership between Skagit Regional Health and Skagit Radiology. This new center near Skagit Valley Hospital will bring the latest in mammography technology – Digital Breast Tomosynthesis – to Mount Vernon. This is a true advancement of health care services in our community. The Skagit Valley Hospital Foundation will play a role in this project as well, having made a commitment to raise $2 million to help build and equip the new center.

As we celebrate the addition of the latest in technology, we also have brought back the time-honored healthcare specialty of midwifery as a service for women at Cascade Valley Hospital in Arlington. We are pleased to offer this expansion of care options for women.

We have also made a commitment to the Skagit Valley Family YMCA to offer outpatient sports rehabilitation services at the new YMCA facility in Mount Vernon when it opens next year. Another example of a partnership to expand access to healthcare services in our community.

Again, my thanks to the community for the warm welcome.

I am here to assure you that we – the staff, providers and volunteers of our healthcare system – have a dedicated focus on our patients and an eye on the future to provide the quality services our communities need.

Midwifery at a glance: Midwives care for low-and moderate-risk pregnancies but can potentially co-manage higher risk patients with a doctor.

Low-risk patients may have different options during labor: • Intermittent listening to the baby without being secured to a monitor • No IV • Able to walk and move around • Choice of position for delivery • Pain medication or epidural

Midwifery is a time-honored specialty offering care to women that is growing in popularity across the country and is now available in Arlington.

Two Certified Nurse Midwives (CNM) are on staff at the Skagit Regional Health – Arlington Women’s Health clinic near Cascade Valley Hospital in Arlington and a third CNM will join the practice in May. The growing midwifery practice offers an option for women for healthcare at all phases of life, while most known for specializing in prenatal care along with labor and delivery.

“We provide well-woman care,” said Cindy Beske, CNM, who joined the practice in December. “We take a holistic approach with a focus on listening, educating, knowing the whole family and knowing how they want the pregnancy, labor and delivery to go. We use the same approach for non-pregnant gynecology care.”

Beske has been a midwife for 23 years, choosing to seek additional training after spending time as a registered nurse in labor and delivery.

“I knew a piece of the process and I wanted the whole picture,” Beske said.

Karen Landon, CNM, joined the practice in September. Landon became a midwife three years ago after working as an intensive care nurse and after her own experience in having a midwife attend the births of her three children.

“I feel like I am drawn to it for the whole care aspect. I can take care of the woman and her family. I can care for women from puberty to geriatric age. And, I get to catch babies,” Landon said.

Midwives in Washington are independent health care providers

who are licensed by the state and can offer a full range of healthcare services. Beske and Landon are located in the same office with OB-GYN David Gran, MD, should any patient needs arise that warrant input from a physician.

Beske and Landon say they enjoy the opportunity to work with women to explore their options and set their goals for health, pregnancy and childbirth.

“We are really problem solvers,” Landon said. “We work with our patients to build the type of health and well-being that they are looking for and are open to alternative therapies and options.”

“This is very personal care. We offer a very high level of services and a more holistic approach,” Beske said. “During labor, our patients have someone with them through their entire labor.”

A high level of personalization was a reason Jessica Forman of Marysville chose to see the Skagit Regional Health midwives and deliver her second child at Cascade Valley Hospital in February.

Forman noted that the midwives treated her as a mother, were attentive and helpful in meeting her personal

goals in concert with her husband, Zachary. They took cues from her, were only as hands on as she desired during her labor and took control as the birth was imminent.

“They helped me and let me be myself,” Forman said, who also praised the entire staff at the Cascade Valley Hospital Family Birthing Center. “Cindy and Karen were there every step of the way, helping me through contractions and encouraging me between them. I felt like I had a good team around me. It kept me from dwelling on my fears of delivery. I was confident they would help me get through this. Once it was time to push, and it was one of the most painful things I ever had to do in my life, Karen and Cindy made me feel like I could be myself in my most vulnerable moments. There were many times I could have allowed the intensity of the moment to sweep me into hyperventilation, sadness, or panic; but these midwives reminded me that I can still control myself and make this a successful natural birth.

And sure enough, it was.”

The midwives are also there to support mom and family after the birth and routinely schedule a twoday post-partum visit for the whole family.

“There is a real need in this community for this service and that’s what we’re here to provide,” Beske said. “We want women to know their bodies well enough to be comfortable and natural in their care.”

Third midwife joins Arlington practice Debbie Miller, CNM joins the midwifery program based at Cascade Valley Hospital in Arlington in May. Miller received her Registered Nursing degree from Montana State University Northern in 2007. Upon graduation, she worked as a labor and delivery nurse for six years followed by four years working side-by-side with Certified Nurse Midwives as their practice RN. During her stint in the midwifery clinic, Miller became a certified lactation counselor and assisted many mothers with successfully breastfeeding their babies.

In 2015, she decided to go back to school at Frontier Nursing University in Hyden, Kentucky to pursue a Masters of Science in Nursing with a specialty in Midwifery and Women’s Health, graduating in 2017.

Born and raised in the Flathead Valley nestled in the Rocky Mountains in Montana, she was drawn to Arlington as a place to practice.

“Wanting to stay close to family, I kept my midwifery position search narrow and found Arlington to be a great match for us,” she said. “I was

immediately drawn in by the small town feel and sense of community.

I am joining an excellent team of providers at Skagit Regional Health – Arlington Women’s Health.”

Miller said she is drawn to midwifery as a specialty to care for women in all stages of life.

“My mission will be to provide holistic care to women across the lifespan while focusing on patient-centered care and education,” she said. “I will serve my patients and their families with dignity, respect and without judgment.”

As a midwife, Miller’s practice features obstetrics care, including prenatal care, labor management, childbirth, postpartum care and lactation support and women’s healthcare including contraception education and options counseling and management, annual exams, screening and diagnostic labs, sexually transmitted infection screening and treatment, perimenopause and menopausal care.

Miller and her husband Stephan, have two grown children, a granddaughter and a grandson coming soon.

Shalyce’s Story

Generations of Care Growing up fifth generation in Skagit County gave Shalyce Pocock confidence in choosing Skagit Regional Health’s Family Birth Center and staying close to home. When it came time to have her own baby, the choice was simple. Like her and her mother and father before her, the birth of her son marked a third generation to be born at Skagit Valley Hospital.

Pocock, a resident of Bow, describes her experience as “flawless,” demonstrating a level of care and comfort the women of her family have experienced for generations.

Skagit Regional Health is a family tradition.

She said she had confidence throughout her pregnancy, labor and delivery because the staff felt like

family and the hospital felt like being at home. The nurses and staff put her needs and wants first, and created a safe and comforting space for Pocock and her family, allowing her to stick to her birth plan and welcome the next generation.

Arriving two weeks earlier than expected, and with her primary doctor out of town, Pocock attests the birth of her son was still “flawless.”

Pocock’s water broke at 2 a.m., but by the time she and her husband, Ian, reached Skagit Valley Hospital at dawn, her labor had stalled. Being induced was the next protocol, yet the progression was slow. At no time did Pocock experience any stress related to her circumstance.

Although there were nerves and uncertainty at the start, “they [the hospital staff] gave a sense that you were the only patient in the hospital that day. And I know well that there were lots of other babies probably being born at the exact same time.

And yet every time they came in, whether I had five questions or 20 questions, they took their time to answer. …They just did what I needed, right when I needed it. And so, I would say it was a very calm experience,” Pocock said. After a full day of labor, she was able to have the natural childbirth she’d intended.

Being the first grandchild born into her maternal line, there was a big cheering section stationed at the hospital’s waiting room from both sides of the extended family.

This didn’t distract the nurses in maintaining the wishes of

the expectant parents. “We also had indicated to the nurses that we didn’t want them to go out and tell them [the family] that he was here until we gave the AOK. I wanted to be sure that, as a family, we could bond, the three of us, with the baby, before our family exchanged and took their turn with him,” Pocock stated.

After the visitors left, the couple were left alone to bond with their son, however, newborn bliss was interrupted by a postpartum hurdle.

After two days of coaxing, Pocock’s son struggled to latch and breastfeed.

Determined to claim the time she needed with the hospital’s highly skilled lactation consultant, she voiced her concerns and requested to stay at the hospital an extra day.

The hospital staff, putting Pocock’s needs first, honored her request and allowed her to stay as a patient without giving up. “And when we got home he was great and he breastfed for almost a year. So, I’m really glad I stood up and asked for that time because I think that they were happy to keep me there,” she said.

The early support Pocock received as she struggled to find her rhythm with her firstborn would help develop the maternal confidence she’d maintain throughout her second pregnancy and delivery. “I feel like I was empowered when I left the hospital and I was confident that I could make decisions for my baby,” she recalled. By the time she gave birth to her second son, she’d stay in the hospital less than a day before returning home.

Seattle Children’s providers on duty at Skagit Valley Hospital Pediatricians from Seattle Children’s are on the job 24/7 at Skagit Valley Hospital to care for the youngest of patients.

This team of seven physicians – known as pediatric hospitalists – bring a high level of training in caring for children, plus a close connection to the resources and specialists of Seattle Children’s, ranked by U.S. News & World Report among the nation’s best children’s hospitals. This enhanced connection was established in 2011 when Skagit Valley Hospital partnered with Seattle Children’s to provide Pediatric Hospitalist services locally.

This regional team is in close communication with colleagues at Seattle Children’s and they participate in training and quality improvement projects, all linked to the latest in evidence-based care for children. The doctors are then able to deploy the same standard of care for patients in the community.

“We are an extension of the main campus and we are relied on to keep children safely in our community,

rather than automatically sending children to Seattle,” said Pediatric Hospitalist Barbara Geraghty, MD. “We are practicing the same standard of care as patients would see at Seattle Children’s, but we’re right here.”

“We get patients where they need to be for the right level of care,” said Pediatric Hospitalist Jenny Taylor, MD. “If I keep a patient here, I know that this hospital can provide exactly the same level of care here in our community.”

The connection to Seattle Children’s is reassuring to parents, families and patients.

it,” Dr. Taylor said. “It’s a powerful symbol of quality and compassion in this community.”

The pediatric hospitalists at Skagit Valley Hospital can be found in several areas of the hospital. They attend about one third of all births in the Family Birth Center, if there is fetal distress or other high-risk issue. Newborn weight and jaundice checks occur on weekends and holidays when primary clinics are closed. Sick or premature newborns are cared for in the Special Care Nursery, with back-up telemedicine support when needed from Seattle Children’s Neonatologists. They also see children in the Emergency Department, care for children admitted to the hospital, supervise outpatient medication infusions and provide pediatric consults.

“We’re there for back up in the birth center,” Dr. Taylor said. “We are truly just around the corner if they need us.”

The doctors enjoy the opportunity to work in the hospital and meet the needs of the youngest patients and their families.

“I like the freedom to give every patient the time and attention they need,” Dr. Taylor said. “It is an incredible privilege to partner with families at such an important time.

Even with a normal newborn, if the parents need me to spend an hour, I can spend an hour.”

Serving hospitalized patients is very rewarding, Dr. Geraghty said.

“I very much enjoy helping patients with their acute care needs and supporting families through that time,” Dr. Geraghty said. “Through our partnership with Seattle Children’s, we have the excellent support needed to provide quality care close to home.”

SUMMER safety tips Pediatricians offer tips for a safe and healthy summer Help your family stay safe and healthy this summer by following these 10 tips provided by the pediatricians of Skagit Regional Health.

SUN SAFETY:

Spring and summer are great times to take advantage of nice weather to get outside. However, it is important to follow these sun-safety tips: 1. Apply sunscreen on children 6-months-old and up when going outside. Dress younger babies in long-sleeved clothing and a large brimmed hat.

2. Wearing a hat and light-colored clothing with long sleeves and long pants is a great option for older kids to shield their skin from the sun.

Long sleeves also serve as a layer of protection against insect bites.

3. Apply SPF 30 (or higher) sunscreen on your kids (and yourself) daily.

Reapply every two hours.

4. Don’t assume that your sunscreen will provide 100 percent protection.

Stay out of the sun during the sunniest parts of the day: 10 a.m. – 4 p.m.

WATER SAFETY:

Kids spend more time in and around water in the warm weather months.

Remember these water-safety tips when at the lake, ocean or pool.

5. Learning how to swim is an important skill. Sign your children up for swimming lessons. Make sure that an adult who can swim well is always watching children in the water.

6. Use life vests on kids in open water.

Remember that flotation devices (arms floaties, rafts, etc.) are not meant to keep children from drowning.

MORE SUMMERTIME TIPS:

7. Remember that window screens are meant to keep bugs out, not children in. Children can easily fall out an open window with an intact screen.

8. Children should use a bike helmet every time they ride.

9. When the weather gets warm, it is especially important to drink water regularly to prevent dehydration.

10. This time of year, there is an abundance of fresh fruit and vegetables. Challenge your family to eat at least one meal a day with real ingredients that has never had a label on it.

The latest in pacemakers: Electrophysiologist offers new service

JANUARY COVER STORY:

Update on Wayne Wayne Ramsey, a Sedro-Woolley High School teacher, was featured in January’s edition of HealthQuest regarding his dedication to health, wellness, exercise and good eating habits to prevent the onset of diabetes. Here is an update on Wayne since the magazine was published: When the winter issue came out, I was surprised at how many people noticed me on the cover, and, although I didn’t feel I deserved it, how many people told me I inspired them. I was shocked at all the people who shared their stories of diabetes and prediabetes with me. Their struggles and challenges left me feeling compassion for all they were going through and the thought that I could have it a lot worse. The article opened up a forum of discussion on health issues relating to nutrition and exercise. My students also got involved. Many people wanted to get the names of my Registered Dieticians (RD) so they could start too. I told almost everyone that they should get an RD in their corner. Agriculture teachers and friends posted the magazine all over the nation. Even recently at the store, a fellow teacher wanted to know what I was buying since I am “all healthy now” (my basket had whole wheat/corn tortillas, Zevia, pure cranberry juice and spinach).

I have gotten a lot of support from my family, especially as I work at coming up with creative ways to cook. My wife, Mary, and I make many dishes together and she helps me when I am trying new things.

I send photos of a new dish once a week to my RDs to share what I am up to. I do weights and circuit

training three times a week, do aerobics like swimming or running hills, and started a class of pilates and yoga blended together.

I have finally learned that diabetes and prediabetes are just words. They don’t define who we are, but instead, only describe the functioning of our pancreas. People are defined by their ability to reach deep down inside themselves and push themselves harder than they thought they could.

We all have problems and need one another to lean on now and then.

Technology in pacemakers is advancing rapidly. Today, tiny, batterypowered pacemakers are available for some patients that are rate responsive to the needs of the heart and carry a 10 to 12-year battery life.

These newer models of pacemakers are appropriate for patients with chronic Atrial Fibrillation (AFib) or a low heart rate and are inserted using a catheter rather than requiring an incision to implant.

Skagit Regional Health Electrophysiologist Ramy Hanna, MD, received specialized training in Minneapolis last year to learn the technique for inserting the Medtronic Micra™ pacemakers and started offering the option to appropriate patients in January. Dr. Hanna said he is impressed by the technology.

“I think it’s pretty cool,” Dr. Hanna said. “It’s rate responsive, it senses a patient’s movement and will pick up the heart rate as needed and then revert to base rate and will last 10 to 12 years.”

Patients have been very pleased as well, Dr. Hanna said.

“Patients are impressed by the fact that they have a pacemaker, they have strong heartbeats and they have no incision and no leads or wires,” Dr. Hanna said.

The single-chamber pacemaker is inserted via a catheter in a vein in the upper thigh and the procedure is performed in the cardiac catheterization lab at Skagit Valley Hospital. Patients stay overnight in the hospital for monitoring and typically have fewer medical complications, he said.

Leadless pacemakers will be the trend for the future, Dr. Hanna said, noting work is underway to create technology to pace both chambers of the heart.

Skagit Radiology and Skagit Regional Health to partner on new women’s diagnostic imaging center A new diagnostic imaging center will open next year in Mount Vernon to bring the latest in breast mammography to the community.

Skagit Radiology and Skagit Regional Health are partnering on the construction of the Skagit Imaging Pavilion at the corner of 13th and Division streets in Mount Vernon. The Skagit Valley Hospital Foundation has committed to raise $2 million to help build and equip the Women’s Imaging Center, within the facility. (See page 33 for campaign information.)

The new Skagit Imaging Pavilion will feature state-of-the-art imaging equipment, including Digital Breast Tomosynthesis (3D mammography) in the Women’s Imaging Center along with ultrasound, CT, MRI and stereotactic breast biopsy, bringing new and existing services into a convenient, central location with a full continuum of care.

Digital Breast Tomosynthesis (DBT), according to the American College of Radiology, is an advancement over previous technology, improves accuracy, provides higher cancer detection rates and fewer patient recalls for ABOVE: Concept rendering for the new Skagit Imaging Pavilion in Mount Vernon.

additional testing. 3D mammography is becoming the standard of care.

As with previous technology, tomosynthesis uses low-dose X-rays to produce images of breast tissue to detect abnormalities.

“I don’t think there is any recent development in diagnostic radiology that is as revolutionary as tomosynthesis,” said Radiologist Leah Kiviat, MD, Medical Director of the Breast Care Center. “We have improved accuracy and fewer false positive results. The 3D images are an amazing improvement over 2D mammograms — it’s a very exciting time in women’s health.”

Detection and diagnosis are the first steps. Treatment decisions are made in collaboration with a full team of expert staff and providers, including The Breast Institute at Skagit Regional Health’s Cancer Care Center. Patient navigators work with newly diagnosed breast cancer patients to provide individualized plans including assistance in setting up appointments, answering questions and supporting each patient’s journey.

“We are pleased for this exciting opportunity to provide high-quality, accessible medical imaging, as part of our long-standing partnership with Skagit Regional Health,” said Kelley Cline, MD, President of Skagit Radiology. “The center will not only advance outpatient imaging services, but will also be a centralized, comprehensive women’s imaging center.”

Along with mammography, the new facility will be home to a variety of diagnostic imaging modalities in a central location.

“We are excited that this project will bring state-of-the-art breast imaging technology to the women in our community,” said Barbara Jensen, RN, BSN, MBA, Director of Oncology, Diagnostic Imaging and Palliative Care at Skagit Regional Health. “As an added bonus, this new space will serve to enhance our imaging program for all patients in our area, improving the accessibility of our CT, MRI and Ultrasound departments.”

Construction will commence this summer with completion scheduled for Summer 2019.

When the Skagit Valley community is in trouble, Steven Wade is there to help. Wade has served as a Skagit County Sheriff’s Deputy in Mount Vernon for the past 12 years. In 2011, after developing a malignant melanoma facial tumor, he became the one who needed the support of his community. Even heroes need time to heal.

Wade’s original diagnosis was on November 29, 2011. His tumor was located on his lower lip and chin, and was detected after his local doctor carried out a biopsy and immediately referred him for surgery at the Cancer Care Institute at Virginia Mason, in Seattle. When cancer margins were still not clear after surgery, Wade would undergo both concentrated radiation and chemotherapy treatment. After a year spent in Seattle for daily and then weekly treatment, his surgeon recommended Skagit Regional Health for follow-up care.

As of March 28, 2017, Wade was pronounced cancer free. However, the road to recovery was lined with several hurdles.

Now 58 years old, Wade is in better shape than most people half his age and his yearning and dedication to his health has only

grown stronger along the way.

Before becoming a sheriff’s deputy, Wade spent 26 years in the timber industry. Before his cancer diagnosis, Wade’s part-time job was training for triathlons and competing on the triathlon circuit.

“Anywhere from 10-20 hours a week, I’d spend training, and then racing all around the world,” Wade said.

After cancer, he’s returned to his rigorous athletic schedule, albeit a little slower. “I’m also six years older, and many of my friends tell me I would’ve gotten slower anyway, even without cancer,” he said.

Wade was determined to maintain much of his physical fitness even while undergoing 15 months of treatment. “My doctors have said that my success may have been related to being in good physical condition prior to getting cancer,” he said.

Wade trains one to three hours a day in the activities that may have been responsible for the long-term strength he’d need to survive. routine with hiking and lifting weights. “That doesn’t mean that nowadays I don’t sneak a nap into the middle of my day,” he remarked.

During his cancer treatments, Wade only missed five months of active duty, and was able to tolerate his medications well, especially at the beginning. “With most cancer treatments, the medication creeps up on you and it gets harder and harder as you go,” he recalled.

Feelings of disbelief after diagnosis morphed into receiving loving support. “My wife was a stalwart force in maintaining the attitude that we were going to go about our business, just as we did before, and we were going to get through this, because it was just a bump in the road,” he said.

To make it across the finish line, Wade would need a team of external support. Being housebound for his last five months of treatment presented Wade with both physical and mental challenges. With a diminished immune system, his only option was to limit his exposure to illness by staying indoors. A friend loaned him a treadmill and walking became the extent of his activity. CONTINUED FROM PAGE 11 to heal. “The beginning was tough,” he said. “But, I wouldn’t say it was tougher than the end.”

The immunotherapy drug available at that time, a drug that would lend an extra chance of survival to Wade’s condition, had one uncharted side effect. One day while biking down a county road at 15-20 mph, he had an episode of syncope – fainting – and he lost consciousness.

As a result, he suffered a concussion that lasted several weeks, a dislocated shoulder and clavicle, not to mention extreme road rash. Healing from his wounds while still tolerating injections three times a week injections that produced flu-like symptoms - was a lot to ask anybody to cope with.

“I had to pull back. All my energy was used to fight the cancer and I did very few workouts those last five months,” Wade recalled.

Recovering from his injuries, while

maintaining some semblance of activity, was instrumental in Wade’s psychological as well as physical health. “One of the things that makes people less susceptible to depression is physical exercise,” he said. “I thought it was very important, as did Dr. (George) Gjerset that I try to maintain some physical fitness during treatment in order to push back the side effect of depression, or at least diminish it, as much as possible.”

Oncologist George Gjerset, MD at the Cancer Care Center in Mount Vernon always buoyed Wade’s optimism. After six years of working together, a mutual respect was forged.

Dr. Gjerset was able to figure out what made Wade tick and became “a coach” who presented treatments in a way Wade’s competitive side could overcome and win. Conversely, Dr. Gjerset was also being mentored. “He sees me as a, (even though he’s my senior), as a role model,” Wade said.

Wade said Dr. Gjerset was inspired to implement some of the athlete’s dietary and exercise habits, while

Wade appreciated his provider’s way of communicating. “To be able to speak to me, in a language I identify with, it’s been really good for me,” Wade said. In turn, Wade said Dr. Gjerset was impressed with Wade’s rare ability to stave off other complications and ailments without the use of additional drugs.

For Wade, understanding his condition through the years was easier with Dr. Gjerset’s help. “He’s a numbers guy, like I am,” said Wade.

“When I go to the doctor, I always leave with a piece of paper. It’s usually my blood work and I know what the numbers on that page mean. And that’s the kind of guy he is.”

Dr. Gjerset and Wade bonded over Wade’s trajectory of graphs and charts, and athletic goals that would lead them both to a greater sense of health, all the while positively influencing the course of one another’s lives. He joined the medical staff of Skagit Valley Hospital in 1996, as a partner with North Puget Oncology and served the patients in this community for more than 20 years. His physician colleagues wrote “Dr. Gjerset will be missed and fondly remembered by many.

Physical Therapy Outpatient program opens at Cascade Valley Hospital

Physical Therapy at Cascade Valley Hospital in Arlington has expanded to include outpatient services.

Providing patient-centered therapy, Skagit Regional Health Physical Therapists care for patients from pediatrics to geriatrics with a variety of conditions limiting mobility and function outside of the inpatient, hospital setting.

This growing specialty is located in spacious quarters on the second floor of Cascade Valley Hospital. The department has training equipment and treatment stations that aid in the rehabilitation of various conditions including orthopedic injuries, stroke, arthritis, neuromuscular disorders, work injuries and sports and recreation injuries.

With a hands-on approach, physical therapists diagnose and treat existing health conditions, provide patient education, customize plans of care and offer preventative treatments that can help patients avoid many health problems before they occur.

Cascade Valley Hospital wound care marks

20 YEARS

Cascade Valley Hospital Wound Care and Hyperbaric Medicine Center is celebrating 20 years of service in Arlington this year.

From humble beginnings, this award-winning center has grown from a back, windowless room with a draw-curtain separating patients in the Surgery Center, to its own office with five exam rooms and two hyperbaric chambers.

Joining the center a month after opening in 1998, Clinical Nurse Manager Teri Burt, RN, WCS said “It’s a career that keeps changing and evolving. That’s what has kept my interest over the years. There is always something new and different in the world of wound care.”

Contracted with Healogics, the largest provider of advanced wound care in the nation, Cascade Valley Hospital Wound Care and Hyperbaric Medicine Center relies on a sciencedriven, evidence-based approach to wound healing.

Center staff treat patients from head to toe. From Podiatrists specializing in foot and ankle wounds, to Family Medicine practitioners who examine the full body, plus a general/vascular surgeon who provides vascular and surgical intervention when necessary, patients are matched with the provider and wound care plan that is specific to their needs.

When a patient walks through the door they are paired up with a provider and RN Case Manager.

With weekly visits, the patient will typically see the same team each time, ensuring continuity of care as well as supporting relationship and trust development between the patient and wound care team.

Wound care becomes a personal experience of health and teaching throughout the process. Patients who come to the center are oftentimes those who have had difficulties managing their diabetes and have struggled in the traditional health care system. They are referred to wound care by their primary care provider because they need help not only healing, but with education about what has caused the wound and how to prevent them in the future. Because of this, RN Case Managers spend a significant portion of each patient’s weekly visit on education and training.

“These patients can be very complex and teaching is important at every visit. We work with patients to help them understand why their wound(s) aren’t healing and teach lifestyle changes that will encourage wound healing and prevent them from becoming wounded patients again,” Burt explained. “We care about them and work to help make their lives better.”

With more than 57,000 patient encounters over the last two decades, the Cascade Valley Hospital Wound Care and Hyperbaric Medicine Center has earned the Center of Distinction Award in 2008, 2009, 2012, 2013, 2015 and 2017. The center has also received the Center of Excellence Award in 2009 and 2013. These awards are given to Healogics centers with outstanding records of wound healing, reduced time to heal and patient satisfaction.

“Striving to provide the best care around, the staff is very proud of the work they have accomplished over the last 20 years and are excited to continue their effort to make lives better,” Burt said.

Safety Huddle

Taking the pulse on quality, safety and the patient experience Heathcare is an environment of continuous improvement. There is no end point or final achievement in any health system that completes this work in key areas such as safety, efficiency and quality. Process improvement is part of the daily routine, woven into the organization’s infrastructure as much as any service line or clinical task.

In the past 12 months, process improvement and standardization efforts made way for an important daily touchpoint – the Daily Safety Huddle. This 15-minute stand-up meeting takes place in-person at both Skagit Regional Health hospitals. An additional call-in Daily Safety Huddle happens between clinics, since geography of having clinics in three counties prevents an in-person daily meeting.

“The Daily Safety Huddle is a central touchpoint for both clinical and non-clinical staff at the different locations,” said MJ Tyler, RN, MN, Vice President and Chief Nursing Officer for Skagit Valley Hospital. “A diverse cross-section of employees meet every morning for 15 minutes to take a quick pulse of the facility. This is a daily opportunity to return to center, check-in and make sure we’re focused on all the right things to improve patient care.”

A typical Daily Safety Huddle includes representatives from each clinical care department, case management, engineering, technology services, laboratory, pharmacy, employee health and more. The meetings are brief, a quick opportunity to bring forward any issues that could delay patient care, impact safety or affect workflow.

Huddles can include updates on an influx of flu patients or a check-in with Information Services about the Electronic Health Record system. The group regularly reports out on safety concerns or communication gaps and what can be done to prevent them in the future.

The huddles are also an opportunity to bring forward any reoccurring challenges or process improvement needs. Since most departments are represented in the Daily Safety Huddle, employees can quickly identify the right partner(s) to work toward a solution. These sparks of cross-department collaboration often result in the quick resolution of a challenge that may have taken many steps to resolve in the past. The challenges are documented during the huddle and teams report back the next day, as needed.

“The Daily Safety Huddle is a great example of ‘stopping the line’,” said Jonathan Lyons, Manager of

“Participants have a safe venue to bring forward issues and concerns with the right people in the room, pull a group together and problem solve. This meeting plays a key role in keeping everyone, especially myself as a non-clinical department, with a finger on the pulse.”

The Daily Safety Huddle has already led to a number of positive outcomes. A standardized reporting template was created to track the notes from each huddle and these quick meetings have resulted in the formation of several committees to address challenges brought up in that setting. Additionally, staff participation has grown significantly since the inception of the Daily Safety Huddle, adding to the voices contributing to removing barriers to great care.

“The beauty of this process really is the brevity of the meeting,” said Michelle Sand, MSN, RN, NEA-BC, Vice President and Chief Nursing Officer for Cascade Valley Hospital.

“Many of these barriers can be addressed simply by alerting a few people in the room. And this format inspires quick team formation to address larger challenges that need attention. We’ve already seen great outcomes from these 15 minute windows of focused time.”

Care from the comfort of home Skagit Regional Health has offered MyEClinic – virtual clinic services – since July 2016 to provide the community with on-demand care from the comfort of home, at the office or on-the-go. Patients can request care when and where it is most convenient, 24 hours a day, from anywhere in Washington state.

MyEClinic connects patients with a board certified physician or nurse practitioner over the phone or using video chat for a flat rate of $40.

MyEClinic providers are able to treat a wide range of common conditions, including: pink eye, flu-like symptoms, urinary tract infections, rash, sinus infections and more.

MyEClinic is available for patients of any age, but has been especially popular among parents with young families. The mother of a recent MyEClinic patient shared how grateful she was to have this service available when her son had pink eye symptoms.

Donors fund hospice volunteer program Erin Long began as Hospice of the Northwest’s new Volunteer Manager in October. Hospice volunteers provide companionship, friendship and vigil to people at the end of life. Thanks to the generosity of donors and Erin’s ambitious goals, there have been some big, positive changes to the Hospice of Northwest’s Volunteer Program.

Donor funds have purchased 20 new devices with protective cases including iPads, iPods, Kindle Fires and Bluetooth speakers. Volunteer trainings have doubled, occurring quarterly and featuring an online component so trainees can move forward at their own pace. This has led to a 24 percent increase in active hospice volunteers.

Volunteer support meetings now incorporate time to share stories with fellow volunteers, a memorial meditation on patients who have passed away and training conducted

by rotating Hospice of the Northwest staff members. A new program through We Honor Veterans has trained three veteran volunteers to provide Pinning Ceremonies, paying tribute to hospice patients who are veterans and their commitment to our country. As a final addition, Erin forged a relationship with Pet Partners of America, Dogs On Call and Love on a Leash to bring certified animal therapy to patients. Most critters are dogs, but just recently a visit from “Flight,” the therapy llama, brought furry hugs and gentle kisses to patients at a local nursing facility.

“We are grateful for our hospice volunteers and the community support that makes their meaningful work possible,” Erin said.

Celebration of generosity Dedicated and generous donors, staff, volunteers, trustees and community members came together in February to celebrate the significant impact donor funding has on the care that Hospice of the Northwest provides. In 2017, donor funding provided more than $500,000 in support of expert and compassionate hospice care. Donor funds enhance programs including volunteer services, bereavement services, complementary therapy, staff education, medical equipment and technology, and hospice services for those who are underinsured or have no insurance.

During the program, Hospice of the Northwest staff shared stories and examples of how donor funding helped to enhance patient quality of life. Deana Strom, a hospice spiritual counselor, shared a story about how

donor-funded atypical treatments helped an Air Force veteran join his son on a “bucket-list” Cessna trip around the San Juan Islands.

Hospice of the Northwest Medical Director Anita Meyer, MD explained how atypical treatments, such as blood transfusions, can greatly improve a patient’s quality of life. She further explained that these types of treatments can fall outside of the care that the agency typically receives reimbursement for through Medicare and other insurances, which can create a gap between what it costs to care for a patient and what the agency is reimbursed for that care. She also described the financial impact of new medical and technological advances that have changed the type of care available at the end of life, underscoring the positive impact of donors’ gifts.

Wendy Rohrbacher, Hospice of the Northwest Foundation Executive Director, closed the program by thanking donors for their enthusiasm and generosity. She also reminded them that their “generosity not only helps hospice patients, but also their loved ones and our whole community. Your gifts provide hope, comfort and laughter. Your gifts provide dignity and compassion every moment of life.”

What sparked your interest in the field of medicine? My grandfather was a family practice physician who had an office in the back of my grandparent’s house in Long Island, NY. He saw patients of all types and ages, rounded in the hospital, delivered children and much more. I was fascinated with his office and instruments as a child, though at that time had the greatest interest in the large jar of lollipops he kept on the counter for children after vaccinations.

Dr. Ginoza is board certified in Family Medicine and neuromusculoskeletal medicine. He is a member of Skagit Regional Health’s Family Medicine Residency teaching faculty. Why did you choose medicine and, in particular, pediatrics: I was always fascinated by how the body works and it’s the way that I wanted to help people. I love kids. If you take care of kids, I believe you take care of families too.

It’s true that kids say the funniest things and I find they bring little nuggets of joy to your day.

Why Mount Vernon, Washington? I am from a more rural area and I wanted to find an area where I can feel like I’m serving a community. The Pacific Northwest stole my heart when I came to train at Seattle Children’s, so I decided to stay. Skagit Pediatrics is an amazing group of people who work really hard for the families we serve.

What is your practice philosophy? I take a family based approach and want to learn where a family is coming from and what they can do and how I can support them. Every family is different and has different limitations and abilities. I want to meet them where they are and assist them as best I can.

Dr. Carrasco is a graduate of the University of New Mexico School of Medicine and completed her medical training at Seattle Children’s Hospital and the University of Washington. A native desert dweller, Dr. Carrasco enjoys the natural beauty of the Pacific Northwest and spends time away from work hiking, biking, walking, baking and reading. What sparked your interest in medicine as a career? I first became interested in sleep medicine at the University of Chicago where I went to college. The University of Chicago is the birth place of sleep medicine where REM sleep was discovered.

What do you like most about your practice? The thing I enjoy most about sleep medicine is seeing patients and helping with their sleep issues.

Dr. Park is a graduate of Meharry Medical College School of Medicine in Nashville. He completed a psychiatry internship and residency at DartmouthHitchcock Medical Center in New Hampshire. He remained at DartmouthHitchcock Medical Center for a sleep disorders fellowship. Dr. Park is board certified in psychiatry and sleep medicine. When he is not caring for patients, Dr. Park enjoys spending time with his wife and six-year-old son. Sleep medicine appointments for patients ages 15 and older can be made by calling 360-428-2586.

LifeCenter Northwest honors Skagit Valley Hospital

Local organ procurement organization, LifeCenter Northwest is proud to announce the presentation of their Organ Donation Achievement Award for 2017. Skagit Valley Hospital is being recognized for superior performance in organ donation last year.

Skagit Valley Hospital is one of 15 hospitals in a four-state service area to receive an Organ Donation Achievement Award. This award is presented to hospitals with a 75 percent donation rate. This rate is achieved through making timely referral calls, collaboration on potential donation cases with LifeCenter Northwest, and maintaining the opportunity for donation. These accomplishments translate to more waiting list patients receiving a second chance at life through organ transplantation.

LifeCenter Northwest is the federally designated organ procurement organization serving Alaska, Montana, North Idaho, and Washington-the largest geographic donation service area in United States.

Skagit Regional Clinics – Stanwood now offers primary care appointments during extended hours. Clinic hours are: 7:30 a.m. and 7:30 p.m. on Tuesdays and Thursdays.

Introducing Urgent Care’s “Purple Chart Program” The wait for follow-up care just got shorter thanks to a new process called “Purple Chart,” implemented in Skagit Regional Clinics’ three urgent care locations in January 2018.

Urgent care patients with certain diagnoses need to be seen again in the days after their initial visit.

Follow-up care has traditionally been the role of the patient’s primary care provider; however, a shortage of local primary care providers can mean a several-week wait time for appointments in many offices, which doesn’t allow for timely follow-up treatment.

“Some of our patients need twoor three-day follow-up that their primary care provider just can’t provide right now,” said Dr. Nona Hanson, who has worked at Skagit Regional Clinics’ Urgent Care since 2013.

Urgent care wait times have also spiked in recent months due to limited primary care access and the winter flu season.

“We realized that some patients were having to wait multiple times in one week so that they could get their follow-up care. Some of them weren’t coming back because of the wait,” said Dr. Hanson.

This realization was the motivation to develop a new, streamlined process for urgent care patients who need to be seen in follow-up, soon after their initial visit. They call it the “Purple Chart Program” which aims to improve patient care and satisfaction.

While the goal of the program is lofty, the process is simple. If a patient needs a short followup, which is often helpful for elderly patients, those with pneumonia, a patient in need of a wound check or suture removal, the provider puts the words “purple chart” on the discharge paperwork. When the patient comes back to be seen later in the week, they present the form to the urgent care front

office staff. Purple Chart patients are seen before other non-emergent patients, leading to a shortened wait time.

Though the program has only been running for a few months, the urgent care team has already seen signs of success. Patients get the follow-up care they need in a timely fashion.

Recently, a gentleman came in with a serious leg infection. Dr. Hanson saw him for follow-up care using the Purple Chart process. The patient finished his visit within 30 minutes of arrival. She noted that even included a 20-minute wait time following an antibiotic injection and a dressing change.

“I told him that he needed to come back again in two days for a re-check,” said Dr. Hanson. “He asked me, “Am I going to be that Purple Chart again? Because that was great!”

When asked how the program got its name, Dr. Hanson noted that purple is the color of royalty. “I want our patients to feel that we treat them like royalty-especially when they need extra attention,” she said. “I hope this program helps us to extend that care to those who need it most.”

CELEBRATING THREE DECADES OF GENEROSITY AND GRATITUDE As the Skagit Valley Hospital Foundation enters its’ 30th year, we are so very grateful for your support!

It really has been an incredible 30 years! With your support, we have been able to fund the Skagit Valley Kidney Center, the Cancer Care Center, the Mental Health Center, the Heather Schols Cardiac Rehabilitation Center and many other incredible projects. We have purchased equipment for dozens of care areas at Skagit Regional Health and provided funds to start new programs. In 2017, we were proud to fund new equipment for Surgical Services, including the purchase of a Hana Surgical Table and C-arm, while additionally providing $528,344 in grants for other projects.

Of course, none of this could have happened without you! Thank you for attending, sponsoring and supporting our signature events – the Have a Heart for Kids dinner, the Par Tee Golf Classic and the Festival of Trees – and thank you to the more than 1,000 volunteers who gave their time to help us achieve our success.

So much has changed in the healthcare environment regionally and nationally over

the three decades since this foundation’s formation. As we navigate through the everchanging landscape that is healthcare in the 21st century, we remain focused on our mission of raising funds to ensure that quality healthcare is delivered to the residents of our communities.

In February, the Foundation’s Board of Trustees committed to raising $2 million to assist in the purchase of equipment for the new Women’s Imaging Center! This new facility will create a regional center of comprehensive breast cancer diagnosis and treatment, helping countless women in our community.

The Skagit Valley Hospital Foundation’s 15th annual “Have a Heart for Kids” dinner, held February 26 at Il Granaio Italian Restaurant in Mount Vernon, raised more than $15,500 for Skagit Regional Health’s Children’s Therapy Program. The clinically-based, family-focused program provides one-on-one and small group physical, occupational and speech-language therapy to children with special needs. Services include family education and home programs.

$2 Million is Our Goal!

WOMEN’S IMAGING CENTER PROJECT

Your mother, your sister, your daughter, you. The statistics are hard to ignore. One in eight women will get breast cancer in her lifetime.

We also know that breast cancer is highly treatable when diagnosed early and early detection also allows for effective and less toxic treatment.

With that goal in mind, Skagit Regional Health, in a trusted partnership with Skagit Radiology, is creating a Women’s Imaging Center in a new, modern facility in Mount Vernon.

The Skagit Valley Hospital Foundation is honored to be a part of this project and has announced a $2 million capital campaign to assist in raising funds to make the Women’s Imaging Center a reality.

The $2 million will allow the acquisition of: • Two Digital Breast Tomosynthesis 3D mammography units • Three ultrasound units • One stereotactic biopsy table • One CT Scanner • One state-of-the-art MRI unit with breast coil This new imaging center, along with the knowledge of dedicated radiologists, surgeons, oncologists and the excellence of Skagit Regional Health’s Cancer Care Center, will create a facility for comprehensive breast cancer detection and treatment.

Pulmonary Rehabilitation Program

This Skagit Valley Kidney Center program is for people with reduced kidney function or high risk for renal failure to explore their treatment options. Patients, families and caregivers will receive information about treatment options, such as dialysis options both at home and in-center and transplant. Our healthcare experts will be on hand to discuss diet, health maintenance, financial and social concerns and answer any questions you may have. This intensive program can help patients with Chronic Obstructive Pulmonary Disease (COPD), to reduce respiratory symptoms, improve functional independence and complications and reduce hospitalizations. The program is provided by a multi-disciplinary team plus individualized education to help patients understand and learn how to manage and cope with daily activities. Participants learn selfmanagement and relaxation techniques to improve emotional well-being, skills to help regain control of their breathing and pursue activities to the fullest extent of their abilities.

Nurse-Family Partnership First-time moms who are eligible for Apple Health or WIC can enroll in Skagit County’s Nurse-Family Partnership (NFP) program. Moms are paired with nurses who meet with them from early pregnancy until their child turns two, providing personalized information and support for a healthier pregnancy and a more confident parenting experience.

PCAP (Parent-Child Assistance Program) PCAP is a research-based program that assists women who have abused drugs and/or alcohol while pregnant to access services in the community.

Women who meet the eligibility criteria will receive three years of intensive case management, which includes home visitation, intervention and transportation. PCAP will help clients build and maintain healthy, independent family lives, assuring that children are in safe and stable homes and work with the client to prevent future births of drug and alcohol exposed children. DIABETES EDUCATION SKAGIT REGIONAL HEALTH CERTIFIED DIABETES EDUCATION PROGRAM Skagit Regional Health offers a multidisciplinary, comprehensive education program for adults with diabetes, either newly diagnosed or for those patients who want additional guidance to help reduce the risks of long-term complications. The program includes individual and group education.

Continuous Glucose Monitoring (CGM) is available through the program. A small sensor, worn in the abdominal area, measures and stores data for four to five days.

A1C testing is also available.

Parkinson’s BIG® Program Skagit Valley Hospital physical therapist Bree Corbin, MPT has completed the required comprehensive hands-on training and testing through LSVT Global to become a certified LSVT BIG clinician. LSVT BIG is a researchbased exercise program focused on amplitude (bigger movement) training for individuals with Parkinson’s.

Parkinson’s disease often results in slowing of movements and decreased size of movements leaving the individual struggling with daily tasks and walking. The BIG program focuses on teaching the amount of effort needed to produce normal movements through intensive and high-effort practice. The protocol consists of 16 one-hour sessions, four days a week for four weeks.

Stroke Support Group This caring support group for stroke survivors, their loved ones and caretakers provides up-to-date education and information. Provided

Meets the last Friday of each month from noon – 1 p.m. at Skagit Valley Hospital in the Speech Therapy Office (Room 102) in the Outpatient Rehabilitation Center. No charge. MAIL DELIVERY Volunteers at Skagit Valley Hospital make a daily circuit to pick up and deliver mail.

GIFT SHOP The Skagit Valley Hospital Gift Shop has many wonderful retail gift items for sale. The gift shop is open Monday through Friday from 9 a.m. – 5 p.m.

All proceeds from the Gift Shop are returned back to help Skagit Regional Health in areas of need. Volunteers assist with sales and merchandising.

PIANISTS Music can help to create a healing environment. Accomplished pianists are invited to join the group of volunteers who share their talent playing the grand piano in the Skagit Valley Hospital lobby.

JUNIOR MEDICAL VOLUNTEERS The Junior Medical Volunteer program is for students age 1618. The program allows young people an excellent opportunity to explore the culture of a healthcare organization while giving service to their community. Junior Medical Volunteers work in many areas of Skagit Regional Health. Each year, Skagit Regional Health awards scholarships to outstanding Junior Medical Volunteers.

FOR STUDENTS AND JOB SEEKERS While volunteering can be a great way to explore job opportunities, network or get required school credits, we ask volunteers to complete a minimum of 100 hours before requesting recommendations or receiving credit for hours volunteered.

As the number of positions is limited, prospective volunteers should apply at least a year in advance of any school deadlines that may be applicable.

High school students seeking course or club credit should apply and begin volunteering before their senior year.

OTHER VOLUNTEER DEPARTMENT SERVICES: HOSPITAL AND CLINIC TOURS Tours of the facilities for groups and individuals are available.

We want to hear from you

Feedback from patients is one of the primary ways we as a healthcare system obtain information to help us improve. It’s our job to provide safe, quality and compassionate care. We need our patients to let us know how we are doing.

We use a pair of survey tools to gather feedback from randomly selected patients who have received care in our hospitals and clinics.

HCAHPS, which stands for Hospital Consumer Assessment of Healthcare Providers and Systems, goes out via U.S. Mail to those who have been inpatients at Skagit Valley Hospital or Cascade Valley Hospital. For those who receive care in one of our outpatient clinics, you may receive a CG CAHPS survey via email – that’s short for Clinician and Group Consumer Assessment of Healthcare Providers and Systems.

According to the Centers for Medicare and Medicaid Services, these surveys provide a national standard for collecting and publicly reporting information about patient experience of care and allows valid comparisons to be made across hospitals locally, regionally and nationally.

That’s why it’s so important for our patients to fill out a survey when you receive one. Surveys may be frustrating to fill out and return, but

they are a very important tool for us and impact how we can improve our services as well as how we are reimbursed for our services. This information allows us to know how we are doing, how our scores are trending and how we compare to other healthcare systems.

These are standardized questions asked about important topics:

• Communication with nurses and doctors • Were you treated with courtesy and respect • Listening by staff and providers • Staff responsiveness • Cleanliness of the environment • Quietness • Pain management • Medications Most importantly, you are asked if you would recommend our facilities to others and for an overall 1-10 rating. In this case, a one is not the number we are looking for.

Receiving feedback can be a difficult, but rewarding, thing. It’s what we do with the feedback that really matters. At Skagit Regional Health, we monitor these scores, share them with leaders and staff and discuss them in Board meetings.

These scores tell us where our opportunities for improvement are and also give us moments of celebration. In all cases, we take these results to heart and use them to help us help you.

So, when you receive one of our surveys, we ask you to complete it and return it, we truly value them. Being honest in your assessment is how we are able to improve.

Most importantly we value each and every patient for their loyalty and the only way to continue having patients who return, is to listen and improve. Of course, we love to hear when we get it right the first time too.
