Cardiac cath lab upgrades

Bringing the latest in imaging to Skagit Valley Hospital For patients experiencing heart disease, unusual heart rhythms or vascular issues, the cardiac catheterization lab is a likely destination as the place where interventional cardiologists, electrophysiologists and interventional radiologists provide state-of-the-art diagnosis and treatment.

Skagit Regional Health has embarked on a project to upgrade two of the three catheterization labs at Skagit Valley Hospital with the latest in cardiac imaging services.

The new equipment will replace systems installed 13 and 14 years ago. The first upgraded lab was opened in July 2023 and the second will open in 2024. Once the project is complete, all three labs will provide the latest technology and be equally equipped to handle the needs of all patients.

Among the cutting-edge procedures performed at Skagit Valley Hospital are: • 3D mapping and ablation of heart arrhythmias • Leadless pacemaker implants • Placement of the Optimizer® heart failure treatment device implant - the first in Washington state • CardioMEMS heart failure monitoring device implants

“I am extraordinarily excited to see the remodel of our catheterization laboratories as we continually expand our cardiovascular and interventional radiology services,” said Cardiac Electrophysiologist Ramy Hanna, MD. “This project highlights our strong commitment to the community and our drive to always stay at the cutting edge of procedural care for electrophysiology, structural heart and interventional radiology services.”

The number of procedures in the catheterization labs has increased in recent years from 2,961 in 2018 to 3,611 in 2021. A referral center for the surrounding areas, patients hailing from 15 different Zip Codes received care in the labs in 2020.

After suffering a cardiac arrest in 2012, Michael, husband and father, received life-saving care from the cardiac catheterization lab at Skagit Valley Hospital.

He was only 24 Michael Garcia's story of life-saving cardiac care Per his usual weekday morning routine, Michael Garcia was at his local health club. Suddenly, he dropped to the ground going into cardiac arrest. Friends nearby rushed to his side and began CPR. Michael was transported to Skagit Regional Health’s Heart and Vascular Institute where it was determined Michael’s heart attack was brought about by ventricular tachycardia, or “VT.” This abnormal heart rhythm occurs when the lower chamber of the heart beats too fast to pump well and the body doesn’t receive enough oxygenated blood.

Michael’s cardiac issue required urgent treatment. “The team first implanted a defibrillator to regulate my heart rhythm. Next, they performed a catheter ablation to pinpoint where the arrhythmia started,” he explained. This minimally invasive process stops the electrical impulses that cause the arrhythmia. Michael’s life-saving procedure is one of many performed in the cardiac catheterization lab.

“This experience was an eye-opener for me. At 24, I thought, what is my life going to be like?” he said.

Because of the quick CPR performed by friends, his physicians’ diagnosis and the catheterization treatment available right here, Michael leads a joy-filled, robust life raising three daughters and a 10-month-old son while running a landscape supply business with his family.

In 2022, on the 10th anniversary of his cardiac arrest, Michael had a new defibrillator implanted. “It’s unreal to think I have already lived a decade plus since my first procedure. I received a second chance at raising a family, achieving wealth and, most importantly, another shot at leading a full life!”

One step at a time

A patient's journey of breast cancer and the 500-mile Camino de Santiago In 2020, Whidbey Island resident Marilee Pike’s travel plans were put on hold. COVID-19 disrupted countless journeys and ambitions across the world, and hers was no exception. But little did she know, her journey wasn’t ending - it was only beginning.

Feeling inspired by the film The Way, Pike and a friend began to dream in 2020 about their own adventure to walk the Camino de Santiago – a 500-mile pilgrimage journey to the Cathedral of Santiago de Compostela in Galicia, Spain.

While COVID-19 stalled their travel plans, an unexpected chapter in Pike’s life began in September 2021 when she received a phone call from her primary care provider.

“I went in for my annual mammogram…they saw something and scheduled a follow-up mammogram, ultrasound and biopsy,” said Pike.

October is Breast Cancer Awareness Month

Pike was diagnosed with Invasive Ductal Carcinoma with invasion into her lymph nodes. She soon heard from a Skagit Regional Health patient navigator and met with her oncologist and surgeon at the Skagit Regional Health Cancer Care Center in Mount Vernon. Her treatment involved surgery, eight sessions of chemotherapy and 30 sessions of radiation.

Pike said she initially felt overwhelmed by the process, but reminded herself to take it one step at a time. And the support she received from her care team at Skagit Regional Health – from receiving homemade hats for warmth during the winter after losing hair, to the hugs and celebrations upon ending her treatment – helped her make it through.

“I cannot say enough about the care I received at Skagit Regional Health,” said Pike. “This was a scary period of my life and they made me feel strong. Everyone, and I mean everyone, was so kind to me and concerned about me.

I really liked how all my caregivers were on the same page for my treatment. There wasn’t any question or confusion about what the plan going forward was going to be.”

Mammograms can detect changes in the breast up to two years before they can be felt by a patient or provider during an exam - making mammography critical in the fight against breast cancer.

Skagit Regional Health follows the American Cancer Society in recommending assessment of breast cancer risk in all women at age 40.

This October, make your health a priority and make the call to schedule your mammogram. Extended hours are offered in October and convenient appointment hours are available throughout the year.

She was thrilled and decided to give back by participating in the Susan G. Komen 3-day 60mile walk in Chicago occurring a few months later. She began the walk one year and one day after receiving her cancer diagnosis.

“I don’t think my outlook changed right after I was given my NED,” said Pike. “I was ready for my body to start healing. I think my outlook on life changed less than a year later when my older brother was diagnosed with throat cancer and my younger brother was diagnosed with mesothelioma…my life is truly a gift from God. I don’t question why I had cancer.

There must be a reason, so I am grasping onto every minute of it.”

In May of 2023, Pike and her friend fulfilled their dream to walk the Camino de Santiago.

Their trip is one they will never forget and offered a chance for Pike to reflect and learn from others on a similar journey.

“The most memorable events were the people I met along the way,” said Pike. “They were from all over the world, sharing their stories as we walked…some were breast cancer survivors, which gave me hope for my future.”

Although Pike recalls feeling overwhelmed when she first began her cancer journey, like the Camino de Santiago, she reminded herself to take it one step at a time.

“I would absolutely recommend Skagit Regional Health to anyone facing this challenge,” said Pike, reflecting on her cancer treatment. “The folks are wonderful and the care is outstanding.”

Surgeons at Skagit Regional Health started using a new robot in August to assist with precise back procedures.

The Globus ExcelsiusGPS® Robotic Navigation system uses diagnostic images of the patient, that are displayed on a computer screen, to help guide surgeons in providing precise, minimally invasive procedures. The images help the surgeon create a surgical plan based on the patient’s individual anatomy and determine the size and placement of screws.

“The robot is a precision guide for hardware placement,” said spine surgeon David Mourning, MD. “It does not do nerve decompressions, such as discectomies or laminectomies, as those still require the steady hand of a surgeon combined with tactile feedback to safely operate.”

For patients, the main benefit is safety and efficiency, said spine surgeon Dylan Morris, DO. “The robot will allow us to perform complex spine surgeries in a safe and efficient manner,” Dr. Morris said. “This will allow us to continue to give the best care possible for our patients at Skagit Regional Health.”

Dr. Mourning also noted the efficiency of hardware placement with the precision of the robot.

“Using the robot can greatly decrease time in the operating room by 10 to 20 percent as the screws go in like clockwork, eliminating delays due to visualization, difficult anatomy and other issues,” Dr. Mourning said. “Less time under anesthesia and shorter time in the OR is better for the patient.”

The robotics and navigation reduce the length of procedures, improves accuracy and dramatically lowers the level of radiation exposure from fluoroscopy, which is good for the patient and the OR team. In addition, the smaller incisions mean less tissue damage and a shorter hospital stay.

“We won’t do any new surgeries, we’ll just do the procedures safer and faster,” Dr. Mourning said. “I’ve been looking forward to getting this for several years now. I’ve used several other systems for guidance (image guidance, artificial reality headsets) but nothing compares to the reliability and efficacy of this system.”

This is not the first surgical robot in use in the operating rooms at Skagit Valley Hospital. Skagit Regional Health’s general, colorectal and thoracic surgeons, OB/GYNs and urologists use the da Vinci® Xi™ Surgical System, brought to the hospital’s operating rooms in 2018.

Mount Vernon Surgery Center boasts efficiency and state-of-the-art equipment Patients can expect an improved overall experience.

The Skagit Regional Health – Mount Vernon Surgery Center opens to patients in December, providing a modern, stateof-the-art outpatient facility for surgery and endoscopy.

The 60,000-square-foot building, just northwest of Skagit Valley Hospital, also serves as a medical office building and home to clinics for Audiology; Ear, Nose and Throat (ENT); Gastroenterology; Orthopedics and Sports Medicine, Interventional Pain Medicine and Spine Surgery.

The three-story building features four operating rooms, three endoscopy suites and on-site diagnostic imaging.

“We will have an abundance of technology in place that will protect our patients and allow the staff to focus on patient care,” said Matthew Ornelas, MBA, Ambulatory Surgery Center Administrator. “We are very excited about how the Mount Vernon Surgery Center will expand our organization’s surgical capability, allowing surgical services to continue growing and providing care to the patients of this region.”

With a growing number of surgical procedures now offered in the outpatient setting, including many total joint replacements, the patient-focused facility features the latest in technology and offers a convenient location, easy access, patient pick-up area and ample parking.

Natural light and calm, neutral colors are part of the patientcentered approach to the Mount Vernon Surgery Center.

The goals of the project are focused on the patient, said Plant Operations Program Manager Michelle Gjersee, who has played an integral role in the project planning.

“Calming and welcoming colors with cleanable materials, large lobbies, waiting spaces flanked with windows and a quick patient pick-up location are design elements that show how we have achieved a patient-centric facility and demonstrate that Skagit Regional Health puts our patients first in all that we do,” Gjersee said.

Providers who will work in the building have been very involved in the planning of the facility, which will benefit the patients and staff. Specialists in orthopedics, urology, general surgery, foot and ankle, plastic surgery, ENT, gynecology, orthopedic spine surgery, gastroenterology and other surgical specialties will provide services in the new surgery center operating rooms.

“Patients will have a faster and more seamless experience at the surgery center,” said Orthopedics and Sports Medicine specialist Christopher Sheu, MD, FAAOS.

Dr. Sheu said he looks forward to more streamlined care, the “latest and greatest technology” and state-of-the-art instrumentation and facilities, including video towers in the operating rooms.

Patients will benefit from better outcomes, less complication and infection rates, along with the advanced technology, he said.

Dr. Sheu plans to perform minimally invasive shoulder, hip and knee arthroscopy at the Mount Vernon Surgery Center, as well as reconstruction/replacement of the shoulder, hip and knee at the outpatient center.

Ear, Nose and Throat specialist Christopher England, MD notes that the new facility will provide “access to a highly efficient operating suite which will make things more convenient for our patients and increase access to care without sacrificing quality or safety.”

Gastroenterologist Suresh Alagugurusamy, MD looks forward to the new facility opening.

“The patients will appreciate the attractiveness of the new spaces and modern equipment, which means more patients will be interested in getting services,” said Dr. Alagugurusamy. “There will be more privacy in the preand post-op areas for our endoscopy patients.”

“This facility is designed for this purpose. This is purely an outpatient facility and will have the efficiencies and processes for each patient,” he said. “I am very excited we will be moving into this new facility. I think for the patients and providers, having this state-of-the-art endoscopy unit will be awesome.”

Dr. Alagugurusamy will perform upper endoscopy and colonoscopy in the new facility, which also features the latest processing equipment for the scopes.

“The surgery center will be far more efficient as we are dealing primarily with healthy patients and outpatient procedures and this will allow us to be more efficient in how we treat our patients,” Dr. England said. “The center has a modern design which I also feel will be more comfortable for patients. Plus, the technology going into the clinics and operating rooms is state-of-the-art. The new facility will have the most up-to-date version of almost everything.”

Skagit Regional Health’s planning process for the highly anticipated new facility has included input from a variety of providers.

Dr. England will perform a full range of procedures in the new facility, including thyroid, sinus and ear surgery, plus cosmetic reconstruction. ENT providers use an image guidance system to visualize during sinus surgery compared with a patient’s CT scan. “It makes surgery safer, faster and, in my experience, provides superior outcomes,” he said.

“Skagit Regional Health has had physicians involved in the design and implementation of the surgery center since the beginning,” Dr. England said. “This includes everything from discussing floor plans to how the operating rooms are used and shared between services. There are a lot of moving pieces and it is a complex process, however, physician input has been used throughout the planning and construction.”

“I have been a proponent of the surgery center since I started here seven years ago,” Dr. Sheu said. “It’s finally here. I’ve been fortunate enough to be involved in the entire process from its inception, design and served on the strategic planning team.”

The addition of the Mount Vernon Surgery Center will relieve pressure on the six operating rooms at Skagit Valley Hospital, where the focus will remain on patients who need to receive hospital care following surgery and emergent procedures.

“Patients will receive the same fantastic care at the surgery center as they do at the hospital,” said Kelly Bradford, MBA, RN, CNOR, Regional Director of Surgical Services. “The addition of a surgery center expands our ability to continue to care for the people of our community.”

Your wishes, your voice: The power of advance care planning

What if you experienced a sudden accident or illness that prevented you from making your own medical decisions? Would your family or loved ones know what kind of care you want or don’t want?

An advance directive is a legal document that allows you to state your wishes for medical care in the event you become unable to speak for yourself. This can include decisions about cardiopulmonary resuscitation (CPR), life support and other life-sustaining treatments. By having an advance directive, you can help ensure your wishes are followed and that your loved ones and healthcare providers know what you would want.

In Washington (and many other states), the advance directive also enables you to assign someone to act as your healthcare durable power of attorney. This is a person you trust to make healthcare decisions for you when you are unable to communicate or make decisions for yourself. Often people don’t consider filling out an advance directive until they are of a very advanced age or facing a significant medical diagnosis. However, we highly recommend every adult complete this form. Anyone who is at least 18 years old can and should have an advance directive.

Some people are reluctant to complete an advance directive because they are unsure about some of the choices, or they worry, “What if I change my mind?”

The advance directive can be updated as frequently as you desire. We recommend people review their advance directive at least every ten years, any time they experience a significant life change (e.g., a loved one’s death, a significant move or divorce), or if they have received a new life-limiting diagnosis.

Sometimes as we age, have more life experiences or watch loved ones endure tough situations, we may reassess our feelings about things like CPR or life support. This is completely natural. Advanced directives do not lock you into any decisions.

• The heating, ventilation and air conditioning system filters air at the point of use, just above the ceiling, to ensure the most freshly filtered air is entering the facility at all times.

People of an advanced age, who have a serious illness or who possess a very strong opinion about not undergoing CPR should also consider completing a Portable Order for Life-Sustaining Treatment (POLST) form. The advance directive is really only valid inside a hospital or other healthcare facility.

Having a POLST signed by your healthcare provider helps ensure emergency medical service (EMS) providers can follow your wishes should you need help at home or in a public place.

Since we never really know when one of these moments may arise, it’s best to get started on your advance directives now to make sure your voice is heard.

OB/GYN and Midwifery care At its core, the Women’s Health department at Skagit Regional Health is committed to compassionate medical care, support and guidance for every patient. Recently, the Women’s Health clinic at Skagit Regional Clinics – Mount Vernon has expanded into two separate clinics – Obstetrics/ Gynecology (OB/GYN) and Midwifery.

Both OB/GYN and Midwifery care are also available at Skagit Regional Health – Arlington Women’s Health.

Midwifery For patients who desire a holistic approach and present as low-risk, midwifery is a good option.

“Midwifery focuses on patientcentered care,” said Tami Schnell, Regional Director for Women’s and Children’s Services. “The philosophy of empowering women to make informed decisions about their health…CNMs (Certified Nurse Midwives) strive for longterm relationships with their patients.”

The new midwifery clinic at Skagit Regional Clinics – Mount Vernon is set up well to enable this.

“Midwifery care involves providing care during pregnancy, childbirth, and the postpartum period as well as reproductive and gynecologic health services, with a focus on education and care of the whole person,” said Madeline Harris, CNM, Certified Nurse Midwife at Skagit Regional Health.

“Midwives primarily care for healthy, low-risk individuals, but can also manage many of the most common pregnancy complications, including gestational diabetes that is controlled with diet, mild preeclampsia and advanced maternal age.”

A unique benefit to midwifery care at Skagit Regional Health is the ability to deliver in a hospital setting while maintaining a holistic approach. “(Midwives’) philosophy is to provide women with the delivery of their choosing in an atmosphere of compassion, skill and health promotion,” said Schnell.

Obstetrics and Gynecology (OB/GYN) For patients with complicated or high-risk pregnancies, OB/GYN care is necessary. OB/GYN providers are also trained to partner with patients experiencing complex gynecologic issues.

“OB/GYN (providers) are dedicated to the health and well-being of women as well as their unique needs,” said Schnell. “Their focus is on the clinical aspects of care…from preteens through menopause. (They) are trained to do surgeries and manage pregnancy-related complications.”

One benefit to receiving gynecologic and pre-/post-natal care at Skagit Regional Health is that the Midwifery and OB/GYN services are closely connected in both Mount Vernon and Arlington.

“Although midwives at Skagit Regional Health typically care for low-risk pregnancies, they are adept at identifying any complications of childbirth and closely work with consulting OB/GYNs when necessary,” said Carlyn King, DO, OB/GYN provider at Skagit Regional Health.

“The (OB/GYN and Midwifery) providers are a team, in two different clinics,” said Schnell. “The admiration and respect of each other’s skill levels and their philosophy creates the best care for patients.”

A growing need for advanced cardiovascular care The statistics are hard to ignore. Heart disease continues to be the leading cause of death in the U.S. for both men and women. Each year, one in seven adults die from the disease.

You probably know someone who has struggled with heart disease. A parent. A sibling. A neighbor. A friend. The good news is that cardiovascular disease is highly treatable when diagnosed early.

Skagit Regional Health’s Heart and Vascular Institute provides outstanding cardiovascular care to more than 3,000 patients from 15 different Zip Codes each year. And the need continues to grow.

An investment in having the best cardiac imaging services available is what our community deserves. Imaging technology has improved immensely and has proven to increase the accuracy of early detection. It allows for more precise interventional cardiac procedures such as those to open blocked arteries or place the heartbeat back into rhythm. Most importantly, this new diagnostic technology will provide our cardiologists with quicker results and improve overall outcomes for Skagit Regional Health patients.

What is a cath lab?

A catheterization lab or cath lab is a procedure room with imaging equipment used to visualize arteries and chambers of the heart to diagnose heart disease, treat any blockage identified and prevent heart attacks (see page 4).

Many treatment options With the planned upgrades, our multidisciplinary cardiovascular team will continue to provide the following services: • Angioplasty • Cardiac catheterization • Arrhythmia and atrial fibrillation management • Electrophysiology • Pacemaker insertion And, as an added benefit, these non-surgical, minimally invasive treatments usually do not require an overnight hospital stay. Orthopedic providers bring hands-on learning to Arlington high school students The Arlington School District (ASD) has recently partnered with Skagit Regional Health providers as part of their Career and Technical Education Advisory Board, bringing medical expertise to their Health Science program to offer better resources and information for students interested in pursuing future career paths in healthcare.

Orthopedic Surgeon Ryan Vellinga, MD is one Skagit Regional Health provider who offers up-to-date information to educators to aid teaching and help enhance the overall student learning experience – all with the goal of enabling students with interest in a career in healthcare.

“If (students) in high school get a chance to be around something in medicine and see something cool and surprising, that could alter their career path,” said Dr. Vellinga. “There is a whole host of kids who may also be great physicians but never had inspiring exposure to it – so providing the opportunity to explore their interest could help some pursue something they’d really like to do for their own career.”

The teachers see tremendous benefit for both their students and themselves as they plan their program.

“Dr. Vellinga has shared things that helped him in school and medical school and that has helped me as a teacher to change or add things to my lessons to help the kids,” said Nicole Roskelley, Arlington High School Health Science instructor. “It is so appreciated and we love having them as part of our group and an active part of our community/ board.”

School district officials see the benefits and plan to keep it going – for several career fields.

“This is an example of the kind of partnership we seek in all our Career and Technical Education programs,” said Brian Long, College and Career Readiness Director for ASD. “We have extensive programs best supported by a vast array of careers and people. Our goal is to align our courses with industry standards so that our classrooms look less like classrooms and more like worksites. We cannot do this without the input of industry content experts like Dr. Vellinga.”

As a public hospital district, it’s important for you to know that the organization needs a margin – or profit.
