We are excited to have you on campus for a St. Jude Lifetime Cohort (St. Jude LIFE) study visit. You help us learn how childhood cancer and its treatments can affect our survivors as they age. This information can help us find ways to reduce the late effects of treatment. Late effects are treatment-related conditions that happen months to years after treatment is over.

St. Jude LIFE is a research study. This means the study will provide comprehensive research-based testing that requires signing a legal consent form.
If the St. Jude LIFE participant is under age 18 years or cannot give consent for themselves, a legally authorized adult must come with them to research visits. This adult must be able to give legal permission for the research tests.
Before anyone can take part, St. Jude hospital staff must make sure that proper permission is given. Consent can come from:
• A parent with legal custody
• A legal guardian
• A legal custodian
• Another legal representative appointed by a court or state agency
• A person with power of attorney for health care
• A health care agent or surrogate
Please bring legal papers to your visit. St. Jude may ask to see them. Legal papers are documents that specify who can legally give consent for a child or dependent to take part in the study.

• MyChart: If you have MyChart, link your St. Jude account. If not, download MyChart from your phone’s app store and make sure your contact information is correct.


• Our St. Jude app: Download the Our St. Jude patient app for your appointment calendar, maps, housing/ transportation info, and more. You must have MyChart (the same password works for both apps).

• Children must be supervised by an adult at all times: Participants CANNOT supervise children and attend study visits.
Schedule confirmation: The St. Jude LIFE team will confirm your schedule by email, text, or phone about 2 weeks before your visit. At any time, if you need to reschedule or cancel, email SJLIFEvisit@stjude.org as soon as possible.
Mail back surveys and forms in the yellow envelope (by the due date in your packet).
Book travel

Note: Book U.S. flights 3 weeks before your visit and international flights 2 months before your visit.
• Bus/car/housing: Call Patient Services 901-595-4501
Gather what you will need: Photo ID, Social Security number or tax ID number, and any legal documents for consenting (if applicable).
Download St. Jude apps: MyChart and Our St. Jude (Our St. Jude requires MyChart).
Check your lab day: Fasting labs mean no food or drink (except water) after midnight before labs. Take prescription medicines with water as usual.
Pack athletic wear for visits: You will need clothes and shoes for physical tests that involve walking, exercise bikes, and treadmills. Examples: T-shirt or athletic top, shorts or sweatpants/ leggings, and comfortable closed-toe athletic/tennis shoes.
• Arrive 30 minutes before your first appointment each day.
• Start at Patient Registration in the Patient Care Center
• Follow your schedule in order and check in at the front desk at each location.
Your visit begins at Patient Registration in the Patient Care Center. Depending on your arrival time, you may check in on the day you arrive in Memphis or on your first day of appointments.
You will get an ID badge when you arrive. You must wear it while on campus. You need the badge to:
• Buy food in Kay Kafe, The Domino’s Village, or Family Commons
• Use housing elevators
• Unlock housing rooms
During your study visit, you will have:
Health evaluations
Blood and urine lab tests
Heart and lung function tests
Thinking and memory skills tests
Physical fitness and function tests
Eye exam
Hearing exam
Bone density scan (if you are 18 or older)
Depending on the cancer treatment you received or other studies, you may have other tests.
• Use your hospital ID badge to buy food at Kay Kafe, The Domino’s Village, or Family Commons.
• Daily allowance is $25. Remaining funds do not carry over to the next day.
• St. Jude will cover transportation for one adult to travel with adult St. Jude LIFE participants.
• Anyone under 18 years cannot stay alone in St. Jude housing at any time.
• No more than 5 people may stay in a room.

Please follow these St. Jude policies for personal and campus safety.
• Adult supervision:
Anyone under 18 years old MUST always be supervised by an adult. That adult CANNOT be a study participant. St. Jude does not provide child care.
• No alcohol or drugs:
Alcohol and drugs are not allowed on campus or in housing.
• No weapons:
Weapons are not allowed anywhere on campus or in housing, even if you have a license. Only on-duty officers are allowed to have weapons at St. Jude.
• Pets and service animals:
Pets are not allowed in patient housing. Service dogs are allowed. St. Jude understands that your service dog is an important part of your health and well-being.
According to the Americans with Disabilities Act (ADA), a service dog is trained to perform a task related to a person’s disability. All service dogs at St. Jude must meet the needs of the person they assist. Service dogs are checked when they arrive at St. Jude. You can start the process before your appointment by calling the nursing house supervisor at 901-595-3300. Many test results will appear in MyChart as they are ready. Your St. Jude LIFE team will discuss results with you and answer your questions. Compensation
You will be paid for your participation in the study. You must register to receive payment. You will start this process at your consent appointment on your first day. You will be paid using a paper check, mailed to the address you provide on your first day. Payment may take up to 6–8 weeks after your visit.

The St. Jude Lifetime Cohort Study (St. Jude LIFE) began in 2007. It has more than 6,000 participants. St. Jude LIFE participants visit St. Jude about every 5 years for comprehensive health exams, assessments, and survey completion.
Information from the study helps St. Jude and pediatric cancer facilities worldwide:
• Learn how common late effects are and how to predict who will have them
• Know more about what actions childhood cancer survivors can take to prevent and manage late effects
• Adjust treatment plans to reduce late effects in future patients
• Suggest follow-up care plans for local healthcare providers
Information from the study helps survivors’ local health care providers:
• Know what health screenings childhood cancer survivors should have
• Know more about what actions childhood cancer survivors can take to prevent and manage late effects to optimize their health
As a St. Jude LIFE participant, you help improve the lives of fellow childhood cancer survivors and learn about your own health and ways to stay healthy.
