Welcome Home The Hodroff Pavilion is a separate, supportive environment specially designed and staffed for the distinctive needs of adults with dementia and memory loss. The Pavilion is made up of 24 studio apartments with services, amenities and programs that support each person’s dignity and uniqueness and give families peace of mind.

SERVICES The monthly rate for tenants in the Hodroff Pavilion for Memory Care includes: • Weekly housekeeping and laundry s­ ervices, daily bedmaking and trash removal • Individualized personal cares • Escorts, cueing and redirection • Home care services 24/7 • Licensed nursing personnel on-site daily and on-call 24-hours per day • Response to pendant calls • Structured and purposeful memory care activities that fulfill tenants’ physical, mental, spiritual and social needs • Transportation for outings • All utilities

Each potential resident will be assessed by a Registered Nurse prior to admission and assigned a Care Level. There is a one-time initial assessment to determine services needed costing $245.00. Potential residents will be placed according to the highest need assessed. For example, one need in Care Level 3 places the potential resident in that level. Care Levels may be reevaluated and reassigned as care needs change.

Extensive dressing and grooming assistance or two-person assistance Extensive Medication Management: Weekly setup. Medication Administration 4 times per day by Home Health Aide: *includes when needed and nebulizer medications (cost of medications not included, additional charges for injected medications) where 24/7 nursing is not available Dining — Hands-on assistance with set up and eating Extensive bathroom assistance (i.e. more than eight or more times per day, two-person assistance and/or incontinence with behaviors (i.e. resistiveness)** + catheter care included Two-person and/or mechanical lift transfers for ambulation or bed mobility Oxygen management and CPAP assistance Frequent redirection (multiple times daily) Safety checks — 3 or more per day (due to elopement or fall risk)

* Injected medications (i.e. monthly B-12 injections) will be charged the incremental licensed nurse fee.
