How braces move teeth Anyone with misaligned or crooked teeth may have considered orthodontic braces at some point. In fact, getting braces has now become a rite of passage for many youths. The American Association of Orthodontics reports that roughly four million people are wearing braces at any given moment, and one in five of those people is an adult.

Orthodontists often share with their patients what to expect from their treatments, including how long they can expect to be in braces. While many know the basics of braces, they may be curious as to why braces are so effective.

Anyone who has had a tooth extraction knows that teeth don’t move around willingly, but they’re not cemented into the jaw, either. In fact, teeth are held in place by what is known as the periodontal ligament. This ligament connects the roots of each tooth to the surrounding alveolar bone of the jaw that contains tooth sockets, explain the experts at Burke & Redford Orthodontics in California. The ligament is strong but it’s also malleable. This means that when pressure is applied for certain periods of time, teeth can move rather easily.

Braces consist of various components, all designed to put pressure on teeth so they move gradually. Brackets are the metal or ceramic materials that are glued to the surface of the teeth.

Archwires are fitted over the brackets and are the primary pressure-makers of the braces system. O rings, also known as ligatures or ligature elastics, are small elastic bands that hold the bracket onto the archwire and will be changed at each adjustment, indicates Dillehay Orthodontics in Kansas.

When braces put pressure on the teeth, the periodontal membrane stretches on one side and compresses on the other, which essentially loosens the tooth in the socket. The orthodontist uses techniques and skill to manipulate each tooth into better position. This happens while the jaw bone grows in to support the tooth in its new position — called bone remodeling.

Some discomfort is to be expected during bone remodeling, but it’s usually a dull ache. It takes months for new tissue to come in while teeth move. Each patient is unique, but the process typically takes one to three years.

Patients are urged to wear a retainer following orthodontic treatment to keep teeth in their optimal positions, as teeth will move as people get older and may try to crowd at the front of the mouth.

Chances are, you’ve heard these enough times to know that they are warning signs of a heart attack.

Still, too many people dismiss their symptoms, delaying emergency medical care when minutes really do matter and fast action saves lives.

Read on to hear what Morris Hospital Cardiologist Athar Saeed, M.D., has to say about heart problems and why it’s important to get help if your heart is giving you signs that something’s not right.

Q: What are signs that someone might have a problem with their heart?

Dr. Saeed: When it comes to your health in general, it’s important to know what your “normal” is and to pay attention to any deviation from what’s normal for you. We have some patients tell us that they used to be able to walk around the house without any difficulty, but now they experience significant shortness of breath just walking to the bathroom. For those patients, that’s not normal and could be a sign that something’s not right. Another common symptom that brings patients into our cardiology practice is dizziness, which can sometimes be a sign of heart disease. Palpitations is another one and just a fancy way to say you feel like your heart is racing, or you can actually feel your heart beating. Palpitations can be a sign of a heart rhythm problem, especially in women.

fatigue can be a sign of heart disease. Unfortunately, the older people get, the less obvious the symptoms of heart disease become. This is where it comes back to being in tune with your symptoms and your body and knowing what’s normal and what’s not normal for you.

Q: What are the most common heart attack warning signs?

Dr. Saeed: Chest pain is certainly one of the most common. I have had some patients report chest pain on the left side of the chest and some with chest pain on the right side. Some have said the pain feels like it’s in the back of the chest. What’s most important to remember is that any type of chest pain is a concern, especially if the pain gets worse with exertion.

Shortness of breath is another very common warning sign of a heart attack. Some people experience pain or discomfort in one or both arms, pain in the back, or the neck, jaw, or stomach. Cold sweat, nausea and lightheadedness sometimes occur with a heart attack.

Q: What should someone do if they experience any of the heart attack warning signs?

A: Calling 911 is always the fastest way to get lifesaving treatment for a heart attack. No one should ever hesitate to use a hospital emergency department if they think they might be having a medical emergency, including a heart attack. That’s what we’re here for. If it turns out to be nothing, that’s okay. We’d rather have you come and find out it’s nothing than have you not come when you should have.

Q: Why is it so important to get to a hospital quickly if it’s a heart attack?

Dr. Saeed: At Morris Hospital, we are very fortunate to have a cardiac catheterization laboratory, which is unusual for a hospital our size. The technology allows us to use x-ray guidance to take pictures of the arteries directly around the heart so we can see if there are any severe blockages. We can open up most blockages with a stent right in the cath lab. If somebody is having a major heart attack, the most important thing is to get the artery open as quickly as possible because permanent damage starts occurring within 90 minutes. At Morris Hospital, we have a team on call to respond to heart attacks 24 hours a day, 7 days a week. If we get the call from the ER, we’re at the hospital and ready to go within 30 minutes. We can also implant pacemakers and defibrillators in the cath lab.

The decibel levels of common noises Noise is everywhere. Though some noise can be very harmful, many noises are associated with something positive. For example, for many people the sound of birds chirping marks the arrival of spring, while others may instantly associate the sound of waves crashing on a shore with the relaxing spirit of summer. But not all noises should be welcomed with open arms, especially by people who are not protecting themselves from noise.

The Environmental Protection Agency notes that noise is often expressed in decibels, a unit of measurement that indicates the volume of sound. The greater the number of decibels, the louder the noise and the more harmful that noise is to a person’s ears. And no one is immune to the side effects of exposure to excessive levels of noise, which include hearing loss. For example, the Hearing Loss Association of America estimates that one in five teenagers in the United States experience some degree of hearing loss. That’s unfortunate, as hearing loss can have negative short- and longterm consequences. For example, the HLAA reports that even mild hearing loss can cause a child to miss as much as 50 percent of classroom discussion. The long-term consequences also are significant, especially for those who don’t seek treatment for hearing loss. According to the HLAA, people with unaided hearing loss earned an average of $20,000 less per year than people who used hearing aids or cochlear implants.

Many people suffer hearing loss after attending rock concerts or special events like fireworks shows. But various other noises also can have an adverse effect on a person’s hearing. The experts at Johns Hopkins Medicine report that normal conversation is usually around 60 decibels. These noises often measure at much higher decibels than normal conversation, potentially putting people’s hearing in jeopardy.

• Jet plane, siren, jackhammer: No one wants to walk past a jackhammer in use, and for good reason. Jackhammers measure at roughly 120 to 130 decibels, putting them on par with noises made by jet planes and sirens. Johns Hopkins notes that unprotected exposure to these noises can contribute to permanent hearing loss.

• Personal music players: Maximum volume on some personal music players, including smartphones, is 10 times as loud as

recommended listening settings. That’s concerning for everyone, but especially for parents of young people who grew up listening to music primarily through personal music players. Few people would put chain saws or radio-controlled airplanes directly next to their ears, and the same principle should apply when using personal music players. Exposure to noises around 110 decibels can lead to permanent hearing loss.

• Motorcycle: Many people have waited next to motorcycles at stoplights. When the light turns green, the sound from motorcycles as they speed off can be especially loud, averaging about 90 decibels. That can contribute to gradual hearing loss over time.

Morris Hospital Orthopedic Spine Surgeon Provides Help for Neck and Back Pain If you’re reading this article, there’s a good chance you’re going to experience neck or back pain at some point in your life. Some estimates say upwards of 85 percent of people will have neck or back pain in their lifetime, whether the result of activity, trauma or age-related conditions. The likelihood only increases after age 40.

“As we get older, on the other side of 40 for sure, most of us get to a point where we realize our neck or back is not what it was when we were 16,” says Dr. Mir Ali, board-certified orthopedic spine surgeon with Morris Hospital Orthopedics & Sports Medicine who received advanced training in spine surgery through a fellowship with Rush University Medical Center, Midwest Orthopedics. “I think most of us learn to live with it and manage it as best we can.”

But the question that next arises is: At what point do neck and back symptoms require help from a primary care doctor? Or even an orthopedic spine surgeon?

When to Seek Help The first sign medical help is needed for neck or back pain can be as simple as having lingering or reoccurring pain significant enough that traditional treatments or over-the-counter medications provide little to no relief. Other indications of needing to see a doctor include having to miss work due to the pain or experiencing numbness and tingling.

Dr. Ali says more concerning red flags start with shooting pain down the arms or legs, weakness in the upper or lower extremities, bowel or bladder problems, or having trouble walking.

“The areas of the spine are very delicate areas,” he says. “The spine obviously controls your whole body, so there are things that really worry us as doctors. If someone has any of those symptoms, they need to see their primary care doctor right away, their spine doctor right away, or they can go to the emergency room.”

Advancements in Treatment While Dr. Ali hopes people seek a doctor before severe symptoms set in, he is concerned that fears or misconceptions may keep them away. For instance,

while his specialty is called orthopedic spine surgery, the vast majority of his patients are at least initially treated without surgical intervention.

Depending on the patient’s condition, treatments can range from physical therapy, to medications for short-term pain relief, to cortisone injections, and as a last resort, surgery. Dr. Ali believes his approach to care delivers two essential components to his patients – options and education – so they feel they have evaluated all possibilities before surgery.

“At least in the short run, 75 to 90 percent of our patients are able to avoid surgery,” Dr. Ali says.

“For someone to come in to visit a spine specialist, obviously the pain has gotten to a point where it’s affecting their daily lives and their quality of life, but we really work up from the least invasive approach, saving surgery as a last resort.”

While the least invasive approaches can help in many situations, there are some conditions that cannot be truly resolved without surgical intervention. But even that prospect shouldn’t be feared, according to Dr. Ali. A procedure like anterior cervical fusion for arthritic/ herniated discs in the neck is typically a one-hour, outpatient surgery that provides patients immediate relief and results. Another procedure -- lumbar fusion for severe deterioration of lumbar discs -- has advanced from being a 6-hour surgery with a week-long hospital stay to now being a one-hour surgery with the possibility of going home the same day.

“Technology is always improving,” Dr. Ali says. “There are surgeries we do on 80- to 90-year-olds which used to take six hours 10 years ago. The fact is, we can do that surgery in an hour and a half today with less anesthesia so the patient can get up and go home the same day or the next day. It’s quite remarkable.”

Warning signs of mental health issues A 2017 study from the Institute for Health Metrics and Evaluation estimated that 792 million people across the globe lived with a mental health disorder. If that figure is startling, it’s likely even greater as a result of the pandemic.

• Difficulty understanding or relating to other people

an inability to carry out activities or handle problems and stress that arises each day

• Changes in sleeping habits or feelings of fatigue and low energy

• An intense fear of weight gain or concern with appearance

• Changes in eating habits, such as increased hunger or lack of appetite

Pre-adolescent children also can experience mental illness, which can manifest itself through these symptoms:

Data from the Kaiser Family Foundation indicated that about four in 10 adults in the United States reported symptoms of anxiety or depressive disorder during the pandemic.

That marked a significant uptick since the summer of 2019, when one in 10 adults reported such symptoms.

The pandemic undoubtedly posed significant challenges that affected the mental health of people across the globe. But those challenges won’t necessarily go away if and when life returns to pre-pandemic normalcy. Nor is there any guarantee that new challenges that pose a threat to individuals’ mental health won’t arise in the years to come. That reality underscores the importance of learning to recognize signs of mental illness in both adults and children.

The National Alliance on Mental Illness notes that each mental illness has its own symptoms, so anyone concerned about their own mental health or the health of a loved one can speak with a mental health professional for more information. In the meantime, the NAMI notes that these are some common signs of mental illness in adults and adolescents.

• Changes in sex drive • Difficulty perceiving reality (delusions or hallucinations, in which a person experiences and senses things that don’t exist in objective reality)

• Feeling excessively sad or low

• Inability to perceive changes in one’s own feelings, behavior or personality. Some individuals experience a lack of insight known as anosognosia, which NAMI describes as unawareness of one’s own mental health condition.

• Feelings of confusion or difficulty concentrating and learning

• Overconsumption of substances such as alcohol or drugs

• Extreme mood changes, including uncontrollable “highs” or feelings of euphoria

• Multiple physical ailments without obvious causes, such as headaches, stomach aches and vague and ongoing aches and pains

• Excessive worrying or fear

• Prolonged or significant feelings of irritability or anger • Avoiding friends and social activities

• Suicidal thoughts • Difficulties handling daily life, including
