FOX RIVER GROVE – About a year ago, Dr. Bruce Bell added a new payment model to his practice.

Under this model – a more affordable offshoot of concierge medicine – called direct primary care, about 40 of his patients, about 2 percent of his practice, pay a flat rate to ensure they get quality time with him.

An estimated 5,000 to 5,500 doctors and clinics have adopted direct primary care or concierge medicine systems, according to the Concierge Medicine Research Collective.

The most common specialties to use the model are primary care, family medicine, cardiology and pediatrics.

While a majority of doctors who enter concierge medicine or direct primary care would make the same decision again, Bell hasn’t seen it take off at his own Fox River Grove practice.

In fact, he has fewer patients going the direct primary care route – which at Bell’s practice is called concierge plus – than when he initially adopted it in August because they’re typically getting the same amount of time with him.

Dr. Stewart Segal, the founder of the Lake Zurich Family Treatment Center, has had more success with the model.

About 10 percent of his patients pay $1,800 a year to be in his direct primary care program, which takes up about 20 percent of his time mostly before or after his office hours.

He expected his wealthier patients to sign up, but found it was his average, fairly healthy patients who took advantage

of the program for its focus on preventative care.

He also was concerned his other patients would feel slighted, but he said that’s gone OK, too, because he still spends enough time with patients to develop good relationships.

In exchange for the flat payment, Segal’s direct access patients get to skip the front desk, dealing directly with one primary care nurse who will schedule their appointments and tests. They also have Segal’s cellphone number if they want to talk directly to him.

They’re also provided with a zip drive with all their medi-

cal records in case they need to see a doctor when they’re out of town.

The major benefit, though, is the physical, Segal said.

The nurse conducts an extensive physical with the patient, running an “extensive battery of tests,” which Segal then reviews in conjunction with the patient’s history to develop an action plan for improving the patient’s health.

Segal then refines that plan after meeting with the patient and conducting an examination.

He’ll check in a month later, depending on the patient, to make sure progress is being made.

“My direct access patients are a lot of fun to take care of,” he said. “To take care of somebody who truly wants to be healthy and who is truly going to follow what you prescribe … is a breath of fresh air.”

A physical like this would not be possible under an insurance plan, Segal said. Some insurers only cover one wellness visit a year, and some don’t cover any.

By taking the insurance company out of the equation, doctors can lower their overhead costs and therefore devote more time to fewer patients, advocates argue.

More than 40 percent of a primary care providers’ revenue goes to reimbursement costs, according to the advocacy group Direct Primary Care Coalition.

Cargo Equipment plans move to Huntley

By JIM DALLKE jdallke@shawmedia.com HUNTLEY – After more than 20 years in Elgin, Cargo Equipment Corp. is moving its business to Huntley.

The securement products supplier will announce the decision at Huntley’s Village Board meeting Thursday.

Cargo Equipment President Jeff Iden said the company’s need to expand, plus Huntley’s new interchange at Interstate 90 and Route 47, were the main reasons for the relocation.

“Huntley seems like a really nice town that has had

tremendous growth over the last 20 years,” Iden said. “It’s a great opportunity for us.”

The company plans to build a 40,000 square foot building – twice the size of its existing facility – in the Huntley Corporate Park and hire around 20 additional employees.

Construction will begin next spring. The building is expected to be completed by fall 2014.

Iden said his business was outgrowing its existing Elgin building. The facility didn’t even have enough parking for all of the company’s employees.

“We just needed more space,” Iden said. “Elgin is a nice town, but when we

jumped to Huntley, the town was very receptive … And being off of I-90 was really important to us.”

Cargo Equipment is the seventh major firm to announce operations in Huntley during the past two years, Huntley Village President Charles Sass said in a news release.

“Cargo Equipment’s move to Huntley not only brings new jobs but also verifies the significant positive economic impact of the new I-90/Route 47 interchange,” Sass said in the statement. “We’re privileged to have Cargo Equipment select Huntley for its operations.”

Cargo Equipment Corp. makes load securement equipment and supplies such as ratchet straps, tie down assembles, and winch straps. The company also makes its own line of custom ratchet straps and tie down assemblies.

The company’s planned Huntley facility, 13700 George Bush Court, represents the significant growth the company has made since it first began in 1962, Iden said.

“It’s going to be a whole new ballgame for us when we move out there,” Iden said. “We’ll have the space and room to grow. That’s what we’re really doing it for.”

By LINDSAY WEBER editorial@nwherald.com HUNTLEY – Six brain surgeries, five rounds of chemotherapy, stage 4 high-grade glioma and one tumor. This would be a challenge for any adult, yet it all rests behind the smiling eyes of 22-month-old Huntley boy, Matthew Erickson.

“When Matthew was born we were given two options: hospice or treatment. We chose treatment,” said Matthew’s mother, Sue Erickson.

At just four days old, Matthew underwent his first brain surgery and soon began chemotherapy. He spent the first 13 months of his life in the hospital fighting this aggressive disease. Now, Matthew is 14 months chemotherapy-free but will never live a life without cancer.

“The doctors are floored by how well he’s doing. When we go to the hospital they say ‘here comes the miracle,’ ” Erickson said.

The Erickson family attended the Flyweight Bowl in the afternoon of Oct. 26 at Marlowe Middle School, where they were given a check for $3,600 from Tackle for Pink.

Parents in the Huntley Youth Football program nominated local families in need of assistance because of the financial burdens of a family struggling with a loved one affected by cancer. The Erickson family was chosen as the 2013 recipient.

Bouncing and giggling in the arms of Huntley Mustangs coach, Chris Behles, one would never know the challenges this little fighter has overcome.

“The kids know there’s a little boy in the community that has challenges that they don’t and they need to help him out,” Behles said.

Tackle for Pink was founded by Huntley Youth Football President Ron Ricciardi in memory of his younger brother, Pasquale, who at age 21 lost his battle with pediatric cancer. Ricciardi has seen firsthand the struggles of families dealing with pediatric cancer and wanted to bring aware-

Erickson said Matthew is doing well, and they’re looking forward to celebrating his second birthday Dec. 11.

“He’s always happy and smiling,” Erickson said. “He wakes up in the morning and pops his eyes open and just

Matthew’s tumor has not grown in 14 months, but he continues to suffer from hydro-

cephalus, which is a buildup of fluid in the cavities within the brain increasing the size of the ventricles and putting pressure on the brain. While there still

are many challenges ahead, the family remains hopeful.

“He’s thriving,” Erickson said. “There’s a light in his eyes.”

Bus issue lingers in Rutland Township

jbustos@shawmedia.com As people age into their golden years, they have a desire to remain independent and live at home. If driving is no longer an option, many rely on public options or volunteer services.

One such program is the diala-ride Senior Express in McHenry Township, which operates in McHenry, Johnsburg, Ringwood and Wonder Lake.

“It keeps seniors in their home and out of institutions and gives them the quality of life they deserve,” McHenry Township Supervisor Donna Schaefer said.

The program provides seniors and people who are disabled anywhere with door-to-door service within the service area.

In MCRide, people 60 and older and people with disabilities are eligible for trips in the service area. They are able to reserve a ride at least an hour before pickup time.

Service goes to McHenry, Woodstock and Crystal Lake, to the Fox Lake Metra station and along Route 120 between Woodstock and McHenry, Route 14 between Woodstock and Crystal Lake, and Route 31 between Crystal Lake and McHenry.

Schaefer said MCRide helped improve senior transportation in the area.

“It’s getting a little better,” Schaefer said. “There are still barriers, but the MCRide program made it much better.”

The MCRide program is paid for with money from Pace, Crystal Lake, McHenry, Woodstock, Dorr Township, Greenwood Township, a McHenry County Senior Services grant and two federal grants.

Fares are based on a person’s trip length.

With MCRide, trips cannot be booked after 7 p.m., Pace spokesman Patrick Wilmot said.

“We haven’t yet seen a level of demand that would require that,” Wilmot said. “If we reach a point where there is demand to extend those hours, we can always do that if need arises.”

For Pace, 6 percent of its riders in the region are seniors, Wilmot said.

In Pace’s dial-a-ride programs, which are curb to curb, the drivers pick up riders in front of their houses or buildings in fully accessible buses. Drivers generally don’t pull into someone’s driveway or exit the vehicle to help someone in, Wilmot said.

There are liability issues, he said.

unattended can be an issue as well, especially if there are other passengers on board the 15-person buses.

“It can make a process of picking up and dropping off passengers a little longer and can cause delays,” Wilmot said.

Algonquin Township also offers senior transportation with curb-tocurb service. Riders need to give advanced notice, however.

Taking a bus isn’t the only way to get around the county. Sarah Ponitz is the executive director of the Senior Care Volunteer Network, which coordinates volunteers to provide rides for seniors, among other things.

The rides could be for medical appointments, grocery shopping, volunteer opportunities or social engagements.

When volunteers provide the lifts,

they help the senior from the door to the car and inside the senior’s destination. The service is free, but seniors have to register ahead of time, and reservations have to be made three days in advance for trips within the county and five days in advance for trips outside the county.

The network limits trips to up to 25 miles outside the county.

“I think there are a variety of ways for them, but there are a lot of challenges,” Ponitz said. “Options are available, but there’s some restrictions to some programs out there as well.”

Ponitz said the Senior Care Volunteer Network provides up to 1,200 rides a month.

“I think No. 1 is it’s not a stranger providing them with transport; there’s a friendship formed,” Ponitz said. “We help make sure they get through the door. Some [drivers] will wait at the appointment.”

Having services that pick up seniors at their doorstep is helpful, especially in situations where the nearest fixed-route bus stop may be blocks away.

“If a Pace stop is two or three blocks away, if it’s 10 degrees below zero, and you’re with a walker, it can be difficult,” Ponitz said.

HUNTLEY – Rutland Township officials still disagree on a bus service for their senior and disabled residents, even after some of their Sun City Huntley residents were officially cut from nearby Grafton Township’s system.

The Grafton Board earlier this week unanimously agreed to end service Nov. 30 to the Rutland riders in Huntley’s retirement community. But the board’s decision left open the opportunity for the service to continue, if Rutland officials decide to fund a portion of Grafton’s $80,000-a-year bus system.

“All indications from Rutland are they don’t seem interested in moving forward. ... At this point in time, as far as we are concerned, we are behind it,” Grafton Supervisor Jim Kearns said.

Kearns first approached the Rutland Township Board in August about funding $24,000 toward Grafton’s bus service. Nearly 57 percent of all the riders for Grafton’s service live in Rutland Township.

Kearns earlier this month revised his offer and asked Rutland to contribute $14,400 annually. In exchange, Grafton would hike Rutland rider fees to cover the remaining $9,600.

But Rutland trustees delayed a vote on Kearns’ proposal and passed on the opportunity to join a Kane County transportation program, which would have provided bus service to Rutland residents outside of Sun City.

Despite the resistance, Rutland Supervisor Margaret Sanders plans to take a vote on both proposals when the board meets Nov. 12 to address the issue. She said an average Rutland homeowner would see a $2 increase to their township property-tax bill if the board approved Kearns’ proposal and joined the Kane County program.

If service is cut Nov. 30, Grafton also would return some unspent dollars to Huntley.

Deer season poses driving hazards

By LAWERENCE SYNETT lsynett@shawmedia.com It happens every fall – an unsuspecting motorist is driving along and strikes a deer that is trying to cross the road.

Deer mating season, combined with hunters combing the woods for big bucks, around this time each year means more four-legged grazers frolicking near the roadways, creating a potentially hazardous situation for motorists traveling in McHenry County.

“They never look both ways because they have other things on their minds,” said Tom Micetich, deer project manager for the Illinois Department of Natural Resources. “Deer figure out it’s not safe to be in the woods with the guys in orange, so they take their chances with a Buick.”

Despite a decline in the number of deer-related crashes in McHenry, Lake and Kane counties combined the past

two years, numbers are up in McHenry County, according to data from the Illinois Department of Transportation.

The three-county area has accounted for about 6 percent of the total number of accidents statewide in 2011 and 2012.

McHenry, Lake and Kane counties had 888 deer-related accidents with 36 injuries in 2012 compared with 1,006 and 47 in 2012, according to IDOT. In McHenry County, there were 279 accidents with 13 injuries in 2012, up from 251 and 13 the previous year.

The year-over-year IDOT data track deer-related accidents involving vehicle damage of more than $1,500. Prior to 2009, accidents involving damage of more than $500 were recorded by the state.

Statewide, there was a 14 percent decline in traffic crashes involving deer, down to 15,489 in 2012 compared with 18,044 in 2011, data show. That includes four deaths and 608 injuries in 2012, down from

six deaths and 613 injuries the previous year.

Deer-related accidents in the three counties were responsible for one death in the past three years, according to the state, none in McHenry County.

The chance for more deer on the roadway typically increases in mid-October through December with the beginning of mating season as well as hunting season. Crop harvesting season also moves deer from one locale to another.

Early mornings and evenings – especially dawn and dusk – are considered to have the highest deer activity near roadways, experts said. Areas with creeks and rivers, woodlands, and wooden fence rows intersecting roads tend to have the most accidents.

“The road isn’t going to stop them from moving around,” Micetich said. “They can pop up anywhere.”

A sign of relief this deer season may come in the form

of a recent survey released by auto insurer State Farm that predicts a 10 percent decrease in the likelihood of a collision with a deer to 1-in-214 chance, compared with a 1-in-162 chance last fiscal year.

The survey was based on claims data and state licenses driver counts from the Federal Highway Administration.

Nationally, the auto insurer calculated the chance of a motorist striking a deer over the next year at 1-in-174.

Deer hunting began in October with archery season, which runs through Jan. 19, 2014. Firearm season has two time periods, Nov. 22 through Nov. 24, and Dec. 5 through Dec. 8.

Harvest numbers taken in January were similar to last year, when more than 200 deer – 20 percent of the total harvest – were killed during archery and firearm season, according to the McHenry County Conservation District.

“We expect hunters to have

a really good year again,” said Brad Woodson, natural resource supervisor for the conservation district. “It’s a very balanced population throughout the county.”

Factors that could affect deer population this season include a lower presence of those found with chronic wasting disease and epizootic hemorrhagic disease after an outbreak last year.

Chronic wasting disease attacks the brains of infected deer, moose and elk, causing them to become emaciated, display abnormal behavior and loose coordination, and eventually die.

Epizootic hemorrhagic disease, a viral disease, is spread through biting gnats, which cause high fever and severe internal bleeding in deer.

All hunters are encouraged to get their deer tested for disease, but there has been no connection between human illnesses from eating the infected meat, Micetich said.

The successful candidate will also understand and embrace the idea that newspapers are part of the 24-hour news cycle.

Ideal candidates will be willing to take on a broad range of assignments, from spot news and government meeting coverage to features, and must tell stories that show readers how the events and people are relevant to their lives. Our reporters are expected to generate their own story ideas and develop sources on their beat, as well as take assignments from the news editor.

At least one year of professional experience preferred, but recent graduates with outstanding internship experience are encouraged to apply. Solid knowledge of AP Style and grammar required, as is ability to write clear, concise copy.

Must have a valid drivers license, dependable transportation and proof of insurance. Experience with the Web and multimedia is a plus.
