4. Nominations must be submitted on official forms provided in ads published in the Creston News Advertiser and Southwest Iowa Advertiser and available at the Creston Chamber of Commerce office, 208 West Taylor St. Only one nomination submission per person, please.

5. Nominations must be signed.

6. A committee comprised of representatives of the Creston Chamber of Commerce, the Creston News Advertiser and the community at-large will select the winner from among the nominees.

7. The judges will consider, among other things, the candidates’ personal integrity and commitment to the community.

8. The Chamber of Commerce will host the winner and up to three guests at the Chamber’s annual meeting and dinner. The winner will receive a plaque from the Chamber.

9. The CNA will contribute $100 in the winner’s name to his/her local charity of choice.

10. Judges’ decisions are final.
