The focus of this position is to facilitate an interactive, decision-support process where aging consumers, adults with disabilities and their families receive guidance in their deliberations to make informed choices about available support services and long-term care options. Responsibilities include remaining knowledgeable of the services and supports available in the service area, conducting in-home assessments, participate in multi-disciplinary teams to enhance collaboration of services, deliver presentations to consumers, providers and community groups, maintain records, enter data, prepare reports. Must be able to work independently and maintain consumer confidentiality, possess creativity and problem solving skills, possess good oral and written communication skills and operate a computer.
