or local families dealing with the emotional, physical, and psychological effects of Alzheimer’s disease and other types of dementia, the feelings of stress and loneliness can become overwhelming.

It helps, however, to know these feelings are normal and there are resources to help those with dementia and their families.

The incidence of dementia is surging by 50 percent every 20 years, and affects many

in the Rock River Valley, according to Helen Slagle, a registered nurse and owner

of Home Instead Senior Care. The Rock River Branch of the Alzheimer’s Association provides assistance to those in need of support or knowledge about the changes affecting their loved ones.

One such form of assistance is the lending library in the Dixon branch, which includes informative books, puzzles and videos. The Association’s seminars and programs are especially important and are provided at little or no cost.

esearch shows that approximately one-third of all American adults are home caregivers, according to Helen Slagle, 60, a registered nurse and co-owner of Home Instead Senior Care in DeKalb.

Slagle, along with her husband Wayne, 66, and daughter Samantha, 35, help on average 40 people a year, many with Alzheimer’s and other types of dementia.

Home Instead strives to provide care to seniors in need of assistance with daily activities such as grooming, meal preparation, light housekeeping, and running errands. Most important, their trained caregivers provide companionship and support for seniors and their families.

Slagle says there are many free and inexpensive resources available to caregivers. It is important for caregivers to realize they are not alone and that they need to take the time to care for themselves.�

Such assistance comes in several forms.

Many seminars and online forums are available in the area. The Alzheimer’s Association provides seminars to help caregivers understand their roles as well as help them gain knowledge of what is happening to their loved ones.

Perspectives on Caregiver Stress is one of the many seminars available. The seminars are held throughout the year and cost only $5.

nities to learn about the legal issues that are sometimes put off by families as they try to deal with the emotional and physical aspects of dementia.

Accountants and lawyers provide advice regarding wills and power of attorney – extremely important issues that many families forget in the aftermath of finding out that their loved one has been diagnosed with this disease, Sweitzer said.

The programs are designed for those afflicted and their loved ones – even children.

“Support and understanding are among the greatest needs for those with dementia,” Sweitzer said.

For those interested in websites, HelpforAlzheimersfamilies.com offers families free downloads of more than 500 solutions to dementia-related problems.

The solutions cover a range of topics, from grooming to behavioral and personality changes, that can occur in seniors and those with dementia. The site also helps families find 24/7 assistance.

“Our programs are available at little to no cost for those who attend,” Sweitzer said. “If the program does have a fee, it is normally only $5. This is to help cover the cost of materials, but we will never turn anyone away for their inability to pay.”

Another source that Sweitzer recommends for caregivers is becoming active in the support groups available through the Association.

“These groups allow caregivers to unwind and discuss the issues they are going through without any judgment,” she said. “Every person in the group recognizes the feelings that each other is expressing. For many, just talking to someone else in a similar situation helps them to relieve stress. It has been shown that caregivers have a significant chance of dying before those they are caring for because they begin to neglect themselves.”

Caregiverstress.com provides free assistance and information for family caregivers to help them determine whether they

are at risk for health problems related to stress.

A smartphone application called the “Alzheimer’s and Other Dementias Daily Helper” offers help with problem solving.

Slagle finds the increase in assistance for families dealing with Alzheimer’s and other forms of dementia promising.

“As a family caregiver myself, and as a nurse, I wish I had had more information,” she said. “Treatment and care is very important for those with dementia.

Boomers 2013 Hundreds of thousands of Americans are receiving medical devices that were once considered nearly exclusive to the elderly. The shift is profoundly changing patient care and expanding the fortunes of the medical-technology industry while amplifying concerns over the safety and oversight of some products.

The movement is so dramatic that the futures of major medical device companies such as Medtronic, St. Jude Medical and Boston Scientific are increasingly tied to younger groups and the new markets they represent.

Middle-age Americans, in particular, are driving this trend as they seek ways to remain physically active. The number of patients ages 45 to 64 who had a hip replacement more than doubled from 2000 to 2010, according to a Star Tribune analysis of data from the U.S. Department of Health and Human Services. The increase was more pronounced for knee replacements, rising 213 percent. “This is huge,� said Dr. Robert Hauser, a cardiologist at the Minneapolis Heart Institute who has studied the safety and effectiveness of heart devices for years. “I think it’s a tremendous step forward, but there are issues that need to be dealt with.�

Though widely celebrated, some treatments have been shadowed by reports of devices or other medical products faltering – defective wires in defibrillators, failing artificial hips and leaky drug pumps, among them. Patients have suffered complications, severe pain and even death. Every year, 25 to 40 medical devices are recalled for high risk – meaning a patient’s life could be in jeopardy.

Device companies are facing thousands of patient lawsuits challenging the safety of some devices, and federal regulators are under greater pressure to intensify their oversight. At the same time, device makers are spending millions to promote their products to doctors and patients while simultaneously pushing to simplify governmental reviews to quicken their products’ path to market.

9 are converting technology to regulate heart rhythms or treat diabetes into new tools for a wider range of ailments, including overactive bladders, anxiety and migraines. The result is a wider range of devices and other medical products being implanted or connected to more nonelderly patients.

But this growing use of medical hardware should be tempered by a call for safety, public disclosure and caution, consumer advocates say. “We’re not talking about computers or cars or toasters,� said Lisa McGiffert, director of the Consumers Union’s Safe Patient Project, a group that campaigns for better medical practices. “We’re talking about things that go inside people’s bodies.�

No area of medicine is seeing more nonelderly patients turn to medical devices than orthopedics.

In 2000, one-third of the estimated 157,000 Americans who had hip or knee replacements were younger than 65. Ten years later, the number had nearly tripled to 430,000, meaning almost half of those procedures were performed on these younger patients.

Two contradicting forces are pushing patients toward artificial joints. The rising rate of obesity has led to more cases of deteriorating hips and knees from excess weight. And yet, more Americans are playing sports or exercising in their 30s, 40s and 50s, which puts more wear and tear on their joints.

To get active again – and quickly – patients are more willing to consider joint replacement at a younger age than they were a decade ago, said Dr. Daniel Berry, chairman of orthopedic surgery at the Mayo Clinic in Rochester, Minn. “We’re seeing higher demand,� Berry said of younger patients who want a new hip. “And they are going to use it a lot harder than somebody who is older.�

Patients who get a device at a younger age usually must replace it more often. Each replacement means an expensive surgery, possible complications and significant rehabilitation.

Boonstra, 59, retired in May. She plans to read (her favorites are mysteries with a humorous touch, such as those by Janet Evanovich) and sail. She also plans to remodel a house that has been in her family for 75 years – and eventually move into it.

I can’t say enough about the wonderful attention she receives from the staff. There is always a smile and a hug. Mother loves the food made right there in their kitchen. It’s just like home and if Mother needs something special, they go out of their way to provide it even when they have many other meals to prepare.

Her room is bright and cheerful and of course getting her hair done inhouse always makes her day.

Ever been on the “Market Timing Roller Coaster�? Many people find themselves drawn in by the allure of beating the market and end up on an emotional roller coaster of financial decision-making. A big surge in the market occurs, the roller coaster goes up and perhaps a bit of adrenaline and greed sets in and you think, “Buy!� Since what goes up must come down, the market sometimes dives, the roller coaster drops, fear takes over and you exclaim, “Sell!�

Allowing emotions and investing to mix is a dangerous game and it often results in poor financial decisions being made.

Of the many variables that can influence the performance of a portfolio, for a lot of investors their behavior is probably the most important. Emotions can lead investors to take the wrong action at the wrong time. It is easy to stay disciplined and adhere to your financial plan when the markets are doing well, but it is much tougher to remain on course when the markets become volatile.

Most people are familiar with the investment adage, “buy low and sell high.� It’s a simple enough concept, but when human emotions get involved, simplicity often goes out the window. Knowing the precise time to buy and sell a stock is a rare skill. Most people end up executing trades at the wrong time and instead buy high and sell low.

The role that emotion plays in the success or failure of an investment strategy should not be overlooked. How do you put aside emotional urges and stay

on track financially? It’s generally known that investors are highly unlikely to be able to exploit the market, and further that market timing usually results in lower returns than those generated from a buy, hold and rebalance strategy. “How much did the average investor lag the average fund over the past 10 years ended 2012? A total of 0.95% annualized. The average fund returned 7.05%, but the average investor netted 6.1%. That’s a good chunk of the return.�[1] So why is this? Shouldn’t investors be able to make smarter investment decisions with all the technology and information available today? It appears that the constant flow of information is actually causing more harm than good for the average investor by creating an environment where investors are inclined to buy and sell based on pure emotion.

The 2006 study, “All That Glitters: The Effect of Attention and News on the Buying Behavior of Individual and Institutional Investors� found that individual investors buy stocks that grab their attention, meaning “stocks in the news, stocks experiencing high abnormal trading volume and stocks with extreme one-day returns.�[2] Because investors are so overwhelmed with the many stocks to potentially buy, many investors consider purchasing only the stocks that really catch their attention.

The study found that the attention-driven buying patterns they documented “do not generate superior returns.� In fact, they concluded that “most investors will benefit from a strategy of buying and holding a well-

diversified portfolio.�[3] So how can you stay on track?

HICAGO – One day in 2011, Virginia Koerber, Lake Villa, recalled, she “woke up with a new life.” At 65, she got divorced.

“At an age when I had thought we’d travel and be retired, I was on my own,” she said.

People may marry with “forever” intentions, Koerber said, noting that her parents were married for 56 years, but “it doesn’t always turn out that way.”

Now, Koerber is part of a growing club: people who divorce after age 50.

The divorce rate for this group doubled between 1990 and 2010, according to a study by the National Center for Family and Marriage Research at Bowling Green State University in Ohio.

“This surprised us, because the rate for younger people has leveled off,” said lead researcher Susan Brown, a sociology

Brown attributes the increase to more seniors (“that huge segment of baby boomers”), more women with careers (“they don’t have to stay in empty-shell marriages for the money”) and more people ignoring their churches’ nodivorce rules.

Ironically, Brown said, some of the

same factors that invigorate longtime marriages, such as children leaving home, cause others to end.

“The kids are gone, the marriage has been over for a while and one spouse decides she can’t take it anymore,” said James Pritikin, a Chicago-based lawyer and a fellow of the American Academy of Matrimonial Lawyers.

The over-50 divorce rate is even higher among those who remarried.

“They have a lot of complications because of stepchildren and financial and health care decisions,” Brown said.

“But they also know they can divorce, that life goes on.”

Demographics affect late-in-life divorces, the study found. Blacks have the highest rates, followed by Hispanics, then whites and Asians. The more education, the lower the rate. The unemployed divorce more than workers do; retirees have the lowest rate.

Botox approved to treat crow’s feet

t’s getting easier for baby boomers to hide their years.

Federal regulators for the first time have approved Botox injections to temporarily ease the appearance of crow’s feet.

The Food and Drug Administration recently approved the drug, made by Irvine, Calif.-based Allergan Inc., to treat wrinkles that form on the outside edge of the eyes.

The decision comes 11 years after the FDA approved Botox for the temporary improvement of wrinkles between the eyebrows, known as frown lines.

Now, Botox can be used to treat frown lines and crow’s feet at the same time.

The FDA approval came after Allergan studied the use of Botox in 833 adults with crow’s feet who randomly received Botox or a placebo injection. Patients who received Botox had fewer visible wrinkles than those who received the placebo drug, the FDA said in a news release.

Lack of planning leaves childless boomers wondering who will handle long-term care

ACRAMENTO, Calif. – Karen Spencer was single until she was 48. Like more than 20 percent of baby boom generation women – twice the number as in previous generations – she never had children.

The retired political consultant, now 60 and married, has diligently pieced together a long-term care insurance policy, a range of investments and a small pension to make sure her financial needs will be covered in the future.

But the question that concerns her and many of America’s 15 million childless baby boomers is more emotional and poignant: Without offspring to help them, who will take care of them when they grow old?

“I have nieces and nephews who would show up, but I don’t want them to feel like it’s necessary to take care of me,” said Spencer, who lives in Granite Bay, Calif., with her husband, 68-year-old retired contractor Mike Twigg, and a

Whose love and attention, in short, will help keep them anchored in the world?

So far, aging experts don’t have good answers.

“These are issues that we’ll have to grapple with as a country,” said Lynn Feinberg, AARP policy expert on caregiving.

In a real way, facing old age alone is a women’s issue. Well ahead of the aging of the baby boom, U.S. Census figures show that almost 11 million Americans have already lost their spouses – and two-thirds of them are women who outlived husbands requiring months or years of caregiving before death.

Lacking a spouse, they count on their kids and grandkids to help. After all, family caregiving has traditionally been the heart of long-term care, with more than 65 million Americans today providing care to ill or aging relatives.

As Feinberg said: “When somebody needs long-term care, they typically turn to their children.”

But already, 16 percent of frail adults aged 85 and older have no surviving children to help provide their caregiving, according to AARP Public Policy Institute figures. By 2040, when the oldest boomers are in their 90s, that number will reach 21 percent.

On top of the more than one-fifth of boomers who are childless, census statistics indicate that another 17 percent had only one child.

That demographic reality leaves experts worrying about the country’s dwindling supply of potential family caregivers in years to come.

“The expectation on a nephew to care for his elderly aunt as well as his own parents and children presents a real challenge,” Feinberg said.

“There’s an increasing reliance on fewer family members to take care of older people.”

Even so, both baby boomers and their elders indulge in a rather startling lack of planning for their care needs in old age: A recent Centers for Disease Control study shows that only 37 percent of older adults who aren’t in nursing homes or hospice care – and only 15 percent of all adults – have completed legal proxies specifying who can make health care decisions on their behalf.

“I haven’t made that arrangement with anybody yet,” said Jan Haag, a Sacramento City College professor.

“Perhaps it would be my sister, but I haven’t done that. That’s something we need to think about at any age. It’s important.

“I’m embarrassed I haven’t taken care of that.”

At 54, she is widowed and never had children, although she has from time to time taken in young adults whom she calls “loaner kids” – a niece, for example, and the son of friends who, all these years later, still sends her a Mother’s Day card each May. “I have a great belief that help will be there as I need it,” she said.

Not long ago, a circle of her friends banded together to help another friend – an elderly, childless widow with encroaching dementia – find residential care, she said. Haag’s 81-yearold mother, meanwhile, has set aside funds to assist with her placement in senior living when she can no longer live alone.

“I applaud that,” said Haag. “Please, God, let me have enough money that I can have in-home care or go to a nice facility one day.

“Maybe I need to think about longterm care insurance.”

Only 10 percent of California residents have bought those plans, according to the California Partnership for Long-term Care – but studies show that most childless older adults will probably need them.

Recent University of Southern California research on aging indicates that, although people 75 and older without children score equally well as those with offspring in terms of comfort and contentment, the childless were far more likely to end up in costly nursing care.

‘Mortality index’ helps doctors make decisions on costly medical procedures CHICAGO (AP) – Want to know your chances of dying in the next 10 years?

Here are some bad signs: getting winded walking several blocks, smoking, and having trouble pushing a chair across the room.

That’s according to a “mortality index” developed by San Francisco researchers for people older than 50.

The test scores may satisfy people’s morbid curiosity, but the researchers say their 12-item index is mostly for use by doctors. It can help them decide whether costly health screenings or medical procedures are worth the risk for patients unlikely to live 10 more years.

It’s best to take the test with a doctor, who can discuss what the score means in the context of patients’ own medical history, the study authors say.

The index “wasn’t meant as guidance about how to alter your lifestyle,” said lead author Dr. Marisa Cruz of the Uni-

versity of California, San Francisco.

Instead, doctors can use the results to help patients understand the pros and cons of such things as rigorous diabetes treatment, colon cancer screening and tests for cervical cancer. Those may not be safe or appropriate for very sick, old people likely to die before cancer ever develops.

The 12 items on the index are assigned points; fewer total points means better odds.

The highest, or worst, score is a 26, with a 95 percent chance of dying within 10 years. To get that, you’d have to be a man at least 85 years old with all the above conditions.

For a score of zero, which means a 3 percent chance of dying within 10 years, you’d have to be a woman younger than 60 without any of those infirmities – but at least slightly overweight.
