Recommended men’s health screenings (MS) Men need to be proactive in regard to monitoring their overall health.

This includes seeing their doctors for regular wellness visits and keeping up with recommended screenings to catch and/or prevent illnesses. While men and women can experience the same conditions, health experts recommend specific tests and screenings for men in various age groups.

Johns Hopkins Medicine advises that men are at risk for certain conditions, including several types of cancer, such as prostate, colon and lung cancers. However, Harvard Medical School states men are less likely than women to get routine physical exams and screenings. An American Academy of Family Physicians survey found that 55 percent of men had not seen their doctors for a physical exam in the previous year, even though 40 percent had a least one chronic condition.

While many screenings are recommended starting at age 40 or 50, men should discuss family histories and risk factors to determine if testing should begin earlier. The following are important health screenings to consider.

• Prostate-specific antigen test: A PSA is a blood test that measures how much prostate-specific antigen is in the blood. Measuring PSA has been a standard for prostate cancer screening for 30 years. General guidelines indicate PSA screening begin at age 55. However, having at least one first-degree relative with prostate cancer could necessitate earlier testing.

• Colorectal cancer screening: Colorectal cancer screening generally occurs between ages 50 and 75. Tests include fecal occult blood tests, stool DNA tests, colonoscopy, and contrast barium enemas. Doctors will determine which tests are applicable and how often to conduct them.

• Diabetes: Men who have a BMI over 25 are overweight and should consider a diabetes screening. In addition, blood pressure above 130/80 mm Hg or other diabetes risk factors could require a blood test to check for elevated glucose levels.

• Screenings from page 3 • Abdominal aortic aneurysm: Guidelines recommend a one-time screening for men who have smoked between the ages of 65 and 75.

• Hepatitis B and C: Men are at increased risk for infection if they have had unprotected sex with multiple partners, received blood transfusions or transplanted organs before June 1992, are healthcare workers who have been stuck by needles, or travel to regions with high rates of the hepatitis B virus.

• Lung cancer screening: Men should undergo a lung cancer screening through low-dose computed tomography if they are over age 55, have a 30-pack-per-year smoking history and currently smoke or quit within the past 15 years.

• Testicular cancer screening: Testicular cancer is one of the most common cancers among young men, says Johns

Hopkins Medicine. Early screening can include self-checks. Routinely checking the testicles for any lumps or unusual features while showering can help detect testicular cancer. Doctors may order a painless ultrasound if something is discovered.

Health screenings are an important component of men’s health care. Now is the time to have a discussion with the doctor about which screenings are necessary.

What is body mass index?

People who are attempting to improve their physical fitness may hear a lot about body mass index, or BMI, and wonder just how important a good BMI is to their overall health. That can be hard to determine, especially for people who are unsure just what BMI measures.

According to the Centers for Disease Control and Prevention, BMI is a person’s weight in kilograms divided by the square of his or her height in meters. For those unfamiliar with the metric system, that explanation of BMI might not be too helpful. So how is BMI used and what purpose does it serve? BMI might be most useful in a screening capacity, as the CDC notes it is an inexpensive and easily performed method of screening for weight category.

For example, by calculating a patient’s BMI, doctors can quickly determine if the

patient is underweight, has a normal or healthy weight, or is overweight or obese.

Conditions associated with being overweight include heart disease and diabetes, so an easily calculated index determining if a person is overweight (or underweight or obese) can be an effective tool in combatting some potentially dangerous conditions. But physicians won’t base their opinions on patients’ overall health based on BMI alone. In fact, if physicians determine a BMIs is a health risk, they will likely perform further assessments, including skinfold thickness measurements and evaluations of things like eating habits, physical activity and family history.

So while BMI might be somewhat confusing, it can still be a valuable tool for people looking to determine just how healthy or unhealthy they are.

Chiropractors can help with various ailments Anyone who has experienced pain understands that it can be debilitating. Pain can make it challenging to manage everyday tasks and can affect relationships and physical and mental health in many ways.

Pain medications were once the norm, but with the rise of the opioid epidemic, it may be wise to take a different approach to pain treatment.

Statistics from the Centers for Disease Control and Prevention, the Agency for Healthcare Research and Quality and the United States Department of Health and Human Services reveal that as many as one in five people received prescription opioids long-term for noncancer pain in primary care settings in 2016. While opioid

use for pain does not always lead to abuse of these medications, opioid addiction has become a widespread problem across North America. These same agencies report that approximately 1,000 people are treated in emergency departments in the United States each day for misuse of prescription opioids. Between 2016 and 2017, opioid poisoning hospitalization rates went up by 8 percent in Canada, resulting in an average of 17 hospitalizations each day.

Chiropractic care may be one avenue to pursue as a viable alternative to pain medications. While chiropractic manipulations are most often associated with back and neck pain, there are many different benefits — some of which may surprise

• Migraines and headaches: Research shows that spinal manipulation can be an effective way to treat migraines, tension headaches and headaches that begin in the neck. In clinical trials conducted at Macquarie University, 72 percent of migraine sufferers had noticeable or substantial improvement after a period of chiropractic treatment.

• Accident injuries: Chiropractors can assess musculoskeletal anomalies caused by motor vehicle accidents as well as slips and falls. Whiplash is a common issue in car crashes, and this may be addressed by a chiropractor.

• Anxiety and stress: The fight or flight mechanism that is activated when a person experiences stress and anxiety can take its toll on various parts of the body.

Because stress involves nerve impulses, chiropractors may be able to work primarily with the spine where these impulses

originate to help release tension and nerve irritation. In addition, adjustments can improve blood circulation, which may help signal the brain to turn off its “rally to action” response to stressful situations.

• Fibromyalgia: Fibromyalgia is marked by widespread musculoskeletal pain in the joints, muscles, tendons, and other soft tissues. It can be combined with temporomandibular joint syndrome, or TMJ, as well as irritable bowel syndrome, among other illnesses. Chiropractors can remove subluxations, which are misalignments of the vertebrae in the spine and joints that lead to pain responses. A Florida State University study found that those with fibromyalgia who combined resistance training with chiropractic treatment two times a week saw improvements in their symptoms.

Chiropractic care may be an asset for pain management, as well as a way to improve overall health.

Determining breast cancer stage

When receiving treatment for breast cancer, women will learn about cancer staging. According to the nonprofit organization Breastcancer.org, determining the stage of the cancer helps patients and their doctors figure out the prognosis, develop a treatment plan and even decide if clinical trials are a valid option.

Typically expressed as a number on a scale of 0 through IV, breast cancer stage is determined after careful consideration of a host of factors. The staging system, sometimes referred to as the TNM system, is overseen by the American Joint Committee on Cancer and ensures that all instances of breast cancer are described in a uniform way. This helps to compare treatment results and gives doctors and patients a better understanding of breast cancer and the ways to treat it.

Breastcancer.org notes that the TNM system was updated in 2018, but before then was based on three clinical characteristics: • T: the size of the tumor and whether or not it has grown into nearby tissue • N: whether the cancer is present in the lymph nodes • M: whether the cancer has metastasized, or spread to others parts of the body beyond the breast While each of those factors is still considered when determining breast cancer stage, starting in 2018, the AJCC added additional characteristics to its staging guidelines, which make staging more complex but also more accurate.

• Tumor grade: This is a measurement of how much the cancer cells look like normal cells.

• Estrogen- and progesterone-receptor status: This indicates if the cancer cells have receptors for the hormones estrogen and progesterone. If cancer cells are deemed estrogen-receptor-positive, then they may receive signals from estrogen

that promote their growth. Similarly, those deemed progesterone-receptor-positive may receive signals from progesterone that could promote their growth. Testing for hormone receptors, which roughly two out of three breast cancers are positive for, helps doctors determine if the cancer will respond to hormonal therapy or other treatments.

Hormone-receptor-positive cancers may be treatable with medications that reduce hormone production or block hormones from supporting the growth and function of cancer cells.

• HER2 status: This helps doctors determine if the cancer cells are making too much of the HER2 protein. HER2 proteins are receptors on breast cells made by the HER2 gene. In about 25 percent of breast cancers, the HER2 gene makes too many copies of itself, and these extra genes ultimately make breast cells grow and divide in ways that are uncontrollable. HER2-positive breast cancers are more likely to spread and return than those that are HER2-negative.

• Oncotype DX score: The oncotype DX score helps doctors determine a woman’s risk of early-stage, estrogen-receptor positive breast cancer recurring and how likely she is to benefit from post-surgery chemotherapy. In addition, the score helps doctors figure out if a woman is at risk of ductal carcinoma in situ recurring and/or at risk for a new invasive cancer developing in the same breast. The score also helps doctors figure out if such women will benefit from radiation therapy or DCIS surgery.

What seniors should know about herbal medicines A lifelong commitment to healthy living can improve life expectancy and quality of life while reducing a person’s risk for various conditions and diseases, including cancer and heart disease.

Healthy lifestyle choices like eating a nutritious diet and exercising regularly can greatly reduce a person’s risk for various ailments, but such choices don’t eliminate that risk entirely. As a result, even healthconscious men and women may need to rely on medication to stay healthy. That’s especially true for seniors since age is a risk factor for various conditions.

A 2014 analysis that appeared in the Archives of Gerontology and Geriatrics examined nearly 1,300 published articles

that focused on the use of herbal supplements by elderly patients. Though only 16 of those articles met the researchers’ criteria, the analysis concluded that herbal supplement usage is common among elderly patients, as was a lack of dialogue about such supplements between medical professionals and seniors. A concerted effort to initiate such dialogue on the part of both patients and health care providers can help shed light on herbal medicines so people taking them, including seniors, can learn more about what they’re putting into their bodies.

Kingdom, a widely respected and publicly funded health care system, describes herbal medicines as those with active ingredients made from plant parts like leaves, roots or flowers. Because herbal medicines are made from plant parts, many people assume they’re safe to take without consulting a physician. However, the NHS urges people to treat herbal medicines with the same care and respect as they would more conventional medicines. Herbal medicines can affect the body in various ways. A frank discussion with a physician can shed light on the potential side effects of herbal medicines and whether or not they’re safe.

and Integrative Health, which is part of the U.S. Department of Health and Human Services, notes that more well-designed studies are necessary to fully evaluate interactions between herbal medicines and prescription drugs. However, the NCCIH notes that some evidence points to some harmful effects of mixing the two. For example, the NCCIH reports that prolonged exposure to concentrated garlic extracts may reduce the efficacy of some drugs. The potential for these types of interactions should be enough to compel seniors to think twice before taking herbal medicines without first consulting their physicians. In addition, the NHS has developed a list of various types of people for whom herbal medicines may not be suitable, and that list includes the elderly.

Seniors considering herbal medicines should discuss the pros and cons of such products with their physicians prior to taking anything.
