President’s Message

Those who do not learn from history are doomed to repeat it. Understanding where we came from can help us more clearly identify our vision and clarify where we are going. The American Dental Association was created with the purpose of protecting and delineating our profession. In the early 1800's, dentistry had no parameters. The dental association did not exist. However, in August 1856, a group of twenty-six dentists got together and created an organization to protect and give structure to dentistry as a profession. Another purpose of the group was to limit those who practiced illegally, thereby protecting the public. This is no different than today. One hundred and fifty years later, our principles remain very similar.

Steve Jobs was an innovator capable of recognizing opportunities and was an inventor of the future. As he once said, “Don’t be trapped by dogma which is living with the results of other people’s thinking. Don’t let the noise of others’ opinions drown out your own inner voice. And most important, have the courage to follow your heart and intuition.” We are a smart, creative, skilled, hard-working and caring group of people. We all need to become proactive in the creation and planning of our future. Don’t be afraid of expressing your opinion. Your voice needs to be heard. You might be our own inventor of the future of our profession. Your ideas could be the spark for innovative changes that could lead to a better and more powerful organization that will represent us.

We have created a solid group that protects us as a profession and advocates for the public. The vision of the founders of our profession was key. Their foresight has helped by keeping us strong and unified. However, as the time passes, our priorities and threats are changing. The changes we are facing today are happening at a very fast pace. We have to be vigilant and participate in all the developments that are occurring at all levels that could affect our profession.

We have a unique and rewarding profession that we need to protect. If you are a member of organized dentistry - “THANK YOU!” If you are not a member, I encourage you to join. Your membership is important to your profession, your practice, and to you personally. Why would you risk allowing others to decide your future? Join organized dentistry, get active, and shape our destiny.

Note from the Editor: As a tribute to one of our most beloved and esteemed leaders within organized dentistry, and former editor of both the SFDDA and North Dade-Miami Beach Dental Society newsletters, who unfortunately suffered an untimely passing in November, 2007, it gives me great pleasure to honor Dr. John Tabak by reprinting one of his editorials from the SFDDA archives (JulyAugust, 2003). This could not come at a more timely moment when, just as in 2003, the issue of members dropping out of organized dentistry was a concern.

Forget that our ADA lobbyists have been battling against the excesses of OSHA, HIV, ergonomic, insurance, and, now, HIPAA rules on our behalf for years. And, with many great successes. Forget that no one buys restorative, impression and luting materials that don't meet ADA Specification # whatever. Children's Dental Health Month, Senior's Dental Health Week, Give Kids A Smile. Promotion after promotion getting the word out: see your dentists.

might be legal, but I sure feel like I've been robbed. When my painstaking treatment has been down-coded, I sure feel like my patient has been robbed. When my x-rays are often “lost’ and I am challenged to justify my treatment again and again, I sure feel like I've been robbed. NO MORE! It has taken several years of painstaking work for ADA legal staff to gather sufficient documented information to file these suits. We are going to hold their collective feet to the proverbial fire. And, we will win. But, many of your dentist friends will get a free ride, as they have been for years. Reread the list above. Now, the bad news. It costs huge amounts of money to pursue a lengthy lawsuit against such deep pockets as the insurance companies. We need new members in order not to

Defendants include Cigna, Connecticut General, MetLife and Mutual of Omaha." Mama Mia, that was music to my eyes. Now, that was not the first lawsuit. It was the third, but the first filed here in our South Florida Federal District Court. Everyone has stories about how he/she has been taken advantage of (read: "cheated") by the insurance industry. If you think cheated is too strong a word, consider this. When the money owed to me stays in the insurance company's bank account earning interest for it instead of being paid in a timely manner so it earns interest in my bank account for me, that

That includes many of the specialists to whom you GPs refer thousands of dollars-worth of work, but who think all they need to belong to is their specialty societies. And, do not forget your GP colleagues who feel the same about their AGD membership. The American Dental Association represents the majority of American dentists. It represents the interests of ALL American dentists. That majority needs to be, must get to be, closer to 100% of American dentists. Dental History Trivia... Paul Benjamin, D.M.D. An early operation to repair a cleft lip. Most of us remember Dr. Horace Wells, the Boston dentist who in 1844-45 demonstrated the first use of nitrous oxide to sedate patients for an extraction. Unfortunately, his first live demonstration in front of a medical class did not go off as planned when his patient, not sedated deep enough, woke up, causing the medical audience to jeer and ridicule Wells. Even though Well’s life went downhill from there, he did start the medical profession on the journey to find an anesthesia that would make surgery tolerable. The quest continued with his former associate, Dr. William Morton, who is usually given credit for being the first to use ether for an extraction in 1846. Unlike Wells, this demonstration went quite smoothly. On a side note, Morton neither finished dental school nor medical school, but still worked first as an apprentice for Wells, and then 1 for himself, both as a “dentist” and as a “physician.”

The surgery was successful. No one would ever notice the small scar on the boy’s lip. It was then left up to the boy’s mother to train her son to speak without the tell tale sounds of a cleft. Although a tedious process, the boy grew up with a flawless southern drawl and his cleft palate went unnoticed by everyone. The experience left its mark, however, because the boy ended up attending and graduating from the Pennsylvania College of Dental Surgery in 1872. He even won a dental contest in Dallas, Texas, “for having the best set of artificial teeth… vulcanized rubber and dental ware.” But, like Horace Wells, the once cleft-lip boy did not live happily ever after. Ultimately, he contracted chronic pulmonary tuberculosis and died at the young age of 36. He was named after his uncle, who gave him a normal smile and made sure he would never be ridiculed and traumatized while growing up. He became one of the most famous dentists ever to live. However, the fame came not from his skilled use of an explorer, but from his deadly use of a gun. His name was John Henry Holliday. But, you might know him as Doc Holliday.

Besides Morton, others have claimed to be the first to use ether as an anesthetic. One was Dr. Crawford Long of Georgia (of the Crawford Long Hospital, later to be become Emory University), who used ether for tumor removals at the same time as Morton, but never got the full credit he deserved. Ether Day: The Strange Tale of America's Greatest physician, he had just delivered his own nephew, Medical Discovery and the Haunted Men Who Made It. a healthy boy in all respects, except that the baby Julie M. Fenster, 2001 had this “defect,” which was unacceptable in the southern gentry he was born into. So when the child was 3 months old, Long, using his “newly” discovered ether, helped his cousin repair the boy’s lip. But, Long and his cousin did not attempt the more difficult procedure of closing the boy’s palate. 1.Morton’s career took another interesting turn when he was called as an expert witness at the famous Parkman-Webster murder trial, where dentistry was first used as identification evidence. But, that is a tale for another day

The 1920’s The good old days… In October 1922, there were just thirty-five charter member dentists in the newly organized East Coast District Dental Society. They all were white, male and individually declared “ethical” by the Board of Censors and Ethics. Dr. D. E. Sheehan, Miami was the first president. Their interest in quality continuing education and their concerns for their patients and each other were paramount. As it is today, the dental politics of the times related to licensing laws, fees and dues. Issues of those times In the October 11, 1924 minutes of the society’s Executive Committee, the elected Secretary recorded in his beautiful Spenserian hand that a motion passed directing. … delegate to the state meeting be instructed to object to the sum of $3 for annual license renewal as excessive. Annual dues to the district were $4, FDA $3 and ADA $2. They contributed $1 a year to the relief fund. The total was $11. It is interesting that the Relief Fund contribution designated to help needy fellow dentists was 15% of the dues…a biblical tithe. About Budgets In the second quarter of 1928, the SecretaryTreasurer reported, “…there is $111.60 cash in the First National Bank of Miami, and $209.25 was on deposit when the Ft.Lauderdale Bank and Trust closed its doors.” The great depression was at its zenith, but by 1921 the financial report showed $561 cash on hand. The membership had more than doubled to 72 paid dentists. In 1934, a Treasurer’s report lists paying $14 for 56 dinners at .25 cents per person. What a bargain compared to 1997’s usual $30-$45 person!

The 1930’s Tragedy in a Miami park. On February 15, 1933, Dr. Rupert H. Gillespie, then East Coast Secretary, wrote descriptively of the unexpected ending to a General Membership meeting being held in

“Due to the lateness of the hour, and further due to the fact the President elect of the United States who was in a city park, but two blocks distant, had escaped attempted assassination, the election of the officers was deferred to a called meeting at which time a larger attendance is hoped for.” The incident occurred in Bayfront Park, Miami, when a disgruntled citizen tried unsuccessfully to shoot Franklin D. Roosevelt. Tragically, the Mayor of Chicago took the bullet intended for FDR and died. Growth in the district By July 1936, there were 125 East Coast members. That is about a 280% growth in 14 years and 85% of the market share. Dr. E. C. Kunsford reported on the statistics with a bit of wry humor,“…146 (dentists) in the district; 125 in good standing, 21 good, bad and indifferent.” Dentists were known as golfing aficionados in those early days. It was officially recorded in June 1937 that, “Our good friend and fellow member, Dr. Arthur Kellner, Mayor of Hollywood, caused a near riot when he invited the East Coast District members to a barbeque and all day golf match at Hollywood Country Club.” That they took Dr. Kellner at his word is evidenced by the crowd of nattily dressed members gathered for a group picture the following year at the Hollywood Beach Hotel. (See photo right) On the far right is Dr. Ruth Tichenor, the first woman dentist member of the ECDDS. A graduate of the University of Louisville in 1923, she joined in 1924. Dr. Tichenor practiced general dentistry in downtown Miami.

The 1940’s In 1940, a banquet in Coral Gables marked the 100th anniversary of organized dentistry. Dr. T.A. Price introduced a night club entertainer from “The Drum” for a series of “stories and imitations.” One can only guess the possible rowdy content. The fearsome forties and fabulous fifties

brought far reaching changes during and after WWII. The dental population all but disappeared when Uncle Sam called, creating a critical shortage of dentists to serve the civilian population. Procurement Boards were set up during the national emergency to assign practitioners to areas of need. ECDDS adopted a policy that, “…those dentists who were licensed in Florida after the declaration of war, and who would not accept the critical need area assignment would be dropped from membership.” Minutes of a June 6, 1942 meeting recorded by Secretary Dr. William. J. Hughes tells of showing a Technicolor, sound film entitled. “Fighting the Fire Bomb.” The movie explained procedures for fighting incendiary bombs in the home. A German invasion was feared. “This was educational and most interesting,” wrote Dr. Hughes, who was later seriously injured while serving in action as an US Navy dental officer. After the war, those who served their country resumed their interrupted practices. Other dentists from colder climates passed the Florida board and migrated to South Florida where the population was exploding. There was room for all. The district dues were now $50.

The 1950’s District offices By 1958, the Executive Council decided it was time for a public presence in the community. A lease was signed with the Dade County Medical Association for two small rooms in its new building at Brickell Avenue and Southeast 13th Street, south of downtown Miami. A full-time Executive Secretary was hired at an annual $3000 salary. The society struggled to administer programs and services in a diverse geography and population group – from Key West in the south to Vero Beach in the north. Meetings were a long ride for many leaders and discussions were sometimes heated. In 1966, outgoing president Dr. Andrew A. Henry remarked he was tempted many times to quote a fellow being tarred and feathered and ridden out of town on a rail, “If it weren’t for the honor, I’d just as soon walk.”


The 1960’s and 70’s Changes In 1966, the ECDDS was redistricted, creating the Atlantic Coast District from the Dania cutoff canal to tiny Sebastian north of Vero Beach. East Coast was centralized to include the Greater Hollywood area in Broward County, all of Dade County and south through the Florida Keys. More than 40 years later the geography is the same. Barbara Simms was Executive Director of the society. The refugees came During the 1960’s and 1970’s , thousands of Cuban refugees arrived in South Florida seeking asylum from the tyranny of Fidel Castro’s harsh, Communist regime. Several hundred dentists were among them. A few were allowed to treat fellow refugees in a clinic in the Freedom Tower in downtown Miami. Others were tutored by volunteers from the ECDDS to pass the National Board and entered US Dental schools. Forced to relocate temporarily in colder climates for two to three years, most returned to South Florida and private practice. Many of these hardy scholars eventually joined leadership of the society. Long in the planning, the “June-In-January” Miami Winter Meeting opened to an enthusiastic audience in January 1971. A tradition of Southern Hospitality continued through the next decade. The society took a giant step in 1976, becoming a property owner with the purchase of 1,500 sq.ft. condominium office suite in Coral Gables.

The 1980’s and 90’s Enlarged twice, the headquarters in 1997 included 2,500 sq.ft of attractively decorated executive offices evaluated on the tax rolls at $200,000. The space was owned free of a mortgage. A full time staff of seven, under the direction of Barbra Sims, who retired after 37 years at the helm, administered the society’s policies and programs. She was later replaced by Rosalie Small

who ran the association from 1999 to 2006. New avenues for licensure opened to dental graduates from other countries. By 1997, East Coast’s dentists included graduates of more than 50 foreign dental schools and boasted a culturally diverse dental community speaking many languages. 1n 1995 Dr. Ramon A. Sanchez became the first Cuban exile dentist elected President of the ECDDS. The membership included a wide ethnic mix, about 20% of whom were female, and among them, Dr. D. Georgina Garcia, a Miami Beach general dentist, became the first woman president in 1998.

The New Century and “9-11” Fresh on the heels of the start of the 21st century, our nation would be attacked in a most unlikely and tragic scenario, when two airplanes crashed into the World Trade Center more commonly known as the “Twin Towers”. That was Tuesday, September 11, 2001. By Thursday September 13th, a group of men had been deployed from the Fort Lauderdale Airport to New York, their destination being “Ground Zero,” where the towers once stood. Among those men was our own, Dr. William E. Silver who was one of four dentists from Florida activated by the Office of Emergency Preparedness of the United States Public Health Service – DMORT (Disaster Mortuary Operational Response Team). Society Gets a New Name Also in 2001, our Executive Council, at the suggestion of Dr. Richard Mufson, voted to change the name of our dental association, “...to appear less confusing and more user friendly to the public, and to more accurately reflect the geographic location of the members our society represents,” as then stated by Dr. Mufson. The name change would require, however, approval by the FDA, and after much heated debate by the House of Delegates (as reported in FDA publications), the change from "East Coast"

District to "South Florida" District was approved by the January, 2001 House. The Wave of Technology During this last decade, technology has advanced more quickly than expected. With electronic mail, websites and social media being utilized more and more, the South Florida District Dental Association created a website, made the move to “green” meetings where announcements and agendas were sent by e-mail to the members. It is now common place to see laptops, iPads and e-readers being used by most everyone at meetings from local council meetings to the state and national meetings. Diversity We’ve come a long way from the original thirty-five white male dentists who founded the East Coast District Dental Society. In 2009, Dr. Joseph S. Gay became the first African American President of the association. Today, our current excutive director, Yolanda Marrero, and the board of officers, which is comprised of four women and three men (Dr. Irene Marron, President, Dr. Beatriz Terry, President Elect, Dr. Jeannette Peña Hall, Vice President, Dr. Marcos Diaz, Secretary, Dr. Orlando Dominguez, Immediate Past President and Dr. Rodrigo Romano, Treasurer), are all Hispanic. Our membership is rich in diverse cultures and represents dentists in all phases of their professional lives. The Message Remains the Same The South Florida District Dental Association faces this century with optimism. The challenges are many, and the stakes are high. 2012 LEGISLATIVE SESSION STARTS IN JANUARY Mark your calendars! The 2012 Legislative Session will commence on Tues., January 10 and is scheduled to end on Fri., March 9. During redistricting years where the Legislature is required by the constitution to redraw the congressional and state district lines after the census, the legislative session dates are moved up in order to have the proposed districts approved by the Legislature in time for the 2012 elections.

LEGISLATURE PREPARES FOR REDISTRICTING Every 10 years following the release of the United States Census, the Florida Legislature is charged with redrawing congressional and state boundaries based on the change in population. Because of an increase in population, Florida will add two congressional seats for a total of 27 seats. The state Senate will continue to have 40 districts and the House will keep 120 districts; however the boundaries will change. Districts determine which voters participate in which elections for choosing representatives in the U.S. House, State Senate, and State House. Districts must be geographically compact, respect municipal boundaries as much as possible and keep intact minority voting blocks. The Legislature must approve the redistricting plan during the 2012 Legislative Session which begins in January. It is likely that lawsuits will challenge the Legislature’s plan and that the Florida Supreme Court may ultimately determine the new boundaries. The Florida Legislature has taken steps to make the redistricting process more transparent to the public. The Florida House has launched a website for the public at http://www.floridaredistricting.org/ and the Senate also has a website and district builder which is accessible to the public at www.flsenate.gov/Session/Redistricting/.

PRELIMINARY ISSUES OUTLINED FOR THE 2012 LEGISLATIVE SESSION FDA staff has already begun the process of reviewing bills that have been filed for the 2012 Legislative Session. The

SAVE THE DATE! DENTISTS’ DAY ON THE HILL 2012 The annual Dentists’ Day on the Hill will be held on Wednesday, February 1, 2012. This event is a couple of months earlier due to the legislative session convening in January rather than March because of redistricting. Each year, FDA members from around the state travel to the Capitol to meet with legislators on issues that directly impact dentistry. Legislators

Legislature will be taking on a momentous task of redrawing the congressional and state district lines in preparation for the 2012 elections. This endeavor will be their main focus and will consume much of their time during session. As such, advocacy groups will be advised to be less aggressive in their efforts to push forward controversial legislation. Many times in this process success comes from playing defense and at the end of the day, defense may bring home a better overall outcome than always focusing on offense. At this time, the FDA’s legislative agenda includes the following issues: • Support for maintaining the current law that requires graduates of non-accredited dental schools to complete a 2-year supplemental education program before taking the Florida licensure exam. • Support allowing dental hygienists to administer local anesthesia (after 60-hours of training) to a non-sedated patient over 18 years old under the direct supervision of a dentist. • Support creating a public records exemption for dentists and dental hygienists for information provided in the voluntary dental workforce survey completed during licensure renewal. • Support legislation prohibiting insurance companies from telling dentists what they can charge for non-covered services. • Support clarification that dentists (in specified circumstances) do not need to purchase a $255 health care clinic establishment permit to purchase prescription drugs. There are many other legislative issues FDA staff will be monitoring this year, including Medicaid and budget proposals, which may have an impact on the dental profession. Remember to read your weekly Capital Report and watch your legislative video updates during session for the most current information. There will be a legislative briefing held the evening before Dentists’ Day, Tues., January 31, to help prepare you for your meetings with legislators. The FDA hopes to have a strong showing at the Capitol on Dentists’ Day on the Hill. Mark your calendars for Wed., February 1!
