WHO WE ARE

Established under the Older Americans Act (OAA) in 1973 to respond to the needs of Americans 60+ and over, Area Agencies on Aging successfully develop, coordinate and deliver aging services in every community across the nation, providing assistance to more than 11 million older Americans and caregivers annually.

INFORMATION & REFERRAL Provides older adults and their caregivers with timely information and referrals for area resources.

NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM Provides services to help family caregivers care for their loved ones at home. Services are available to family members who provide in-home care for persons 60 and older or to grandparents age 55+ who serve as caregivers for children 18 and younger.

MEDICARE & STATE HEALTH INSURANCE ASSISTANCE PROGRAM (SHIP) Provides free, in-depth, one-on-one insurance counseling and assistance to Medicare beneficiaries. Call with questions or concerns about Medicare, Medicare benefits, or how to see if you’re eligible for the Medicare Savings Program.

DENTAL AND VISION ASSISTANCE PROGRAMS Medicare and Medicaid often do not pay for senior’s dental and vision needs. The AAA offers programs for adults 60 and older for dental and vision needs.

LONG-TERM CARE OMBUDSMAN People living in long-term care facilities, including nursing homes and assisted living facilities, require support and advocacy. Ombudsmen are resident advocates and ensure residents’ rights are respected. Ombudsmen provide information to individuals and families regarding facilities, quality care and are trained in problem resolution.

The Area Agency on Aging (AAA) provides and connects the community with supports and services that promote aging with independence and dignity for individuals sixty and older and their caregivers in their community of choice. AAAs provide programs directly and through contracted providers.

CASE MANAGEMENT A person-centered process where individuals, family members, and caregivers are supported to develop a plan for addressing long-term services and supports needs that align with their preferences, strengths, values, and needs.

EVIDENCE BASED HEALTH & WELLNESS EDUCATION Health and fall-prevention classes and activities that encourage healthy living and wellness are offered to older adults in the region. Examples include N’Balance, Matter of Balance, Tai Chi for Arthritis, and Chronic Disease Self-Management.

NUTRITION PROGRAM Includes congregate meals, home delivered meals, and nutrition education and counseling.

LEGAL ASSISTANCE Through contracts with local legal service providers, AAAs offer access to quality legal services, including living wills, power of attorney documents, family law and other civil law services.

Thoughts on Covid’s Impact Recently, research began coming out about the terrible impact isolation has on seniors’ health.

Just in time for the world to be hit with being home and isolated to varying degrees. For some at home means being with several family members or friends. For some it has meant being home alone with limited community sources or family/friends able to come into your home. Or maybe you are in a facility of some type and you are limited to seeing just the staff (in person).

It is so important for us all to have social interaction in our life for physical and mental wellbeing. New technologies and some not so new are trying to help us all adjust in healthy ways to this new way of living. Such as phones, shopping apps, house party app, zoom etc. We are also learning new words like Social distancing, window visits, zooming, tele- health visits, and new normal.

We are all being forced to look at our world a little if not a lot, differently. We are integrating old school types of communicating and going about our day. Writing letters, checking in with neighbors, sewing (masks), making bread and soup for ourselves and others. Some are learning new hobbies and expanding interests. Music, pets, board games are all being used to help us maintain the balance in physical and

mental health we all need!

Handwritten letters are being passed between seniors in facilities and school children. What a wonderful connection for both generations.

This reminds me of stories from senior clients who persevered WW2 and made personal sacrifices for the

wellbeing of fellow citizens and the country. There was a recent picture in a paper of a 1918 photo of people at a baseball stadium. They were all wearing cotton masks! How did they phase into regular life again?

Somehow they did and we will also.

The seniors I have known who have already lived through many changes and adversaries, have the best wisdom and patience.

Reach out to one another.

Why Would I Consider A Trust For My Everyone needs an estate plan, Estate Planning? it doesn’t matter what your socio-economic circumstances are.

A simple estate plan will typically contain at least three documents.

This will generally include a durable medical power of attorney and a durable financial power of attorney.

The other document typically found is a way to pass your property upon death such as a will.

However, there are times when a trust might be an option in your estate planning instead of or in addition to a will. So why should you consider a trust? Trusts are useful estate planning tools that can accomplish a variety of goals. They can help avoid probate, minimize taxes, and be used to give property to minor or disabled loved ones. Trusts can be created during a person’s lifetime (Living Trusts) or at the person’s death (Testamentary Trusts). Some different types of Trusts from both categories are discussed below.

Living Trust A person can transfer their assets to a Living Trust and, as trustee, continue using their assets as they always have.

Tax Planning Trusts Several different types of Living Trusts provide flexible alternatives for minimizing capital gains and estate taxes, including the Charitable Remainder Trust, Irrevocable Life Insurance Trust, Qualified Personal

Residence Trust, Grantor Retained Annuity Trust,and Grantor Retained Unitrust.

Testamentary Trusts A person can create a Trust under a Will, called a Testamentary Trust, which does not take effect until they are deceased.

Disability Trusts (also known as Special Needs Trusts) A Disability Trust is a type of Living Trust that allows a disabled person under the age of 65 to use their own assets for their special needs, other than food and shelter, and keep public benefits, such as Medicaid and Supplemental Security Income (SSI).

To Gift or Not to Gift…?

When someone is thinking of how to get Long Term Care help for parents or their spouse, one reaction is to disburse assets so they can apply for Medicaid.

What many people do not realize is that by disbursing assets, they may jeopardize the in-need person’s immediate eligibility for assistance by causing a penalty period.

Transfers Without Fair Consideration (TWFC), or gifting, of any kind are not allowed for Medicaid eligibility. Various types of gifting may include but are not limited to: n

Adding children to the home’s ownership; Giving a car away or Selling a car for less than 2/3 of the vehicle’s fair market value; Church tithing; Donations to various entities; Reimbursing family for travel expenses; Birthday gifts to kids, grandkids, etc.; Selling a home for less than 2/3 of its fair market value (which is determined by the realtor using comparables); In-home care payments to a family member or other third party without a notarized contract in place; Gifting cash to anyone; Adding someone to a bank account’s ownership; Changing life insurance ownership to someone other than a spouse.

The State of Colorado and its counties has a 5 year look-back period. This means that if ‘dad’ added his kids as co-owners on his home four years go, and now dad needs Long Term Care (LTC) Medicaid, the State and counties can look back five years into dad’s financial transactions. Because the home’s ownership changed four years ago, the addition of other owners will cause a gifting situation. Based on dad adding a son and daughter to the home’s ownership, this would be equivalent to 2/3 of the home’s ownership going to the two kids, and 1/3 remaining with dad. If the home’s value is $300,000, the gift portion in this transaction would be $200,000 and the penalty period would be based on this amount.

Penalty periods, otherwise known as Periods of Ineligibility (POI), are based on the amounts gifted. This can be anything from a single gift of cash to multiple transactions involved. Some transactions, or gifts, can be reversed relatively easy and others cannot be reversed at all, so it’s very important to consider if the family is in a financial position to private pay while the penalty is being served. In 2020, a gift of $8,758 will result in a penalty period of one (1) month. If the sample transaction is revisited where a $200,000 TWFC (gift) occurred, the penalty would be nearly 23 months long. Not many families are in a

The penalties cannot be started until the LTC Medicaid applicant is otherwise eligible. What this means for a single or widowed individual is that the individual’s assets cannot exceed $2,000, and he or she would be otherwise eligible if it were not for the Transfer Without Fair Consideration. Once the individual is determined to be otherwise eligible, the penalty period can begin. As always, financial eligibility and functional criteria both must meet the State’s requirements for Long Term Care Medicaid to be approved.

To avoid transfers and the sub-

sequent penalty periods, mom and dad’s money should be used to pay for their own needs. If a spend down of assets is necessary, paying their medical bills; paying off credit card balances in their names; paying facility rent; or upgrading their inhome bathroom to meet their needs are all potential options. Beware though, because some expenses that are thought to be appropriate can back-fire. Navigating the Medicare can seem like a confusing Medicare Maze web of options. Many seniors choose to rely on licensed agents for help navigating an average of 24 plans available to choose from each year.

It’s important to keep in mind that an agent may not be contracted with all insurance companies offering plans in your area. Working with a local agent that is licensed to represent multiple carriers is ideal.

With plans changing from year to year there could be a better choice for you in terms of cost and coverage. Your agent should check whether your doctor, hospital, preferred pharmacy and other providers are considered in network.

Also, some Medicare agents focus on selling only supplemental insurance, or Medigap. Those policies help you with copays, deductibles and other out-of-pocket expenses, and can only be paired with original Medicare. While this might be the option for you, you won’t know if your agent doesn’t check. Work with an agent that represents both Medicare Advantage and Medigap plans to be aware of your options.

There are federal and state laws governing agents who sell Medicare plans, which prohibit things such as them showing up uninvited at your house or trying to lure you with a cash offer. They also cannot legally

charge you a fee to process your enrollment, as agents are compensated through commissions from the insurance carriers they represent.

Make sure you are working with a licensed agent. Unlicensed volunteers can cause you issues if they help you choose and enroll in a plan and there is a mistake. They aren’t held accountable like a licensed agent would be, because it appears as if you enrolled on your own, rather than with assistance. They may not receive the same training as licensed agents or be aware of plan nuances and features, because they do not receive carrier to agent communications.

If you feel you enrolled in the wrong plan, contact an agent to see if you qualify for a special enrollment period to correct the situation.

The Power of Your Words Words, you see them on paper, you hear them used by family, friends, and foe.

But what do they mean?

Well they mean whatever YOU believe they mean.

WHAT? Yes, you give the meaning to words which is what causes much confusion and conflict.

Let me give you an example: You have a definition that you’ve given the word such as marriage. Maybe you have an idealistic view so to you that word means, a committed relationship, honor, support, companion, someone who always has your back. But, your experiences in that area of life are far from the idealistic. Let’s say that you married someone who cheated on you, took money, insulted you or hurt your feelings. So now your definition is all mixed up. The part of you that wants to believe in the ideal versus the experiences that has colored your definition. You no longer have the same definition.

In life we experience many hurts.

Some of those experiences we have defined as ‘mean, hateful, vengeful, hurtful’ and we have written off those people. But what if, how you perceived what they did or said was not the intent that they intended?

I watched several conversations between my Mother and Stepfather that was destructive to their relationship. He would say something, she

would hear what he said, she would put her ‘spin’ on it, and the upset was on. I knew what he meant by what he said but the same words did not MEAN the same thing to my Mother. Sadly, towards the end of his life they were not close and they both had damaged the relationship based on different definitions of words.

So, remember, words are powerful, how are you meaning them and how is someone else hearing them? We do have the option of changing our definitions which also shifts our relationships, not only with a partner, but how we see ourselves, money, success, exercise and more. Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Area Aging on Aging Services Support Aging in Place These days there is such an emphasis on aging in place.

Aging in place appeals to individuals as it allows them to comfortably live in their own home or community while maintaining their dignity and independence for as long as possible compared to the alternative of moving to a more costly older adult living arrangement.

Since the 1970’s the Area Agencies on Aging (AAA) have been playing a crucial role in supporting older adults to be able to age in place. AAAs successfully develop, coordinate, and provide direct services in their communities or contract them out. Typical core services offered by the AAAs include case management, caregiver support, chore services, evidence-based disease prevention and health promotion, homemaker and personal care services, legal assistance, long-term care ombudsman, nutrition programs (congregate and home-delivered meals), reassurance, and transportation services. These foundational services support the needs of older adults living at home.

Each AAA has the ability to offer customized services based on local community needs. There is no onesize-fits-all approach.

However, as the older adult popu-

lation continues to increase so does the demand for AAA services. It is important that older adults not be put on a waitlist for these supportive AAA services. AAAs receive their primary funding through the Older Americans Act and State funds. Funding for AAAs has not kept pace with inflation and the older adult population growth. It is vital that AAAs have sustainable funding in order to continue to meet the needs of older adults who are aging in place.

Most older adults want to age in place. The opportunity to age in place is an exciting and rewarding one, and older adults should be provided with the necessary assistance and resources to make the most of the experience. Having the ability to age in place improves older adults’ quality of life emotionally, physically, socially, and spiritually. Maintaining a good quality of life affords older adults the optimal aging experience. ~

Using Assistive Technology in a 21st Century The Center for Independence Virtual World (CFI) has been serving western slope people with disabilities, including seniors, since 1982.

We receive our funds from contracts with state and federal agencies which allow us to provide case management and demonstration services for free.

These last few months have showed us that CFI and other human services need to change our service model from facility based to community based outreach.

We’re deeply concerned about the isolation and personal impact of COVID-19 on people with pre-existing conditions. The service model of the future will be both virtual and face to face.

The obvious gap in this design is access to reliable and usable technology. Smart phones and tablets will be needed more and more to request medical appointments, order food, inquire about pensions, insurance and disability benefits, and communicate with distant family members.

CFI can help. We have specialists who can demonstrate apps that enhance visual clarity on a tablet.

We have screen reading technology, captioned telephones, and memory supports like Echo and Alexa which can record appointments, shopping lists and repeat telephone numbers.

There is a world of opportunity for

public communication and personal supports available right now for little to no cost.

While CFI is still under limitations from public health, it will not always be so. We will be looking for ways to add more technology services in the months ahead as well as finding funders that will pay for tablets and smart phones. We want to be able to hold peer support meeting again on virtual platforms. To do so means that we will be inviting people who need these tools to remain safe and still be independent in the community to contact us to create a plan for self-sufficiency.

COVID-19 has been a wake-up call to us all that the world is a 21st Century operation and we need 21st Century tools and training to continue to have seamless services. Fortunately, we at CFI believe in life-long learning and that there is a solution to the barriers that have arisen. 24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job.

If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health.

Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.

Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. HOSPICE (See definition in home health grid definitions) Services Provided: N-Nurse – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed.

SSW-Social Worker – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs.

Chaplain – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

Home Health Aide – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. Volunteer – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care: RO-Routine Care – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s residence, this can be their private home, a retirement community, an assisted living, or a skilled nursing facility.

RE-Respite Care – Provided when the family needs time to rest or go out of town for a short time.

Ideally, this service is provided for

five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs.

Continuous care also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be.

The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

Understanding PTSD It’s estimated that 70-90% of in Seniors people over the age of 65 have been exposed to at least one traumatic event in their life.

Post-Traumatic Stress Disorder (PTSD) develops in approximately 1 in 3 people who go through serious trauma.

This can include anything from the death of a loved one, military combat, car accidents, illness or injury, natural disasters, a disease diagnosis or other events that may have significant physical or psychological impact.

Many people experience negative feelings after experiencing a traumatic event. However, when these negative feelings last for a long period of time, cause someone to relive the trauma or have daily impacts, that person could be suffering from PTSD. For seniors, it’s not uncommon for symptoms of PTSD to be a result of traumatic events that happened earlier in their lives or to occur after a major fall, a heart attack or other serious health events.

For seniors with PTSD, symptoms can include flashbacks, depression or anger, fear, self-isolation, out-

bursts or trouble sleeping. These symptoms can be difficult to identify or mistaken for other age-related conditions.

There are treatment options for seniors experiencing PTSD. It is recommended that anyone experiencing PTSD symptoms consult their physician to establish a plan of treatment to successfully manage symptoms.

Lifestyle changes – for some, keeping active and engaged can help manage PTSD symptoms. This can include anything from starting a physician-approved exercise regimen, volunteering, getting involved in community activities, or finding ways to increase mental stimulation. n

Medicine – there are medications that can help seniors with PTSD. For seniors that are prescribed medication, it’s important to schedule regular check-ins with a physician and to be open about the positive and negative effects of medications. n

Therapy – individual or group therapy can be a great resource for seniors with PTSD. There are n

Sharing experiences – talking about the traumatic event that caused the PTSD can also bring relief and foster connections that may be beneficial for a senior’s physical and mental health. This can be a conversation with a friend or joining a support group – it’s important for people to share in an environment where they feel supported and understood. It can be difficult for someone with PTSD to talk about their experiences, but for many this may be a necessary step for managing symptoms. n

Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief.

Each hospice agency will have a grief and bereavement support program.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company.

LICENSED MASSAGE THERAPY – Massage has become one of the fastest growing trends in therapy for hospice patients. Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare; it is an added benefit that select hospice companies have decided to provide for their patients.

MUSIC THERAPY – Music Therapy is an established healthcare pro-

fession that uses music to address physical, emotional, cognitive, and social needs of individuals.

A music therapist is not just a music performer, but actually trained in counseling and music.

A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed

by Medicare; it is an added benefit that select hospice companies have decided to provide to their patients.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings.

For some patients, contact with these gentle animals evokes memories of

pets in their lives, past or present.

Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed

ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths.

Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.

DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SHORT-TERM CARE – Acute care provided on a short term basis.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service,

cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy.

A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

VENTILATOR - Moves breatheable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable to breathe, or is breathing insufficiently.

WANDERGUARD - Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering.~

From Their Eyes Our Elders are facing a period where visiting their loved ones puts them at risk.

They must battle internally the decision whether to venture out to the store, wrap their arms around their family or stay in their homes and visit thru windows, online meeting groups, or phone calls. These are not easy choices for our Elders to make, but we can do things to help make these choices easier and safer for them.

Studies have shown that there are key steps to take to reduce passing COVID-19 onto our Elders. Perform frequent hand washing using hand sanitizers with 70% alcohol or higher. Use Social Distancing and wear a mask or other face and nose covering, these are key factors to helping reduce the spread of COVID-19. Masks maintain “source control” and protects others. If we all wear a mask and use social distancing, we can prevent the spread of viruses and protect one another.

If you know you have been exposed to someone with COVID-19, you need to self-quarantine in your home for 14 days. It is still good to quarantine if you have travelled somewhere that has large numbers of COVID-19 or rapidly increasing. Avoid large gatherings and perform “self-screen-

ing” each day including taking your temperature and monitoring for any shortness of breath, change of taste, sneezing, or coughing. Remembering that many people that are spreading the COVID-19 virus has no symptoms. While you are waiting to see if any symptoms develop, stay in close contact with your Elder so they do not worry and know that you are doing these things to help protect them. When they know you are taking extra precautions it helps to support their and your decision on which kind of visit is best.

With our elders being the population at greatest risk from COVID-19, they know it may be a life or death situation and knowing their loved ones are taking all precautions to help minimize the risk to their Elder, puts their mind at a greater ease. This Pandemic will be around for several month and if we all get in the habit following these simple guidelines we can really help our Elders and still enjoy all the love, the stories, and those precious moments with them without putting them at risk.~ Editor’s Note: This article was submitted by Melissa Latham RN, NHS, Administrator at Larchwood Inns and The PARC. Knowing Community Resources Can We do not always know what Improve a Life we need until the exact moment we need it.

Most people do not usually know what resources are available in our own communities if we are healthy and financially able to provide the basics of daily living – food, shelter, clothing, etc. Only when our circumstances change, which could be due to illness, injury or some other event, perhaps the loss of a spouse, do we start to wonder how will we fill these gaps and provide for all our daily needs.

Fortunately, in Western Colorado, our community has numerous resources for older adults. Region 10 and their Community Living Services Program supports the needs of older adults who wish to remain living independently. Under the Area Agency on Aging and the Older Americans Act, funding is provided for in home support services to adults aged 60 and older who qualify through a quick, telephone assessment.

These services include homemaking, personal care, caregiver respite, home delivered meals, transportation vouchers, and companionship.

AdvantAge Health Resource Centers collaborate with Region 10 to provide the options counseling and assessments for these supportive services. Care Navigators in both Montrose and Delta Counties, including a Spanish speaking Care Navigator,

can review options for benefits and assistance programs along with other care options, provide application forms for services, and offer referrals and helpful information.

A local, toll free number, 1-844-VOA-4YOU (1-844-8624968) is available for potential clients Monday through Friday. Callers can ask questions about health care including; where to go for rehab after surgery, home health, adult day care, memory care, assisted living or longterm care. Help is available to link people to sources for assistance with groceries or meals programs, durable medical equipment, support groups, legal aid, independent housing, family housing or veterans housing assistance and to complete Medicaid and Medicare applications. Connecting the needs of any age group to a service that can fill the need is the goal of the program.
