Long time Grand Junction resident, Opal Jaquette, is a mom to scores of people around the world.

Through her generous spirit, Opal remembered everyone’s special occasions, making her legendary and treasured by all.

Opal, with her husband Frank and their two children Mark and Julie, moved to Guam in 1967. Both Frank and Opal worked as teachers on Guam for over twenty years. Despite Frank’s passing 12 years ago, Opal continued to travel to the warmer Guam climate each winter to be with close friends.

This past winter however, she stayed home; she tried to keep up with normal household duties, but Opal started having some difficulties.

In March, Julie and Mark both returned to Grand Junction to stay with her after family members and church friends noticed signs of increasing dementia. The next two months proved challenging but very providential!

Looking back at the month of April, it was only ‘providence’ that helped the three of them get Opal’s finances in order. By May, Opal’s vascular dementia had escalated quickly, and she was hospitalized three weeks in order to stabilize her condition.

When she couldn’t return to her home, and needed a dementia center, Mark and Julie felt she would like a smaller, more home-like atmosphere with close, immediate, personal care. Walking into The Vineyard’s, they knew it was the perfect place to be mom’s new home. Thus

far, Opal has truly enjoyed her independence, coupled with the attentive care at The Vineyards. She told her family, “I feel like I’ve fallen into a tub of butter here!”

WHO WE ARE

Area Agencies on Aging are national organizations funded through federal and state dollars and authorized under the Older Americans Act. We serve people 60+ and their caregivers throughout Colorado.

INFORMATION & REFERRAL Provides older adults and their caregivers with timely information and referrals for area resources.

NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM Provides services to help family caregivers care for their loved ones at home. Services are available to family members who provide in-home care for persons 60 and older or to grandparents age 55+ who serve as caregivers for children 18 and younger.

MEDICARE & STATE HEALTH INSURANCE ASSISTANCE PROGRAM (SHIP) Provides free, in-depth, one-on-one insurance counseling and assistance to Medicare beneficiaries. Call with questions or concerns about Medicare, Medicare benefits, or how to see if you’re eligible for the Medicare Savings Program.

DENTAL AND VISION ASSISTANCE PROGRAMS Medicare and Medicaid often do not pay for senior’s dental and vision needs. The AAA offers programs for adults 60 and older for dental and vision needs.

LONG-TERM CARE OMBUDSMAN People living in long-term care facilities, including nursing homes and assisted living facilities, require support and advocacy. Ombudsmen are resident advocates and ensure residents’ rights are respected. Ombudsmen provide information to individuals and families regarding facilities, quality care and are trained in problem resolution.

The Area Agency on Aging (AAA) provides and connects the community with supports and services that promote aging with independence and dignity for individuals sixty and older and their caregivers in their community of choice. AAAs provide programs directly and through contracted providers.

CASE MANAGEMENT A person-centered process where individuals, family members, and caregivers are supported to develop a plan for addressing long-term services and supports needs that align with their preferences, strengths, values, and needs.

EVIDENCE BASED HEALTH & WELLNESS EDUCATION Health and fall-prevention classes and activities that encourage healthy living and wellness are offered to older adults in the region. Examples include N’Balance, Matter of Balance, Tai Chi for Arthritis, and Chronic Disease Self-Management.

NUTRITION PROGRAM Includes congregate meals, home delivered meals, and nutrition education and counseling.

LEGAL ASSISTANCE Through a contract with Colorado Legal Services, AAAs offer access to quality legal services, including living wills, power of attorney documents, family law and other civil law services.

The western slope town of Grand Junction is home to not only 3 main hospitals, including a regional VA medical center, but also to the entire spectrum of care from support to pregnant mothers to an award winning Hospice organization and everything in between. This enables residents of all ages to benefit from the care not only for themselves but also for their aging parents.

Being able to age in place has always been a priority to most people. Here in Grand Junction that can become a reality due to not only high tech advances but also to the many and varied services and agencies in the valley that can assure families of quality care in their own home.

These services include the skilled (medical) and non-skilled (non-medical) agencies that can assist seniors from transportation, shopping and meal preparation, to the more intimate care such as assistance with dressing, bathing, skin care, and medication management. There are meals on wheels, home visits from the library, restaurant taxi service, bookkeeping and housekeeping services. All focused on maintaining the senior’s independence in their own home.

If you are a senior or have a loved one in the area who is a senior, you can make one call to gather all your resource needs by calling a local geriatric care manager, licensed and certified to help families navigate through the community for all their needs. Why Would I Consider A Trust For My Everyone needs an estate plan, Estate Planning? it doesn’t matter what your socio-economic circumstances are.

A simple estate plan will typically contain at least three documents.

This will generally include a durable medical power of attorney and a durable financial power of attorney.

The other document typically found is a way to pass your property upon death such as a will.

However, there are times when a trust might be an option in your estate planning instead of or in addition to a will. So why should you consider a trust? Trusts are useful estate planning tools that can accomplish a variety of goals. They can help avoid probate, minimize taxes, and be used to give property to minor or disabled loved ones. Trusts can be created during a person’s lifetime (Living Trusts) or at the person’s death (Testamentary Trusts). Some different types of Trusts from both categories are discussed below.

Living Trust A person can transfer their assets to a Living Trust and, as trustee, continue using their assets as they always have.

Tax Planning Trusts Several different types of Living Trusts provide flexible alternatives for minimizing capital gains and estate taxes, including the Charitable Remainder Trust, Irrevocable Life Insurance Trust, Qualified Personal

Residence Trust, Grantor Retained Annuity Trust,and Grantor Retained Unitrust.

Testamentary Trusts A person can create a Trust under a Will, called a Testamentary Trust, which does not take effect until they are deceased.

Disability Trusts (also known as Special Needs Trusts) A Disability Trust is a type of Living Trust that allows a disabled person under the age of 65 to use their own assets for their special needs, other than food and shelter, and keep public benefits, such as Medicaid and Supplemental Security Income (SSI).

Two Sides to Long Term Care The Long Term Care Medicaid Medicaid program for the elderly and disabled can help pay for in-home care, assisted living situations, or even skilled nursing facilities.

For this reason, the Long Term Care (LTC) program has both functional and financial requirements that must be met by the applicant to be eligible. Potential clients must meet a “nursing home levelof-care” determined by the county in which they live. The client must also meet financial criteria in which income and resources are reviewed to be sure limits are not exceeded.

The nursing home level-of-care criteria will be determined by a county-approved case manager. The case manager will meet with the client and family to find out exactly what the applicant’s physical limitations are and even his or her mental capacities, such as dementia or Alzheimer’s. Family, caregiver, and doctor input is also collected to make the functional assessment complete. The documentation from the case manager is required by the financial technician for the county to complete the application process.

The financial eligibility process is usually the most complicated to clients and families because of the amount of paperwork involved.

This is where a Medicaid consulting agency can help. These agencies can offer clients another option

besides working with the county directly or a costly attorney.

Some basic information regarding financial eligibility in 2019 (*) is:

Applicant must be 65 or over and receiving SSA benefits, OR

If under 65, the applicant must have been deemed disabled by SSA and now receiving SSDI.

For single individuals, a $2000 resource limit applies.

An applicant’s gross monthly income (income prior to any deductions) cannot exceed $2313. a. However, if gross income is between $2313 - $8117, an income trust may allow client LTC benefits if State approved. b. If the applicant has gross income over $8118/mo, LTC is not available due to being over the income limit.

(*) A mounts listed are for 2019 and are subject to change annually.

For Long Term Care Medicaid, giving resources away will create a period of ineligibility (POI) – aka a penalty period. The penalty, based on the total amount of gifts, will begin when the client is “otherwise eligible” and must be served in its entirety before the applicant can receive LTC benefits. The best way to avoid this scenario is for the applicant (and spouse) to use the funds for their own needs and care in this “rainy day” time of their lives.

All income and resources must be reported at the time of application.

The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Are You Unhappy?

Many of us have experienced unhappiness or even clinical depression at different times in our lives.

Maybe the issues were around grief/ loss, relationship conflicts, financial concerns, or loneliness.

The conventional thinking is that brain chemistry plays a part, many people take a pharmaceutical to alleviate these feelings. This is one option.

However, I would like to suggest other options to enhance happiness that you may not be aware of such as Emotional Freedom Technique (EFT or Tapping Technique), anger management techniques, journaling, meditating and taking nutritional supplements.

Unhappiness can also be a habit.

Science has shown that it is much easier for the brain to respond to negative emotions than positive ones and to ‘replay’ the negative over and over. So, we have to “work” sometimes to get ourselves out of the doldrums. We must become the subconscious parent who says to our negative thoughts, “I’ve heard you and that’s enough. It’s time for you to focus on something to be grateful for.” Yes, we all have things we can be grateful for even when things look the bleakest.

We know that negative emotions create chemical changes in our body, ones that do not promote health and happiness. There are things we can do to change our chemistry with better eating, exercise, meditation and therapy.

Research shows eating more fruits and vegetables will make you happier the next day. This supports that old saying, “An apple a day keeps the doctor away.”

Many people who are suffering from stress overload are not getting the B vitamins that are essential to managing stress. Extremely low levels of B12 have been shown to cause mental breakdowns.

An easy technique for stress and unhappiness is meditation. Sit and do “Box Breathing,” where you breathe in to a count of 4, hold for 4, breathe out for 4, and hold for 4. Tapping (EFT) with or without a practitioner works like a charm as well. Most importantly, recognize that you matter and take time to nurture yourself and get help if you need it. REHABILITATION FACILITIES Rehabilitation Facilities offer full restorative therapies such as Physical, Occupational, Speech and/or Respiratory therapies and can be offered in a variety of settings including stand-alone buildings or as part of a clinical practice or existing skilled-care nursing facility.

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Orthopedic Rehabilitation: What The Rehab facilities can take a variProcess Could ety of patients. The primary goal at a rehab facility is to help the Look Like patient’s get stronger and achieve their highest level of functionality.

Rehab facilities often take patients with conditions such as cardiology complications, strokes, wounds, and many other different conditions that has caused a patient to experience increased weakness.

Rehab facilities also see a large population of orthopedic patients. Estimated rehabilitation times can be different for patients depending on their diagnoses along with how their body heals. It is very normal for patients to have questions about their rehab and please don’t hesitate to ask your care team. Due to the higher predictability of orthopedic patients, this article is designed to walk you through a brief synopsis of what the rehab process could look like for orthopedic rehabilitation.

One of the most frequently asked questions regarding total joint replacement surgery is the estimated time for healing. To answer this question, we need to separate

the various total joint surgeries as they all have different healing and recovery times based on the medical research available and the surgeons’ specific protocols.

For total hip joint replacements, you should expect a quick recovery being able to resume normal walking within days. Physical therapy treatments for this surgery focus on walking, balance, coordination, and general strength of the lower body and is generally completed within one month.

Total shoulder joint replacement surgery has a more involved recovery time due to the shoulder joint being highly mobile.

You should expect some time in a sling following surgery according to your surgeon’s protocol. Once you are cleared from the sling, physical and occupational therapies include gentle range of motion restoration, coordination training of the shoulder muscles, and performance of activities of daily living. Therapy should be completed within 1-2 months.

Continuing a life of health, wellness, and engagement as one ages requires regular adjustments and adaptations through the body’s aging process. Living independently in the community late into life is very possible and requires a curious mind and a commitment to life-long learning of new skills. We are your free resource to explore the skillful adaptations and adjustments with tools, techniques, and peers who have life experience to guide the way.

Across Colorado, Independent Living Centers, or ILCs, help seniors and their families and friends adjust and adapt to aging in place. As the body ages there is a diminishing of six core functions that many will experience: Hearing, Vision, Mobility, Speech, Cognition, and Self-care. Our unique services have been delivered to Coloradoans since the advent of the Rehabilitation Act of 1978. CFI is one of 9 ILC’s and serves 12 Counties on the western slope. We are a non-profit organization, highly skilled and trained in working with seniors and their families to create solutions

for a richer, independent life at home. CFI provides community solutions including paratransit, disability benefits, accessible affordable housing location assistance, accessibility training to mitigate sensory loss, support groups, computer training, loaner DME equipment, and specialized services for hearing or vision loss. In the event of a nursing home placement, CFI has transition services to move back into an independent living situation in the community. The transition team ensures a successful move through a comprehensive, multi-agency team approach that addresses risks and solutions.

These services are available for free under subsidized government contracts. Antibiotic Stewardship is the active promotion of appropriate antibiotic use to treat bacterial infections.

Antibiotics should only be taken when necessary. Like all medication, antibiotics have side effects and sometimes does more harm than good. They may give you a skin rash, allergic reaction, diarrhea, C-Diff (clostridium difficile), yeast infection, or worse, and you will still feel ill.

Reactions from antibiotics caused 1 out of 5 medication-related visits to the emergency room.

Antibiotics only kill bacteria and cannot treat viral illnesses like a cold or flu. Over-use and incorrect use of antibiotics causes bacteria to become resistant to medications and harder to treat. Not taking antibiotics exactly as prescribed includes stopping them to early, not taking the complete prescription or skipping doses. According to the CDC (Center for Disease Control) each year, in the United States, at least 2 million people get infected with antibiotic-resistant bacteria. At least 23,000 people die as a result. Antibiotic-resistant MRSA, or methicillin-resistant Staph aureus, is one of the most common. MRSA is the bacteria, Staph aureus which is commonly present on the skin and in the nose and has become resistant to penicillin, amoxicillin, and other antibiotics in that same class.

There has been a frequent overuse of antibiotics for treatment of suspected Urinary Tract Infections,

or UTIs in older adults. Older adults commonly have bacteria in their urine, but not causing a real infection. Yet there may be complaining of vague symptoms, such as increased confusion, falls, or irritability. The doctor orders a urine test that shows bacteria which leads the doctor to order an antibiotic and the vague symptoms will remain. This has led to the CDC to more specifically define the criteria of diagnosis of a UTI and when a urine test is needed.

If needed, and significant for bacteria, a culture is also ordered to define what bacteria is present and what antibiotic would kill that bacteria to ensure treatment with the right antibiotic avoiding use of an antibiotic that would not be effective.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs.

HOSPICE (See definition in home health grid definitions) Services Provided: N-Nurse – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed.

SSW-Social Worker – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs.

Chaplain – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

Home Health Aide – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. Volunteer – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care: RO-Routine Care – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s residence, this can be their private home, a retirement community, an assisted living, or a skilled nursing facility.

RE-Respite Care – Provided when the family needs time to rest or go out of town for a short time.

Ideally, this service is provided for

five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs.

Continuous care also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be.

The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

When your loved one passes, you may go through a period of bereavement and experience grief. Each hospice agency will have a grief and bereavement support program. Many provide grief groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients?

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their insurance.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care.

Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can

determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness.

Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care.

PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence.

PUBLIC HOUSING (PH) – Affordable apartments for low-income families, the elderly and persons with disabilities.

REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term

rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses.

Occupational therapy services in this setting are generally reimbursed

by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients.

RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from live-in caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes.

Ask about their services when you tour. They are also referred to as personal board and care homes.

Most accept private pay; Medicaid in some instances.

SHORT-TERM CARE – Acute care provided on a short term basis.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance

with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. ~

Orthopedic Rehabilitation: What The Process Could Look Like continued from page 56 surgery is the most rehabilitation intensive surgery of the total joints primarily due to scar tissue build up. Following surgery, you should expect heavy emphasis on walking and improving the range of motion of the knee joint. The total estimated time for full rehabilitation for a total knee is 3-4months in order to regain a full range of motion and to have sufficient leg strength.

Be sure to ask your surgeon for estimations of recovery for your specific case as well as ideas related to the type and duration of therapy

needed to get the most out of your new joint.

Wrap Around Services Help Older Adults Many older adults would prefer Age in Place to age in place instead of being transferred to a long term care center or nursing home.

The Program for All-inclusive Care of the Elderly (PACE) has been providing wrap-around care to participants in Montrose and Delta Counties since 2008. PACE is a Medicare/Medicaid managed care benefit for adults aged 55 or older who choose to live in their homes despite being certified by the state as nursing home eligible. The goal of the PACE program is help program participants remain as independent as possible while living in their own home and community.

Senior CommUnity Care is the Western Slope’s PACE provider and manages the program with a staff of paid professionals of various disciplines. There are two PACE day centers and office locations in Montrose and Eckert. Participants must be able to live safely at home within a PACE site’s geographic area.

The core of the PACE program is the interdisciplinary team. The team works to provide comprehensive medical and social services to the PACE participants. This team includes a primary care physician, nurses, dietician, nursing aides, drivers, physical therapists, occupational therapists, recreational therapists and social workers. The interdisciplinary team meets daily and is responsible for determining care needs, allocating resources, coordination of all services, and development of a comprehensive care plan, which is developed for each

individual participant.

The care plan addresses a set of functional issues that are relevant to the overall health of the older adult participant. These include cognition, emotional status, social activity, mobility, diagnoses, medications, nutrition and other activities of daily living. Payment for services and monitoring of the quality of the services is authorized and directed by the interdisciplinary team.

A PACE Day Center is a location where participants can receive primary care by meeting with a doctor or nurse in the clinic, receive rehabilitation treatments to improve strength and balance, enjoy recreational and social activities, participate in one-on-one or group therapy, receive spiritual support and enjoy a meal. The number of days a participant attends the day center will be discussed with the participant and determined through the assessment process.

By caring for frail, older adults through this model, prevention of acute illness and conditions can often be prevented, thereby reducing the number of emergency room visits and hospital readmissions while allowing the individual to age in place in their own home.
