REHABILITATION FACILITIES
There are two types of rehabilitation facilities; in-patient where you live at a facility for a specific amount of time and out-patient where you visit a facility for services. Most hospitals have an associated rehabilitation department, and all skilled nursing facilities offer rehabilitation. Rehabilitation Hospitals, however, exclusively provide intense rehabilitation care 24/7, frequent physicians’ visits, and rehabilitation team experts directly supervised by a Registered Nurse.

HOSPICE CARE
Hospice Care Definitions
HOSPICE Hospice provides individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort, rather than treatment.
INTERDISCIPLINARY CARE TEAM
Hospice provides care for a patient with a team approach. All disciplines work together to help the patient, caregiver, and family. The team provides for the physical, social, emotional, and spiritual needs of the patient.
Physician – Medical Director –
Oversees the care of a patient by working closely with the RN to make sure the patient’s needs are met. The Medical Director reviews a patient’s chart every 14 days with the Interdisciplinary Care Team.
Registered Nurse – Manages the individual Plan of Care for each patient. Visits the patient to assess needs. Coordinates visits by the other team members.
Aide – Assists patient and family with personal care needs and light housekeeping.
Social Worker – Assesses patient and families’ emotional and social needs. Provides counseling, referrals, and support to meet identified needs.
Chaplain – Supports patient and family with spiritual needs.
Volunteer – Provides companionship and support to the patient and family.
Bereavement Specialist – Offers counseling and grief support to a family for a year after a loved one passes.
LEVEL OF CARE
RO-Routine Care – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the patient’s home, retirement community, assisted living or skilled nursing facility.
RE-Respite Care – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility.
GIP – General In-Patient Care – Provided in a skilled nursing facility when symptoms cannot be managed elsewhere. GIP is usually short in duration.
CC-Continuous Care – Provided for end of life or time of crisis that requires a constant presence by a nurse or care team. The criteria for CC may include: Severe pain or symptoms which are out of control, psychotic episodes, break-down of caregiver, imminent death, or family unable to cope.
24/7 ADMISSION Admission to hospice is 24/7, including weekends and holidays.

a resident to have a short-term stay in a community that can meet their needs. Many assisted living communities and nursing homes have a respite care program. SKILLED NURSING CARE –
Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring a high level of need and routine services. Residents can have a private or a shared room. Meals are served in a dining area, activities are scheduled, and therapies are available as prescribed by a physician. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Medicare 2024 “In A Nutshell”
(Parts A, B & Medigap Plans)
Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”
Medicare Part A Hospital Insurance
Generally no cost (If you paid Medicare taxes while working)
$1,632 deductible (per benefit period)
It is possible to pay the $1632 deductible and other daily charges for multiple stays & extended care.
Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.
Providers are paid by Medicare assignment.
No coverage outside USA. No out-of-pocket maximum.
Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.
Medicare Part B Medical Insurance
Most people pay monthly $174.90 (Premium could be higher based on income)
$240 annual deductible. (this can increase each year)
80/20 after deductible.
Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits
Most doctors accept Medicare.
No coverage outside the USA. No out-of-pocket maximum.
Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.
A & B are designed for people over age 65 and those with certain disabilities
Supplemental Policy Plans A - N Medigap
Premium varies by company, age, location & other factors.
Guarantee Issue at initial enrollment
(Other special enrollment periods may be avalable)
These plans generally pay the 20% not covered by Part B & other Medicare eligible benefits
When this coordinates with Medicare, many benefits are 100% covered
Can provide benefits outside USA
Plans are available through insurance companies.
Important to add Part D plan to cover prescription drugs & avoid penalties.
Must have Part A & B to enroll.
There is NO additional cost to use the services of a full time insurance professional. Please call us or a broker in your local area for additional assistance.
