REHABILITATION FACILITIES

There are two types of rehabilitation facilities; in-patient where you live at a facility for a specific amount of time and out-patient where you visit a facility for services. Most hospitals have an associated rehabilitation department, and all skilled nursing facilities offer rehabilitation. Rehabilitation Hospitals, however,

exclusively provide intense rehabilitation care 24/7, frequent physicians’ visits, and rehabilitation team experts directly supervised by a Registered Nurse.

Our Wound Medicine Specialists™ Provide Advanced Treatments, Personalized Care, and Faster Healing

We believe everyone deserves to reclaim their lives. With over 6.5 million people around America suffering from chronic wounds, we’ve chosen to dedicate our practice to helping our patients find long-term relief from such debilitating conditions. Our Wound Medicine Specialists™ use advanced, proven techniques and state-of-the-art technology to give our patients the best chances for recovery. When you choose to be seen in one of our many private clinics at MedCentris, you’re choosing a team of compassionate, experienced, and knowledgeable professionals solely dedicated to seeing you improve. We provide individualized treatment plans catered to your needs, affordable care covered by most major insurers, accessible treatment with at-home options, and much more to make your road to recovery as smooth as possible.

Our Personalized, Holistic Approach to Wound Medicine

If you suffer from chronic wounds, you’ve likely already been through rounds of different treatments and long hours at stressful medical facilities. At MedCentris, we specialize only in Wound Medicine, so we’re able to provide you with the highest level of wound healing available. Our cutting-edge approach to wound healing starts with understanding the wound’s root cause. Because understanding the underlying cause of a wound can reveal what obstacles a patient might have to overcome. This provides us with a much clearer picture and helps us determine the best plan of care.

Access to Care

To provide the most effective care possible, in addition to our private clinics, we offer access to our Home to Heal® program, which enables us to assist most patients who are unable to leave their place of care or if they are unable to be transported due to extraneous circumstances. We also have our TeleWound® platform, where we can treat patients remotely via the internet. This means we can provide Wound Medicine in places we cannot physically be. So, if a patient is in another city, a remote location, or is isolated, we can still provide care.

Reach Out to Us

If you’re suffering from a non-healing wound, we want to help you find relief. Schedule a consultation at our wound care clinic today by calling MedCentris at 1.855.HEAL.DAT or go to our website MedCentris.com.

MedCentris utilizes advanced wound healing protocols, technology, and innovative products that help wounds heal in the shortest time possible. Our network of highly trained Wound Medicine Specialists™ includes physicians, nurse practitioners, physician assistants, nurses, and clinical support staff in more than 40 locations across Louisiana and Mississippi. We partner with hospitals, physician practices, home health and hospice agencies.

residents’ safety. Staff is usually specially trained. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care. Payor sources usually accepted are private pay or some insurance plans.

RESPITE CARE – Provides a temporary break for caregivers by allowing a resident to have a short-term stay in a community that can meet their needs. Many assisted living communities and nursing homes have a respite care program. Residents typically stay from a week to a month, depending on the situation, and receive all the services of the community to meet their needs. Respite care services are usually private-pay.

SKILLED NURSING CARE – Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring a high level of need and routine services. Residents can have a private or a shared room. Meals are served in a dining area, activities are scheduled, and therapies are available as prescribed by a physician. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Most facilities provide rehabilitation following a hospital stay, and accept Medicare, Medicaid, and private pay. ~

Medicare 2023 “In A Nutshell” (Parts A, B & Medigap Plans)

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Medicare Part A Hospital Insurance

Generally no cost (If you paid Medicare taxes while working)

$1,600 deductible (per benefit period)

It is possible to pay the $1600 deductible and other daily charges for multiple stays & extended care.

Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.

Providers are paid by Medicare assignment.

No coverage outside USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Medicare Part B Medical Insurance

Most people pay monthly $164.90 (Premium could be higher based on income)

$226 annual deductible. (this can increase each year)

80/20 after deductible.

Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits

Most doctors accept Medicare. No coverage outside the USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Supplemental Policy Plans A - N Medigap

Premium varies by company, age, location & other factors.

Guarantee Issue at initial enrollment

(Other special enrollment periods may be avalable)

These plans generally pay the 20% not covered by Part B & other Medicare eligible benefits

When this coordinates with Medicare, many benefits are 100% covered

Can provide benefits outside USA

Plans are available through insurance companies.

Important to add Part D plan to cover prescription drugs & avoid penalties.

Must have Part A & B to enroll.

There is NO additional cost to use the services of a full time insurance professional. Please call us or a broker in your local area for additional assistance.
