What is a Long-Term Care A Long-Term Care Ombudsman Ombudsman? is an advocate for individuals living in a nursing home, an assisted living, or similar licensed adult long-term facility.

This free advocate service is resident directed and requires resident consent to investigate a complaint or to refer a complaint to another agency.

Federal law mandates the LongTerm Care Ombudsman program and every state is required to have and administer an Ombudsman program.

In Colorado, there are approximately 81 Certified Long-Term Care Ombudsmen. These programs are supported by regional Area Agencies on Aging and Disability Law of Colorado both, which receive funding under the Older Americans Act. Ombudsmen advocate and work to resolve problems that residents face.

Our goal is to positively effect change and improve the quality of care at a local, state, and national level for individuals living in long-term care facilities.

The Ombudsman program is responsible for protecting and advocating for Residents’ Rights, which include: n To be treated with dignity and respect n To make independent choices n To participate in social, religious, and community activities outside the facility including the right to vote

Consider a Discount Dental Plan Everyone wants to sell you dental for Savings insurance; but a discount plan can save you hundreds and sometimes even thousands of dollars each year.

When shopping for Dental Coverage make sure you are asking at least four questions. What are my annual premium costs? What are my maximum benefit pay outs? What is the waiting period before I can use those benefits? And does this plan cover specialty procedures?

Traditional Dental Insurance typically have low annual limits on benefits; averaging around $1200 per year. Basic insurance plans with a $1000 maximum benefit can cost $478 per year and a slightly better

Today, the average life expectancy is at a record high of 79 years. As we get older, even the most independent among us may experience physical declines or financial hardship that strips away our independence. Add to that an increase in geographic mobility of our families, and the result is millions of seniors left behind, hungry and alone.

Using Walls or Objects to Balance. Unsteadiness can lead to bracing objects, like furniture and walls, it’s common and dangerous. Holding the wall in a stairwell provides little security in the event of a fall. If you notice these behaviors, it may be time to install grab bars or additional interior railing along walls or stairways to provide support throughout the home.

Avoiding Showering or Bathing. Stepping over tub can be difficult and dangerous. Remodeling the bathroom to remove an old tub and install a walk-in shower or tub is a great option. However, if budget is a concern, our team has excellent and cost-effective options that can restore the ability to bathe safely such as equipment or simple solutions that don’t involve construction.

How Will You Pay for That? What to Know and What to Do Now It is estimated that 70% of all of us who reach age 65 will need some level of Long-Term Care.

Here in Colorado the average cost for Long term care is $8758 per month according to 2020 state information. By nature, only about 15% of us are wired for advanced planning. So, this article is for the rest of us. Not planning in advance causes us to avoid the issue until we can no longer care for our loved one or worse yet, we become a caregiver who is injured in the process or worn to a frazzle by 24 hour caregiving.

Often we avoid planning because we are afraid of learning that we may lose everything paying for care.

Here’s some practical things to know and do now so you can stop worrying about the cost of Long Term Care for yourself… or for ones you love.

The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Find Help Navigating the Medicare can seem like a confusing Medicare Maze web of options. Many seniors choose to rely on licensed professionals for help navigate an average of 24 plans they have to choose from.

The amount of choices can be overwhelming. Adding to the confusion not all plans are available in all areas or to all beneficiaries.

If you work with an agent, it’s important to keep in mind that the agent might not be contracted with all insurance companies that offer plans in your area. This means the agent might not take a look at every coverage choice available to you. Make sure they have evaluated your options through multiple carriers. With plans changing from year to year and new ones available in many areas, there could be a better choice out there for you both in terms of cost and coverage. Your agent should check whether your doctor, hospital, preferred pharmacy and other providers are considered in network.

Be aware that if you call a number on an ad you see on TV or elsewhere, you will not have a face to face, local agent to assist you if there are problems in future.

Also, some Medicare agents focus on selling only supplemental insurance, or Medigap. Those policies help you with copays, deductibles and other out-of-pocket expenses, and can only be paired with original Medicare. While original Medicare with Medigap might be the option for you, you won’t know if you or your agent doesn’t check. You should work with an agent that represents both Medicare Advantage and Medigap plans to be aware of all of your options.

There are federal and state laws governing agents or brokers who sell Medicare plans, which include things such as barring them from showing up uninvited at your house to pitch a plan or trying to lure you with a cash offer. They also cannot legally charge you a fee to process your enrollment as agents generally are compensated through commissions from the insurance carriers they are licensed with.

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Nutrition Programs Are Delivering More Today, the average life expectancy Than Just a Meal is at a record high of 79 years. As we get older, even the most independent among us may experience physical declines or financial hardship that strips away our independence. Add to that an increase in geographic mobility of our families, and the result is millions of seniors left behind, hungry and alone.

1 in 4 older adults lives alone, with 1 in 5 feeling lonely.

Social isolation among older adults is associated with an extra $6.7 billion in Medicare spending each year.

Nearly 9.5 million seniors are threatened by hunger, with more than 5.5 million of which are food insure or very low food secure.

MEALS ON WHEELS Meals on Wheels has been guided by a single goal since its inception, to support our senior neighbors to extend their independence and health as they age. What started as a compassionate idea has grown into one of the largest and most effective social movements in America, currently helping nearly 2.4 million seniors annually in virtually every community in the country.

Frequent in-home visits provide unique opportunities to meet nutritional needs, combat social isolation, address safety hazards and provide holistic care.

CONGREGATE MEALS Congregate meals offer nutritional and social benefits. They bring seniors together to enjoy meals.

Seniors often have a difficult time cooking interesting meals for themselves. SRDA’s delicious meals provide great nutrition that will keep seniors healthy as well as providing an outlet where seniors can create friendships and get to know other seniors in their area. SRDA offers transportation for seniors that are interested in congregate meals, but do not have the ability to drive themselves.

In addition to serving healthy meals, the program presents opportunities for social engagement, information on healthy aging, and meaningful volunteer roles, all of which contribute to an older individual’s overall health and well-being. SRDA has over 10 sites throughout Pueblo County to join.

Senior Nutrition programs help decrease food insecurities that many older adults experience, in addition to decreasing isolation. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

INSURED – This refers to what happens if someone gets hurt on the job.

If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent

with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

MINIMUM HOURS PER VISIT – This refers to the minimum number of hours that an agency provides service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above.

Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness.

SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments.

Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor

functions and reasoning abilities, but also to compensate for permanent loss of function.

Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

HOSPICE CARE DEFINITIONS

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is

provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs.

It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. When Americans prepare to celebrate Thanksgiving, we focus on appreciating the things that truly matter — family and friends, meaningful moments, conversation and laughter.

It’s only fitting that National Hospice and Palliative Care Month also is observed in November, an ideal time for those already coming together to discuss matters that are important to the family as a whole.

Palliative care can be given at any point during the course of illness, while hospice is a form of palliative care provided when physicians determine that a person is nearing the last six months of life. Support extends to family members, too, helping them cope with their loved ones’ illnesses and caring for them at home when possible.

Each year, nearly 1.4 million Americans receive these lifechanging services, which help ensure dignity, respect and peace during life’s most difficult journey.

Rather than focusing on finding a cure, the emphasis shifts to getting the best quality of life, regardless of the quantity of days left.

But many people still don’t know about hospice and palliative care, or they have misconceptions that

become barriers to care. As a result, many individuals still die alone or in pain.

So why does all this matter and why should you consider starting a conversation about end-of-life at your Thanksgiving table? Because Americans are aging at a rapid pace.

According to the U.S. Census Bureau, there are more than 46 million Americans over the age of 65, and that number is expected to double in the next 30 years.

More importantly, research has shown that eight out of 10 Americans would prefer to die in their homes, free of pain, surrounded by family and loved ones. And thankfully, hospice care can grant this wish — when people know about it and fully understand its many benefits.

End-of-life discussions at Thanksgiving need not spoil the other traditions that we’ve grown to care so deeply about. As you gather around your own Thanksgiving table with friends and family this year, my greatest hope is that you will take the time to talk about your wishes. ~

How do you know if a move to senior retirement living is right for you? Sometimes I find myself sitting at my desk trying to make a decision and I’m stuck! Three things have helped me…

Gather more information: Retirement and care communities LOVE to show off through tours, lunches, and your participation in events. Yes, it is okay to ask questions of the residents you meet. Find the activity calendar and see if you can observe something listed for “right now”. Let your trusted senior advisor accompany you so you don’t feel pressured or overwhelmed with details. Your advisor can also screen your options, so you don’t get frustrated considering communities that don’t fit your needs.

Talk to somebody: Sometimes just saying out loud what you are thinking, or questioning, or feeling, can help to sort out what is important to you. A family member or friend can be a good listener.

Your senior living advisor can also listen, answer questions, and clear up misconceptions. Or give ideas regarding homecare, a realtor, home modifications, and other solutions. It may feel like everybody has a differ78 SOUTHERN COLORADO – SPRING 2020-2021

ent opinion on what’s right for you, so it helps to get impartial feedback from an independent source.

Can I try it?: You don’t sign a one-year lease. Most communities and care settings simply require a 30-day notice if you aren’t happy. Of course, use all your tools to make the best senior living decision, but don’t feel like you are trapped if your first decision doesn’t provide the quiet enjoyment, care, and community you deserve. Also, consider Assisted Living respite care for a 2-4 week stay for a trial run, or as coverage for a vacation or event.

I encourage you to START with a senior care advisor, but even if you are already into your decision process, let her/him hear your plan. It is likely there are options and considerations they can add to your search.

Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept.

These restrictions vary by community.

INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths.

Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence.

PUBLIC HOUSING (PH) – Affordable apartments for lowincome families, the elderly and persons with disabilities.

REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilita-

tion services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for longterm residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients.

RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from livein caretakers. RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities.

RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/ location of apartment.

SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SHORT-TERM CARE – Acute care provided on a short term basis.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

Get to Know The Center at What happens if you go to Park West the hospital with an injury or illness and you are unable to return home right away?

The Center at Park West is an 80-bed short-term facility that has been serving Southern Colorado since November 2017. We are rated a 5-Star facility by Medicare and provide rehabilitation and nursing services in a hotel-like atmosphere.

Each patient is provided with a private room, including their own bathroom with walk in shower. Our highly qualified, professional staff offer a comprehensive and interdisciplinary approach to your care. Patients can expect training and education provided by our nursing and therapy teams, medical professionals and our Registered Dietitian, to ensure your safe return home after a hospitalization.

To optimize your functional independence and to facilitate your safe return home, we offer a state-of-theart therapy gym. The 5,000+ square foot rehabilitation gym offers life skills training in our ADL (activities of daily living) apartment, physical, occupational, and speech therapy services. Home Safety Assessments prior to discharge, within a 30-mile radius.

The interdisciplinary team at The Center at Park West strives to provide outstanding patient-centered care and services, from your medical and rehabilitation needs, to your dietary requirements and psychosocial needs.

Our full-time Registered Dietitian assesses every patient and provides individual dietary education, working closely with the kitchen to provide palatable, healthful, and nutritious meals to aide in recovery.

Unlike many skilled-nursing centers that operate inside traditional long-term nursing homes, the Center at Park West is a freestanding building focused solely on shortterm rehabilitation. With our individualized patient-centered treatment plans, family-like atmosphere, and our passion for your success, we are the best choice for short-term skilled nursing care in the area.
