DEAR VALUED READERS: Hello and welcome to the 1st edition of Seniors Blue Book for the Greater Orlando Metropolitan area. My name is Lee Sapp and I am the owner/publisher of this exceptional senior resource guide. First some background on me. I am a native born Floridian and a University of Florida graduate. I am the proud father of David and Jennifer, and the grandfather of Austin and Haley. (Hint) Austin and Haley, along with my mom, are on the cover of this, our 1st edition. My career choices have always been related to healthcare. I started as a salesman in pharmaceuticals, continued into medical sales, and after earning an MBA, I became a hospital administrator. It has been a wonderful, challenging, and constantly evolving career. After 35 years in healthcare, I began to look to see what other opportunities I might pursue, and discovered Seniors Blue Book (SBB). While in a different professional category, Seniors Blue Book still affords me with the opportunity to provide assistance to people in need of important information and educational articles related to healthcare choices and MUCH more. Meet Our Cover Girl – “My Mom” Thank you for selecting this, our 1st edition of Seniors Blue Book for the Greater Orlando Florida Metropolitan area. Our goal is to become the selection of choice for anyone searching for information related to senior citizens. Please indulge me for a few moments. I elected to present my mom (my favorite senior citizen), and her great grandchildren (my grandchildren) on the cover of this edition. My thoughts were to help us realize that becoming a senior citizen allows us to both live in the present and enjoy the future. My mom is 82 years old and lives in Valrico, Florida. In addition to being the mother of my 2 sisters and me, she has 6 grandchildren and 16 great grandchildren. She became a widow in July 2013 and she says the joys she experiences from her children, grandchildren, and great grandchildren are her greatest blessing. My mom was born in Alabama and moved to Winter Haven, Florida when she was 5 years old. She graduated from Winter Haven High School in 1951. In addition to raising 3 children, she was employed as the executive secretary to the hospital administrator in Bartow Florida. She excelled in and enjoyed that position for more than 10 years. Upon encouragement from the administrator, she made the decision to pursue her own career as a business woman/entrepreneur and created her own medical dictation business. She grew this business for many years before

electing to retire with my dad. Both my parents were very involved with me and my sisters while growing up and participated, even if only as spectators, in all our activities. Mom was very active in her church and loved to sing. My mom has always been an attractive woman and upon urging from her friends, she entered the Mrs. Florida contest and while she did not win, she was thrilled to be named 1st runner up. Losing my dad more than 2 years ago was very hard. To deal with the pain, she stayed busy in her church and kept in close touch with friends and of course, her family. As time passed, she decided that she had to continue living her life. Her first truly ‘solo’ decision was to buy a new car. As a surprise to all of us, she came home in a brand new RED 2 door Cadillac CTS. You might notice it on the cover. She said she always liked Cadillacs and the color red. So, there you go. My mom has embraced life and is living it to the fullest. She is living proof, that no matter what your circumstances or age, you get out of life what you put into it. So go ahead – whatever it takes to remind you to live life to its fullest – choose that. It does not matter is it’s a Red Car, a Red Hat, or Ruby Red Slippers. It just needs to be meaningful to YOU. Starting an Exercise Program: We recognize and have heard time What Do and again that the key to staying healthy is proper diet and exercise. Seniors Need? But “exercise” can be quite vague to those who encounter age-related physical obstacles or have never been part of a fitness regimen.

Few activities are more important to senior health than those which help improve balance, flexibility, strength and endurance. Simple practices around the house can help improve balance or strength without disrupting daily routine. Standing up from a chair or walking up stairs can improve a sense of balance and build muscle strength when practiced and repeated. Even bending over to pick something up can be beneficial. In addition to building strength and balance, endurance exercises are important to maintaining heart health and metabolism. This can mean anything from raking leaves to going for a bike ride –it’s simply important to increase the heart rate. And for individuals with joint problems, swimming allows seniors to access all of the challenges and benefits of exercise while easing strain on problematic areas. It’s also good to remember that keeping the mind active and atten-

20 GREATER ORLANDO AREA – WINTER 2015-SPRING 2016

tive is just as important as physical health, often driving the body’s overall health. Some of the best exercises for both body and mind are derived from Eastern cultures. Tai chi is an excellent practice for older adults. Yoga has many different forms, but all include a two-fold approach: holding and maintaining a series of postures while focusing on breathing. Before attending just any yoga class, make sure it is specifically tailored for older adults to ensure comfort and safety. I can say over and over how important it is to our physical wellness to practice balance, strength, endurance and stretching, but perhaps the ultimate benefit of exercise programs is an increase in self confidence. Medicare 2016 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. Hundreds of seniors and their families are struggling with this question every day! There is probably no VA program more misunderstood than “Aid and Attendance.” In fact, “Aid and Attendance” isn’t even the right name…and that’s just for starters. We’ll ask some typical questions and provide the answers with references cited for those of you who want to dig deeper into the actual law itself. 1. No “Aid and Attendance?” What most people refer to as “Aid and Attendance” is the Non-Service Connected Disability Pension, also sometimes called the Improved Pension. Pension is a guarantee by the VA that eligible wartime veterans and their un-remarried surviving spouses will be granted a minimum monthly income.[38 C.F.R. 3.3] Aid and Attendance is just a special, additional benefit that can increase the funds paid to Pensioners if they have a medical problem requiring the regular help and assistance of another person in safely performing daily living tasks.[38 C.F.R. 3.352] 2. What are the eligibility requirements for Pension and who can apply? There are four points of federal law that must be met: (i) minimum wartime military service, (ii) marital status (if applicable), (iii) limited

net income, and (iv) limited net assets.[38 C.F.R. 3.3 and 3.272]. A single veteran, married veteran or surviving spouse of a veteran can be considered for benefits. The veteran must have served at least 90 days of active, federal, military service, oneday of which must have been during a wartime period, and cannot have been un-desirably discharged.[38 C.F.R. 3.3] 4. How much income is “too much?” Gross household income greater than the amount of benefit guaranteed in law makes a claimant ineligible. However, gross income may be reduced dollar-for-dollar by un-reimbursed, regularly occurring, predictable medical expenses.[38 C.F.R. 3.272(g)] Each case is unique and well worth a phone call to check potential eligibility. 5. Can just anyone help me with a VA claim? No. Anyone who represents a claimant or even gives advice concerning a VA claim must be accredited by the General Counsel of the VA. The VA accredits attorneys, agents or veterans service officers. VA law is complicated and there is much to know to adequately represent a potential client. Be careful and use good common sense! 6. Is it legal to charge fees for representation before the VA? No,

However, a VA General Counsel opinion from May, 2004 allows attorneys to charge for what VA terms a “pre-filing consultation” as this legal work takes place before a person becomes an actual claimant for benefits.[38 C.F.R. 14.617(h)] 7. I applied for benefits and was denied. Is there anything I can do? Yes. It is best to have the claim reviewed by a knowledgeable accredited individual and if advisable, file a request to reopen the claim. Osteoporosis “Breaking” News Osteoporosis is a bone condition that makes bones thinner and more fragile secondary to loss of bone density.

There are 40 million people affected by osteoporosis, women more often then men. Half of all postmenopausal women will have an osteoporosis related fracture during their lifetime. Fractures occur most frequently in the spine, hip and wrist. Taking measures to avoid falls is important in the prevention of fractures for those already diagnosed with the disease. Risk factors are numerous, some modifiable and others unchangeable. Non-modifiable risk factors include race (Caucasian and Asian), age, previous low-energy fracture, small bone structure and a family history of osteoporosis. More important are the modifiable risk fractures including tobacco smoking, excessive alcohol intake, inactivity, reduced calcium and vitamin D intake and reduced sex hormones. Taking medications such as prednisone, heparin and excessive thyroid replacement can also affect bone density negatively. Diagnosis is made by measuring bone mineral density with DXA scanner. There are no symptoms of osteoporosis other than fracture. A score on the DXA below -2.5 is diagnostic for osteoporosis. A score between -1.0 & -2.5 is indicative of osteopenia which

represents mild bone loss. Treatment involves lifestyle changes including getting 1200 mg of calcium daily thru diet and supplementation. Getting adequate vitamin D approximately 800iu thru diet, sunlight and supplementation. Weight bearing exercise daily. Quit smoking and drink alcohol in moderation only. Additionally, establish a fall prevention program including night lights, wear sensible shoes, de-clutter space in your home, and use assistive devices like canes and walkers as needed. Also review medications that cause sedation or lightheadedness with their physician. There are numerous medical treatments for osteoporosis. Most drugs like Fosamax block resorption of bone. Drugs like Evista effect the hormone estrogen and can reduce spine fractures. Forteo is the only drug I am aware of that increases bone formation. The newest treatment is a form of immune therapy called Xgeva. There are several options you can discuss with your physician; ultimately combination therapy may be the most effective. Recognizing Physical Signs of Most people are familiar with the Alzheimer’s cognitive symptoms of Alzheimer’s Disease; memory loss, inability to remember names and places, difficulty speaking, and mood swings.

However, the physical signs of Alzheimer’s are not as well known. By becoming familiar with the physical signs, it is easier to identify Alzheimer’s in its early stages, allowing treatment to begin as soon as possible. The first alerting sign is repeating unusual actions, like opening the refrigerator repeatedly, aimlessly walking between rooms, or looking for an item that has already been found. Other signs, especially when uncharacteristic, are lack of personal hygiene, and wearing the same clothing repeatedly, either from forgetfulness or apathy. Another symptom of Alzheimer’s is having fresh bruises and cuts with no memory of how they got there. Common cognitive symptoms of Alzheimer’s are sundowning and wandering, dangerous activities that could result in these injuries. It is important to manage these behaviors to reduce injury risks for the person suffering and caregiver. Loss of fine motor skills is another alerting factor. This lack of precise

movements is particularly noticeable at dinnertime when trying to grip utensils. Having a difficult time using dinner utensils could be a sign that something isn’t right. Facial expressions such as frowning, grimacing, keeping eyes tightly closed, or rapidly blinking could all be signs of Alzheimer’s. Bodily expressions such as rigid posture, fidgeting, rocking, or changes in walking patterns are also warning signs. Because Alzheimer’s Disease is easiest to manage when detected early, it’s important to recognize these symptoms to get help as soon as possible. Don’t hesitate to call a physician, as they will be able to help find the appropriate resources. Respite, hourly, or live-in caregivers can be good options to help a family member, friend or loved one feel comfortable and safe in the home. to deliver nutrients through a tube if you cannot take food or drink through your mouth. HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare

guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team. 24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive,

psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. Why Choose Hospice? The word hospice often brings negative emotions like fear and failure and worries about the cost. Many families are not ready to let their family member go or they are angry that they do not have more time. I experienced these emotions as my mother-in-law, Barbara, was struggling to breath with her chronic condition of heart failure. Her son was very adamant that if she got up and walked around with the assistance of a physical therapist or stopped taking her medications she would get better and live a long time. I could see the signs of her decline and realized she needed additional help with basic activities like bathing, dressing and transfers. After several family conferences, the family agreed to place her on hospice. The hospice services provided a nurse on a regular basis to help manage her pain and other health conditions that come with living with a chronic illness. A physician trained in palliative care directed the management of her care. Hospice aides helped in keeping her clean and well groomed. Additional emotional support came from an involved Medical Social worker.

Barbara enjoyed having the opportunity to unpack her life with a Chaplin and resolving past issues with her spirituality. An added bonus was the music therapist, who played songs of her youth and often would ease her breathing by the type of music which he played. Barbara became very close to those who served her at this critical time of her life. The biggest miracle of hospice was mending of family relationships which had become strained. Forgiveness was given for mistakes of the past and the family was able to heal. When Barbara passed there was a sweet peace that filled the room. Gandhi said “Where there is love there is life.” Hospice neither hastens nor prolongs life. Hospice is a way of caring that brings comfort to persons living with a terminal illness, and gives support to them as well as to their families. As a family member, I am grateful for the hospice services which are available. What is so helpful to families in this situation, Medicare pays 100% of all of these necessary services. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary

care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. As a senior living professional who has helped several thousand clients navigate senior housing and service options, this one tip can significantly simplify the process. Create a personalized list of determining factors before you begin your search. Some of the factors to consider are: Location. Many out-of-state seniors choose Florida due to the weather and lack of a state income tax. Locally, a desire to be in close proximity to family, a hospital or convenient access to doctors may be key. Furthermore, consider the preference of a rural setting with wildlife and nature versus a fast-paced urban lifestyle. Services. If you enjoy cooking or light yard work, this might be a short list. However, it’s helpful to anticipate these and list the specific services and health care options you may need in the future. Licensed CCRC’s are not the only place where comprehensive senior services are offered that can change as your needs do. By finding a place where you can live independently with like-minded

peers but have access to higher service levels if and when needed, can provide a social safety net and peace of mind for you and your family. Price. Project a budget but don’t forget that many current monthly expenses like lawn maintenance, utilities, association dues, cable TV, expensive health and golf memberships, and even groceries may no longer be necessary. A senior community may also help avoid expensive home repairs, home owner’s insurance and property taxes which can fund future care needs. Many companies may appear similar but financially, for-profit and not-for-profit companies operate very differently. Amenities. Creating a list of current hobbies or interests tends to be easy. After all, having a place for your boat or RV is important. Besides the obvious, take time to consider past interests stretching back to your child-rearing years or an empty nest period as these could be rekindled. Listing possible future endeavors might be the hardest to obtain. Keep watch for these while touring a potential community as amenities are simply tools for building friendships. If you do, these could be symptoms of depression and should not be ignored. Six million people age 65 or older in our country are affected by depression. For some, this may be their first experience with these feelings of sadness. For others, they may have experienced episodes of this illness for as long as they can remember. Feelings of depression in older adults, may certainly be associated with a significant loss of a loved one. Receiving a diagnosis of macular degeneration, experiencing a heart attack or a hip fracture may also cause a sudden onset of depression.

It is alarming to see that less than 10% of those individuals actually receive treatment. Many older adults may be ashamed to admit their feelings. The good news is that upon diagnosis, 80% of these individuals can be effectively treated and relieved symptoms. Although clinical depression may be common in the elderly, it should not be referred to as a normal of aging. Some of the most common symptoms are:
