Case Management

Are you experiencing an unexpected challenge? Do you need help to stay independent in your home? If you’re 60 or older, an Area Agency on Aging case manager may be able to provide one-on-one help by phone or at your home. Your case manager will work with you as long as they can help you. State Health Insurance Assistance Program (SHIP)

Do you currently have Medicare, or will you soon be eligible for Medicare? Area Agency on Aging Medicare counselors provide unbiased information, counseling and enrollment assistance at no cost. SHIP counselors can help you learn more about your benefits and understand your summary notice. They are available to provide presentations on enrollment and preventing fraud to community groups. Veteran-Directed Care

Veteran-Directed Care serves eligible veterans of any age who are at risk of moving to long-term care facilities. Through the program, eligible veterans receive home and community-based services, including allowing them to pay a trusted caregiver who can help them continue to live at home. The program is administered in partnership with the Department of Veterans Affairs, which determines eligibility. For more information in Spanish and English, call 303-480-6700.
