Larimer County Office on Aging (LCOA) Ombudsman If you live in a long-term care setting, there might be a time where you need someone who can help you understand your rights. An ombudsman advocates for the rights of residents in assisted living communities and nursing homes. Ombudsmen work with residents to make sure they receive the care they want and need. They help residents maintain their quality of life and receive quality care in licensed long-term care communities.

The Ombudsman program can help with: u Investigating and resolving complaints u Mediating conflicts u Finding a long-term care home u Providing unbiased consultation and education on the daily operations, resident rights, and regulations of long-term care homes.

Guardianship and Conservatorship: What You Need As we age, issues of guardianship and conservatorship may come up. to Know If at some point an individual is no longer able to make financial or medical decisions a court may designate a guardian, conservator, or both.

The court’s decision to appoint a guardian or conservator removes the individual’s rights to make decisions due to the individual’s inability to manage safely and effectively on their own. Those entrusted with these roles take over making decisions for the protected person.

Colorado separates guardianship, appointing someone to make health and welfare decisions, and conservatorship, appointing someone to manage finances and assets.

Separating these roles allows for an individual to continue making health and welfare decisions when able, while appointing someone to take over financial decisions. The opposite could occur, depending on an individual’s situation and ability.

When both a guardian and conservator are needed the court may appoint two different people required to work together making decisions in the best interest of the protected person.

A conservator works to ensure the protected person’s assets are used wisely for housing, medical, and other needs. Allowing bills to be paid and bank accounts managed. The guardian manages medical needs, support, care, welfare, housing, etc.

Leaving the decision of who takes on these critical roles to the court may not be in your best interest. By naming individuals within powers of attorney you allow yourself a say in the decision.

A financial power of attorney allows you to name a conservator, and indicate which financial decisions they are allowed to make for you. As your situation and needs change this document may need updating to fit your circumstances.

A medical power of attorney names someone to act as guardian, making health and welfare decisions when you are unable.

When appointing a guardian or conservator the court takes several factors into consideration, including those named within powers of attorney. Ensuring someone appointed guardian or conservator will act in your best interest is crucial. You want someone who will manage your assets and medical care wisely. A Checklist for When a Spouse or Parent Passes When you lose a spouse, partner, or parent, the grief can be overwhelming.

In the midst of that grief, life goes on.

There are arrangements to be made, things to be taken care of – and in recognition of this reality, here is a checklist that you may find useful at such a time.

First, gather documents. Ask for help from other family members if you need it. Start by gathering the following.

A will, a trust, or other estate documents. If none of these exist, you could face a longer legal process when settling the person’s estate. n A Social Security card/number. Generally, the person’s Social Security number will be retired shortly following the death. If you are uncertain, consider checking with the Social Security office. n

Then, gather these additional highly important items. ny account statements A Deeds/titles to real estate n Car titles or lease agreements n Storage space keys/account records n Any bills due or records of credit card statements n Any social media platform information, if applicable n n

Last, but not least, look for a computer file or printout with digital account passwords. Prior to their loved one’s passing, some family members may try to centralize all this informa30 NORTHERN COLORADO – OCTOBER 2021-2022

tion or state where it can be found.

In addition, see if the person left a letter of instructions. A letter of instructions is not a legal document; it’s a letter that provides additional and more-personal information regarding an estate. It can be addressed to whomever you choose, but typically, letters of instructions are directed to the executor, family members, or beneficiaries.

Following these steps, address financial, insurance, and credit matters. Investment and retirement plan accounts and insurance policies should have beneficiaries, so reach out to the financial and insurance professionals who helped your loved one as well as the person overseeing their workplace retirement plan. Talk with these professionals to learn about the possible tax implications from inheriting these assets.

State and federal taxes for your loved one will also need to be paid, and possibly, other taxes for the year of their death.

Keeping Your Brain Healthy Many people expect to live to age 90 and beyond. The chances of developing Alzheimer’s (the most common type of dementia) or other types of dementia increase as we get older. Many of my patients ask me, “How do you keep your brain healthy?”

It would be nice if there were a simple supplement or medication to prevent dementia, but unfortunately, this is not the case.

The good news is there are things you can do to improve your overall health and well-being as you get older, and these things also help keep your brain healthy.

First, it’s important to take care of your physical health. Avoid tobacco or too much alcohol. For people over 65, that means no more than one drink per day. Some medications, including over the counter, can harm the brain, so review your medications with your doctor or pharmacist at least once a year. If you have high blood pressure, diabetes, or high cholesterol, work with your health care team to keep these conditions under control. Enjoying a healthy diet with plenty of fresh fruits and vegetables is also helpful.

Second, get plenty of physical activity – this is one of the best things you can do for your brain health. With more physical activity, you’ll feel better, sleep better, have

less stress, and have more energy.

Choose an activity that you enjoy and invite a friend or family member to join you. If you are just starting

out, go slow and gradually increase, with a goal to get to at least 30 minutes of moderate activity (like a brisk walk), at least five days a week.

Third, learning new things helps form new connections in your brain.

Dusting off an old musical instrument, learning a new language, or solving puzzles are examples of fun activities to try.

Finally, it’s essential to stay socially connected. Spend time nurturing friendships and family relationships.

The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/medications that need to be introduced into the body via a patient’s veins.

Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendant or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Stand-Alone, Hospice Inpatient Care Center Being Added When a person makes the difficult decision to no longer continue curafor Northern Colorado tive measures for a terminal illness and seeks the help of a hospice care provider, most of the time they can receive all the care they need in the comfort of their own home or the care community where they live.

However, there are times when a person needs a higher level of care to keep them comfortable. More support than what can be provided in a home setting. Many times, this higher level of care is provided in a hospital or other acute care setting.

While this can be an adequate way to manage a patient’s pain and symptoms, it is not the best setting for complete care of the patient’s emotional and spiritual needs and, not the best setting for family and loved ones to help and be involved. These are the times when an inpatient care center or “hospice house” provides the medical support needed to manage acute pain and other symptoms while offering a comfortable, home-like setting.

To meet the needs of patients who require inpatient level hospice care, and future growth of the northern

Colorado community, a 12-bed inpatient care center designed to care for hospice patients with the most acute end-of-life needs is being added to the Pathways campus at 305 Carpenter Road between Fort Collins and Loveland.

In this stand-alone facility, each of the 12 rooms will be designed to accommodate groups of visitors as well as family members who wish to stay overnight with a patient. There will be access to outdoor garden spaces where patients will be able to enjoy the outdoors from the comfort of their bed. Four Common Myths Myth #1: Hospice is a place.

About Hospice Fact: “Hospice is not a place but rather, a philosophy of care,” Dr. Payne said. “Hospice comes to you wherever you call home, surrounding you and your caregivers with the support you need at the end of life.”

Myth #2: Hospice is giving up.

Fact: “Hospice is not giving up,” Dr. Payne said. “It is enhanced care at the end of life, to maximize quality of life at the end of life.”

Myth #3: Hospice is only for those who are actively dying.

Fact: “Hospice is for any person with chronic illness whose life expectancy is six months or less,” Dr. Payne said. “The true benefit of electing hospice care is seen when hospice is brought in sooner rather than later.”

Myth #4: Can patients actually improve?

Fact: This one is actually true!

“To remain on hospice, a patient needs to have a life expectancy of six months or less should the disease process run its normal course,” Dr. Payne said. “Many times, we see hospice care come into a patient’s life and wrap their arms around a patient and family, getting them the right care at the right time, and

the patient actually improves. The disease process may still be there, but perhaps the patient has learned to manage it differently, they now have the resources they need.

Some patients do not continue to decline but rather, improve.

Big takeaway Hospice care is generally an appropriate option for those who are terminally ill with approximately six months or less to live. While there are generally some misconceptions about hospice care, first and foremost, it is about maximizing a patient’s quality of life at the end of life. In addition, hospice care can provide support, resources and information to you and your family during this difficult time, including bereavement counseling.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs.

It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope.

Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays.

AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being.

This is not reimbursed by Medicare.

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief.

Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.

The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

Alzheimer’s disease can be one of the most emotional and devastating diagnoses that effects an individual and their family.

As a progressive brain disorder, the disease depletes memory and thinking skills, making even daily life tasks difficult. Estimates vary, but experts suggest that more than 5 million Americans may have some form of Alzheimer’s. It is currently ranked as the sixth leading cause of death in the United States.

Although extensive global research continues to explore the causes and treatments, unfortunately no cure currently exists.

Alzheimer’s disease damages the hippocampus, the part of the brain that controls the formation of memories. Vital nerve cell connections to the brain also begin to deplete which prevent the brain from sending messages to the body’s muscles and organs. Over time, as nerve cells perish, other parts of the brain are altered.

Simple activities like preparing a small meal, spending time outdoors or driving a car become more difficult for your loved one. As the disease progresses, your loved one can get lost easily, misplace items and even become confused doing the most basic of tasks. As the dis-

ease advances, some personalities can develop into a worried, angry, or even violent state.

Treatments for Alzheimer’s disease can include a variety of options and modalities. Working with professionals such as an Occupational Therapist, Neurologist and Geriatrician can help greatly. Cognition-enhancing medication can also prove helpful for some people. Research that is helping loved ones and caregivers establish a better quality of life with Alzheimer’s disease is on-going but keeping your loved one safe is often the greatest challenge.

The Final Season I’ve often heard the phrase, “Getting old isn’t for sissies”, from my mom and her friends. When someone reaches their end-of-life season, they frequently need extra help with things like cooking, cleaning, personal care, household chores, and more. It’s exhausting, and it’s hard.

When this final season begins, it puts extra stress on family who may be providing the care themselves or trying to find quality caregivers to help. Elderly people may move in with a relative or find placement in a facility or a home where assistance is provided.

tor said we could be in “COVIDmode” for the next two years.

What? I thought to myself, please don’t say that out loud. Don’t send it out into the universe!

Deep down, I knew she was right.

I also knew COVID would become part of the health care fabric of diseases we need to manage and treat for the older adults we serve.

As we have walked the COVID journey, we saw our residents, staff, and families become strong and then stronger. Their resiliency and dogged determination were an inspiration. Everyone was in the trench fighting together. Then the light came at the end of the tunnel…. vaccines. What a blessing.

As the COVID positive numbers declined, so did everyone’s energy.

Their enthusiasm for the fight had waned. People were tired. Residents. Staff. Families. They were emotionally worn out. Our resiliency meter was low. Some had experienced trauma. COVID has brought trauma.

According to the National Council for Behavioral Health, 70% of adults have experienced some kind of traumatic event. COVID is not even in this equation, yet.

For those of us in senior care, there is a high likelihood that many of our residents and staff could

be affected by trauma. Would it be safe to say, we should approach each thinking there may be a response from something that occurred in their life? We think so.

We think COVID has added to our mental health and resiliency, and not always in the best way.

Should we become a Trauma-Informed organization? We answered with a resounding, YES! The Trauma Informed Care Project of Orchard Place/Child Guidance Center defines Trauma-Informed Care as an organizational structure and treatment framework that involves understanding, recognizing, and responding to the effects of all types of trauma; emphasizing physical, psychological, and emotional safety for both consumers and providers, and helps survivors rebuild a sense of control and empowerment.

Realizing there may be trauma, recognizing the signs and symptoms, responding with trauma informed policies/practices, and resisting re-traumatization are the key principles to a successful trauma-informed effort. ~

Occupational therapy services in this setting are generally reimbursed

RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from livein caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances.

RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities.

RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are

typically based on size/ location of apartment.

SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SHORT-TERM CARE – Acute care provided on a short term basis.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians

MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed

by Medicare.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump.

If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness.

This service is not reimbursed by Medicare.

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
